A Case Report of Advanced Gastric Cancer Patient Treated with Bojungikgi-tang Gamibang (절제불가능한 위암의 위장관 출혈에 대한 보중익기탕 가미방 투여 1례)

  • Approximately 10 percent of advanced stomach cancer patients have internal bleeding in their stomach. It is crucial to treat internal bleeding since it severely deteriorates patient's condition, and disturbs process of chemotherapy. There are hemostatic agents and dressing, radiotherapy, endoscopic ligation and coagulation, surgical methods to treat bleeding. However, these methods cannot be executed in some cases in which patient is in terminal stage or not in desirable condition to take these treatments. We are going to introduce a case of advanced gastric cancer patient having stomach bleeding who made a choice to take oriental medical treatments. The patient was on third stage of stomach cancer, had 3200cc of blood transfusion for five times. After diagnosed as not being able to have resection, he started to oriental medical treatments to stop bleeding. From March 28, 2009 to April 16, 2009, we administer 120cc Bojungikgi-tang gamibang to the patient in 90minutes after each meal. During 19days of having Bojungikgi-tang gamibang, we observed that the patient needed significantly lower amount blood transfusion, to 640cc. The patient improves in performance, and was able to eat more. Despite of short period of observation, this case indicates that Bojungikgi-tang gamibang treatment to the patient could work as hemostat, further improved quality of patient's life.

Patient Satisfaction Related to Increased Nursing Personnel Staffing (일 종합병원의 간호인력 증원에 따른 입원환자 간호만족도)

  • Purpose: This study was to examine patient satisfaction related to an increase in nursing personnel staffing. Methods: The subjects of this study were 317 hospitalized patients in the general wards of a medical center in Kyungki Do. Data were collected at 2 and 8 months following an increase in nursing personnel in the hospital. Patient Satisfaction Scale developed by Lee(1986) and revised by Seo(2000) was used. The data were analyzed by descriptive analysis, t-test, ${\chi}2$ test, and one way ANOVA. Results: Although the patient satisfaction scores at 8 months were higher than the scores at 2 months, there were no significant differences in patient satisfaction between the two periods Among the patient satisfaction subscales, the scores of the education subscale at 8 months were marginally higher than at 2 months (t=-1.97, p=.050). Conclusions: Adequate nursing staffing is important in patient satisfaction and patient outcome. The results provide data that suggests a positive direction for nursing intervention and development of nursing professionals.

A Case Report of Non-small Cell Lung Cancer with Brain Metastasis Patient Treated with Banhabaekchulchoenma-tang Gagambang (반하백출천마탕 가감방을 투여하여 비소세포성 폐암의 뇌전이에 의한 두통이 호전된 1례)

  • Lung cancer is the most common primary tumor of brain metastasis. Metastasis to the brain is a frequent complication of non-small cell lung cancer (NSCLC) occuring in 12 ~ 18% of patient. The incidence of brain metastasis is rising because of longer survival of cancer patients as a result of the increase in early diagnosis of primary tumors and aggressive management, and improvements in imaging quality and accessibility such as widespread use of MRI. The most common symptoms are gradual onset of headache, focal weakness, and mental changes. Surgery, chemotherapy, radiotherapy and steroid therapy are treatment of NSCLC with brain metastasis. We report a patient with NSCLC metastasis to the brain. This patient underwent chemotherapy, radiotherapy and steroidtherapy. However, the patient requested oriental medical treatment for the patient's continuous headache and disease. We administered Banhabaekchulchoenma-tang gagambang with Allergen-removed Rhus Verniciflua Stokes(aRVS). The patient showed remarkable improvement in terms of frequency and severity of headache. Further study will be needed in order to determine the long-term effectiveness of oriental medical treatment on cancer patient with headache.

Cancer Pain Relieved by Acupuncture on a Patient with Colorectal Cancer: Single Case Report (대장암 환자의 암성 통증에 사암침법을 적용하여 통증 감소를 보인 증례 1례)

  • Most of patients who have cancer still suffer from various forms of pain that significantly impact their quality of life. Cancer pain is a difficult problem for clinicians because analgesic drugs do not always procure complete relief. Acupuncture has been used as complementary treatment of cancer pain. We report a patient with colorectal cancer who suffer from cancer pain. The patient underwent chemotherapy, radiotherapy and opioid therapy. However, the patient insisted on being treated for his continuous pain with oriental medical treatment. The patient was treated with acupuncture(especially SAAM acupuncture) for 2 months, additional herbal medication was administered. After those treatments, the patient showed remarkable improvement in terms of intensity and severity of cancer pain. Further study will be needed in order to determine the long-term efficacy of acupuncture on cancer pain.

