Analysis of Image Distortion on Magnetic Resonance Diffusion Weighted Imaging

  • The purpose of this study is to improve diagnostic efficiency of clinical study by setting up guidelines for more precise examination with a comparative analysis of signal intensity and image distortion depending on the location of X axial of object when performing magnetic resonance diffusion weighted imaging (MR DWI) examination. We arranged the self-produced phantom with a 45 mm of interval from the core of 44 regent bottles that have a 16 mm of external diameter and 55 mm of height, and were placed in 4 rows and 11 columns in an acrylic box. We also filled up water and margarine to portrait the fat. We used 3T Skyra and 18 Channel Body array coil. We also obtained the coronal image with the direction of RL (right to left) by using scan slice thinkness 3 mm, slice gap: 0mm, field of view (FOV): $450{\times}450mm^2$, repetition time (TR): 5000 ms, echo time (TE): 73/118 ms, Matrix: $126{\times}126$, slice number: 15, scan time: 9 min 45sec, number of excitations (NEX): 3, phase encoding as a diffusion-weighted imaging parameter. In order to scan, we set b-value to $0s/mm^2$, $400s/mm^2$, and $1,400s/mm^2$, and obtained T2 fat saturation image. Then we did a comparative analysis on the differences between image distortion and signal intensity depending on the location of X axial based on iso-center of patient's table. We used "Image J" as a comparative analysis programme, and used SPSS v18.0 as a statistic programme. There was not much difference between image distortion and signal intensity on fat and water from T2 fat saturation image. But, the average value depends on the location of X axial was statistically significant (p < 0.05). From DWI image, when b-value was 0 and 400, there was no significant difference up to $2^{nd}$ columns right to left from the core of patient's table, however, there was a decline in signal intensity and image distortion from the $3^{rd}$ columns and they started to decrease rapidly at the $4^{th}$ columns. When b-value was 1,400, there was not much difference between the $1^{st}$ row right to left from the core of patient's table, however, image distortion started to appear from the $2^{nd}$ columns with no change in signal intensity, the signal was getting decreased from the $3^{rd}$ columns, and both signal intensity and image distortion started to get decreased rapidly. At this moment, the reagent bottles from outside out of 11 reagent bottles were not verified from the image, and only 9 reagent bottles were verified. However, it was not possible to verify anything from the $5^{th}$ columns. But, the average value depends on the location of X axial was statistically significant. On T2 FS image, there was a significant decline in image distortion and signal intensity over 180mm from the core of patient's table. On diffusion-weighted image, there was a significant decline in image distortion and signal intensity over 90 mm, and they became unverifiable over 180 mm. Therefore, we should make an image that has a diagnostic value from examinations that are hard to locate patient's position.

On board imager를 이용한 치료간 환자 셋업오차 평가 (Evaluation of the Interfraction Setup Errors using On Board- Imager (OBI))

  • 영상유도방사선치료(image guided radiation therapy: IGRT) 시 환자를 1차적으로 skin marker를 이용하여 위치시키고 2차적으로 OBI(on board imager)를 이용하여 해부학적 위치를 확인 후 couch를 움직여 set up을 보정하게 되는데, 이때 발생하는 오차에 대한 평가를 하려고 한다. 치료계획시 $0^{\circ}$$270^{\circ}$방향의 DRR(digital reconstructed radiography) 영상과 OBI로 촬영한 영상을 2차원-2차원 정합(2D-2D matching)으로 비교하여 치료계획시 환자의 셋업과 치료시 환자의 셋업의 오차를 비교하였다. Head&Neck 및 Spinal cord와 같은 주요장기 부위의 치료에서는 치료때 마다 OBI에 의하여 셋업시 확인하였으며, Chest 및 Abdomen&Pelvic 는 일주일에 2~3회 확인하였다. 그려나 보정 값은 모두 OIS(oncology information system)에 기록하여 160명의 환자를 대상으로 각각 Head&Neck, Chest 및 Abdomen&Pelvic으로 나누어 피부 지표를 이용한 셋업의 정확성을 평가하였다. Head&Neck 환자의 평균 셋업 오차는 각각 AP, SI, RL 방향에서 $0.2{\pm}0.2cm$, $-0.1{\pm}0.1cm$, $-0.2{\pm}0.0cm$ 로 나타났으며, Chest의 경우 $-0.5{\pm}0.1cm$, $0.3{\pm}0.3cm$, $0.4{\pm}0.2cm$ 로 나타났고 Abdomen의 경우 $0.4{\pm}0.4cm$, $-0.5{\pm}0.1cm$, $-0.4{\pm}0.1cm$로 나타났다. Pelvic 의 경우 $0.5{\pm}0.3cm$, $0.8{\pm}0.4cm$, $-0.3{\pm}0.2cm$ 나타났다. Head&Neck 같은 강체 (rigid body)는 셋업 오차가 Chest 및 Abdomen 부위에 비하여 상대적으로 작게 나타났다. Chest에서는 횡축 방향의 오차가 컸으며, Abdomen&Pelvic 에서는 AP 방향의 오차가 크게 나타났다. Chest에서 횡축오차가 크게 나타난 이유는 환자 셋업시 환자 몸의 휘어짐에 기인한 것이며, Abdomen에서의 AP방향의 오차가 큰 이유는 환자의 호흡으로 인해 앞뒤 위치의 변화 때문으로 사료된다. 환자 셋업 시스템에서는 systematic error는 나타나지 않았다. OBI는 해부학적 위치를 확인하기 때문에 병소가 피부에 위치해 있을 경우 피부마커로 셋업을 하는 것이 정확할 것으로 생각된다. 2차원-2차원 정합은 3차원-3차원 정합과 비교하여 rolling 오차를 찾아내지 못하나 환자의 피폭이 적다는 장점이 있으며 셋업 확인 시간이 짧기 때문에 실제 임상에서는 2차원-2차원 정합이 유용하였다.

