Estrogen-secreting adrenocortical carcinoma

  • Adrenocortical carcinoma is a rare type of endocrine malignancy with an annual incidence of approximately 1-2 cases per million. The majority of these tumors secrete cortisol, and a few secrete aldosterone or androgen. Estrogen-secreting adrenocortical carcinomas are extremely rare, irrespective of the secretion status of other adrenocortical hormones. Here, we report the case of a 53-year-old man with a cortisol and estrogen-secreting adrenocortical carcinoma. The patient presented with gynecomastia and abdominal discomfort. Radiological assessment revealed a tumor measuring $21{\times}15.3{\times}12cm$ localized to the retroperitoneum. A hormonal evaluation revealed increased levels of estradiol, dehydroepiandrosterone sulfate, and cortisol. The patient underwent a right adrenalectomy, and the pathological examination revealed an adrenocortical carcinoma with a Weiss' score of 6. After surgery, he was treated with adjuvant radiotherapy. Twenty-one months after treatment, the patient remains alive with no evidence of recurrence.

A Case of Postherpetic Neuralgia Treated with Korean Medicine, including Soeumin Bojungikgitang-gagambang (소음인 보중익기탕 가감방 투여를 포함한 대상포진 후 신경통 환자 한방복합치료 1례)

  • Objectives: This study reports on the effect of Korean medicine treatment, including Soeumin Bojungikgitang-gagambang, on a patient with postherpetic neuralgia. Methods: The patient received Korean herbal medicine, pharmacopuncture treatment, and acupuncture treatment for 22 days. We assessed changes in symptoms due to the treatment using the Numeric Rating Scale (NRS), the McGill Pain Questionnaire-Short Form (SF-MPQ), and the European Quality of Life-5 Dimensions (EQ-5D) scale. We also measured the Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) to distinguish neuropathic pain. Results: After treatment, the patient showed decreased NRS, SF-MPQ, and EQ-5D scores. Conclusion: The results indicate that Korean medicine treatment, including Soeumin Bojungikgitang-gagambang, is an effective management of symptoms in patients with postherpetic neuralgia.

A Review of the Assessment Tools in Clinical Trials for Patients Injured in Traffic Accidents in South Korea (국내 교통사고 후유증 환자의 평가도구에 대한 임상연구 실태조사)

  • Objectives : The aim of this study is to review the instruments used to assess patient condition and the effects of Korean medical treatment for patients injured in traffic accidents in clinical studies in the Korean medical field published in South Korea. Methods : A literature study was performed, and clinical studies on patients injured in traffic accidents visiting Korean medical clinics or hospitals were examined. Data about chief complaints, tools used to assess either patients' condition or the effectiveness of Korean medical treatment, and time point of assessment were extracted. The studies were classified according to symptom. The use-frequency and the parent category of the assessment tools were also analyzed. Results : 78 studies were selected and classified into 5 categories based on the chief complaints, neck pain, lower back pain, headache, psychological symptoms and unspecified symptoms. Various scales were used to report the patient's pain severity, and functional or psychological status resulting from traffic accidents. Pain index, in particular VAS, was most frequently used and a few mechanical instruments and Korean medicinal assessment tools were mentioned. Conclusion : Most instruments used in Korean medicine clinical studies on patients injured in traffic accidents are self-reported tools. To assess both the patient's condition precisely and the effectiveness of Korean medical treatment on patients injured in traffic accidents, more appropriate and unified tools should be designed that consider pain, functional disability, psychological status and objective assessments measured by devices together.

A Case of Advanced Gastric Cancer with Multiple Leptomeningeal Metastasis (진행성 위암의 추적 관찰 도중 다발성 수막내 전이가 발견된 환자 1례)

  • Leptomeningeal carcinomatosis occurs in approximately 5% of patients with cancer. The most common cancers involving the leptomeninges are breast, lung cancer and melanoma. However, gastric adenocarcinoma has been rarely reported with leptomeningeal carcinomatosis. The presenting manifestations are usually headache, visual disturbances and seizures. We report a case of leptomeningeal metastasis that presented as a gastric cancer. A 75-year old man was transferred to our hospital for further evaluation and treatment after being diagnosed with adenocarcinoma through endoscopic biopsy during a regular health examination. An abdominal computed tomography (CT) showed AGC, stage IA (cT1N0M0), while an endoscopic examination showed AGC, Borrmann type 2. The patient is currently under observation after undergoing radical subtotal gastrectomy with gastroduodenostomy and subsequent administration of oral chemotherapeutic agents. As an abdominal CT response assessment performed after surgery revealed new metastasis to the liver, the patient received palliative chemotherapy as recurrence was suspected. After receiving chemotherapy in the order of DP (Cisplatin + Docetaxel), FOLFIRI (5-FU + Leucovorin + Irinotecan), an abdominal CT response assessment showed complete response. Since decreased mentality maintained throughout the follow up period based on outpatient clinic, brain MRI was performed and revealed multiple leptomeningeal metastasis. The Patient died 2 days after the diagnosis.

