파킨슨병 환자의 e-운동프로그램이 균형과 보행 그리고 지구력에 미치는 영향 (The Effect of e-Exercise Program on Balance, Gait, and Endurance in Patients with Parkinson's Disease)

  • Purpose : Patients with Parkionson's disease have a temporal and spatial restriction for their's exercise treatment. The aim of this study is to verify the effectiveness of e-exercise program on balance, gait, and endurance ability in people with Parkinson's disease. Methods : Eighteen participants, with Parkinson's disease that ranged from stage to on the Hoehn & Yahr scale, were assigned to two group. Two group were divided into on-line group that exercised in home by computer video, and off-line group that exercised in clinical center. Exercise program that included 3 sections (warm-up, main exercise, cool down)-5 parts(stretch, strength, balance, gait, stretch) was used to train for participants. We used the functional reach test for balance ability, timed up and go test for gait ability, 6 minute walking test for endurance ability. Wilkoxon sign lank tests were used to assess the difference of balance, gait, and endurance ability in pre-and post test each groups. Mann-whitney tests were used to assess the difference of balance, gait, and endurance ability between groups. Results : Wilkoxon sign lank tests revealed that both group were improved the balance, gait and endurance agility by e-exercise program intervention. Mann-whitney tests showed that there were no differences between groups in the variation of balance, gait, endurance ability. Conclusion : We believe that exercise program with on-line(e-exercise program) is an effective intervention for people with Parkinson's disease and is no different than off-line exercise group in the variation of balance, gait, endurance ability.

  • Purpose: The purpose of this paper was to verify effects of the team approach rehabilitation program on balance, gait, and muscle strength of lower extremities of elderly people with Parkinson's disease. Method: Subjects of this paper were 40 elderly people with Parkinson's disease, 20 control and experimental groups respectively, who could walk independently and were less than the 2.5 Hoehn &Yahr stage. The team approach rehabilitation program was applied to the experimental group for 12 weeks. Results: There was significant decrease in second at timed up &go test (p= .008), but no significant difference in reach length at functional reach test (p= .201) with partial improvement of balance. There was no significant difference in second at 10-meter walk test (p= .070), but showed tendency of improvement of gait. And number of times at 30s-chair stand test, indicating muscle strength on lower extremities, increased significantly (p= .029), Conclusion: The team approach rehabilitation program has demonstrated its effectiveness on improving balance, and muscle strength of lower extremities for the elderly with Parkinson's disease.

  • Objectives : Postural instability and gait difficulty(PIGD) can develop in all Parkinson's disease patients, especially late in the disease course, but does not respond well to conventional dopamine treatment. This study aimed to report three cases of PIGD in Parkinson's disease patients treated with Korean and Western medicine treatment. Methods : We used acupuncture, bee venom acupuncture, herbal medicine and moxibustion to treat patients during hospitalization. They continued Western medication and received rehabilitation treatment. We observed the changes of PIGD using the Unified Parkinson's Disease Rating Scale(UPDRS). Results : After treatment, PIGD symptoms improved and UPDRS scores were decreased. Especially, scores of falling, walking, gait and postural stability subsections related with PIGD were decreased. In addition, general conditions of patients were improved. Conclusions : This study suggests that Korean medicine treatment could be effective in the treatment of PIGD in Parkinson's disease patients.

  • 파킨슨병 노인은 낙상의 신체적, 정신적 위험요인을 가진 비율이 비 파킨슨병 노인에 비해 높아, 파킨슨병 노인의 낙상 경험 유무에 따라 낙상에 영향을 주는 신체적, 정신적 위험요인을 한국 노인 전체를 모집단으로 하는 국가 데이터를 이용하여 규명하였다. 2017년 보건복지부의 노인실태 자료조사를 이용한 2차 분석연구로, 파킨슨병 노인은 총 103명이었으며 결측값을 제외하고 총 96명의 대상자를 분석하였다. 파킨슨병 노인에게서 낙상에 가장 큰 영향을 미치는 요인은 IADL로 나타났으며, IADL은 운동 조절기능과 관련이 있다. 운동 조절기능의 저하는 일상생활에 필수적으로 요구되는 신체 움직임에 제약을 가하며 위급상황에서의 자기 보호적 행동까지도 영향을 미쳐 낙상에 영향을 미친다. IADL이 낙상에 영향을 줄 수 있다는 본 연구결과를 바탕으로 파킨슨병 노인의 낙상 예방 중재를 위한 다양한 운동요법을 제안할 수 있을 것이다.

