수면과 시토카인 (Sleep and Cytokine)

  • Cytokines are the main regulatory molecules of not only immune system but also sleep system. Research on the role of cytokines on sleep has greatly been expanding since the first report of sleep-promoting effects of interleukin-1, the first cytokine molecule. Interleukin-1 and tumor necrosis factor are most widely studied among various cytokines. Studies over about twenty years demonstrate that most cytokines promote sleep but several cytokines inhibit sleep. Slow wave sleep is the main part that cytokines have effects on. Besides normal sleep physiology, cytokines have more major roles on pathophysiology of various sleep disorders. Obstructive sleep apnea is the representative sleep disorder that shows how deeply cytokines are involved in their pathophysiologic mechanisms of sleep disorders. Though there are many controversial issues on this topic, more mysterious part of normal sleep physiology and sleep disorders will be revealed in near future through thorough studies on sleep and cytokine.

Contralateral Superior Cerebellar Artery Syndrome : A Consequence of Brain Herniation

  • Vascular compromise is a well-known consequence of brain herniation syndromes. Transtentorial brain herniation most often involves posterior cerebral arteries. However, isolated involvement of contralateral superior cerebellar artery (SCA) during unilateral impending brain herniation is reported only once and we present another case of this exceedingly rare entity. A 24-year-old man was referred to us with impending herniation due to a multiloculated hydrocephalus, and during the course of illness, he developed an isolated SCA ischemia in the opposite side of the most dilated entrapped horn. In the current article we discuss the probable pathophysiologic mechanisms of this phenomenon, as well as recommending more inclusive brain studies in cases suspected of Kernohan-Woltman notch phenomenon in unilateral brain herniation. The rationale for this commentary is that contralateral SCA transient ischemia or infarct might be the underdiagnosed underlying pathomechanism of ipsilateral hemiparesis occurring in many cases of this somehow vague phenomenon.

Sex-Gender Differences in Irritable Bowel Syndrome

  • Because of the sex-gender differences that are shown in a diversity of physiological and psychological factors, it can be speculated that the clinical presentation of symptoms as well as treatment strategies in women and men with irritable bowel syndrome (IBS) may differ. Studies have revealed that IBS is more common in women than men. As for the IBS subtype, IBS with constipation is significantly more prevalent among women than men. Sex hormones and gender differences may play important roles in the pathophysiology of IBS. However, its pathophysiologic mechanisms still remain largely unknown, and therapeutic implications are limited. Moreover, women IBS patients have been reported to feel more fatigue, depression, anxiety, and lower quality of life than men IBS patients. Furthermore, there has been evidence of differences in the appropriate treatment efficacy to IBS in men and women, although relatively few men are enrolled in most relevant clinical trials. A more sex-gender-oriented approach in the medical care setting could improve understanding of heterogeneous patients suffering from IBS. An individualized and multicomponent approach including sex and gender issues might help improve the treatment of IBS.

  • While the rising epidemic of obesity is primarily attributed to a sedentary lifestyle, poor dietary habits, and the aging of the population, secondary causes of obesity generally go undetected and untreated. These include endocrinological disorders, such as Cushing's syndrome, polycystic ovary syndrome, hypothalamic obesity, hypogonadism, and hypothyroidism, as well as genetic, syndromic, and drug-related obesity. We present an overview of the major disorders associated with obesity, highlighting the pathophysiologic mechanisms and discussing the diagnostic and treatment strategies that are most helpful to practicing physicians in recognizing and treating these generally under-detected and undertreated disorders.

신생아 종아리신경병증 1례 (A case of neonatal peroneal neuropathy with intrauterine onset)

  • 출생시에 나타나는 신생아 종아리신경병증은 아직 국내에는 보고된 적이 없는 매우 드문 질환으로, 대부분이 자연 치유되는 양성 경과를 취한다. 신생아의 홑신경병증은 대개 산과적 합병증이지만, 출생 전 원인도 고려하여야 한다. 저자들은 볼기태위로 인해 제왕절개술로 만기 태어난 신생아에서 발견된 발처짐을 보고한다. 생후 4일째 시행한 전기생리학적 검사에서 종아리신경의 복합근육활동전위 소실과 앞정간근과 긴엄지폄근의 탈신경전위가 관찰된 것으로 미루어 자궁 내에서 발병한 종아리신경병증으로 추정한다. 조기에 전기생리학적검사를 시행하고 추적검사를 하면 발병시기와 병리적 기전 및 예후를 판단하는데 도움이 된다.

