Psychiatric Treatment of Ruminative Vomiting and the Associated Weight Loss in an Adolescent Boy with Autism : A Case Report (자폐증 청소년에서 체중감소와 연관된 반추적 구토의 정신과적 치료)

  • We treated the persistent ruminative vomiting of a 13-year-old autistic boy by using a behavioral and psychopharmacological approach in a closed ward. Before the interventions, the patient had lost about 10kg of body weight due to very high-frequency ruminative vomiting. Together with psychopharmacological trials, the psychiatric treatment included a combination of a behavioral approach and food regulation that emphasized food restrictions, and we provided regular light meals. There was a considerable reduction of the ruminative vomiting and 2kg weight gain was achieved during the admission. These therapeutic gains were maintained at the 4-month follow-up assessment and the vomiting symptom reached a near-zero level and the weight loss were recovered by about 8kg. The issues related to the treatment approaches for ruminative vomiting with autism are discussed.

A Case of Muscular Sarcoidosis diagnosed by Gallium-67 Scintigraphy and Magnetic Resonance Imaging (갈륨 신티그라피와 자기공명영상으로 진단한 근육 유육종증)

  • Gallium-67 scintigraphy is helpful in the assessment of active extrapulmonary sarcoidosis. Muscular involvement of sarcoidosis is often asymptomatic or nonspecific, and laboratory examinations do not provide convincing evidence of muscular involvement We report a case of muscular sarcoidosis, which was detected by gallium-67 scintigraphy. In a patient who was suffering from fever and arthralgia of knee joint, gallium-67 scintigraphy showed mediastinal and hilar involvement of sarcoidosis with unexpected extensive muscular uptake. Magenetic resonance imaging revealed the detailed depiction of intramuscular infiltration of sarcoid granuloma. Gallium-67 scintigraphy is useful in detecting inflammatory muscular involvement of sarcoidosis as well as other multiorgan involvement.

  • A herniated intervertebral disc is the most common type of soft tissue mass lesion within the lumbar spinal canal. Magnetic resonance imaging (MRI) is a useful tool for the assessment of patients with lower back pain and radiating pain, especially intervertebral disc herniation. MRI findings of intervertebral disc herniation are typical. However, from time to time, despite an apparently classic history and typical MRI findings suggestive of disc herniation, surgical exploration fails to reveal any lesion of an intervertebral disc. Our patient underwent lumbar disc surgery with the preoperative diagnosis of lumbar disc herniation; however, nothing could be found during the surgical procedure, except a swollen nerve root.

  • Objectives : The purpose of this study is to report the effect of traditional Oriental hair care products on inhibition of extensive alopecia areata. Methods : We have experienced a 29-year-old woman with extensive alopecia areata to get worse quickly. The patient was treated by acupuncture and traditional oriental hair care products in 9 months. We used HRV(Heart Rate Variability), APG(Accelerated Photoplethysmograpy) and SALT(Severity of alopecia tool) scale for the assessment of alopecia areata and her physical condition. Results & Conclusions : Her hair loss and physical condition was completely improved.

  • Motor aphasia is an affection frequently caused by insult of the left middle cerebral artery and usually accompanied by a large lesion involving the Broca's area and the adjacent motor and premotor areas. Therefore, a patient with motor aphasia commonly shows articulatory disturbances due to failure of the motor programing of speech sound. Objective assessment and treatment of phonologic programing is one of the important aspects of speech therapy in aphasic patients. We analyzed the speech disorders acompanied with motor aphasia in a 45-year-old man using a computerized sound spectrograph, Visi-$Pitch{\circledR}$, and Multi-Dimensional Voice $Program{\circledR}$. We concluded that a computerized speech analysis system is a useful tool to visualize and quantitatively analyse the severity and progression of dysarthria, and the effect of speech therapy.

  • Eleven years old cocker spaniel referred to Veterianry Medical Teaching Hospital, Seoul National University was diagnosed as pheochromocytoma. The patient was transferred to the hospital after hit by car and the main clinical signs were tachypnea, tachycardia, edema of hindlimb and lethargy. On ultrasonographic examination, hyperechoic foci was detected and it was invaded into the caudal vena cava. Venography showed the filling defect and local dilation of the caudal vena. The diagnosis of pheochromocytoma is challenging due to the nonspecific clinical signs and severe concurrent disease in dogs. This report describes one dog with invasive pheochromocytoma in which radiography, ultrasonography and venacavography were key diagnositc tools. Ultrasonography was a usuful method for assessment of size, shape and margination of the tumor and invasiveness to the major vessels.