파킨슨병 환자의 인구학적.임상적 특성에 따른 우울, 일상생활 수행능력 및 인지기능 (Influence of Clinical and Demographical Variables on Depression, Instrumental Activities of Daily Living, and Cognitive Function in Patients with Parkinson's Disease)

  • Purpose: To investigate the influence of clinical and demographical variables on depression, instrumental activities of daily living (IADL), and cognitive function in patients with Parkinson's disease. Method: Using a structured questionnaire data were collected from 100 participants registered in the neurology department of C university hospital. Duration and stage of disease, fall history, vision impairment, duration and quality of sleep, orthostatic hypotension, ambulation impairment, and use of walking aid were included in clinical variables. Depression, IADL, and cognitive function were assessed using Kee's GDSSF-K, Cho's scale, and K-MMSE. Collected data were analyzed using the SAS program. Results: The depression score for the participants was 7.78, higher than cut-off score(5). Participants who were male, living with spouse, above high school education, high economic status, with no vision impairment, and no ambulation impairment revealed high cognitive scores. Average IADL score were significantly higher for participants who were male, who had high economic status, low stage of disease, and no ambulation impairment. Depression IADL & cognitive function scores were significantly different according to ambulation impairment. Scores for ADL and cognitive function were positively correlated. Conclusion: It is recommended that make programs for patients with Parkinson's disease, clinical and demographic variables should be considered according to their individual needs.

파킨슨병 노인을 위한 수정된 강제-유도운동치료: 사전연구 (Modified constraint-Induced Movement Therapy (CIMT) for the Elderly With Parkinson's Disease: A Preliminary Study)

  • This study was designed to examine a 3-week modified constraint-induced movement therapy (CIMT) to the less-affected arm of patients with Parkinson's disease (PD) would improve function of the more-affected arm in PD. The subjects were 6 institutional older adults with PD and clients of the social welfare facilities. The subjects (2 men, 4 women) ranged in age from 66 to 90 years (mean age 77.2 yrs). Three clinical tests were used to determine the improvement of functional activity between before and after modified CIMT. The tests included Unified Parkinson's Disease Rating Scale (UPDRS). Wolf Motor Function Test (WMFT), and Action Research Arm Test (ARAT). There were significantly differences after the modified CIMT for time performance in WMFT and pinch in ARAT (p<.05), No significant difference was noted after the modified CIMT for UPDRS and functional ability scale in WMFT. Therefore, the modified CIMT might improve time performance and is available to therapeutic program helping them improve functional ability for upper extremity in Parkinson's disease.

  • Purpose: The purpose of this paper is to provide nurses with a concise review on neurodegenrative dementias. This review includes pathophysiology, clinical course, and tips on management of dementias from Alzheimer's disease (AD), Parkinson disease (PD) and lewy body dementia (LBD). Considering increasing numbers of dementia cases among older adults, nurses who are cognizant about dementia care are instrumental in maximizing daily activities and quality of life of patients with cognitive impairment and dementia.

