재활운동 프로그램이 뇌졸중 환자의 기능상태 회복에 미치는 효과 (Effects of Rehabilitation Program on Functional Recovery in Stroke Patients)

  • Stroke is a major cause of death and long-term disability. Because muscle weakness is one of the most prominent consequences of stroke, it was considered important to determine whether exercise in order to improve muscle strength and range of motion could have an effect in limiting the learned disuse of the affected side. The purpose of the study was to identify the effects of an 8 week rehabilitation program on physical and cognitive ability in stroke patients. A total of 18 patients who were admitted to the oriental medicine unit of a K medical center in Seoul were recruited : ten for the experimental group and eight for the control group. The rehabilitation program consisted of three level's of active and passive exercises for prevention of muscle contracture and at range of motion. Muscle strength, flexibility of the upper and lower extremity, perceived balance, functional independence, depression, and quality of life for the two groups were compared at the pretest and 4 and 8 weeks after the rehabilitation program. The results are as follows : 1) When measuring muscle strengths of shoulder abduction and elbow flexion, hip flexion and knee extensor, ankle dorsi-flexor and muscle strength of knee flexor. Muscle strength of knee flexor for the experimental group was significantly higher than the comparison group at the 4 weeks. 2) Muscle strength and flexibility of the ankle dorsi -flexor for the experimental group was significantly better than for the control group at 8 weeks. 3) Functional independence, perceived balance, and Tinetti balance for the experimental group as measured at 4 and 8 weeks were better than for the control group. Also, there were changes over time in physical balance and functional ability, but there was no significant differences between the groups. 4) The experimental group showed a higher quality of life and lower depression than the control group at 8 weeks. 5) Muscle strength and flexibility of ankle dorsi -flexor were significantly changed over time and an interaction between group and time. The findings suggested that the rehabilitation program would improve the physical and psychological status of the stroke patients. Thus, the gains in actual or perceived ability to perform physical activities was marked.

에비안스-함베르크 스트레칭이 뒤넙다리근 단축이 있는 성인의 근긴장도 및 유연성에 미치는 영향 (The Effectiveness of Evjenth-Hamberg Stretching with regards to Muscle Tone and Flexibility in Adults with Hamstring Tightness)

  • PURPOSE: In adults with hamstring tightness, stretching is considered a therapeutic exercise that improves flexibility. However, previous studies have reported efficacy of numerous methods, and are inconclusive in determining the most effective stretching techniques. This study aims to compare the outcomes of Evjenth-Hamberg stretching (E-HS), proprioceptive neuromuscular facilitation stretching (PNFS), and static stretching (SS), on muscle tone and flexibility. METHODS: A total of 30 subjects were assigned to each of the three stretching groups:; E-HS (n1 = 10), PNFS (n2 = 10), and SS (n3 = 10). Muscle tone of the hamstring muscle, active knee extension (AKE), and passive knee extension (PKE) were assessed by range of motion. RESULTS: After the intervention, statistically significant differences were obtained between groups for muscle tone (p < .05). The post hoc test, showed statistically significant differences in muscle tone between the E-HS and PNFS groups (p < .05), and PNFS and SS groups (p < .05). Post hoc test after intervention, also revealed statistically significant differences in flexibility (p < .05) between the E-HS and PNFS groups (p < .05), E-HS and SS groups (p < .05), and PNFS and SS groups (p < .05). Moreover, a statistically significant difference was observed in PKE scores between the E-HS and SS groups (p < .05), and PNFS and SS groups (p < .05). CONCLUSION: This study indicates that E-HS may be the most effective stretching technique for muscle tone and flexibility, in adults with hamstring tightness.

Effect of Game-Based Balance Training with CIMT on Pain, Muscle Strength, Range of Motion and Dynamic Balance in Female Patients with Total Knee Replacement

  • Purpose: The aims of this study were to determine if game-based training with constraint-induced movement therapy (CIMT) is effective in improving the balance ability in female patients with a total knee replacement, and to provide clinical knowledge of CIMT game-based training that allows the application of total knee replacement. Methods: Thirty-six patients who had undergone a total knee replacement were assigned randomly to CIMT game training (n=12), general game training (n=12), and self-exercise (n=12) groups. All interventions were conducted 3 times a week for 4 weeks. All patients used a continuous passive motion machine 5 times a week and 2 times a day for 4 weeks. The visual analog scale (VAS), muscle strength of knee flexion and extension, and range of motion (ROM) of knee flexion and extension were assessed, and the functional reach test (FRT), and timed up and go (TUG) test were performed to evaluate the balance ability. Results: All 3 groups showed significant improvement in the VAS, knee flexion and extension muscle strength, FRT, and TUG test after the intervention (p<0.05). Post hoc analysis revealed significant differences in FRT, and TUG of the CIMT game training group compared to the other group (p<0.05). Conclusion: Although the general game training and CIMT game training improved both the knee extension muscle strength and dynamic balance ability, CIMT game training had a larger effect on dynamic balance control.