Integrated Effect of Non-Invasive Neuromodulation on Bladder Capacity in Traumatic Spinal Cord Injury Patient: Single Case Report

  • Objective: To evaluate the changes in bladder capacity and storage through non-invasive neuromodulation by application of repetitive Trans magnetic stimulation (rTMS) and genital nerve stimulation (GNS) in traumatic spinal cord survivors. Design: A Single Case Study. Method: The Patient was registered in trail with the clinical trial registry of India (CTRI/2022/05/042431). The Patient was interposed with rTMS on lumbar area, from T11-L4 vertebrae with 1 Hz and the intensity was 20% below that elicited local paraspinal muscular contraction for 13 minutes. GNS was placed over dorsum of the penis with the cathode at the base and anode 2 cm distally at 20 Hz, 200 microseconds, Continuous and biphasic current was delivered and amplitude of stimulation necessary to elicit the genito-anal reflex. For assessment, Neurological examination was done for peri-anal sensation (PAS), voluntary anal contraction (VAC) and bulbocavernous reflex (BCR), deep anal pressure (DAP), and American Spinal Injury Association Impairment Scale (ASIA scale). Outcome assessment was done using Urodynamics, Spinal Cord Independence Measure Scale Version-III (SCIM-III), American Spinal Injury Association Impairment Score (ASIA Score), Beck's Depression Inventory Scale (BDI). The baseline evaluation was taken on Day 0 and on Day 30. Results: The pre-and post-data were collected through ASIA score, SCIM-III, BDI and Urodynamics test which showed significant improvement in bladder capacity and storage outcomes in the urodynamics study across the span of 4 weeks. Conclusion: rTMS along with GNS showed improvement in bladder capacity & storage, on sensory-motor score, in functional independence of individual after SCI.

  • Background: The dosage of the anesthetic drugs is generally determined by the total body weight of the patients. However, the drugs can be overdosed when the patient is morbidly obese. We have determined anesthetic induction dose based on lean body mass estimated from bioelectrical impedance analysis (BIA). Case: We report a case of morbidly obese patient (161 cm, 138 kg and body mass index 53.1) who had an elective laparoscopic cholecystectomy. The dose of induction agent was determined by lean body mass estimated by BIA, and the sedation was assessed by the observer's assessment alertness/sedation scale. Conclusions: Dose determination through lean body mass measured by BIA is useful in highly obese patients.

A Case report of Idiopathic Scoliosis Patient : Comparison of X-ray and Digital Infrared Thermographic Imaging (특발성 척추측만증 환자의 X-ray와 적외선 체열촬영의 비교 1례)

  • Purpose : The purpose of this study is to report the comparison of X-ray and Digital Infrared Thermographic Imaging (DITI) of a patient with idiopathic scoliosis. Method: The patient in this case was a 25-year-old female. Her chief complaint was a pain in lumbar, scapular and shoulder regions. We examined her with DITI and X-ray. And then We compared DITI and X-ray. Results: Cobb's angle of thoracic spine was $24.78^{\circ}$ as a primary curve. Cobb's angle of thoracolumbar spine was $17.63^{\circ}$. Temperature on convex side of the thoracic spine was $0.3^{\circ}C$ higher than the other side. Temperature on convex side of cervical spine was $1.4^{\circ}C$ higher than the other side. There was no correlation curvature degree with temperature difference. Conclusion : There was a tendency that the temperature on convex side of the spine is higher than the other side of the spine. DITI is a useful assessment tool when it is used with X-ray as a diagnostic tool of idiopathic scoliosis. Further studies are needed.

A Case Report of Symptom Improvement in Stage IV Metastatic Colorectal Cancer Treated with a Korean Medicine-based Integrative Cancer Treatment (전이성 4기 대장암 환자의 한의기반 통합암치료를 통한 증상 호전 증례보고)

  • Objective: This case report shows symptom improvements in stage IV metastatic colorectal cancer treated with a Korean medicine-based integrative cancer treatment (ICT). Methods: A 61-year-old male patient diagnosed with colorectal cancer in November 2017 and metastasis to the lung, peritoneum, and liver in September 2020 was treated with Integrative Cancer Treatment (ICT) for abdominal pain, abdominal discomfort, and anorexia for 1 month. Clinical outcomes were measured with the National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), a numeral rating (NRS), the Eastern Cooperative Oncology Group (ECOG), and the Functional Assessment of Cancer Therapy-General (FACT-G) scales. Results: After treatment, the patient's abdominal pain was improved from NRS 8 to 1, and the ECOG score of the patient was improved from grade 3 to 2. Integrative Cancer Treatment. Also the score improved on the FACT-G test, and there were no serious side effects of grade 3 or higher according to the NCI-CTCAE. Conclusion: This case study suggests that Korean medicine-based ICT may help to improve abdominal pain and quality of life in patients with metastatic colorectal cancer.

A Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Warm Needling (온침으로 호전된 항암화학요법 유발 말초신경병증 치험 1례)

  • Objective: The purpose of this study is to report the effects of warm needling in a patient with cancer who had chemotherapy-induced peripheral neuropathy (CIPN). Methods: A 46-year-old Korean female outpatient diagnosed with malignant ovarian cancer was treated with warm needling therapy on the foot acupuncture points for CIPN. Neuropathic symptoms and quality of life were assessed using the numeric rating scale (NRS) and the functional assessment of cancer therapy/Gynecologic Oncology Group neurotoxicity (FACT/GOG-NTX) score. Results: After 6 months of treatment, the patient showed a reduction in the severity of CIPN symptoms and an improvement in the quality of life, although the severity of symptoms fluctuated as the patient underwent chemotherapy sessions. Conclusion: This study suggests that warm needling may be an effective treatment for CIPN.

  • ■ Objectives This clinical study is to report the effect of Korean medicine on patient with hot flushing ■ Methods A patient who suffered from hot flushing was treated with herbal medicine 'Gami Ahnjeonbaekho-tang', acupuncture, moxibustion and pharmaco-acupuncture. Degree and frequency of flushing were measured. Digital Infrared Thermal Imaging (DITI) was used to estimate the temperature of skin surface for the outcome assessment ■ Results After taking Gami Ahnjeonbaekho-tang, degree and frequency of hot flushing reduced and skin temperature of patient was reduced on DITI. ■ Conclusion This clinical case study showed the effect of Korean medicine on hot flushing
