Assessment of cardiac function in syncopal children without organic causes

  • Background: Syncope is a common problem in children and adolescents. However, a large proportion of syncope cases have no underlying cause. Purpose: This study aimed to identify the factors affecting the severity of syncope using tissue Doppler imaging (TDI). Methods: This retrospective study included 61 children and adolescents with syncope who underwent echocardiography. The head-up-tilt test (HUT) was performed when there was a more severe syncopal event. We compared the echocardiographic findings between the execute HUT and nonexecute HUT, negative HUT result and positive HUT result, and normal electrocardiogram (ECG) and abnormal ECG groups. Data were analyzed using an unpaired t test post hoc analysis. Results: In the execute and nonexecute HUT groups, the odds ratios were 0.55 for medial E/E' (P=0.040) and 0.64 for lateral E/E' (P=0.049). Comparison of the results of the decreased, normal, and increased groups for lateral E/E' revealed a significant difference in the execution HUT and nonexecute HUT groups (overall, P=0.004; decreased vs. increased, P=0.003; normal vs. increased, P=0.050). Conclusion: Medial E/E' and lateral E/E' were decreased in patients with severe syncopal events. These findings suggest that the presence of left ventricular diastolic deterioration may cause hypoperfusion even in the absence of organic causes and, consequently, increase syncope severity and frequency. The TDI measured by echocardiography can be used as an index to predict syncope recurrence and/or severity.

Risk factor for pituitary dysfunction in children and adolescents with Rathke's cleft cysts

  • Purpose: This study evaluated the clinical manifestations of and risk factors for pituitary insufficiency in children and adolescents with Rathke's cleft cysts. Methods: Forty-four patients with Rathke's cleft cysts younger than 19 years who visited Seoul National University Children's Hospital between January 1995 and September 2009 were enrolled. Rathke's cleft cysts were confirmed histologically through an operation in 15 patients and by brain magnetic resonance imaging (MRI) in 29 patients. The clinical, hormonal, and imaging features were reviewed retrospectively. Results: The clinical presentation of symptomatic patients was as follows: headache (65%), endocrinopathy (61%), and visual disturbance (19%). Endocrinopathy included central precocious puberty (18%), diabetes insipidus (14%), general weakness (11%), and decreased growth velocity (7%). After surgery, hyperprolactinemia resolved in all patients, but growth hormone insufficiency, hypothyroidism, and diabetes insipidus did not improve. Pituitary insufficiency except gonadotropin abnormality correlated significantly with severe headache, visual disturbance, general weakness, and cystic size. Suprasellar extension of cysts and high signals in the T2-weighted image on brain MRI were related to hypothyroidism, hypocortisolism, and diabetes insipidus. Multivariable linear regression analysis showed that only general weakness was a risk factor for pituitary insufficiency ($R^2$=0.549). Conclusion: General weakness is a risk factor for pituitary insufficiency in patients with Rathke's cleft cysts. When a patient with a Rathke's cleft cyst complains of general weakness, the clinician should evaluate pituitary function and consider surgical treatment.

비만아에서의 leptin, adiponectin, interleukin-6, tumor necrosis factor-α에 대한 연구 (Leptin, adiponectin, interleukin-6 and tumor necrosis factor-α in obese adolescents)