Gastrointestinal hemangioma in childhood: a rare cause of gastrointestinal bleeding

  • Gastrointestinal (GI) hemangiomas are relatively rare benign vascular tumors. The choice of an appropriate diagnostic method depends on patient age, anatomic location, and presenting symptoms. However, GI hemangiomas are not a common suspected cause of GI bleeding in children because of their rarity. Based on medical history, laboratory results, and imaging study findings, the patient could be treated with either medication or surgery. Herein, we report 3 cases of GI hemangioma found in the small bowel, rectum, and GI tract (multiple hemangiomas). Better knowledge and understanding of GI hemangioma could help reduce the delayed diagnosis rate and prevent inappropriate management. Although rare, GI hemangiomas should be considered in the differential diagnosis of GI bleeding.

Rupture of Giant Superficial Femoral Artery Aneurysm in a Leukemic Patient Submitted to Chemotherapy

  • The superficial femoral artery (SFA) is a relatively rare location for lower limb aneurysmatic disease. In the literature, this disease is described an association between a relatively high growth rate and/or the rupture of aneurysms and chemotherapeutic agents. We report a case of the rupture of a giant SFA aneurysm in a patient during chemotherapy for acute lymphatic leukemia.

폐동정맥루 (Pulmonary Arteriovenous Fistula -One Case Report-)

  • Pulmonary arteriovenous fistula is a congenital vascular malformation In the lung, various synonyms including Pulmonary cavernous vascular malformation, Pulmonary arteriovenous aneurysm, Cavernous hemangioma of the lung0 Pulmonary telangiectasia, Pulmonary hamartoma, etc. The pathogenesis of its symptoms is that unoxygenated, desaturated arterial blood enters into the pulmonary venous system directly. Recently we have experienced one case of the pulmonary arteriovenous fistula which was diagnosed as the pulmonary cystic lesion of the lung preoperatively in 20 years old, 61 kg, male patient. Operation was revealed well circumscribed cystic lesion filled with blood, subpleural and anterior mediobasal location, and bright red colored aspirates on two times needle aspirations. Microscopic finding shows ill circumscribed vascular lesion composed of varying sized blood vessels with irregular thickening of wall and final pathological diagnosis is Pulmonary Arteriovenous Fistula. Basal segmentectomy was done and the patient shows good postoperative course.

Delayed Type III Endoleak Caused by Fabric Erosion after Endovascular Repair of an Abdominal Aortic Aneurysm

  • A 74-year-old patient presented with recurrent aneurysms in the infrarenal abdominal aorta and right common iliac artery 6 years after endovascular aortic repair using endografts in the same location. The patient underwent an aorto-bi-iliac replacement with removal of the stent graft. Two holes measuring 2 mm each were found in the removed graft, and they appeared to have been caused by wear from continuous friction between the endograft and the aortic wall.

  • The purpose of this preliminary report was to describe the operating procedure of T-scan system and to identify the location, timing and force of occlusal contact in patient with normal occlusion using computerized T-scan system. From the preliminary observation , the author obtained the following results. 1. T-scan system displayed 2 dimensional and 3 dimensional description of occlusion: contact locations, timing (sequence) and forces of occlusal contacts. 2. The T-scan sensor was the most important part of the T-scan system. 3. The data of T-scan system cannot be stored in computer diskett. 4. The T-scan system is thought to be the most effective system to detect occlusal contacts and can be applied to the followings : occlusal diagnosis, occlusal equilibration, crown and bridge restorative procedures, denture adjustment, implant procedures, splint adjustment, laboratory procedures, periodontal treatment, orthodontics, TMJ treatment and patient education etc.

Guide flange prosthesis for early management of reconstructed hemimandibulectomy: a case report

  • Surgical resection of the mandible due to presence of benign or malignant tumor is the most common cause of the mandibular deviation. Depending upon the location and extent of the tumor in the mandible, various surgical treatment modalities like marginal, segmental, hemi, subtotal, or total mandibulectomy can be performed. The clinicians must wait for extensive period of time for completion of healing and acceptance of the osseous graft before considering the definitive prosthesis. During this initial healing period prosthodontic intervention is required for preventing the mandibular deviation. This case report describes early prosthodontic management of a patient who has undergone a reconstructed hemi-mandibulectomy with modified mandibular guide flange prosthesis. The prosthesis helps patient moving the mandible normally without deviation during functions like speech and mastication.

흉부 손상의 임상적 고찰 (Clinical Analysis of Chest Trauma; Analysis of 247 patients)

  • Clinical analysis were performed on 247 cases of thoracic trauma, those were admitted & treated at the department of thoracic & cardiovascular surgery,Hanyang University Hospital during the period from Jan,1989 to June,1992. Age distribution of those was from 2 to 80 years old & mean age was 38 years old. The ratio of male to female patient was 186:61 [3:1].This ratio revealed high incidence in male patient. The most common cause of trauma was traffic accident in this series.The modes of injury were as follows: traffic accident 124 cases[50.2%],fall down 52 cases[21.05%], stab wound 47 cases[19.03%] and gun-shut wound 1 case.Ellapse time from accident to admission were 141 cases [57.09%] under 6 hr.Rib fracture were observed in 159 cases[64.37%], hemo or pneumothorax were observed 134 cases[54.25%] of total cases and location distributed Right:Left:Both[74:112:37], in left predominant. Conservative,non-operative treatment were performed in 128 cases and operation[open thoracotomy] 32 cases.Mortality was 1.6%[4 cases] & most common cause of death were due to irreversible shock with brain edema. Conclusively, more evaluation & co-operation of other department were expected treatment & better prognosis.
