Background : The clothing is considered as second skin. The patient clothing needs basic clothing demands which are comfortable, functional and aesthetical and special demeands which are associated with medical examination, treatment and management of daily-life inconveniences. The patient is weak and labile to psychic trauma and has limitation of movement. So the help of others is needed. But recent patient clothing does not satisfy above demands. So the design, making and management of patient clothing to satisfy those demands are very important. The purpose of this study' is to develop multipurpose clothing which is comfortable, functional, aesthetical and satisfying patients and medical personnels. Method : The questionnaires of existing patient clothing were tested by patients, medical doctors, and nurses. After analyzing two types of questionnaires, eight types of experimental patient clothing were made. After preference test, shortcomings were removed and merits were adapted. One type of multipurpose patient clothing mixing eight above experimental clothing was made. The state of wearing and moving were videotaped. The videotape-recoding was watched by patient, caregivers(medical doctors, nurses) and clothing specialists. Result : The general reaction for experimental clothing were positive by the group of patients, medical personnels and clothing specialist, there were significant differences among three groups. Conclusion : It was thought that to develop one type of multipurpose clothing is possible. If the studys for patient clothing which are free-size and economic matters that are type of clothes and method of washing are made in depth, the desirable patient clothing will appear.
This study investigated the differences in patient experience by arrangement type of medical tourism facilitators(MTF) from the pre-visit to visit stages. Specifically, patient experiences from each stage with different service providers (MTFs and medical institutions) were measured: provision of information and respect for patient preferences as pre-visit experiences with the facilitators, communication and concierge services as visit experiences with the facilitators, and medical services as medical institution experiences. The scale to measure foreign patients' experiences was modified from the 'Picker in-patient questionnaire(PPE)' and the 'Picker patient experience questionnaire(PPE-15)'. Quantitative data were collected by conducting a self-administered questionnaire on 173 patients from China, Russia, Mongolia, and Kazakhstan. Qualitative data were collected by conducting in-depth interviews with 9 patients and 9 service providers. The data were collected between January and October in 2019. Quantitative data was analyzed by SPSS 25 for Chi-squared test and ANOVA, and qualitative data were analyzed based on keywords. The main results are as follows. When foreign patients used only overseas MTFs, they had a relatively positive patient experience in respect of receiving pre-visit information(F=7.47, p<.01) and respect for patient preferences(F=3.11, p<.05). Looking at both domestic and overseas facilitators during the visit, the patient experience was relatively negative for communication(F=3.75, p<.05). Regarding medical institutions, patients had a relatively negative patient experience with regards to medical services when they used both domestic and overseas facilitators(F=6.49, p<.01). The implications of this study are as follows. Patients should have a seamless and high-quality experience regardless of the facilitator arrangement type. This can be prepared through service standardization for the service providers. It would be also necessary to consider each other's features and problems at the institutional level and to improve service coordination by having service providers periodically communicate with each other.
Objectives: This study is to report a case that has an effective result to systemic type juvenile idiopathic arthritis patient with an oriental medicine treatment. Methods: We treated the patient with an oriental medicine and tapered down with the western medicine. We followed up the laboratory blood tests every two or three months and through telephone at least six days in a week. Results: The symptoms of systemic type juvenile idiopathic arthritis were vanished and the patient maintains his condition with oriental medicine treatment after discontinued all Disease-Modifying Antirheumatic Drugs(DMARDs). His ESR, CRP levels and other blood test results were back to normal range. Now he is in clinical remission status. Conclusions: According to the result, the oriental medicine treatment is considered to be effective on the systemic juvenile idiopathic arthritis and further studies will be needed with more cases.
Objectives This study is to report a case that has an important meaning as a result of treating Juvenile Rheumatoid Arthritis. We investigated a patient who had to maintain his life with western medicines such as DMARDs, NSAIDs and steroids for a long time. The patient has recovered from all symptoms and his ESR, CRP has been back to normal range with oriental medicine treatment. Methods The patient had fever, especially repeated fever during the afternoon, pain and swelling of joints, generalized skin eruption, anorexia, delayed growth, weight loss, fatigue. So we treated him with herbal medicine and reduced his western medicine. The aim of treatment was recovery from Juvenile Rheumatoid Arthritis after discontinuance of all western medicine. Results The symptoms of systemic type Juvenile Rheumatoid Arthritis was vanished and the patient maintains his condition with oriental medicine treatment after stopped all DMARDs such as MTX(methotrexate) and NSAIDs. His ESR and CRP levels were back to the normal range. After this treatment the patient's height and weight has been increased which showed a significant meaning in growth to the child. Conclusions This study showed that oriental medicine can elevate the Juvenile Rheumatoid Arthritis patient's quality of life with continuous health care and treatment for major problem. For more accurate studies, further studies would be needed with more cases.
In this study, we developed five mobile units and an integrated system which can manage vital signs from each unit using Bluetooth wireless communication. The five kinds of mobile unit were so designed that each has different function to be applied according to the condition of patient properly. The mobile units can measure ECG signal of single or 12 channel, blood pressure, pulse and SpO2 signal from a patient. Also, to reduce the uncomfortable measurement, several types of units such as belt type, wrist type and necklace type were designed. Our proposed system can integrate and monitor several biological signals from different patient by using Bluetooth wireless communication simultaneously. The developed system was evaluated in the simulated emergent situation and showed the system can monitor 5 patients in maximum according to the data quality. It showed the possibilities that the developed system can be used effectively for emergency situation or in- or out-hospital transport of patient. In future, with the combination of mobile communication technique, a patient who is in emergency situation can be provided with proper first-aid and a doctor can pile information of patient and give better diagnosis and treatments.
