Purpose: The aim of this study was to elucidate clinical nurses' ethics germane to information literacy and perception of patient data privacy and thus help nurses to develop more positive and consolidated ethical values. Methods: For this study a descriptive survey design was used. Participants were 142 nurses who worked in a hospital and completed self-report questionnaires. Data were collected from August 1 to 5, 2016 and were analyzed using independent t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and stepwise multiple regression with SPSS 22.0. Results: Ethical value had a positive correlation with information needs (r=.25, p=.002) in information literacy as well as in direct patient care (r=.27, p=.001), shift work (r=.20, p=.016), patient information management (r=.39, p<.001), and communication (r=.24, p=.004) in perception of patient data privacy. Patient information management, educational background, and age were significant variables predicting the level of ethical values and accounted for 21% of the variance. Conclusion: Ethical values education with particular emphasize on managing patient information should be encouraged for nurses who are younger and have a lower education level. Findings indicate a need for education programs to guide clinical nurses to utilize appropriate information when solving ethical challenges in every day nursing practice.
Among the countless elements for healing environment in healthcare facility, 'privacy' has been rather neglected in general hospital designs in Korea. The fact may be due either to present situation of excessively crowded patients or to cultural origin which more stresses on the mass than on individuals. However, as patient amenity has increasingly become important, privacy should be viewed as one of the key elements in the general hospital design, in order to improve the quality of patient service as well as to compete with other hospitals. Also it is very important that those methods for privacy improvement should be not only theoretically correct but also practical enough to be compatible with medical efficiency, space availability or construction cost within a particular hospital. The purpose of this case study is to present an actual examples to improve patients' privacy adopted in the schematic design of the Seoul Asan Hospital renovation project, along with comparisons with other hospital cases.
본 논문은 RFID를 이용하여 환자, 의료진, 의약품을 관리하는 UHISRL(Ubiquitous Healthcare Information System based on Real Time Location)을 제안하였다. 제안하는 UHISRL은 환자의 건강상태를 모니터링하고, 그 결과를 스마트 폰과 태블릿 PC로 확인할 수 있다. 또한, 본 논문에서 설계된 ERHL(Extended Randomized Hash Lock) 인증 기법을 사용하여 재전송공격과 스푸핑 공격을 차단하였고, 환자의 프라이버시는 CP-ABE(Cipher Text - Attributed based Encryption)기법을 이용하여 UHISRL DB 접근을 속성에 따라 제한함으로써 보안을 강화시켰다. 특히, UHISRL는 만성질환자의 응급 상황을 실시간으로 모니터링 함으로써 불의의 사고를 방지할 수 있도록 하였다.
A major accident occurring on the bed is falls that occur during at times when the care of nurses or protectors is inadequate, which is fatal to patients or the elderly. In particular, Enuresis or sleepiness caused by sleep apnea increases the risk of falls. Therefore, it is very important to detect falls and sleep apnea of patients without infringing privacy in the bed to patient's safety and accident prevention. In this paper, we reviewed the technologies developed for bed monitoring and implemented a non-intrusive monitoring system. The Occupancy Sensor allows the temperature of the bed and surrounding area to be extracted to enable track of the patient's motion. The Doppler Radar detects the patient's movements at normal times and the respiration state when patients have no movement during sleeping. It is specially designed for real-time monitoring of falling and respiration during sleeping through contactless multi-sensing while solving patient's privacy problems.
In this paper, we propose a privacy-preserving emergency call scheme, called PEC, enabling patients in life-threatening emergencies to fast and accurately transmit emergency data to the nearby helpers via mobile healthcare social networks (MHSNs). Once an emergency happens, the personal digital assistant (PDA) of the patient runs the PEC to collect the emergency data including emergency location, patient health record, as well as patient physiological condition. The PEC then generates an emergency call with the emergency data inside and epidemically disseminates it to every user in the patient's neighborhood. If a physician happens to be nearby, the PEC ensures the time used to notify the physician of the emergency is the shortest. We show via theoretical analysis that the PEC is able to provide fine-grained access control on the emergency data, where the access policy is set by patients themselves. Moreover, the PEC can withstandmultiple types of attacks, such as identity theft attack, forgery attack, and collusion attack. We also devise an effective revocation mechanism to make the revocable PEC (rPEC) resistant to inside attacks. In addition, we demonstrate via simulation that the PEC can significantly reduce the response time of emergency care in MHSNs.
