An Index of Visual Function in Patients with Cataract - Pilot study - (백내장 환자의 시기능지표 - 사전 연구를 중심으로 -)

  • Background : There is increased recognition that rigorous approach to functional assessment will complement the assessment of clinical status. This study is to develop the appropriate visual function index in Korean patients with cataracts by the assessment of the reliability, validity of visual function indexes including VF-14. Methods : An prospective study was performed with 92 patients who had undergone either one eye or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. Patients were interviewed. and clinical data were obtained. Doctors were questioned with self-entered questionnaire forms. Medical record was examined to understand surgery process. The survey was conducted at 1-2 days before cataract surgery. Results : We used following measures : 7 visual function indexes, global measures of patients' trouble and satisfaction with vision, and best-corrected visual acuity(VA) in each eye. 7 visual function indexes showed a high internal consistency reliability (Cronbach's alpha coefficients ${\geq}$ 0.86). The better visual acuity(operated eye, better eye), the higher patient satisfaction with vision, the lower symptom score, the lower patient trouble with vision was correlated with preoperative visual function index scores. Among 7 indexes, VF-12 with the exclusion of 2 items drivings from VF-14 is a high Cronbach's alpha coefficient and correlation coefficient and the highest $R^2$. Conclusion : We conclude that VF-12 is reliable and valid in Korea. So we suggest that in Korea, all items of VF-14, the visual function index, which is in use internationally, ought to be surveyed and VF-12 which has been excluded by 2 items of drivings from VF-14 ought to be used for analysis.

  • Dental sedation plays a pivotal role in alleviating patient anxiety during various procedures. Remimazolam, a benzodiazepine derivative, stands out for its distinctive attributes, particularly its rapid onset of sedation coupled with a brief duration, making it an invaluable option for dental applications. The patient was admitted for the extraction of impacted third molars via patient-controlled sedation and not only demonstrated stable vital signs but also expressed a high level of satisfaction with the procedure. An in-depth analysis of plasma remimazolam concentrations and changes in the Patient State Index revealed negative correlation patterns, highlighting the inherent potential of remimazolam in achieving effective sedation. This expanded research scope aims to provide a more nuanced understanding of the pharmacological responses to remimazolam in dental sedation scenarios. This case report offers valuable insights into the evolving landscape of dental sedation methodologies and paves the way for a more informed and evidence-based approach to the use of remimazolam in patient-controlled sedation.

The evaluation of implementing smart patient controlled analgesic pump with a different infusion rate for different time duration on postoperative pain management

  • Background: Control of postoperative pain is an important aspect of postoperative patient management. Among the methods of postoperative pain control, patient-controlled analgesia (PCA) has been the most commonly used. This study tested the convenience and safety of a PCA method in which the dose adjusted according to time. Methods: This study included 100 patients who had previously undergone orthognathic surgery, discectomy, or total hip arthroplasty, and wished to control their postoperative pain through PCA. In the test group (n = 50), the rate of infusion was changed over time, while in the control group (n = 50), drugs were administered at a fixed rate. Patients' pain scores on the visual analogue scale, number of rescue analgesic infusions, side effects, and patients' satisfaction with analgesia were compared between the two groups. Results: The patients and controls were matched for age, gender, height, weight, and body mass index. No significant difference in the mount of drug administered was found between the test and control groups at 0-24 h after the operation; however, a significant difference was observed at 24-48 h after the operation between the two groups. No difference was found in the postoperative pain score, number of side effects, and patient satisfaction between the two groups. Conclusions: Patient-controlled anesthesia administered at changing rates of infusion has similar numbers of side effects as infusion performed at a fixed rate; however, the former allows for efficient and safe management of postoperative pain even in small doses.

The Affective Factors of Case Managers' Occupational Stress (사례관리자의 직무스트레스에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to learn the affective factors of case managers' occupational stress. Method: A total of 986 participants responded to a self-administered questionnaire that included 24 items of Korean Occupational Stress Questionnaire Short Form (KOSQSF), job satisfaction index, health behaviors, general characteristics, and variables related to work. The data collection of research was done from 17th to 21th of September, 2007. The data were analysed by t-test, ANOVA, and multiple regression with SPSS 11.1 package program. Result: The results of this study were as follows: The overall job satisfaction rate of men and women was highly than that of the Korean worker's standard. There were statistically significant differences in occupational stress in work place, work department, work position, smoking, treatment in out-patient clinic, subjective & relative health-status, work load, supervisor's review on work-ability, and job satisfaction. According to the multi-variate analysis, occupational stress(47.6%) was related to job satisfaction, workload, individual work-ability, supervisor's review on work-ability, health status and gender. Conclusion: Occupational stress varied depending on the ten variables and was influenced by job satisfaction(35.7%) and 5 other variables. The results suggest that further follow-up study on case managers is necessary to relieve their occupational stress.

Clinical Reports on Atrophic Acne Scar Treatment with Sae-ssack Therapy (새싹테라피 3단계를 이용한 위축성 여드름 흉터 치료 20례)

  • Objectives : The purpose of this study was to evaluate the therapeutic efficacy of Sae-ssack(sprout) therapy on atrophic acne scars. Methods : Twenty patients had atrophic acne scars on the face were treated with Sae-ssack therapy. Sae-ssack therapy consists of a total of three treatments, a first-stage Sae-ssack acupuncture, a second-stage texture treatment, and a third-stage anti-pigmentation therapy. The evaluation of the treatment effect was made by the Korean medical doctor who did not participate in the procedure, and the PGA(Physician's Global Assessment), QGASC(Qualitative Global Acne Scarring Grade System) and patient satisfaction were used as the evaluation index. Results and Conclusions : According to the above indicates, the Sae-ssack therapy was evaluated to be effective in the repair of atrophic acne scars. There were no visible or consistent side effects and patient satisfaction was also very high.