Hospitals in Korea are having trouble with economic matters because of the low cost of medical insurance and the consumer's demand for high-quality care. The management of patient clothing is very difficult Especially for the management of size is very difficult because the mix of patients in the wards is very different from the stocked clothing sizes. Because of economic matters the ward dose not prepare enough clothing for every size. The nurses in the wards have trouble managing the clothing. Therefore the nurses want free-size patient clothing for all patients. The problem of managing the patient's clothing were surveyed by nureses in the wards. It was known that the free-size patient clothing was needed. The clothing designed from 150cm, to 180cm by 5cm were made. The different sizes of clothing were pretested. The experimental clothing were selected. The experimental patient clothing are designed for between from 165cm and 170cm in height which refers to Korean average. The fitness and sensory test were made by medical personnel outpatient who will be future client for hospital and by specialist in clothing. Finally The free-size patient clothing was made. It was said that there is no difference between the C-type (170cm) and the D-typer(165cm) clothing by specialists in clothing. The outpatient future client for hospital liked the C-type was selected. The questionnaires of the C-type patient clothing was tested by outpatient patients and by medical personnel. Eighty-four point-eightprecent(84.8%) of evaluators liked free-size clothing 75.8% liked unisex use.
Purpose: Objective of this study was to investigate how often nurses to perform patient safety activities. Methods: 521 observations were collected in 9 hospitals by 107 nursing students. Nurses' patient safety care activities were measured 0 (not at all) to 10 (all the time) scores. Descriptive statistics, independent t-test, one way ANOVA, correlation, multiple regression used to analyse data. Results: Items like 'Initial nursing assessment', 'drug management bring on', 'preparation for radiology test', 'falls assessment', 'nursing record' got high scores. But, scores of 'patient identification', 'verbal order management', 'hand hygiene' were lower than others. Each scores were different significantly according to institutions and departments. Within a same institution, the variance of scores, especially in 'patient identification', 'hand hygiene' were great. Scores of activities were different according to characteristics of institutions like type, location, number of beds, teaching hospital, number of accreditation, JCI accreditation. Predictors influencing nurses' patient safety activities were type, location and accreditation. These predictors account for 19.4% of variance. Conclusion: Performance of nurses' patient safety activities were different significantly according to characteristics of institutions. The important items like 'patient identification', 'hand hygiene' had achieved lowest performance. Further researches are needed to improve the basic safety activities.
A modified Fontan procedure was performed on two patients with Univentricular heart. The first patient had UVH of right ventricular type with trabecular pouch and had various associated anomalies, such as common atrium, common atrioventricular valve and combined pulmonary stenosis. The second patient had UVH of left ventricular type with outlet chamber and the associated anomalies were atrial septal defect, tricuspid stenosis and combined pulmonary stenosis. Postoperative hemodynamic insufficiency, fluid retention and renal insufficiency were occurred in the first patient, but relieved with the aid of inotropics and vasodilators. We thought that the good postoperative course and surgical result were gained from the widely patent atriopulmonary anastomosis.
본 연구는 치과위생사 269명의 근무특징과 환자유형에 따른 감정노동이 직무만족도에 미치는 영향을 조사하였다. 일반적인 특징에 따른 감정노동 전체평균은 2.85이며, 성별은 남성, 음주는 하는 경우 높았다. 일반적인 특징에 따른 직무만족도 전체평균 3.02이며 성별은 여성, 연령은 많은 경우, 결혼은 기혼이 높았다. 근무특성에 따른 감정노동은 치과위생사가 다른 직위보다 높았으며, 근무특성에 따른 직무만족도는 경력이 높을수록, 직위는 치과위생사, 수입은 많을수록 높았다. 환자유형별 감정노동은 남성환자, 연령은 20-64세, 진료시간대는 오후 7시 이후가 가장 높게 나타났다. 환자유형별 직무만족도는 환자연령은 65세 이상, 치료 종류는 치아우식 및 치주질환치료, 진료시간대는 오후 7시 이상, 경제수준은 '상'인 경우 높았다. 감정노동 정도에 따른 직무만족도는 감정노동이 '보통'일 때 상승하였으나 '높은군'에서는 가장 낮게 분석되었다. 근무특성, 환자유형, 감정노동이 직무만족도에 미치는 영향을 알아보기 위한 위계적 회귀분석결과 월 수입이 많은집단(t=1.994, p=.047), 환자나이가 많은집단(t=3.027, p=.003), 감정노동이 낮을수록(t=13.858, p=.000) 직무만족도가 높은 것으로 나타났다. 즉 치과위생사들의 직무만족도를 높이기 위해선 감정노동이나 환자의 특성을 고려하여 쾌적한 근무환경을 위한 노력이 이루어져야 할 것이다.
Objectives: The aim of this study was to explore and describe how fasting blood sugar (FBS), postprandial 2 h Glucose (PP2h), and glycated hemoglobin (HbA1c) of a patient diagnosed with type 2 diabetes mellitus can be reduced by treatment with Galgeun-tang herbal medicine. Methods: The patient was administered herbal medicine to reduce serum glucose levels. The prescribed herbal medicines included Galgeun-tang and Galgeun-tang-gami. Results: The therapeutic outcomes were control of blood sugar and glycated hemoglobin (HbA1c) levels and decreased insulin administration. Conclusion: The herbal medicine, Galgeun-tang, appears to be a valid treatment for type 2 diabetes mellitus. Serum glucose (FBS/PP2hrs) and HbA1c were well controlled and insulin administration was decreased. Galgeun-tang was effective in controlling the daily glucose levels in a patient with type 2 diabetes mellitus.
