The Effect of Defecation on Blood Pressure in Stroke Patients (배편이 중풍 환자의 혈압에 미치는 영향)

  • Background : Patient, family, society and country are badly damaged by stroke (CVA : cerebrovascular accident). To reduce sequelae and return rapidly to society, treatment in the acute stage is very important. In many studies on purgation therapy (下法) used in acute stages of stroke, the changes of blood pressure, pulse and so on before and after purgation therapy were reported. However, the changes in the human body according to the day elapsed after defecation were not reported, so study was needed to confirm such changes. Methods : Seventy-six patients were studied. We observed forty-three patients at acute stroke and thirty-three patients at convalescent stroke. To confirm the importance of regular defecation in stroke patients, we analyzed blood pressure according to the day elapsed after defecation. Results : The following result were obtained. 1. In stroke group, compared with the day after defecation, the mean blood pressure of the third day after defecation increased. It increased the most in the acute stroke group. 2. Especially in stroke and acute stroke groups, compared with the second day after defecation, the mean systolic blood pressure of the third day after defecation increased significantly. 3. In the infarction group, compared with the day after defecation, the mean blood pressure of the third day after defecation increased. It increased the most in the acute infarction group. 4. Especially, in the infarction and acute infarction groups, compared with the second day after defecation, the mean systolic blood pressure of the third day after defecation increased significantly. Conclusion : In stroke patients, regular defecation is important, so we think that purgation therapy (下法) is needed in stroke patients over the third day elapsed after defecation. We think that the bowel movement control in acute stroke patients is more important than in convalescent stroke patients.

Analysis of setup error at rectal cancer radiotherapy technique (직장암 방사선치료기법별 자세오차에 관한 분석)

  • Radiotherapy of rectal cancer requires a stabilized image but the movement of patients is almost unavoidable in radiotherapy. In this study, the setup error using the radiation treatment technique was compared according to the loading time and BMI(Body Mass Index) for 14 patients with rectal cancer. In addition, the variation of the dose by the average setup error was compared. Therefore, the technique of a selective standard was established. As a result, 3DCRT(3-Dimensional Radiation Therapy) and VMAT(Volumetric Modulated Arc Therapy) showed a similar time and error. In comparison, IMRT(Intensity Modulated Radiation Therapy) increased the time two fold and the error four fold. In BMI, a more pyknic patient showed a larger error for all techniques. Regarding the dose, IMRT and VMAT increased much more than 3DCRT in the average error at the small bowel. Therefore, 3DCRT of the short time will be applied to pyknic rectal cancer. Moreover, VMAT selects than IMRT in the overexposure of the small bowel.

  • Objective : Hemichorea-Hemiballism is a violent, involuntary movement restricted to one side of the body. It is usually associated with stroke, and infrequently associated with infections, drug usage, metabolic derangement, and hyperglycemia. In this study, we performed to evlauate the effects of Saamchimbeop Ganjeonggyeok on the the 1 case of Hemichorea-Hemiballism with diabetes mellitus. Methods : We performed Saamchimbeop Ganjeongyeok which consists of KI10(陰谷), LR8(曲泉), LU8(經渠), LR4(中封). The effect of Saamchimbeop was assessed by comparing the counts of the movements of ankle and metatarsal joint for 1 minutes before and after acupuncture therapy. Results : The hemiballismic movements was reduced after acupuncture therapy. Conclusions : The Saamchimbeop was effective on the patient of Hemichorea-Hemiballism with diabetes mellitus. And Further studies with more population are necessary.