  • Restless legs syndrome (RLS) is a sensorimotor neurological disorder in which the primary symptom is a compelling urge to move the legs, accompanied by unpleasant and disturbing sensations in the legs. Although pathophysiologic mechanism of RLS is still unclear, several evidences suggest that RLS is related to dysfunction in central nervous system involving brain and spinal cord. L-DOPA, as the precursor of dopamine, as well as dopamine agonists, plays an essential role in the treatment of RLS leading to the assumption of a key role of dopamine function in the pathophysiology of RLS. Patients with RLS have lower levels of dopamine in the substantia nigra and respond to iron administration. Iron, as a cofactor in dopamine production, plays a central role in the etiology of RLS. Functional neuroimaging studies using PET and SPECT support a central striatal D2 receptor abnormality in the pathophysiology of RLS. Functional MRI suggested a central generator of periodic limb movements during sleep (PLMs) in RLS. However, to date, we have no direct evidence of pathogenic mechanisms of RLS.

  • Oral prostaglandin E1 (PGE1) is a medicine that is clinically applied during a treatment of patients suffering with vascular disease with chronic arterial obstruction because it has vasodilation and anti-platelet effects. The mechanisms of lumbosacral symptoms associated with spinal stenosis probably include vascular insufficiency with hypoxic injury to the cauda equina and the nerve roots. Thus, increasing the blood supply would be beneficial to improve the pathophysiologic condition. Several studies on the improvement of clinical symptoms of spinal stenosis by PGE1 treatment have been reported on. In this case, 47-year old female underwent posterior compression and posterolateral fusion with a cage at L2-4 due to L3 compression fracture, and she did not show improvement of the radiating pain of her right leg after the operation. Therefore, she received repetitive epidural catheterization and adhesiolysis, epidural block and physical therapy, but her symptoms deteriorated after temporary improvement. Finally, she was given PGE1 and the radiculopathy was completely improved, although some muscle weakness still remained.

  • In addition to classic cholinergic and adrenergic pathways, the existence of a third division of autonomic control in the human airways has been proved. It is called a nonadrenergic noncholinergic(NANC) nervous system, and difficult to study in the absence of specific blockers. Neuropeptides are certainly suggested to be transmitters of this NANC nervous system. It is very frustrating to understand the pathophysiologic role of these peptides in the absence of any specific antagonists. However, further studies of neuropeptides might eventually lead to novel forms of treatment for bronchial asthma. Another study of the interaction between different components of the autonomic nervous system, either in ganglionic neurotransmission or by presynaptic modulation of neurotransmitters at the end-organ will elute neural control in airway disease, particularly in asthma. Studies of how autonomic control may be disordered in airway disease should lead to improvements in clinical management. Epithelial damage due to airway inflammation in asthma may induce bronchial hyperresponsiveness. Axon reflex mechanism is one of possible mechanisms in bronchial hyperresponsiveness. Epithelial damage may expose sensory nerve terminals and C-fiber nrve endings are stimulated by inflammatory mediators. Bi-directional communication between the nerves and mast cells may have important roles in allergic process. The psychological factors and conditioning of allergic reactions is suggested that mast cell activation might be partly regulated by the central nervous system via the peripheral nerves. Studies in animal models, in huamn airways in vitro and in patients with airway disease will uncover the interaction between allergic disease processes and psychologic factors or neural mechainsms.

  • It has been said that variable anatomical structures and neural circuits are related to the generation of tremor. There are cerebral cortex, thalamus, basal ganglia, inferior olivary nucleus, midbrain tegmentum, stretch reflex, and musculoskeletal structures. The stretch reflex is related with the physiologic tremor and various peripherally originated tremors. We experienced a case with the post-stroke resting tremor which was induced and aggravated by mechanical stretching stimulation. In the present case, stretch reflex has a major role in the generation and exacerbation of tremor. It is presumed that the development of tremor is attributed to the increased rhythmicity of ventral intermedius nucleus of thalamus. The enhancement of thalamic rhythmicity may be due to the increasement of long latency reflex by post-stroke rigidity. This case suggests that stretch reflex may have a major role in the pathophysiologic mechanisms of a certain centrally originated tremor.
