A STUDY ON THE SELECTION OF ARTIFICIAL TEETH FOR THE EDENTULOUS PATIENTS IN KOREANS (한국인 무치악환자의 인공치아선택방법에 관한 연구)

  • An esthetically pleasing result in denture service is dependent upon several consideration. Esthetic considerations for the edentulous patient include tooth size, shape, arrangement, positioning, gingival contour, age, sex, personality differences, and ethnic type. Especially, the form of anterior artificial teeth is an important factor on the esthetics. The selection of artificaial teeth requires understanding and knowledge on physical and biologic factors and has to meet the indivisul esthetics and functional needs of each patient. However, the selection of artificial teeth is based on the large degree of subjective judgement of the dentist, Therefore, this is one of the most unscientific processes. Many attempts have been made to find a guideline for the selection of artificial teeth. Temperamental theory by White and Hall utillized with the physical characteristics such as body size, body form, color of eyes and hair, and disposition. SPA theory by Frush and Fisher utilized with the basis of sex, personalities, and age of the indivisual. There has not been provided for a guideline and study on the selection of artificial teeth for Koreans yet. This study was aimed to evaluate the William's typal matching theory in Koreans. 1. The facial forms of korean adult were ovoid(46.3%), square(36.7%) and tapered(17.0%) form. 2. The anatomic forms of natural maxillary incisors were ovoid(45.3%), tapered(31.3%), square(23.3%) form. 3. The forms of face and natural maxillary incisors were ovoid(21.9%), squared(11%), tapered(6.3%). 4. The natural maxillary incisors were similar in form to the facial form only in 39%. There was no correlation between the form of the face and the form of natural maxillary central inisors in Koreans. 5. Artificaial teeth which was selected according to the typal matching theory did not represent the form of the natural teeth in 61 per cents of the fatal, but it felt that they harmonized with the form of the patient's face and produced good results.

  • Patient Navigation Program (PNP) is considered as an important implementation of health care systems that can assist in patient's treatment. Due to the feasibility of PNP implementation, a systematic reuse is needed for a wide adoption of PNP computerized system. SPL is one of the promising systematic reuse approaches for creating a reusable architecture to enabled reuse in several similar applications of PNP systems which has its own variations with other applications. However, stakeholder decision making which result from the imprecise, uncertain, and subjective nature of architecture selection based on quality attributes (QA) further hinders the development of the product line architecture. Therefore, this study aims to propose a quality-driven approach using Multi-Criteria Decision Analysis (MCDA) techniques for Software Product Line Architecture (SPLA) to have an objective selection based on the QA of stakeholders in the domain of PNP. There are two steps proposed to this approach. First, a clear representation of quality is proposed by extending feature model (FM) with QA feature to determine the QA in the early phase of architecture selection. Second, MCDA techniques were applied for architecture selection based on objective preference for certain QA in the domain of PNP. The result of the proposed approach is the implementation of the PNP system with SPLA that had been selected using MCDA techniques. Evaluation for the approach is done by checking the approach's applicability in a case study and stakeholder validation. Evaluation on ease of use and usefulness of the approach with selected stakeholders have shown positive responses. The evaluation results proved that the proposed approach assisted in the implementation of PNP systems.

  • The prevalence of anterior cruciate ligament (ACL) injury is continuously increased due to sports activities and traffic accident. Simultaneously ACL reconstruction operations are on the increase. Several kinds of autografts and allografts are used in ACL reconstruction. Although ACL reconstruction using an autogenous bone-patellar tendon-bone graft is the good standard, it might have potential morbidity, anterior knee pain and minimal extension loss. To minimize the complications and disadvantages on each graft and to select appropriate graft for each patient, it is necessary to understand the unique characteristics of each graft for biomechanical aspect, morbidity and disadvantage. Selecting the appropriate graft depends on numerous factors including surgeon's preference and experience, patient's activity level and age, extent of ligament injury, tissue availability, and patient's selection for graft .

  • This article discusses the issues of benefit and possibility of application of patient-controlled sedation (PCS) for the dental treatment. The purpose of administering PCS for patients is to diminish anxiety and to provide cooperative and effective dental treatment. But there are a lot of barriers to application of PCS such as knowledge about pharmacokinetics of sedatives, expensive high speed and patient-controllable syringe pump, and well educated sedationist. And there could be risk of deep sedation, airway obstruction and hypoxic brain damages, etc. In order to decrease incidence of complication, appropriate drug selection, selection of right bolus dose, lock-out time is much important.