  • Client(patient) satisfaction has been used for a variety of purposes in different settings, yet, those receiving home care services have been the focus of few satisfaction studies. Satisfaction is considered an indicator of quality of care and is also being used to assess the performance of home care providers and home health care delivery at multiple levels. The aim of this study was to develop the tool for evaluating client satisfaction in several aspects with home care service while maintaining or achieving acceptable reliability and validity. Firstly, Client Satisfaction Instrument made preliminary of 43 items composed basic 6 criteria and 7 indicators, based on a review of literature, quality improvement and client satisfaction findings in home care. Secondly, there were consisted in two expert panel groups who were made in professionals of research and practice in home care field. The tool was modified to 6 criteria and 7 indicators, 25 items after reviewing by two expert panel groups. The content validity index of the tool was above 0.8 and the results of internal consistency was more than 0.8 in all items. Finally. The Client Satisfaction Instrument in Home Care Service was made up 6 criteria such as client satisfaction of nurses's instruction of home care educational need. knowledge/technique/attitude of homecare nurses's performance. interpersonal relationship and emotional support of nurse. easiness of homecare accessibility, continuity of homecare service. overall efficacy of homecare use, 7 indicators. and 25 items. The results of evaluating client satisfaction will be able to contribute in quality improvement and service marketing in home care.

Satisfaction and demand of the New Prosthesis Treatment according to the oral health condition of the geriatric patient in Daejeon City (대전지역 노인환자의 구강보건실태에 따른 치과보철의 만족도와 요구도)

  • In this study 240 geriatric patients, aged 60 year and over were evaluated using a questionnaire and oral examination for their oral status, demand and satisfaction of prosthesis treatment. 1. The single main complaint was regarding from the existing removable prosthesis. When the complaints were catagorized into larger groups, complaints regarding existing prosthesis were 32% and followed by 30% in caries and endodontic problems. 2. DMFT index woman appeared the man the age when will increase more highly recording where highly and considered statistically. 3. According to the result from questionnaire, 48% of patients wearing removable prosthesis and 43% with fixed prosthesis were not satisfied with existing prosthesis. Thirty-seven percentages of patients were not satisfied with aesthetics and unable to chew food properly. 4. Thirty-nine percentage of patients were wearing removable prosthesis. More complete dentures were found on maxilla and partial dentures on mandible. 5. Seventy-seven percentage of patients requested for new prosthesis and the majority of these patients had complaints of difficulties in chewing and discomfort.

Awareness and satisfaction of therapeutic goals on functional status in persons with hemiparetic stroke

  • Objective: Setting therapeutic goals assist to identify patient priorities and predict problem that may arise in performing daily and functional activities. Goal setting is associated with improved rehabilitation outcomes in individuals with hemiparetic stroke. This study was conducted to assess the effects of having awareness and satisfaction of goals on the functional status of individuals with hemiparetic stroke. Design: Cross-sectional study. Methods: Eighty-one people who had been diagnosed with stroke participated in this study. The study was conducted through a face-to-face surveys to identify participants' awareness and satisfaction of goals and standard assessment tools were used to evaluate patients' functional status. This study measured functional status through three clinical measures including the manual function test (MFT), Berg Balance scale (BBS), and modified Barthel index (MBI). Results: In participants with hemiparetic stroke, the MBI, BBS, and MFT scores were significantly higher in the participants who were aware of the therapeutic goal than in those who were unaware (p<0.05). The MBI, BBS, and MFT scores were not significant between the participants who were satisfied with the goal and those who were unsatisfied. Conclusions: This study suggests that having awareness in therapeutic goal-setting could lead to improvement of functional status in those with hemiparetic stroke. This study highlights how goal-setting can have a positive effect on persons with hemiparetic stroke.

  • The usability evaluation of the remote fluid monitoring device, which was introduced to reduce the work of nurses and increase the efficiency, was performed due to the expansion of the non-face-to-face medical system. Remote fluid monitoring is a fusion of various technologies such as fluid measurement and analysis, error correction technology, and transmission technology. The range of use by users, the information they want to obtain, and the control device, etc. are wide, and the factors that evaluate the product are also diverse. Therefore, it is difficult to improve the product through evaluation. In this study, a quantitative index was developed to help improve the product for commercialization by conducting 20 usability evaluations in three areas of product stability, operability, and satisfaction with the remote sap monitoring system device. It was performed through infrared and load cell-type sap monitoring devices. In terms of stability, there was a difference in installation work such as fixing the pole of the device, and high satisfaction was shown for operability and accuracy. In terms of product satisfaction, the satisfaction of load cell devices was generally high.

  • Objectives This study investigated the effect of camping style Hospitalization program for atopic dermatitis children patients. Methods 6 patients were admitted for camping style hospitalization program. The program includes acupuncture, herbal medicine, examination, education, psychotherapy, dressing, exercise. The patients were analyzed by Severity Scoring Atopic Dermatitis (SCORAD) index and measured skin pH value (pH). Also, patients evaluated themselves by writing an atopic dermatitis diary which consists of diet, emotion, severity of pruritus, and sleep loss. Checked questionnaires which consists of atopic dermatitis, self treatment methods and each program satisfaction. Results After 7 days of hospitalization program, the patient's SCORAD index scores were decreased and most of the patient's symptoms were improved. Conclusions The hospitalization program could be helpful for atopic dermatitis children patients.
