소아 청소년 물리치료 실태 조사: 서울 경기 지역 소아 청소년 재활병원을 중심으로 (Survey About Current Status of Pediatric and Adolescent Physical Therapy: Focus on Pediatric and Adolescent Rehabilitation Hospitals in Seoul and Gyeonggi Province)

  • 목적 : 본 연구는 장애 소아 청소년을 치료 중인 물리치료사를 대상으로 소아 청소년 물리치료 실태, 만족도, 수가 체계, 대상, 시행, 평가 활용에 대해 알아보기 위해 진행되었다. 연구방법 : 서울 경기 지역 소아 청소년 물리치료사 60명을 대상으로 설문지를 활용하여 소아 청소년 물리치료 실태, 대상자, 만족도, 치료 시행 및 평가 활용 등을 조사하였다. 실태, 대상자, 만족도 등은 빈도 및 기초 통계 분석을 활용하여 분석하였으며, 물리치료 참여도-난이도, 소아 청소년 물리치료 주요 요소의 중요도-본인의 능력, 주요 치료 목표-사용 빈도 차이는 대응 표본 T 검정을 통하여 그 차이를 확인하였다. 결과 : 소아 청소년 물리치료 실태는 대부분 독립된 공간(95.0%)에서 하루 11~15건(66.7%), 1대 1 치료(95.0%)로 진행되는 것으로 조사되었다. 대상자의 주요 연령은 학령기, 학령전기, 진단은 뇌병변, 발달지연이 많았으며 20대 이상까지 치료하였다. 소아 청소년 물리치료 만족도는 높았으나(70.0%) 업무강도 역시 높았다(71.7%). 주요 치료법으로는 신경발달치료, 보행치료, 목표지향적 재활이 있으며, 평가는 대동작기능검사 88과 66, 대동작기능분류검사 사용이 많았다. 현 수가 체계는 부적합하다고 하였으며(66.1%), 적정 수가 체계가 필요하다고 하였다. 결론 : 본 연구를 통하여 소아 청소년 물리치료와 관련된 광범위한 내용과 요소를 확인하였다. 현 실태조사연구를 바탕으로 소아 청소년 물리치료사의 전문성 및 지속성 향상, 최신 치료법 적용을 위한 노력이 필요할 것이다.

소아 폐염 환자에서 시행한 흉부 물리치료의 효과 (Effect of Chest Physical Therapy in Pediatric Patients)

  • Object: We evaluated the efficacy of chest physical therapy in pediatric patients with pneumonia. Method: Retrospective study was performed in 89 pneumonia patients admitted to pediatric unit. The subjects were divided into two groups: control group; treatment group. Control group included 42 patients who didn't receive chest physical therapy. Treatment group included 47 patients who received chest physical therapy. The chest physcial therapy employed were postural drainage, chest percussion, deep breathing training and enhancement of coughings. The efficacy was evaluated by x-ray outcome before discharge. Results: There were no significant difference in age, sex, type of pneumonia, and symptom duration between two groups. However the duration of fever after admission, duration of antibiotic use and hospital stay were longer in treatment group. In treatment group, longer the day to initiate chest physical therapy, longer the hospital stay. The final outcome was not different between groups. Conclusion: The results suggest that the referred patients for chest physical therapy tends to be of severe cases. Nevertheless, the result that the outcome was not different in two groups means that the chest physical therapy could be used as a effective treatment method in pediatric patient with pneumonia.

우리나라 소아 물리치료사의 임상적 추론 (Clinical Reasoning by Pediatric Physical Therapists in South Korea)

  • Purpose: The purpose of this qualitative study was to understand the application of clinical reasoning throughout clinical and therapeutic knowledge of clinical pediatric physical therapists under physician prescriptions. Methods: Purposive sampling techniques were used in selection of nine clinical pediatric physical therapists in South Korea. Open and semi structured interviews were conducted, and were thoroughly examined and reviewed, followed by analysis of the clinical reasoning portion. Results: Pediatric physical therapists have been influenced by information gathering, physical function and evaluation of their patients, hypothesis-orientation, and re-evaluation and self-monitoring. Novice physical therapists were more dependent on physician prescriptions and parent's hope than the result of their own evaluation and critical pathway. Middle experienced pediatric physical therapists were more dependent on rapport with children and their parents. Highly experienced pediatric physical therapists were more reliant on hypothesis-orientation and self-monitoring. As reports on clinical experience of pediatric physical therapists have accumulated, clinical reasoning of pediatric physical therapists has been influenced by their experience, such as education, clinical pathology conference, their own clinical experience and rapport with patients and their parents than physicians' prescriptions. Conclusion: The findings of the current study are generally consistent with existing research on clinical reasoning. The results of the current study may be used by educators for enhancement of clinical reasoning abilities and knowledge of students or novices as well as development of a guide for use by suitable novices or students, and could provide important information for use in physical therapy practice and research.