  • At here, we investigated the historical background, current stage, and concrete therapies of alternatives in cancer therapy. We obtained the conclusion below. 1. Alternative medicine(AM) is a unothodoxial medical physiology, medical practice or intervension that overcome the side effects, faults, limits of coventional medicine. 2. Recentely, as emerging New-age movement, collapse of Decarte's mecanical cosmology, and emphasis on patient rights, the interest of AM is increased, and foreign & domestic use rates of AM are risen. 3. After coventional medicine in cancer therapies that depends on operations, radiations, and chemotherapy is bounded, Alternatives in cancer therapy are rapidly developed, and the majority of users are often considered satisfactory. 4. Alternatives in cancer therapy are classified six boundaries; Diet and Nutrition, Mind-body Technique, Integrated System, Pharmacologic and Biologic treatments, Immuno Enhanced Therapy, Natural and herbal medicine. 5. Looking at contents of Alternatives in cancer therapy, we get to know most of them are similar to the treatment of oriental medicine in holistic method, therapy, philosophy. If we synthesize the alternative medicine's treatment mentioned above and investigate it with oriental medical viewpoint, that may help us approach the conquest of cancer and improve the rate of cure. 6. It is certain that foreign AM will be imported reversely, and we must prepare for rebuilding of Korean traditional AM systemically, investing for activity positively, making national policy for medical system.

  • The heifer electrocardiograph is a widely progressing research which is used in judgement to existence for a heart disease and analyzes electric potential changes of heart movement during continuous living of everyday after attaching to body portable holler recorder for twenty-four hours. In order to obtain electrocardiogram clearly, it must eliminate that 60Hz power line interference, baseline wandering, noise of muscle constriction. In holter electrocardiogram, the most big problem which is recorded signal of ECG(electrocardiogram) is a baseline wandering elimination, which is occurred by rhythm of respiration and muscle constriction of part from attaching to an electrode. Such baseline is roughly irregular wandering and shaking up and down therefore the part of baseline wandering elimination is very important because it is difficult of ECG diagnosis. In this study, it is stabilized baseline wandering in analog part as implementation f real-time signal processing digital filter so it is applicable to analyze patient's heart disease by way of design of baseline wandering elimination system.

  • Eagle's syndrome is a disease caused by an elongated styloid process or calcified stylohyoid ligament. Eagle defined the disorder in 1937 by describing clinical findings related to an elongated styloid process, which is one of the numerous causes of pain in the craniofacial and cervical region. The prevalence of individuals with this anatomic abnormality in the adult population is estimated to be 4% with 0.16% of these individuals reported to be symptomatic. Eagle's syndrome is usually characterized by neck, throat, or ear pain; pharyngeal foreign body sensation; dysphagia; pain upon head movement; and headache. The diagnosis of Eagle's syndrome must be made in association with data from the clinical history, physical examination, and imaging studies. Patients with increased symptom severity require surgical excision of the styloid process, which can be performed through an intraoral or an extraoral approach. Here, we report a rare case of stylohyoid ligament bilaterally elongated to more than 60 mm in a 51-year-old female. We did a surgery by extraoral approach and patient's symptom was improved.

Risk Factors of Severity of Pressure Injuries in Acute University Hospital Inpatients (급성기 대학병원 입원환자의 욕창중증도의 영향요인)

  • This study was conducted to identify the factors influencing the severity of pressure injuries of patients with pressure injuries admitted to acute university hospital. This study was a secondary analysis on the data of the study conducted to identify the factors influencing the deterioration of pressure injuries during hospitalization. The data were collected by retrospectively examining the medical records of patients with pressure injuries who were 18 years of age or older and who were admitted to acute university hospital from May 2017 to November 2018. We used data from 472 patients with pressure injuries at admission for this secondary analysis. In order to identify the factor influencing of severe pressure injuries compare to superficial pressure injuries, we analyzed the data using logistic regression analysis. As a result of the study, gender, body temperature, and patient's movement were identified as factors affecting severe pressure injuries. Therefore, special care is necessary to increased the number of position change for inpatients with pressure injuries, especially for patients with decreased mobility.