  • 목 적 : 비만은 지방세포의 축적과 인슐린 내성으로 제 2형 당뇨병을 초래하고, 고 인슐린혈증과 고혈당이 염증 과정의 전구 물질과 함께 작용하여 혈관 내피의 이상, 혈청 지질의 이상, 고혈압과 혈관 염증을 일으켜 동맥경화와 심혈관 질환을 야기하는 것으로 알려져 왔다. Leptin은 adiopnectin과 함께 지방 세포로부터 생산되는 가장 풍부한 adipocytokine으로서, 일차적으로는 식욕 조절과 체내 지방 축적 조절의 기능을 하지만, IL-6, IL-12, TNF-${\alpha}$와 같은 다른 사이토카인과 함께 염증과정의 전구 물질이 되며 여러 면역 관련 질환에서 중요한 매개체가 된다. 이번 연구에서는 비만 청소년에서 leptin과 여러 cytokine과의 상관관계를 알아보고자 하였다. 방 법 : 16세 이상부터 18세 미만의 66명의 중등도 이상의 비만 청소년을 대상군으로 하였고, 같은 연령의 정상 청소년 26명을 대조군으로 하였다. 신장, 체중을 이용하여 비만도와 체질량지수를 산출하였고, 혈청 지질, 간 효소치를 측정하였다. 효소면역 측정법을 이용하여 IL-6, TNF-${\alpha}$를 측정하였고, 방사면역 측정법을 이용하여 adiponectin, leptin, 인슐린을 측정하였다. 이 후 leptin과 각 측정치와의 상관성을 알아보았다. 결 과 : Leptin은 비만군에서 대조군보다 유의하게 높았고, TNF-${\alpha}$와 IL-6도 비만군에서 대조군보다 유의하게 높았다. 인슐린도 비만군에서 대조군보다 유의하게 높았고 adiponectin은 비만군에서 유의하게 낮았다. Leptin은 비만도, 체질량지수, IL-6과 유의한 양의 상관관계를 보였으며, 또한 IL-6와 TNF-${\alpha}$간에 유의한 양의 상관관계를 보였다. 결 론 : 비만 청소년에서 leptin, TNF-${\alpha}$, IL-6와 같은 cytokine과 insulin이 성인병으로 진행되는데 중요한 요소가 될 것이며, leptin을 성인병의 예측 인자 및 임상치료의 척도로 사용하기 위해 더 많은 연구가 필요할 것으로 생각된다.

Hypertensive crisis following mRNA COVID-19 vaccination in adolescents: two case reports

  • In response to the global coronavirus disease 2019 (COVID-19) pandemic, vaccines were developed and approved quickly. However, numerous cardiovascular adverse events have been reported. We present two adolescent cases who developed a hypertensive crisis following NT162b2 mRNA COVID-19 vaccination. Patient 1 was an 18-year-old male and his systolic blood pressure was 230 mmHg one day after the second vaccine. He was obese. No secondary cause of hypertension other than the vaccine was identified. Patient 2 was an 18-year-old male who complained with palpitation after the first vaccine. His blood pressure was 178/109 mmHg. He had autosomal dominant polycystic kidney disease. Both were treated with continuous infusion of labetalol followed by losartan, and blood pressure was controlled. Patient 2 received second vaccination and his blood pressure did not rise. It is warranted to measure blood pressure in adolescents at high risk of hypertension after NT162b2 mRNA COVID-19 vaccination.

Management issues of congenital adrenal hyperplasia during the transition from pediatric to adult care

  • Steroid 21-hydroxylase deficiency is the most prevalent form of congenital adrenal hyperplasia (CAH), accounting for approximately 95% of cases. With the advent of newborn screening and hormone replacement therapy, most children with CAH survive into adulthood. Adolescents and adults with CAH experience a number of complications, including short stature, obesity, infertility, tumor, osteoporosis, and reduced quality of life. Transition from pediatric to adult care and management of long-term complications are challenging for both patients and health-care providers. Psychosocial issues frequently affect adherence to glucocorticoid treatment. Therefore, the safe transition of adolescents to adult care requires regular follow-up of patients by a multidisciplinary team including pediatric and adult endocrinologists. The major goals for management of adults with 21-hydroxylase deficiency are to minimize the long-term complications of glucocorticoid therapy, reduce hyperandrogenism, prevent adrenal or testicular adrenal rest tumors, maintain fertility, and improve quality of life. Optimized medical or surgical treatment strategies should be developed through coordinated care, both during transition periods and throughout patients' lifetimes. This review will summarize current knowledge on the management of adults with CAH, and suggested appropriate approaches to the transition from pediatric to adult care.

Reninoma: a rare cause of curable hypertension

  • The most common type of refractory hypertension found in children is secondary hypertension, which is a potentially curable disease. Reninoma, a renin-secreting juxtaglomerular cell tumor, is a rare cause of severe hypertension that is usually diagnosed in adolescents and young adults. Surgical resection of the tumor completely cures the hypertension of patients with reninoma. The typical clinical presentation of reninoma includes hypokalemia, metabolic alkalosis, and features secondary to the increased activation of the renin-angiotensin system without renal artery stenosis. We report a case of reninoma in a female adolescent with a typical clinical presentation, in which surgical removal of the tumor completely cured hypertension. We discuss here the clinical features, imaging studies, and immunohistochemical examination of the tumor used to establish the diagnosis of reninoma and for the management of the condition.