The Effect of Patient-Engaged Bedside Handoffs Protocol in Comprehensive Care Units (간호·간병통합서비스 병동에서의 환자참여 침상인계 프로토콜 적용 효과)

  • Purpose: The purpose of this study was to apply patient-engaged bedside handoffs in comprehensive care units, and to evaluate the effects of bedsides to nurses and patients. Methods: This study employed a cluster randomized cross-over design.Electrical Medical Record (EMR)-based handoffs and patient-engaged bedside handoffs were alternatively applied to 104 patients, who were assigned to a total of 30 clusters (nursing handoff teams) in 4 comprehensive care units at the S medical center in Seoul, and the patients evaluated each type of handoffs. A total of 139 nurses were also participated in the same units and evaluated each type of handoffs. Data were analyzed using t-test, Wilcoxon rank sum test, ANOVA, and Kruskal-Walls test. Results: The patient's satisfaction of the patient-engaged bedside handoffs was higher than that of the EMR-based handoffs (Z=-5.16, p<.001). On the other hand, the nurse's satisfaction of the patient-engaged bedside handoffs was significantly lower than that of the EMR-based handoffs (t=13.21, p<.001). There were no differences in length of the reporting time between two types of handoffs (t=-0.48, p=.634). Conclusion: Patient satisfaction with the patient-engaged bedside handoffs was higher than that of EMR-based handoffs, and nurses' satisfaction with the patient-engaged bedside handoffs was significantly lower than that of EMR-based handoffs. Future studies are needed regarding the impacts of patient-engaged bedside handoffs on the quality of healthcare by identifying the benefits of the handoffs.

A Case-Report on Taeeumin Patient with Essential Tremor using Melonis Pedicelus (과체로 호전된 태음인 본태성 진전 치험 1례)

  • Objectives The Purpose of this case is to report that a Taeeumin patient diagnosed as essential tremor was treated with Melonis Pedicelus and then patient's symptoms improved. Methods We prescribed Melonis Pedicelus for patient's physical symptoms. The improvement of the patient's essential tremor was evaluated. Results After the Melonis Pedicelus was given, the patient's Essential tremor and physical symptoms were improved. Conclusions This case study showed an efficient result of using Melonis Pedicelus in the essential tremor of Taeeumin.

  • Objective : To investigate the therapeutic effects of HangAm-Plus (HAP) on advanced gastric cancer patient. Methods : A 53 year old male patient diagnosed with advanced gastric cancer (T3N3M1) was admitted to EWCC (East-West Cancer Center) on Nov. 20008. He had refused to go through the standard cancer regimen after having total gastrectomy on Jul 2008. The patient was treated with HAP (3,000 mg/day) for the period of 11 months from Nov 27th, 2008 to Oct 10th, 2009. Computed tomography (CT) and endoscopy were used to evaluate the disease progression of the patient. Results : HAP treatment was well tolerated by the patient. Patient has shown 25 months of stable disease condition up until now. Conclusion : This case study supports HAP's potential efficacy in treating advanced gastric cancer patients.

Clinical Approach of Chukyu Pharmacopuncture and Brainspotting Through a Traffic Accident Patient Case (교통사고 환자 증례를 통한 척유약침과 Brainspotting의 임상적 접근)

  • The purpose of this study was to introduce effects of Chukyu pharmacopuncture and Brainspotting on knee pain, headaches, and acute stress disorder of a patient with a traffic accident. We treated a patient with Chukyu pharmacopuncture and Brainspotting. Numerical rating scale (NRS), Range of Motion (ROM), Hyperextension Position (HEP), Effusion (Eff), Patella Compression Test (PCT), Frontal Flexion Position (FFP), McMurray (MCM), Lateral Joint Line Tenderness (LJLT), Medial Joint Line Tenderness (MJLT), Anterior Drawer Test (ADT), Lachman Test, Varus/Valgus stress, Beck Anxiety Inventory (BAI), Patient Health Questionnaire-9 (PHQ-9), Pittsburgh Sleep Quality Index (PSQI), Core Seven Emotions Inventory Short Form (CSEI-S), Subjective units of disturbance scale (SUDs), and Heart rate variability (HRV) were used to evaluate the patient. After the patient was treated by Chukyu pharmacopuncture and Brainspotting, the patient showed improvement in NRS, physical examination, and psychological symptom assessment. These results indicate that Chukyu pharmacopuncture and Brainspotting are effective on knee pain, headache, and acute stress disorder after a traffic accident.

  • Breast cancer is common in West and Incidence of breast cancer has increased in Korea. According as conventional western medical treatment, breast cancer patient received MRM(modified radical mastectomy) and anti cancer chemotherapy. In this case report, We introduce a case of breast cancer patient who showed lung metastasis(metastatec adenocarcinoma) after MRM(modified radical mastectomy) 4 years before the diagnosis of relapse. After lung metastasis of breast cancer, the patient received 4th chemotherapy and refused to get more conventional western medical treatment including chemotherapy. After 24 month of traditional oriental medical treatment using allegen removed Rhus Verniciflua Stokes(ARV), however, the size of cancer mass decreased and the patient showed improved condition. Further case study will be needed in order to determine the effect of ARV on breast cancer patient.

  • Objectives We report 1 treatment case with Onbaek-won which is based on Sasang constitutional medicine for specific symptoms that leukoaraiosis Soeumin patient has. Methods We treated leukoaraiosis Soeumin patient suffering from specific symptoms. We prescribed Onbaek-won for her physical symptoms. The improvement of her symptoms were evaluated by Manual Muscle Test(MMT) and Visual Analogue Scale(VAS). Results After the Onbaek-won was given, the patient's most of symptoms were improved. And there was not any adverse effect. Conclusions This study shows that using Onbaek-won can be effective treatment for leukoaraiosis Soeumin patient's specific symptoms.