소아 가정방문 물리치료의 인식도와 수요도에 관한 설문조사 (Awareness and Demand for Pediatric Home-Based Physical Therapy in Korea)

  • Pediatric home-based physical therapy (PHBPT) provides professional rehabilitation programs at the patient's home, where the activities of daily life are actually performed. PHBPT also allows to avoid the difficulties of transporting children with disabilities to the clinic. Despite these advantages, PHBPT is not yet widely practiced in Korea. There is little objective information regarding the opinions of the main stakeholders on PHBPT. To investigate the awareness and demand of PHBPT among the main stakeholders, 41 pediatric physical therapists (PT) (of 60 contacted) were recruited from different regions of Korea on the basis of the regional population distribution. The recruited PTs completed their questionnaires and also participated in collecting questionnaires from 35 medical doctors (MD) with whom they worked and from randomly selected 201 parents of children with disabilities recruited. The overall response rate was 85.5%. The awareness of PHBPT differed between PTs (95.1%) and parents (67.2%) (p<.001). The survey showed that 82.9% of MDs had at least heard about PHBPT. Significantly more parents (83.5%) than MDs (57.1%), and 70.0% of PTs, wanted to start PHBPT service immediately (p<.001). Significantly more parents (90.0%) than PTs (73.2%) were willing to participate in PHBPT (p<.001). Opinions on the details of policies and procedures (i.e., necessity for prescription, treatment cost, and treatment frequency) differed among the respondent groups, but all favored a minimal qualification of 6~10 years of pediatric experience and a treatment session duration of 1 hour. These findings provide objective information to support health service administrators to understand the current demand and develop feasible policies and procedures of PHBPT in Korea.

일부 뇌성마비 아동을 대상으로 한 아동균형척도의 검사 - 재검사, 측정자간 및 내의 신뢰도 (Test-retest, Inter-rater, and Intra-rater Reliability of a Pediatric Balance Scale in Children with Cerebral Palsy)

  • Purpose: This study was designed to determine the test-retest, inter-rater, and intra-rater reliability of the Pediatric Balance Scale (PBS) when applied to children with cerebral palsy (CP). Methods: Subjects were out-patient children with cerebral palsy at four CP clinics in Gyeonggyi-do and Chungcheong nam-do. For test-retest and inter-rater reliability studies, the PBS was used twice on 7 separate days by twenty-four children with CP. To assess intra-rater reliability, 10 CP subjects were selected by random sampling. Four pediatric-trained physical therapists with 2-13 years of clinical experience scored the children’s performance blindly, while replaying videotaped data. Results: There was no significant difference in total scores (ICC 3,1=0.89, 0.93, 0.90, and 0.91) measured by each of the four therapists on two occasions. The Inter-rater reliability assessed the 1st and 2nd time was high (1stICC 3,1=0.91, 2nd ICC 3,1=0.93). The intra-rater reliability measured by each of the four therapists using the 2nd scores was also high (ICC 3,1=0.98, 0.99, 0.97, and 0.98). Conclusion: The PBS is reliable. We believe that it can be used in characterizing children with CP.

한글판 아동 균형척도(Pediatric Balance Scale)의 측정자간 신뢰도 (Inter-Examiner Reliability of the Korean Version of the Pediatric Balance Scale)

  • The Pediatric Balance Scale (PBS) is a modified Berg's Balance Scale developed as a balance measure for school-aged children with mild to moderate motor impairments. The purpose of this study was to determine the inter-examiner reliability of the Korean version of PBS when applied to children with developmental delays. In this study, PBS was administrated to a total of 79 children with developmental delays (17 with global developmental delay, 31 with cerebral palsy, and 31 with mental retardation) in the Seoul Community Rehabilitation Center. Two pediatric trained physical therapists with longer than 13 year of clinical experience scored the children's performance blind, while replaying videotaped data. The inter-examiner reliability was statistically determined by intraclass correlation coefficients (ICCs). The results of this study revealed that the Korean version of PBS seems to have high inter-examiner reliability when applied to children with movement disorders such as global developmental delay (ICC=.96) and cerebral palsy (ICC=.97); however, it has relatively lower inter-examiner reliability (ICC=.78) for children with developmental delay secondary to mental retardation. therefore, the results support that the Korean version of the PBS could be a useful clinical measurement to assess the balance skills for children with developmental delay who have an adequate level of cognition to enable them to fol1ow the verbal instructions to complete the test.

