A Case Study of the Creutzfelt Jacob Disease Suspected Patient (Creutzfelt Jacob Disease 의증 환자에 대한 임상보고)

  • 1. Objectives Creutzfelt Jacob Disease is one of a group of neurodegenerative disorders causing spongiform encephalopathies due to a infection of prion or unconventional slow virus on central nerve system. The diagnosis of this disease is not easy and there is currently no cure. This article is to report our case about a female patient who was not diagnosed as CJD at the early period so that we treated her with Yangkyuksanhwa-tang(凉膈散火湯) and Jihwangbaekho-tang(地黃白虎湯). 2. Methods Magnetic resonance imaging(brain MRI), blood test and computer tomography were performed. The treatment for this patient was clinically based on Sasang Constitutional Medicine. 3. Results and Conclusions (1) Visual field defect, ataxia, myoclonus, sweating and dysuria were the main symptoms of the patient. (2) The pathological change in parenchyme was not revealed during the early periods by MR imaging. So the diagnosing CJD was not possible during the time in this case. (3) Jihwangbaekho-tang(地黃白虎湯) improved her myoclonus and sweating. Bur her mental disorder and the progress of the pathological change in the parenchyme was not able to be treated.

Altered expression of mud loach (Misgurnus mizolepis; Cypriniformes) hepcidin mRNA during experimental challenge with non-pathogenic or pathogenic bacterial species

  • Transcriptional response patterns of mud loach (Misgurnus mizolepis; Cypriniformes) hepcidin, a potential ortholog to human hamp1, in response to experimental challenges with non-pathogenic and pathogenic bacterial species were analyzed based on the semi-quantitative reverse transcription-PCR assay. Mud loach hepcidin transcripts were much more preferentially induced by pathogenic bacterial species (Edwardsiella tarda and Vibrio anguillarum) causing apparent pathological symptoms than by non-pathogenic species (Escherichia coli and Bacillus thuringiensis) displaying neither clinical signs nor mortality. However in overall, the induced amounts of hepcidin transcripts were positively related with the number of bacterial cells delivered in both pathogenic and non-pathogenic bacterial species. Inducibility of hepcidin transcripts were variable among three tissues examined (liver, kidney and spleen) in which kidney and spleen were more responsive to the bacterial challenge than liver. Time course expression patterns of hepcidin mRNAs after challenge were different between groups challenged with pathogenic and non-pathogenic species, although the overall pattern of hepcidin expression was in accordance with that generally observed in battery genes appeared during early phase of inflammation. Fish challenged with E. coli (non-pathogenic) showed the significant induction of hepcidin transcripts within 24 hr post injection (hpi) but the level was rapidly declined to the basal level either at 48 or 96 hpi. On the other hand, hepcidin transcript levels in E. tarda (pathogenic)-challenged fish were continuously elevated until 48 hpi, then downregulated at 96 hpi, although the level at 96 hpi was still significantly higher than control level observed in non-challenged fish. This expression pattern was consistent in all the three tissues examined. Taken together, our data indicate that hepcidin is tightly in relation with pathological and/or inflammation status during bacterial challenge, consequently providing useful basis to extend knowledge on the host defensive roles of hepcidin under infectious conditions in bony fish.

  • Although Serbia is recognized as an endemic country for echinococcosis, no information about precise incidence in humans has been available. The aim of this study was to investigate the skeletal manifestations of hydatid disease in Serbia. This retrospective study was conducted by reviewing the medical database of Institute for Pathology (Faculty of Medicine in Belgrade), a reference institution for bone pathology in Serbia. We reported a total of 41 patients with bone cystic echinococcosis (CE) during the study period. The mean age of 41 patients was $40.9{\pm}18.8$ years. In 39% of patients, the fracture line was the only visible radiological sign, followed by cyst and tumefaction. The spine was the most commonly involved skeletal site (55.8%), followed by the femur (18.6%), pelvis (13.9%), humerus (7.0%), rib (2.3%), and tibia (2.3%). Pain was the symptom in 41.5% of patients, while some patients demonstrated complications such as paraplegia (22.0%), pathologic fracture (48.8%), and scoliosis (9.8%). The pathological fracture most frequently affected the spine (75.0%) followed by the femur (20.0%) and tibia (5.0%). However, 19.5% of patients didn't develop any complication or symptom. In this study, we showed that bone CE is not uncommon in Serbian population. As reported in the literature, therapy of bone CE is controversial and its results are poor. In order to improve the therapy outcome, early diagnosis, before symptoms and complications occur, can be contributive.

