Fracture of the Second Rib: An Indirect Sign of Serious Trauma Like Fracture of the First Rib?

  • Background: The first 3 ribs are anatomically well-protected, shielded by the large thoracic muscles, the shoulder girdle, and the arm. A significant force is required to fracture these ribs; thus, such fractures suggest a high-energy trauma and are associated with injuries to vital organs of the thorax, such as the aorta, the heart, the lungs and the great vessels. Methods: A retrospective analysis was conducted over a 10-year period at a single hospital. The study assessed patients with fractures of the second rib, including their concurrent injuries and the overall severity of their trauma. Results: Among the 76 patients included in the study, the average age was 47.35 years, 81.5% were men, and 19.5% were women. Thirteen patients (17.1%) survived their injuries. The most common causes of injury were road traffic accidents (63%) and pedestrian injuries (22%). The patients who did not survive sustained injuries to an average of 5 additional organs, while survivors had injuries to an average of 2.07 additional organs. Left rib fractures were the most frequently observed (46%). The most serious concurrent injuries reported were to the aorta (5.26%), heart (10.52%), lung (52.36%), head (57.89%), liver (30.2%), spleen (26.31%), and kidney (17.1%). Conclusion: As indicators of serious injury to vital endothoracic organs, isolated fractures of the second rib should be considered equal to first rib fractures in clinical importance.

  • In line with the continued growth of car ownership, the traffic problems in central area of metropoles such as Seoul would become increasingly degraded. comparing with most western cities, the problems in Seoul are characterized by the improportionately high rates of intersection delay, station congestion, traffic accidents caused by weaving conflicts and pedestrian congestion. It is caused by the lack of flexibility I street network, which is prerequisite for upholding the efficacy of traffic management and control, resulted from the simplicity of network graph in terms of connectivity, street density and distribution by width. This pattern has been resulted from the prolonged policy pursuing the street-widening of the nagging bottleneck in such a short period since the 1950s, comparing that most western cities had undergone over several centuries an age of horse-and-vehicle transportation. In order to improve the expected traffic problems in central area over the coming periods of motorization, it is imperative to restructure the street network in Central Seoul so that the efficacy of traffic management and control may be operative. Based upon the long-range planning the street network should be restructured by stages so that cenral traffic may be controled by one-way operation and most through-traffic be detoured around fringe area.

Safety Message Transmission Technology for the Elderly Pedestrians at the Conflict Area: Background and Technology Concept (상충지역에서 고령 보행자를 위한 안전메시지 전송기술 : 배경 및 기술 개념)

  • As the elderly are increasing, traffic accidents which are related to the elderly are one of major issues in aged society. In this paper, we analyze traffic accident statistics for the elderly. Based on the analyzed data, safety message transmission system for the elderly pedestrians is introduced. The proposed system is based on the P2V(Pedestrian-to-Vehicle) communication which combines ICT(Information and Communication Technology) with vehicles and pedestrians. We introduce the concept and principle of the proposed system and discuss implementation issues for realizing the proposed system.

Tribological approach for the analysis of the pedestrain slipping accident II

  • The variations of the surface topographical parameters for the analysis of the pedestrian slip and fall accidents during the sliding friction between the specially prepared floor specimens and three working shoes were investigated. The profile ordinate data for each flooring specimen were obtained at 1.1 .mu.m intervals using a laser scanning confocal microscope system along to the direction of sliding. A number of surface roughness parameters, that is, the centre line average (c.l.a.) and root mean square (r.m.s.) roughness, maximum height (Rtm), maximum mean peak height (Rpm), maximum mean depth (Rvm), and average asperity slope were calculated using a computer program and compared with the dynamic friction results. The analysis showed that the surface parameters undergo marked variations during the sliding process, but the variations were statistically significant. It was found that amongst various surface parameters, the maximum depth (Rvm) and the average asperity slope of the asperities were the biggest variation during the sliding proceeding. This result confirms the previous study and may suggests a new approach to monitoring the flooring environments with their service as the effort to reduce the pedestrain slip accident.

  • The LBS(Location Based Service) technology plays an important role in reducing wastes of time, losses of human lives and economic losses by detecting the user's location in order by suggesting the optimal evacuation route of the users in case of safety accidents. We developed an algorithm to estimate indoor location, movement path and indoor location changes of smart phone users based on the built-in sensors of smartphones and the dead-reckoning algorithm for pedestrians without a connection with smart devices such as Wi-Fi and Bluetooth. Furthermore, seven different indoor movement scenarios were selected to measure the performance of this algorithm and the accuracy of the indoor location estimation was measured by comparing the actual movement route and the algorithm results of the experimenter(pedestrian) who performed the indoor movement. The experimental result showed that this algorithm had an average accuracy of 95.0%.