소아용 균형 감각검사(PCTSIB)를 이용한 6세 및 7세 아동의 균형 수행력 비교 (A Comparative Study of Balance Performance in 6,7 years old by means of the Pediatric Clinical Test of Sensory Interaction for Balance)

  • The purpose of this study were to compare the balance performance with 44 children, who aged 6 and 7years old by means of the Pediatric Clinical Test of Sensory Interaction for Balance(PCTSIB), second is to determine whether age and gender-relatad difference were present, third is to found correlation of weight, height with duration of balance performance. All subjects were performed with different foot position that were feet-together, heel-toes. The starting position was that subject placed their hands on the hips. The results of each test were measured by stop watch. These collected data were analyzed by using oneway ANOVA, Sheffe test t-test and correlation. The results of this study were as follows ; 1. There were all significiant difference is all instances except condition 4 with heel-toes(p<0.05). The duration of balance performance of 7-year-olds group was more increased than 6-year-olds group. 2. There was statistically significiant difference in all instances by gender(p<0.05). 3. There was no correlation between weight or height duration of balance performance (p<0.05).

물리치료과 학생들을 대상으로 아동균형척도의 항목별 측정자 신뢰도에 관한 연구 (The Study on the Rater Reliabilities of Physical Therapy Students Pediatric Balance Scales items)

  • PURPOSE: The purpose of this study is to examine assessment of test-retest reliability for 13 items of pediatric balance scales, then the intrarater reliability among the raters. METHODS: Study participants included 6 children with spastic cerebral palsy who could walk. Raters were consist of seventy-four, 2nd year physical therapy major students. The children's ability to achieve physical balance was wideotaped for PBS items. The raters watched the tapes and evlauated each child twice. Test-retest reliability was analyzed using the Spearman correlation, and interrater reliability was analyzed using the Kendall's coefficient of concordance for ranks. RESULTS: The total PBS scores averaged 49.22 and 50.06 for first and second tests. Test-retest reliability of PBS individual items were between low and high. The low itmes were 3 items, and high were 4 itmes. Interrater reliability were low agreement. CONCLUSION: Based on the study, the pediatric balance scale is an effective evaluating tool for measuring functional balance of school age child. However, the agreement of students or beginner's interpretation is necessary in conjunction to this study results. On top of that, it is important to provide detailed explanation and repetitive training, which improves the analysis reliability. Also, each category subscores level for the pediatric balance scale must be examined carefully in conjunction to performing test, only after improving match quality via preceding practice.

The Reliability of a Pediatric Balance Scale Based on the Raters' Clinical Work Experience and Test Experience

  • Purpose: To investigate the rater reliability of a Pediatric Balance Scale (PBS) for children with cerebral palsy, and to investigate possible differences among raters according to their clinical work experience and testing experience. Methods: Study participants included 18 children with spastic cerebral palsy who could walk. They were instructed by pediatric physical therapists, two of whom had ten years of clinical work experience and two who had less than one year of experience. The children's ability to achieve physical balance was videotaped for PBS items. The raters watched the tapes and evaluated each child twice. Rater reliability was analyzed using the intraclass correlation coefficient (ICC). Differences between experienced and novice raters were analyzed using a paired t-test. The statistical significance level was set to 0.05. Results: The total PBS scores averaged 45.78~48.00 and 45.72~47.67 for first and second tests. Intra-rater reliability was very high (ICC=0.89~0.99), and the repeated measurement coincidence was high (p>0.05). Inter-rater reliability was high (ICC=0.83~0.84), but there was a bit of a difference in the coincidence (p<0.05). The experienced raters' reliability and coincidence were higher than those of the novices, and there were differences in reliance and coincidence between experienced and novice raters (p<0.05). Conclusion: Inter-rater and intra-rater reliability is very high. However, rater reliability showed defferences depending on clinical work experience and testing experience. When testing pediatric patients with the PBS, the rater's clinical experience and test experience may affect the test results.

Exploring the Relationship Between International Classification of Functioning, Disability, and Health Items Linked to Clinical Assessments in Children With Cerebral Palsy

  • Background: The International Classification of Functioning, Disability, and Health-Child and Youth version (ICF-CY) is designed to record the characteristics of developing children and examine the influence of a child's environment on their health. Objects: This study was designed to determine the relationship between the clinically extracted ICF-CY items and The Pediatric Evaluation of Disability Inventory (PEDI) and Gross Motor Function Measure (GMFM) items. Methods: Thirty patients (17 males and 13 females) who were hospitalized in a pediatric and youth patient unit of a rehabilitation hospital were included in the study. Four health professionals (two physical therapists and two occupational therapists) working independently linked the PEDI and GMFM-66 items to the activity and participation domains of the ICF-CY. Results: There were strong negative correlations between the ICF-CY subdomains and the PEDI subdomains (r = 0.76-0.95; p < 0.05). There were positive strong correlations between the ICF-CY subdomains and the GMFM-66 (r = 0.76-0.95; p < 0.05). Conclusion: The extracted ICF codes were a valid tool for evaluating the mobility and selfcare conditions of cerebral palsy in the pediatric rehabilitation area.