Lovastatin derivative dehydrolovastatin ameliorates ulcerative colitis in mice by suppressing NF-κB and inflammatory cytokine expression

  • Ulcerative colitis (UC) is associated with intestinal immune imbalance and inflammatory response. Because dehydrolovastatin (DLVT), a derivative of lovastatin, has been recently shown to inhibit inflammation and relieve immune arthritis induced by chemical stimuli, we studied its effect and possible mechanism on UC induced by dextran sulfate sodium. The BALB/c mice were classified into six groups: normal control group, model group, DLVT high dose group, DLVT low dose group, salazosulfapyridine (SASP) group and lovastatin (LVT) group. The disease activity indices of UC and pathological changes were investigated. The myeloperoxidase (MPO) activity in colon tissue and inflammatory factors such as IL-6, IL-10, IL-17, and TNF-α in the serum were analyzed by ELISA, while the expression of NF-κB p65 protein in colon tissue was detected by immunohistochemistry and western blot. DLVT relieved the disease activity indices and pathological damage of the UC mice. Furthermore, DLVT significantly decreased MPO activity and improved the imbalance of inflammatory cytokines through inhibiting the expression of NF-κB p65. Meanwhile, the positive drug of SASP has a similar effect to DLVT, but the effect of DLVT in both decreasing IL-17, TNF-α, and increasing IL-10 was significantly stronger than that of SASP. These results suggest that DLVT may ameliorates the symptoms of UC.

  • Successful treatment of any pathological condition of the shoulder requires an accurate diagnosis. Physical examination represents an integral part of the assessment of shoulder disorders. Pain, loss of strength, decreased range of motion, and instability are the most common symptoms caused by a great variety of different shoulder pathologies. Therefore, a sophisticated clinical approach to the patient with shoulder pathology is essential. A standardized evaluation including a variety of diagnostic tests and clinical assessments improves the diagnostic accuracy by specifically examining one component of the shoulder complex. In most cases a careful physical examination will establish or suggest a diagnosis and special investigative techniques such as imaging procedures can be applied more selectively for confirmation or further evaluation of the pathological entity.

Contents and Severity of Worries in Outpatients with Generalized Anxiety Disorder Compared to Normal Control (범불안장애 환자와 일반인의 걱정 내용 및 심각도의 비교)

  • Objective : Generalized anxiety disorder (GAD) is a common anxiety disorder characterized by excessive worries and anxiety symptoms. To distinguish between pathological and normal worries, this study compared the contents and severity of worries among patients with GAD with those of matched non-psychiatric control Methods : Thirty outpatients with GAD receiving treatment at the psychiatric department of Hanyang University Guri Hospital were recruited. As a matched control group, we selected 30 adults without psychiatric history who participated in the Anxiety Disorder Survey, during the Mental Health Exposition held in Seoul in April, 2014. Matched demographic data were age, sex, education, and household income. Self-questionnaires included demographic data, contents of worry (10 categories of worries and each severity), and the Pennsylvania State Worry Questionnaire (PSWQ). Results : There was no statistical difference in the contents of worry between patient and control groups. However, the total PSWQ scores significantly higher in the GAD group. Conclusion : Our findings suggest that patients and non-patients worry about the similar contents and worries in GAD cannot be differentiated by what people worry about. Only the severity of worry was a distinguishable feature. Our finding supported inclusion of the diagnostic criterion of "excesslve wornes" in GAD.

Morphological Changes of Bones and Joints with Rheumatoid Arthritis and Osteoarthritis

  • Arthritis is a common disease in aged people, and is clinically divided into rheumatoid arthritis (RA) and osteoarthritis (OA). Although common symptoms such as pain are present, the underlying pathological mechanisms are slightly different. Therefore, the objectives of the present study were to compare joint damage induced by RA and OA by analyzing the major morphological and molecular differences, and to propose a suitable therapeutic intervention based on the pathophysiological conditions of bones and joints. For the RA animal model, 8-week-old DBA1/J mice were immunized with bovine type II collagen emulsified in complete Freund's adjuvant (CFA). Normal C57BL/6 mice (over 2 years of age) were used for OA. The clinical arthritis score was calculated using a subjective scoring system, and paw thicknesses were measured using calipers. The serum TNF ${\alpha}$ level was analyzed using an ELISA kit. Micro-CT was used to identify pathological characteristics and morphological changes. In collagen-induced RA mice, there were increased ankle joint volumes and clinical scores (p<0.01). The concentration of TNF ${\alpha}$ was significantly increased from 3 to 7 weeks after immunization. Micro-CT images showed trabecular bone destruction, pannus formation, and subchondral region destruction in RA mice. OA among aged mice showed narrowed joint spaces and breakdown of articular cartilage. This study suggests that a careful therapeutic intervention between RA and OA is required, and it should be based on morphological alteration of bone and joint.