최근 m-헬스케어는 응급상황이 발생할 경우, 환자의 정보가 제 3자에게 쉽게 노출되어 악용될 수 있는 문제가 있다. 본 논문에서는 m-헬스케어의 응급상황 환경에서 환자의 정보를 이용하여 환자의 프라이버시 노출을 최소화하기 위한 속성 기반의 환자 접근 제어 프로토콜을 제안한다. 제안 프로토콜은 환자의 민감한 정보를 제 3자에게 노출시키지 않도록 환자의 민감한 정보를 개인 건강 정보에 포함하여 병원관계자와 환자가 생성한 랜덤수로 해쉬한 서명키로 암호화한다. 또한 제 3자로부터 환자 정보가 불법적으로 악용되는 것을 예방하기 위해서 환자와 병원관계자 사이의 동기화를 유지함으로써 개인 건강 정보의 유출을 예방한다.
Corona 19 minimizes face-to-face contact, and online untact platforms are emerging in the medical sector. However, there are potential risks of medicine expiration, medicine misuse, and responsible materials management for secure delivery. In this paper, we investigate three key functional requirements for online pharmacy, and design the blockchain based online pharmacy to meet the requirements. To protect the patient's privacy and to ensure tamper-free traceability, we incorporate the multi-level access authentication scheme for each participant (governments, medical circles, and patients). We show that our system guarantees patient's privacy without further system modification.
This study aims to find gender differences in inpatients' privacy and stress concerns in multi-bed hospital room. In the literature review, we consider the characteristics of multi-bed hospital room and patients' privacy as a psychological environment factor. In the survey research, the questionnaires were used to understand the inpatients' general characteristics and privacy requirements. A total of 109 copies were analyzed through a cross-tabulations and T-test using the SPSS 18.0. Results and conclusions are as follows: (1) In the case of multi-bed room patients, there were some gender differences in privacy and stress, but the differences were limited. (2) There is a difference in the patient 's preference of the hospital room according to gender, and this is related to the privacy characteristic. (3) The social communication and strolling are commonly effective stress relief methods for both men and women, so space is needed to do this methods. The meaning of this study is to specify the privacy and stress of the inpatients in multi-bed patients' room.
본 연구는 임상실습 전의 간호 학생들의 환자 개인정보보호 인식 정도를 확인하여 정보보호에 대한 관심을 불러일으키고 임상실습 학생들을 위한 환자 개인정보보호 교육 프로그램 마련에 필요한 기반 자료를 제공하고자 시도되었다. 간호학과 2학년 학생 126명을 대상으로 조사한 결과, 간호학생들의 환자 개인정보보호 인식은 높은 수준이었다(4.52점/5점). 일반적 특성에 따른 환자 개인정보보호 인식은 통계적으로 유의한 차이가 없었다. 향후 일부 간호학생으로 제한되었던 대상자를 확대하여 간호학생의 환자 개인정보보호 인식에 영향을 주는 요인을 파악하기 위한 다학제간 융합연구가 필요하고, 임상실습 미경험 간호학생들을 위한 환자 개인정보보호 교육 프로그램 개발과 적용이 필요하며. 환자 개인정보보호 인식을 향상시키기 위한 의료기관과 대학 측의 공동 방안 모색이 필요하다.
최근 IT 기술의 발전과 함께 의료 기술 또한 급격하게 발전하게 되어 헬스케어 서비스를 제공받는 환자의 정보(의료정보, 질병정보 등)가 의료 서비스에서 다양하게 사용되고 있다. 그러나, 환자의 질병정보가 헬스케어 서비스에서 다양하게 사용되면서 환자 치료 방법에 대한 복잡성과 불확실성 또한 증가하여 병원은 환자 치료에 대한 의사결정이 더욱 어려워지고 있다. 본 논문에서는 헬스케어 서비스를 제공받는 환자의 치료를 효율적으로 처리하기 위해서 환자질병정보를 빅 데이터로 관리 할 수 있도록 효율성을 고려한 다기준의사결정벙법 중 하나인 퍼지 AHP를 사용한 헬스케어 환자의 효율적인 빅 데이터 관리 기법을 제안한다. 제안 기법은 환자의 질병정보들 간의 상관관계를 통해 치료기준들 간의 쌍대비교 척도를 삼각퍼지화하여 환자의 현실적 치료 방법을 찾도록 하는 것을 목적으로 한다. 제안 기법은 쌍대비교를 통해 질병 치료들간 퍼지이론의 삼각퍼지수를 적용하여 상대적인 중요도를 산출 한 후 치료 방밥에 대한 효율성을 구한다. 또한 제안 기법은 환자의 질병치료를 결정하기 위한 방법들을 계층화하여 삼각퍼지수를 이용한 쌍대비교 행렬을 구현한 후 질병치료기준별 대안에 대한 중요도를 계산하여 각 치료별 효율성을 분석하여 최종 질병치료 방법을 선택하기 때문에 기존 질병치료 방법보다 판단 및 치료의 애매함과 부정확성 상태를 5.8% 개선하고 있다.