Hepatotoxicity in an Adolescent with Black Iced Tea Overconsumption

  • Tea is the most widely consumed beverage after water in the world. The consumption of iced tea has increased in Western countries and spiked among teenagers for enjoyment, freshening up and alertness. A teenager presented with symptoms of hepatitis. Liver ultrasound revealed sludge in the gallbladder. Laboratory investigations excluded all known causes of hepatotoxicity. Detail nutritional history revealed that the patient had been drinking 1.5-2 liters of black iced tea per day for the last three months. He was immediately advised to stop drinking any tea. Gradually all symptoms disappeared and two months after discontinuation of the tea, all liver enzymes returned to normal and the sludge in the gallbladder disappeared. This case report underlines the importance of a meticulous assessment of a child's dietary behavior when investigating a case of hepatotoxicity and raises awareness about the potential side effects of tea overconsumption.

Influence of Body Weight Support and Walking Speed in the Static Posture of Stroke Patients using Indirect PNF Treatment: A Case Report (PNF의 방산을 이용한 간접치료가 뇌졸중환자의 정적 자세에서의 체중지지 및 보행 속도에 미치는 영향 : 증례보고)

  • Purpose : The purpose of this study was to examine the effects of PNF of irradiation using an indirect treatment of the stroke patient's static weight support and walking speed. Methods : Was carried out a total of nine different the PNF method of treatment for patients with left hemiplegia stroke. PNF of treatment is not in the non-paralyzed side of the trunk and upper and lower extremities by applying resistance and increased strength of the affected side. Assessment of treatment weight support both feet measured and modified using the timed up and go test were compared before and after treatment. Results : Treatment of the paralyzed side of the quadriceps and hamstring, gluteus medius muscle strength increased, and both sides of the foot body weight support improved walking speed was increased. Conclusion : Irradiation using PNF indirect treatment improves the function of the gait of stroke patients.

Improvement of Post-inflammatory Hyperpigmentation, Subsequent to Cold Atmospheric Plasma Treatment, in a Patient with Atopic Dermatitis

  • Cold atmospheric plasma (CAP) is a material that generates free radicals through the ionization of air. Despite the application in various medical fields, the efficacy and safety of CAP on post-inflammatory hyperpigmentation due to atopic dermatitis have not been reported. Herein, we report the improvement in post-inflammatory hyperpigmentation of atopic dermatitis, after CAP treatment of three sessions weekly, 10 minutes per session. We examined the clinical severity indexes before and after treatment, including the Investigator's Global Assessment, modified Atopic Dermatitis Antecubital Severity, Eczema Area and Severity Index, and pruritus visual analogue scores. Our results indicate that CAP alleviates the post-inflammatory hyperpigmentation and clinical severity of atopic dermatitis, and effectively improves the atopic skin lesion without severe safety issues. We believe that the previously reported anti-bacterial effects and production of interleukin-6 after CAP treatment are probably responsible for the underlying mechanisms leading to clinical improvement.

A Case of Palliative Chemotherapy of Advanced Gastric Cancer with Multiple Hepatic Metastasis (다발성 간 전이가 동반된 진행성 위암의 고식적 항암치료 1례)

  • We report a case of a 55-year-old man who diagnosed with advanced gastric cancer (AGC), with A review of the literature. A 55-year old man was transferred to our hospital for further evaluation and treatment after being diagnosed with adenocarcinoma through endoscopic biopsy during a regular health examination. An abdominal computed tomography (CT) showed AGC, stage IIA (T3N3M0), while an endoscopic examination showed AGC, Borrmann type 2. The patient is currently under observation after undergoing radical subtotal gastrectomy with gastroduodenostomy and subsequent administration of oral chemotherapeutic agents. As an abdominal CT response assessment performed after surgery revealed new metastasis to the liver, the patient received palliative chemotherapy as progressive disease was suspected. After receiving chemotherapy in the order of FOLFOX (5-fluorouracil (5-FU)) + Leucovorin + Oxaliplatin), FOLFIRI (5-FU + Leucovorin + Irinotecan), EAP-II (Etoposide + Doxorubicin + Cisplatin), ELF (Etoposide + Leucovorin + 5-FU), TS-1 (Tegafur + Gimeracil) + Cisplatin, an abdominal CT response assessment showed progressive disease for which the regimen was altered to PFL (Paclitaxel + 5-FU + Leucovorin). The patient has currently completed his second cycle of chemotherapy and after an abdominal CT response assessment, further course of therapy will be decided.
