  • 본 증례에서는 난임의 치료방법으로 FCST의 음양균형장치를 포함한 PBT, 평형 침 치료, 부항, 한약 등의 복합치료로 전신 증상의 개선 및 감소에 현저한 치료효과가 나타남을 관찰하였다. 또한 그 결과로 자연 임신에 성공하여 출산하였고 출산한 아기도 건강한 상태임을 확인하였다.

  • 본 증례에서는 섬유근통 증후군을 진단받고 마약성 진통제와 링겔, 패치 등을 다년간 지속적으로 투여받은 환자에게서 한의학적 치료인 턱관절음양균형요법으로 유의한 효과를 얻어 보고하는 바이다.

Surgical treatment of esotropia and unilateral ptosis in a patient with Cornelia de Lange syndrome

  • Cornelia de Lange syndrome (CdLS) is a rare multisystemic disorder that is characterized by mental retardation, prenatal and postnatal growth retardation, limb anomalies, and distinctive facial features, which include arched eyebrows that often meet in the middle (synophrys), long eyelashes, low-set ears, small and widely spaced teeth, and a small and upturned nose. Ophthalmic manifestations include long eyelashes, nasolacrimal duct obstruction, myopia, ptosis, and strabismus. There has been no report of surgical treatment for esotropia and unilateral ptosis in patients with CdLS in Korea. I report a patient with CdLS who underwent surgical treatment for esotropia and unilateral ptosis with a good surgical outcome.

  • The tracheostomy directs external air into the airway tract. This process causes mucosal dryness, irritation and inflammation in the tracheo-bronchial tree. In order to prevent such problems, several methods are applied; ointment application, humidification and careful suction etc. The heat-moisture exchanger (HME) is commercially sold device that assists heating and humidification of the inhaled air. The authors experienced successful treatment outcome of intractable tracheitis caused by repetitive and vigorous intra-tracheal suction by applying HME in a total laryngectomized patient. We report an interesting and didactic case with a brief literature review.

  • Chronic invasive aspergillosis is a life-threatening disease, especially in immunocompromised patients. The diagnosis and treatment of tracheal aspergillosis (TA) are challenging because of its rarity and nonspecific clinical presentations. The treatment standard of TA has been medical treatment like other forms of invasive aspergillosis, but patients with medically resistant TA require surgical intervention. We demonstrated a successful surgical outcome of chronic invasive TA in a 16-year-old patient with immunocompromised status related to acute myelocytic leukemia.

  • Titanium mesh is an alloplastic material widely used for the reconstruction of moderate-to-large skull defects. Repeated wound problems or infection following these reconstructions inevitably lead to the replacement of the cranioplasty material. Among the various alloplastic materials, polycaprolactone implants are usually used for the coverage of small defects such as burr holes. Herein, we present a case of a large cranial defect successfully reconstructed with three-dimensional-printed polycaprolactone implant and a free latissimus dorsi musculocutaneous flap. Until 1-year followup, the patient showed a favorable esthetic outcome with no complications or wound relapse.

  • 본 증례에서는 턱관절 문제가 시작 된 후 연축성 사경증이 발병한 뒤 다년간의 치료로 호전이 없던 환자에게서 턱관절 음양균형요법(Yin-yang Balancing Therapy of TMJ)을 위주로 침, 추나, 약물요법 등의 한의학적 치료를 통해 유의한 효과를 얻어 보고하는 바이다.

  • Gastrointestinal side effects including nausea and vomiting, diarrhea, constipation, oral mucositis and colitis, and hepatotoxicity are common occurrence during chemotherapy. Often they result in unplanned admission and interruption of scheduled therapy. Additionally they have a negative influence on patient's therapeutic outcome and quality of life. The assessment of gastrointestinal side effects is dependent on clinician assignment of a grade established by the National Cancer Institute Common Terminology Criteria for Adverse Events. Continued interest of gastrointestinal side effects has allowed identifying patients at higher risk and providing effective treatments to relieve painful symptom. Finally, proper prevention and management of chemotherapy-induced gastrointestinal side effects will be needed to improve patient's survival and quality of life.

A rare and unique experience of a blunt intrathoracic traumatic injury of the trachea and its management in South Africa: a case report

  • Blunt intrathoracic tracheal injuries are rare, even among blunt chest trauma patients. An early diagnosis based on a high index of suspicion allows for timely surgical management of potentially fatal airway trauma, thereby improving overall outcomes. Diagnosing these injuries can be difficult due to their nonspecific clinical features and the occasional difficulty in radiologic diagnosis. If a patient exhibits respiratory compromise with difficult ventilation and poor lung expansion, despite the insertion and management of an intercostal drain following high-energy blunt trauma, there should be a heightened suspicion of potential airway trauma. The aim of primary repair is to restore airway integrity and to minimize the loss of pulmonary parenchyma function. This case report discusses the rare clinical presentation of a patient with blunt trauma to the intrathoracic airway, the surgical management thereof, and his overall outcome. Although blunt traumatic injuries of the trachea are extremely rare and often fatal, early surgical intervention can potentially reduce the risk of mortality.

  • There are very few case reports of the diagnosis and management of concurrent oral cavity and parapharyngeal space tumors. We present a case involving a 49-year-old female who presented with oral cavity squamous cell carcinoma confirmed by biopsy. Initial diagnostic workup revealed a concurrent parapharyngeal mass. Diagnostic studies and surgical therapy were tailored to account for both pathological entities. The patient was treated with a combination of surgery and adjuvant therapy. The surgical strategy was designed to address both lesions simultaneously. One year post-surgery, the patient had good response to therapy with no evidence of persistent or recurrent disease. This report discusses the outcome and treatment of a rare case of concurrent squamous cell carcinoma with a complicating parapharyngeal space tumor. It explores the diagnostic process, comprehensive workup, and the surgical management.