파킨슨 환자들의 방향전환 보행 향상을 위한 하지의 운동학 및 근육 활동 규명 (Lower Extremity Kinematics and Muscle Activity of Cutting Movement in Older Parkinson's Diseases)

  • 본 연구의 목적은 파킨슨 환자들에게 일상생활 속에서 빈번히 수행되는 방향전환 동작 중에 작용하는 하지의 운동학 및 근육의 활동을 규명하는데 있다. 호엔야르 $1.5{\sim}4$척도의 60대 파킨슨 환자 7명이 본 실험에 참여하였다. 실험실 바닥에 직선($0^{\circ}$) 및 방향전환($60^{\circ}$, $90^{\circ}$, $120^{\circ}$) 보행로를 만들었다. 보행로에 5cm 넓이의 검은색 접착테이프를 붙여 놓아, 그 경로를 따라 무작위 순서로 지나가도록 하였다. 하지의 운동학적 변인을 측정하기 위해 해당부실에 21개의 마커를 부착하였으며, 근전도 변인을 측정하기 위해 우측 하지의 장내전근, 중둔근, 대둔근, 대퇴이두근, 대퇴직근, 비복근, 전경골근에 전극을 부착하였다. 방향전환 각도별(4수준: $0^{\circ}$, $60^{\circ}$, $90^{\circ}$, $120^{\circ}$)로 얻은 하지의 운동학 및 근전도 변인의 차이를 규명하기 위하여 반복이 있는 일원 변량 분석을 실시하였다. 파킨슨 환자들은 방향전환시 활보장과 보간을 감소시키는 것으로 나타났으며, 하지에 작용하는 근육활동에는 차이가 없는 것으로 나타났다.

Relationships between upper extremity performance and unified Parkinson's disease rating scale-motor exam

  • Objective: Clinical measures that quantify upper extremity function are needed for the accurate evaluation of patients and to plan an intervention strategy. The purpose of this study was to examine the relationship between the Unified Parkinson's Disease Rating Scale (UPDRS)-Motor Exam and upper extremity performance as a quantifying clinical tool of upper extremity function in persons with Parkinson's disease. Design: Cross-sectional study. Methods: Thirty-two idiopathic Parkinson's Disease persons participated in this study. To investigate the relationship between the UPDRS-motor exam, Box and Block test (BBT), and Action Research Arm Test (ARAT) by two physical therapists. The examination took up to 1 hour, and the participants were invited to rest between each clinical measure in order to minimize the effects of fatigue. Clinical measures were assessed while the subjects were in the "on" phase of their medication cycle, generally 1-3 hour after taking their anti-Parkinson's medications. Results: In more affected side, the UPDRS-motor exam was significantly negative correlated with the BBT (p<0.05) but it was not significantly correlated with the ARAT. In less affected side, only positively correlation was significantly shown between BBT and ARAT (p<0.05). On the other hand, between BBT and ARAT were not significantly correlated with the UPDRS-motor exam. Conclusions: The UPDRS-motor exam is effective tool which was significantly correlated with manual dexterity in more affected upper extremity. But The UPDRS-motor exam is not effective tool in less affected upper extremity.

Biochemical and molecular features of LRRK2 and its pathophysiological roles in Parkinson's disease

  • Parkinson's disease (PD) is the second most common neurodegenerative disease, and 5-10% of the PD cases are genetically inherited as familial PD (FPD). LRRK2 (leucine-rich repeat kinase 2) was first reported in 2004 as a gene corresponding to PARK8, an autosomal gene whose dominant mutations cause familial PD. LRRK2 contains both active kinase and GTPase domains as well as protein-protein interaction motifs such as LRR (leucine-rich repeat) and WD40. Most pathogenic LRRK2 mutations are located in either the GTPase or kinase domain, implying important roles for the enzymatic activities in PD pathogenic mechanisms. In comparison to other PD causative genes such as parkin and PINK1, LRRK2 exhibits two important features. One is that LRRK2's mutations (especially the G2019S mutation) were observed in sporadic as well as familial PD patients. Another is that, among the various PD-causing genes, pathological characteristics observed in patients carrying LRRK2 mutations are the most similar to patients with sporadic PD. Because of these two observations, LRRK2 has been intensively investigated for its pathogenic mechanism (s) and as a target gene for PD therapeutics. In this review, the general biochemical and molecular features of LRRK2, the recent results of LRRK2 studies and LRRK2's therapeutic potential as a PD target gene will be discussed.