  • Due to the existence of asymmetry of information between doctor and patient, it has been believed that doctor might affect patient's decision making process of purchasing medical care. Based on this notion, doctor's reimbursement method has been suggested as an effective policy device of improving efficiency of patient's medical care use by way of its affecting doctor's practice pattern. By using the Community Tracking Study (CTS) household and physician data set, which includes not only various information on patient's medical care use, but doctor's practice arrangements and sources of practice revenue, this paper investigates the effect of community doctor's characteristics of reimbursement method on community patient's medical care use under the control of patient's socio-demographic characteristics and community doctor's practice type. In the process of estimating econometric model, the endogeneity problem of individual health insurance purchase was corrected by using 2818. And due to the existence of sample selection problem, Heckman's two-step estimation method was used for strengthen the robustness of estimation which was adversely affected by sample selection problem The empirical results show that as the average value of community doctor's portion of practice revenue determined by prospective method out of total revenue increases, the community patient's total out-of-pocket medical cost decreases. This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would be more conscious about practice cost, which might affect doctor's practice pattern and by which his/her patient's use of medical care would decrease.

Analysis of patient transfer status of private emergency ambulance services in Busan (부산 지역 민간 응급 이송업체의 환자 이송 현황 분석)

  • Purpose: This study was conducted as a direct investigation of the data in the dispatch logbooks and status of patient transportation provided by private emergency transport companies in Busan. Methods: This study was conducted using SPSS 23.0 version for a total of 1,000 processed records of private emergency ambulance services in Busan from September 23, 2017 to November 5, 2019. Results: First, 100% of the emergency patient transfers by private emergency ambulances were carried out between medical institutions; 76.4% of all transfer patients had emergency conditions, and 86.0% had serious diseases. Second, 59.3% of the emergency patients were located at distances less than 10 km and 43.2%, at more than 10 km from the medical institutions. Third, 63.5% of the passengers were accompanied by first-class emergency rescuers according to the severity of the condition. Fourth, 92.7% of the reasons for the selection of medical institutions were transferred to places where professional care was available, accounting for most of the reasons for the selection. Finally, the medical institutions were selected according to the severity of the patient's condition; 76.5% patients were transported to institutions with a large number of doctors, and 42.9% of those were transported to specialized care institutions. Conclusion: This study collected data from 1,000 dispatch records of private emergency transport companies in Busan; these records reflect the government's policies to improve the emergency patient transfer system. The current status of emergency patient transfer offered by private transport companies was analyzed. All of the emergency patient transfers were carried out between medical institutions, and 76% of the transferred patients had emergency conditions.

A Study on the Criteria for Selection of Medical Care Facilities by Cataract Patient (백내장 환자의 병원선택기준에 관한 연구 - 서울시내 안과전문병원과 대학병원을 중심으로 -)

  • There are increasing interest and need for information on health care consumer with the significance of hospital marketing and strategic planning being increasingly emphasized. This study was conducted to investigate the criteria for selection of medical facilities according to the characteristics of cataract patient. In order to investigate the major factors affecting selection of medical facilities by cataract patient, 300 subjects of cataract patients who was operated at 5 various medical care facilities were selected. Questionnaires are given on March, 2000 and 273 questionnaires were collected. Two types of hospitals(Eye Hospital and University hospital) were combined and analyzed. The main results of this research is as following; 1. In general characteristics of the patients, the variables shown statistically a significant difference between Eye Hospital and University Hospital; The6 educational level and average income are significantly higher in University Hospital Patient. 2. University Hospitals were preferred by the factors of social relationship, social reputation. The critical factors in reference for Eye hospital were kindness, service speed and convenience. This study can be used as a baseline data for marketing planning of hospital management. But the study may be limited in that the results cannot be generalized due to its small sample size and not being able to reflect demographic variables and life style. Further studies to investigate the hospital consumer behavior will be needed.

  • This presentation is to enhance the pharmacist's role in Over-The-Counter(OTC) drug selection and patient counseling for diversification of pharmacy management after the separation of prescribing and dispensing practice in Korea. Self-medication by OTC drugs may be viewed as one element of the broader self-care treatment. The patient may use a OTC drug to manage a minor ailment, a process that may be supported by counseling from a pharmacist. Pharmacists involved in self-medication decisions have a greater involvement with patients and an enhanced professional role. (omitted)

Understanding of Clinical Trials and Application to the Real Practice (임상시험의 단계별 이해 및 실제)

  • Understanding of a clinical trial is essential in developing clinical guideline and adopting evidence based practice. In designing and executing clinical trials, following ethical requirements should be considered : social value, scientific validity, fair subject selection, informed consent, favorable risk-benefit ratio, institutional review board, and respect for human subjects. According to the stage of drug development, purpose of trials, accumulated scientific data, clinical trials for drug development are classified as phase 1, 2, 3, and 4. Phases of clinical trials can be overlapped and the judgment of entering into the next phase should be considered highly strategically. In reading, evaluating and interpreting clinical trial reports, various skills and challenges exist. Patient sample composition, trial duration, selection of endpoints, responders and non-responders, placebo effect, patient recruitment, and extrapolation to the real world are the examples of those challenges. Treatment success will come from the well balanced approach of evidence based decision making and consideration of specific single case.