A childhood case of spinal tuberculosis misdiagnosed as muscular dystrophy

  • Tuberculosis is primarily a pulmonary disease, but extra-pulmonary manifestations are not uncommon, especially in children and adolescents. Ten percent of extra pulmonary tuberculosis localizes to the bones and joints, and 56% of such cases affect the spine. We treated a childhood case of spinal tuberculosis misdiagnosed as muscular dystrophy in a patient without specific constitutional symptoms. We report this case because the patient had an unusual presentation of spinal tuberculosis.

1형 당뇨병 청소년의 섭식장애 예측요인 (Predictors of Eating Disorders in Adolescents with Type 1 Diabetes)

  • Purpose: The purpose of this study was to identify predictors of eating disorders in adolescents with type 1 diabetes, with the goal of providing data in support of nursing interventions to improve their health. Methods: A total of 136 adolescents aged 13-18 years with type 1 diabetes completed the Diabetes Eating Problem Survey-Revised, Rosenberg Self-Esteem Scale, and the Beck Depression Inventory-II, using structured self-reported questionnaires. The collected data were analyzed using the t-test, $x^2$ test, and binominal logistic regression with SPSS version 23.0 for Windows. Results: The prevalence of eating disorders in adolescents with type 1 diabetes was 39%. Four significant predictors of eating disorders were identified; absence of body satisfaction (odds ratio [OR]=3.87, 95% confidence interval [CI]=1.55~9.65), depression (OR=2.87, 95% CI=1.13~7.28), female gender (OR=2.67, 95% CI=1.09~6.54), and glycosylated hemoglobin type A1c levels (OR=1.47, 95% CI=1.10~1.97). Conclusion: In order to prevent eating disorders among adolescents with type 1 diabetes, programs for managing adolescents' depression and improving their body satisfaction should be developed. Futhermore, more attention should be directed towards programs aiming to prevent eating disorders in female adolescents.

청소년의 신체활동이 성장에 미치는 영향 (The Effect of Physical Activities on the Growth Indices in Adolescents)

  • Objectives The purpose of this study is to investigate the relationship between physical activity and growth indices. Methods 5963 of 12 years old adolescents were participated in the 10th Korea Youth Risk Behavior Web-Based Survey (KYRBWS-V) project in 2014. These data was analyzed to show the relationship between physical activity and height, BMI. Results According to the Physical Activity guidelines, the mean height was higher in the each male and female group with 'moderate physical activity for at least 60 min ${\geq}7$ days/week', 'vigorous physical activity ${\geq}3$ days/week', and 'muscle-strengthening activity ${\geq}3$ days/week' compare to the group who did not. The mean BMI was lower in the male group of 'sitting for watching TV or computer, etc on a weekday < 3 hr/day' compare to group who did not. Conclusions These results showed that physical activity in adolescence is correlated to growth. Therefore, this research can be one of the reference data to show the proper physical activity is important in the growth.

The associations of Urinary Neutrophil Gelatinase-associated Lipocalin (NGAL) and Liver-type Fatty Acid-binding Protein (L-FABP) Levels with Hematuria in Children and Adolescents

  • Purpose: We sought to determine associations of urinary neutrophil gelatinase-associated lipocalin (NGAL) and liver-type fatty acid-binding protein (L-FABP), known markers of renal injury, with hematuria in children and adolescents. Methods: A total of 112 urine samples from 72 patients aged 2 to 18 years with hematuria were enrolled in this study. Urinary concentrations of NGAL and L-FABP were measured by ELISA and compared between subjects with and without proteinuria and between subjects with and without glomerulonephritis diagnosed by renal biopsy. Results: Urinary concentrations of NGAL and L-FABP/creatinine (Cr) in subjects with proteinuria were not significantly different from those in subjects without proteinuria. They were not significant different between subjects with and without glomerulonephritis either. However, both concentrations of urinary NGAL and L-FABP/Cr were positively associated with urinary protein to creatinine ratio. Their levels had a tendency to be increased when proteinuria developed at later visits in subjects with hematuria only at initial visits. Conclusion: Monitoring urinary NGAL and L-FABP levels in addition to conventional risk factors such as proteinuria and serum creatinine might improve the prediction of renal injury in pediatric patients with hematuria.