  • When we see normal gait, gait cycle is seperated as stance phase and swing phase. It needs 6 determinant of gait of pelvic rotation, pelvic tilt, knee joint of stance phase, ankle and foot motion, ankle and knee motion, and pelvic movement to be accomplished. In addition, a joint and muscle action is accomplished biomechanically at the same time with its gait cycle. In oriental medicine, the relationships between chang-fu physiology and meridian physiology are summaried as follows ; ${\bullet}$ chang-fu physiology : Spleen manages the extremities. Liver manages soft tissues. Liver stores blood. Kidney stores essences. Kidney manages bones. ${\bullet}$ meridian physiology : The Leg Greater Yang Meridian and meridian soft tissues The Leg Yang-Myeong Meridian and meridian soft tissues The Leg Lesser Yang Meridian and meridian soft tissues The Leg Greater Yin Meridian and meridian soft tissues The Leg Lesser Yin Meridian and meridian soft tissues The Leg Absolute Yin Meridian and meridian soft tissues Especially, we can find out relations between in a "blood supplied feet can walk well" that explains "blood regulations and by liver nourishing effects"that is the closest concept of muscle. Abnormal gaits are due to three causes as following; first, physical defect secoud, pain third, nervous system or instability of muscle. In oriental medicine, we can know relationship in "atrophy, numbness, stroke, convulsion, muscular dystrophy of knee, rheumatoid arthritis, five causes of infantile growing defects, five causes of softening, sprain". Especially, atrophy is the most important symptom. Gait evaluation should be emphasized where a point can walk 8 feet to 10 feet considering stride width, stride length, the body weight center, stride number, flexion, extension, rotation of a joint as a standard factor. The point is we should find out something strange in a patient's side, front and back view. After that we should find out its cause as an index that we can observe abnormal findings in a joint and muscle.

  • Arteria radialis is a branch of the brachial artery extending down the forearm around the wrist where it closes to skin surface. In the oriental medicine, the skin above arteria radialis has an important role because oriental medicine practitioners put their finger tips on the area, and diagnose patient's health conditions by feeling the pulsation of the arterial contraction. The finger tip diagnostic method relies on subjective decision of the practitioner; and there is a need to develop an objective diagnostic modality. The pulsation of the arterial contraction appears not only a movement on the site but also as temperature fluctuation due to pulsatile feeding of warmer blood. The goal of this study is to demonstrate a feasibility of using an infrared camera quantitatively to detect the temperature fluctuation on the skin. Clinical important three different areas, called chon, gwan, chuk, near a wrist where the arteria radialis reaches close to skin surface are marked with small pieces of surgical tape. A high-speed and high-resolution infrared camera with a 3 cm of field of view measures these areas for 10 second at 200 frames per second with a 320*240 pixel size. The pulsatile temperature fluctuation is calculated after passing a band pass filter to remove any stationary temperature over 10 second. The temperature fluctuation of a healthy male volunteer is measured at a room temperature as a control, and is compared with another measurement performed after 20 minutes staying in a room at a 40 degree Celsius. This comparison is repeated for three times, and indicates that the fluctuation increases after staying 20 minutes in the warm room. This increase becomes smaller when the person stays in the warm room with an acupuncture treatment that decreases body temperature. So that an objective diagnostics on the site may become feasible.

Schwannoma Originating from Infraorbital Nerve (안와하신경에서 기원한 신경초종의 치험례)

  • Schwannomas are well-differentiated solitary benign tumors that originate from the schwann cells of the nerve sheath. They can readily occur in the head and neck regions, but the schwannoma originating from the infraorbital nerve is extremely rare and usually painless, slow-growing, and without specific symptoms. The author experienced a rare case of infraorbital schwannoma, which was completely removed through the intraoral approach. A 20-year-old woman was admitted to our hospital for a painless, solid and circular mass located on the right infraorbital region. The eyeball movement and visual field were normal. There was no globe displacement or proptosis. Preoperative computed tomography demonstrated $13{\times}10{\times}5mm$-sized soft tissue mass. On March 2011, the mass was removed through an intraoral approach. On histopathological examination, the gross specimen consisted of a smooth, well-encapsulated and light yellowish solid mass, measuring $12{\times}7{\times}5mm$. Microscopically, it presented a typical manifestation of schwannoma with Antoni A area with Verocay body, and Antoni B area on H&E stain. The result of the immunohistochemical staining was positive for the S-100 protein. The patient had hypoesthesia of the nasal septum and vestibule in the postoperative period, and this finding confirmed that the internal nasal branch of infraorbital nerve was the nerve in which the schwannoma originated. Infraorbital schwannomas are very rare and must be included in the differential diagnosis of the orbital masses inferior to the eyeball. In the case of early diagnosis, the small-sized infraorbital schwannomas can be completely removed without any scar through an intraoral approach.