  • 노인들이 많이 이용하는 이동 보조 기구인 기존 보행기에 다양한 기능이 부족하여 안전 서비스, 알림 서비스, 스마트폰 애플리케이션 서비스로 분류하여 기능을 추가하였다. 안전 서비스 부문의 기능으로는 경사로 사고 방지 시스템, 장애물 알림 시스템, 밝기에 따른 전조등 시스템, 손잡이에서 손이 떨어지면 자동으로 동작되는 브레이크 시스템, 겨울철 손을 따뜻하게 유지하기 위한 난방 시스템이 있다. 알림 서비스로는 현재의 온·습도 및 미세먼지 농도를 알려준 후 외출하기에 부적합할 시 경고등으로 알려주는 시스템이 있다. 스마트폰 애플리케이션 서비스는 심박 및 체온을 알려주고, 노인의 위치와 SOS신호, 운동량을 알 수 있고, 도난 방지 시스템이 있는 스마트 안전 노인 보행기를 설계하였다.

Traumatic lumbar hernia: clinical features and management

  • Purpose: Traumatic lumbar hernia is rare, thus making diagnosis and proper treatment challenging. Accordingly, we aimed to investigate the clinical manifestations and proper management strategies of traumatic lumbar hernias. Methods: The medical records of patients with traumatic lumbar hernia treated at Gachon University Gil Hospital from March 2006 to February 2015, were retrospectively reviewed. Results: We included 5 men and 4 women (mean age, 55 years; range, 23-71 years). In 8 patients, most injuries were caused by motor vehicle collisions, including those wherein a pedestrian was struck (5 cases of car accidents, 2 falls, and 1 involving penetrating materials); in 1 patient, the probable cause was severe cough. Eight patients underwent hernia repair surgery (5 open and 3 laparoscopic), and a prosthetic mesh was used in 7 patients. Hernia repairs were elective in 7 patients; emergency hernia repair was performed with right hemicolectomy in 1 patient. No severe complication or recurrence was observed. Only 2 patients had mild complications, such as postoperative seroma. Conclusion: Traumatic lumbar hernia is a relatively rare injury of the posteriolateral abdominal wall. Lumbar hernia should be suspected in patients with high-energy injuries of the torso, and all such patients should undergo abdominopelvic computed tomography. After diagnosis, hernia repair can be electively performed without complications in most cases.

A completely non-contact recognition system for bridge unit influence line using portable cameras and computer vision

  • Currently most of the vision-based structural identification research focus either on structural input (vehicle location) estimation or on structural output (structural displacement and strain responses) estimation. The structural condition assessment at global level just with the vision-based structural output cannot give a normalized response irrespective of the type and/or load configurations of the vehicles. Combining the vision-based structural input and the structural output from non-contact sensors overcomes the disadvantage given above, while reducing cost, time, labor force including cable wiring work. In conventional traffic monitoring, sometimes traffic closure is essential for bridge structures, which may cause other severe problems such as traffic jams and accidents. In this study, a completely non-contact structural identification system is proposed, and the system mainly targets the identification of bridge unit influence line (UIL) under operational traffic. Both the structural input (vehicle location information) and output (displacement responses) are obtained by only using cameras and computer vision techniques. Multiple cameras are synchronized by audio signal pattern recognition. The proposed system is verified with a laboratory experiment on a scaled bridge model under a small moving truck load and a field application on a footbridge on campus under a moving golf cart load. The UILs are successfully identified in both bridge cases. The pedestrian loads are also estimated with the extracted UIL and the predicted weights of pedestrians are observed to be in acceptable ranges.

Detecting Jaywalking Using the YOLOv5 Model

  • Currently, Korea is building traffic infrastructure using Intelligent Transport Systems (ITS), but the pedestrian traffic accident rate is very high. The purpose of this paper is to prevent the risk of traffic accidents by jaywalking pedestrians. The development of this study aims to detect pedestrians who trespass using the public data set provided by the Artificial Intelligence Hub (AIHub). The data set uses training data: 673,150 pieces and validation data: 131,385 pieces, and the types include snow, rain, fog, etc., and there is a total of 7 types including passenger cars, small buses, large buses, trucks, large trailers, motorcycles, and pedestrians. has a class format of Learning is carried out using YOLOv5 as an implementation model, and as an object detection and edge detection method of an input image, a canny edge model is applied to classify and visualize human objects within the detected road boundary range. In this study, it was designed and implemented to detect pedestrians using the deep learning-based YOLOv5 model. As the final result, the mAP 0.5 showed a real-time detection rate of 61% and 114.9 fps at 338 epochs using the YOLOv5 model.

  • The recent surge in night walkers and pedestrian traffic accidents has led to an increased interest in retro-reflection-based products, leading to several studies on retro-reflection. Nonetheless, achieving high durability and dispersibility of retro-reflective glass beads in viscous polyurethane coating resin remains challenging. To address this issue, this study conduct to functionalize the surface of glass beads for covalent conjugation to coating resin to enhance their dispersibility and durability in the coating resin. The study evaluated the dispersibility, chemical composition, and retro-reflection properties of the functionalized glass beads and coating resin. The results showed that the functionalized glass beads conjugated to the polyurethane coating resin and exhibited excellent dispersibility, high durability, and maintained retro-reflection efficiency.