The Verification of Concordance Coefficient in Sasang Constitutional Factors by Expert (사상체질 전문가의 진단 우선순위 일치도 검증)

Objective : This study aimed to know the concordance coefficient and diagnostic trend of 6 Sasang constitution diagnosis factors among experts, which are important to diagnose Sasang constitution. 2. Methods : We suggested each 100 cases containing body shape, face, temperament, physiological and pathological symptom and constitutional drug response to 2 groups experts, and researched expert opinions. We analyzed concordance coefficient with Kendall's coefficient of concordance W. 3. Results : 1) Experts agreed with 167(82.5%) diagnosis results of cases, but there were 33 cases(17.5%), with which even one expert disagreed 2) The concordance of priority order of Sasang constitutional factors was significantly associated within expert. 3) Experts put the priority concordance to drug response and Face, body shape, voice and Pathological and physiological symptoms in order. 4) In case by case, concordance coefficient was 47(55.9%) in group 1, 9(10.8%) in group 2, 56 (33.5%) in total. 4. Conclusions : Experts made much account of drug responses and faces in diagnosing Sasang constitution. but there were different opinions in concordance coefficient. So Sasang Constitution Diagnosis Guideline is necessary to integrate experts opinions.

Pathological Mechanistic Study of Conducting Fire Back to Its Origin (인화귀원(引火歸原)의 병기론 연구)

  • The fire not to back to its origin(火不歸原) is said that source yang(元陽) of sea of qi(氣海) rises because fire(火) of lower energizer(下焦) can't return to its origin. Successive medical men regarded the cause of it as yang deficiency(陽虛) or yin deficiency(陰虛) generally, but Jangseoksun(張錫純) presented eight kinds of cause, they are syndrome of upcast yang(戴陽證), deficiency of qi(氣虛), yin deficiency, yin and yang deficiency(陰陽虛), thoroughfare qi ascending counterflow(衝氣上衝), heart fire(心火), yang deficiency with cold fluid retention(寒飮) in middle energizer(中焦寒飮), yang deficiency with sunken cold locked in(沈寒錮冷). The method of conducting fire back to its origin may be the treatment of fire not to back to its origin as an interpretation of the phrase in a broad sense, but it is limited to yang deficiency with sunken cold locked in besides syndrome of upcast yang as the treatment based on pathological conditions. By this standpoint Eunsuryong(殷壽龍) used conducting fire back to its origin to remove hidden cold(伏寒) and make rising false fire(假火) settle. The meaning of conducting fire back to its origin is not just raise yang qi(陽氣) but break sunken cold locked in by using the drugs like Buja(附子), Yukgye(肉桂). Jakyak(芍藥) can concentrate yang qi on the life gate(命門) by converging it, Sukjihwang(熟地黃) can supply yin essence(陰精) and check the intense nature of tonifing yang(補陽) drugs. So if we want to use the method of conducting fire back to its origin, we should confirm the symptoms of sunken cold locked in and yang deficiency not to misdiagnose yin deficiency.

  • Currently, hospitals and medical practices have a large amount of unstructured information, gathered in time at each ward or practice by physicians in a wide range of medical branches. The data requires processing in order to be able to extract relevant information, which can be used to improve the medical system. It is useful for a physician to have access to a patient's entire medical history when he or she is in an emergency situation, as relevant information can be found about the patient's problems such as: allergies to various medications, personal history, or hereditary collateral conditions etc. If the information exists in a structured form, the detection of diseases based on specific symptoms is much easier, faster and with a higher degree of accuracy. Thus, physicians may investigate certain pathological profiles and conduct cohort clinical trials, including comparing the profile of a particular patient with other similar profiles that already have a confirmed diagnosis. Involving information technology in this field will change so the time which the physicians should spend in front of the computer into a much more beneficial one, providing them with the possibility for more interaction with the patient while listening to the patient's needs. The expert system, described in the paper, is an application for medical diagnostic of the most frequently met conditions, based on logical programming and on the theory of probabilities. The system rationale is a search item in the field basic knowledge on the condition. The web application described in the paper is implemented for the ward of pathological anatomy of a hospital in Romania. It aims to ease the healthcare staff's work, to create a connection of communication at one click between the necessary wards and to reduce the time lost with bureaucratic proceedings. The software (made in PHP programming language, by writing directly in the source code) is developed in order to ease the healthcare staff's activity, being created in a simpler and as elegant way as possible.
