골반안정화 운동프로그램이 엉치엉덩관절 통증을 동반한 만성요통환자의 통증과 엉덩관절에 미치는 영향 (The Effect of Pelvic Stability Exercise Program on Pain and Hip Joint of Patients with Chronic Low Back Pain involving Sacroiliac Joint Pain)

  • 본 연구는 엉치엉덩관절 통증을 동반한 만성 요통환자 52명을 대상으로 6주 간 골반운동프로그램 및 전통적 물리치료를 병행한 실험군 27명과 전통적 물리치료만을 중재한 대조군 25명을 무작위 임상 표본 추출 하였다. 두 군 모두 통증의 정도를 알아보는 오스웨스트리-요통장애지수검사와 보행 시 엉덩관절의 각도분석을 한 후, 집단별 실험 전과 실험 후의 오스웨스트리-요통장애지수검사와 보행 시 엉덩관절의 각도분석을 비교하여 골반안정화 운동프로그램이 엉치엉덩관절 통증을 동반한 만성요통환자의 통증과 엉덩관절에 미치는 영향을 알아보고자 하였다. 요통의 변화는 집단 내와 집단 간에서 모두 유의하게 감소되었음을 알 수 있었고, 엉덩관절 각도비교에서는 실험군내 좌측 엉덩관절에서 중간디딤기, 말기디딤기, 유각전기, 초기흔들기와 우측 엉덩관절의 중간디딤기, 말기디딤기, 유각전기, 초기흔들기에서 차이가 있었다. 대조군 내 좌측 엉덩관절에서는 중간디딤기, 말기디딤기, 초기흔들기와 우측 엉덩관절의 체중전달기, 중간디딤기, 말기흔들기, 유각전기에서 유의한 차이가 있었다. 따라서 골반안정화 운동프로그램이 통증을 포함한 요통기능장애수준에 효과적이고 보행 시 입각기와 유각기 동안 엉덩관절의 굽힘과 폄이 리듬 있게 이동하여 보행이 대칭적으로 자연스럽게 이동되는데 도움이 되는 것으로 나타났다.

큰볼기근 근력강화운동이 척추뒤굽음자세를 가진 대상자의 척추정렬 및 동적균형에 미치는 영향 (The Effects of a Gluteus Maximus Strengthening Exercise on the Spinal Alignment and Dynamic Balance of Kyphosis Subjects)

  • Purpose: This study was performed to determine the effects of a gluteus maximus strengthening exercise on both spinal alignment and dynamic balance in subjects with kyphosis. Methods: We measured the kyphosis angle of 150 subjects and selected those whose kyphosis angle was > $50^{\circ}$. The participants included 10 male and 12 female college students. After a gluteus maximus strengthening exercise was applied, the subjects were measured by the formetric 4D and Biorescue systems. The collected data were analyzed by a paired t-test with the SPSS (Ver. 21) program for spinal alignment and dynamic balance comparisons both before and after the gluteus maximus strengthening exercise was completed. Results: The results regarding spinal alignment showed statistically significant decreases in pelvic tilt, kyphotic angle, and lordotic angle after the intervention (p < .05). However, trunk imbalance, pelvic torsion, surface rotation, and lateral deviation were not significantly different after the exercise. The results of the dynamic balance showed statistically significant increases in limits of stability after the exercise (p < .05). Conclusion: The above results suggest that the implemented gluteus maximus strengthening exercise may be effective for spinal alignment and dynamic balance in subjects with kyphosis.

PNF 어깨뼈-골반 패턴이 편마비 환자의 보행에 미치는 영향 -증례보고- (The Influence of Scapular-Pelvic Patterns of Proprioceptive Neuromuscular Facilitation on Hemiplegic Gait -A Case Report-)

  • Purpose: This study examined changes in gait speed and stride length after an intervention involving simultaneous scapular and pelvic patterns of proprioceptive neuromuscular facilitation in a hemiplegic patient. Methods: A 58-year-old woman with left hemiplegia who had complained of slowness of gait speed and weakness of leg strength took part in an intervention involving scapular postdepression patterns on the affected side and pelvic postdepression patterns on the nonaffected side. The intervention was performed with the patient lying on her left side, in a half kneeling position, and in a standing posture. Rhythmic initiation was used for teaching the movements to the patient and improvement of kinesthesia, and a combination of isotonic was employed for increasing strength and irradiation of the scapula and pelvic movement. The intervention took place for 30 min. It was implemented twice a day, 5 days a week, for 3 weeks. After three repetitions, the average time taken to complete the 10-m walk test (10 MWT), in addition to stride length, was measured to determine gait speed. Results: After the 3-week program, the patient's performance in the 10 MWT improved from 21.7sec to 17.1sec, and her stride length improved from 31.4cm to 38.7cm. Conclusion: The results showed that trunk movement exercise, especially coordinative movements of the scapula and pelvis can improve gait speed and stride length by increasing trunk stability and mobility. A combination of pelvic and scapular patterns can facilitate trunk rotation, thereby improving gait speed and stride length.

8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향 (The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain)

  • 이 연구는 8주간의 척추 안정화 운동프로그램이 경부 장애 지수, 균형 능력 및 자세 변화에 어떠한 영향을 주는지를 알아보는 것이다. 운동 프로그램은 8주간 주 3회 척추 안정화 운동을 시행하였으며, 총 60분으로 구성하였다. 연구의 대상자는 6개월간 경부통을 호소하는 만성 경부통 환자 16명을 대상으로 운동 집단(n=8명)과 통제 집단(n=8명)으로 분류하여 운동 프로그램 사전, 사후의 결과를 관찰하여 다음과 같은 결과를 도출하였다. 경부 장애 지수는 시기, 집단, 상호작용에서 유의차가 나타났다(p<.05). 균형 능력은 집단 및 시기 간에서 유의한 상호작용 효과가 나타났다(p<.05). 자세 변화는 골반 경사각에서 시기, 상호작용에서 유의차가 나타났고(p<.05), 경부 및 어깨 위치각은 집단, 시기, 집단, 상호작용 모두 유의차가 나타났다(p<.05). 이상의 결과 척추 안정화 운동이 만성 경부통 환자에게 경부 장애 지수, 균형 능력 변화, 자세 변화에서 유의하게 개선된 결과를 나타내었다. 추후 연구에서는 방사선 영상을 통한 척추 구조 변화에 대한 구체적인 변화를 분석하여 척추 안정화 운동의 타당성을 높이는 연구가 필요할 것으로 사료된다.

뇌졸중 환자에서 자세정렬변화가 족저압 및 균형에 미치는 영향 (Effects of Changes in Postural Alignment on Foot Pressure and Balance of Patients with Stroke)

  • Purpose: This study was conducted in order to investigate the exercise limit that may occur depending on changes in postural alignment by examining the significance of postural alignment changes, foot pressure, and balance of patients with stroke. Methods: In this study, 50 patients diagnosed with a stroke were selected as subjects. Imbalance of postural alignment of the trunk, pelvic tilt of trunk rotation of the body, angle of kyphotic curving of the thoracic, and angle of lordotic curving of the lumbar vertebra were measured. Foot pressure was examined by measuring average pressure and weight bearing. Balance was examined by measuring the center of pressure and limit of stability. Results: The significance of postural alignment, foot pressure, and weight bearing of the non-paretic side was examined. In addition, the significance between postural alignment and balance was examined. Conclusion: It is thought that limits of foot pressure and balance in the standing position can be caused by postural alignment. Thus, both a therapeutic intervention program and postural alignment training should be provided together in order to improve the function of patients with stroke.