Wii 균형 게임과 자가 거울 균형 운동이 만성 뇌졸중 환자의 균형 및 무릎관절 고유수용성 감각에 미치는 효과 비교 (Comparison of the Effects of Wii Balance Games and Mirror Self-Balancing Exercises on Knee Joint Proprioception and Balance in Chronic Stroke Patients)

  • Background: Many studies have reported positive results of the various mirror training and virtual reality games in improving dynamic standing balance and posture adjustment in chronic stroke patients. However, no systematic study has been conducted to compare the effects of virtual reality games and the mirror balance training. Objects: The purpose of this study was to compare the effectiveness of Wii balance games and Mirror Self-Balancing Exercises in improving proprioception of knee joint and standing balance of people with chronic stroke. Methods: Twenty patients with chronic stroke volunteered for this study. The subjects were randomly divided into a Wii balance games group and a Mirror Self-Balancing Exercises group with 10 patients in each group. Each training was performed for 30 mins a day for 4 days. In addition to the balance training, 30 mins neuro-developmental-treatment based routine physical therapy was given to both groups. Proprioception was measured using two continuous passive motion devices, and static balance was measured using a Wii balance board. Dynamic balance assessment tools included the Berg Balance Scale, Dynamic Gait Index, and Timed Up-and-Go test. Results: All measured variables before and after the experimental results showed a significant improvement in both groups (p<.05). Only the improvement of the affected knee proprioception appeared to be significantly greater in the Wii balance game group (p<.05). However, other variables did not differ between the groups (p>.05). Conclusion: The findings suggest that both Wii balance games and Mirror Self-Balancing Exercises may be helpful for improving the proprioception of knee joint and the balance of patients with chronic stroke.

급성 횡단성 척수염으로 진단된 비증 환자의 치험 1례 (Clinical Study on 1 Case of Patient with Arthalgia Syndrome Diagnosed as Acute Transverse Myelitis)

  • Acute transverse myelitis(ATM) is defined as an acute intramedullary dysfunction of the spinal cord, ascendng or static involving both halves of the cord and appearing without any history of previous neurological diseases due to traumatic accident, tumor of all kind, encephalitis and of course excluding all possible viral, bacterial and fungal infection. It is mainly characterized by acute motor disorder of both limbs in respect to which spinal segments are affected as well as sensory disorder and dysuria & dyschezia. The exact cause is unknown, however it is recently suggested that immunological factors are highly involved. It has been reported by several reliable sources that it is often accompanied by immunological diseases such as systemic lupus erythematosus(SLE). As treatments non steroid anti-inflammatory drugs(NSAIDS) are primarily recommended as to steroids, limited doses are injected only with the proper prescription from the physician. Operative methods are not options as traumatic accidents and tumors are excluded as factors. To enhance muscle strength and prevent articular contracture physical therapy and passive exercise is imperative. The following patient whose chief complaints were mainly about hypoesthesia of Rt. lower limb and stiffness of phalanges of both fingers as well as to weakness of lower extermity. Therefore it has been diagnosed as arthalgia syndrome. In oriental medicine factors such as wind evil heat-evil, dampness-heat evil, cold evil cause the arthalgia syndrome. In this case the patient was diagnosed as dampness-heat evil and herbal medicine Chunglijagam-Tang and Dong-Qi acupuncture was applied to treat bladder disorder.

  • This study was undertaken to develop a bed-type cycling system of lower limbs for rehabilitation. This system consists of two modes of cycling: active and passive. Different velocity and loads are provided for improving the muscle function recovery and increasing the muscular strength. To analyze the muscle activity pattern, we measured muscular activity of lower limbs in the rectus femoris (RF), biceps femoris (BF), tibialis anterior (TA), medial gastrocnemius (MG), and soleus (SOL), while cycling in the supine position, and based on the pedaling direction. A total of 18 young and 23 elderly, healthy subjects participated in this study. Muscle activity of MG muscles was significantly different in the two age groups. This study could provide the reference data to develop cycling exercises for lower limbs during rehabilitation of the elderly patients.

PNF 팔 패턴의 각도 변화가 척추세움근의 근 긴장도 변화와 방산의 효과에 미치는 영향 (The Effect of Elector Spine Muscle Tone Using Irradiation According to the Angular Motion of a Proprioceptive Neuromuscular Facilitation (PNF) Arm Pattern)

  • Purpose: This study aimed to compare the elector spine muscle tone using the irradiation of the proprioceptive neuromuscular facilitation (PNF) arm pattern according to angular motion. Methods: Thirty subjects participated in this study. Elector spine muscle tone was measured using a Myotonpro device while in the sitting position according to the angular motion (70°, 100°, 130°) of the PNF arm pattern using a D1 flexion pattern. Each angular motion of the PNF arm pattern was performed with a continuous passive motion (CPM). The change in elector spine muscle tone was statistically evaluated using a repeated one-way ANOVA test. Post-hoc analysis was performed using the Bonferroni method. Results: The results revealed a significant change in elector spine muscle tone when performing the PNF arm pattern using D1 flexion pattern. Specifically, the elector spine muscle tone had significantly increased at 100° and 130° motion in the PNF arm pattern when compared to the initial muscle tone (p < 0.05). No significant muscle tone changes were noted for any of the angular motions of the PNF upper arm pattern (p > 0.05). Conclusion: The results of this study indicate a positive increase in elector spine muscle tone with irradiation of the PNF upper arm pattern exercise with 100° or 130° angular motion. The minimum angle at which the effect of the irradiation of the PNF arm pattern could be seen was 100°.

One Year Experience of the Hand Allotransplantation First Performed after Korea Organ Transplantation Act (KOTA) Amendment

  • The revision of the Korea Organ Transplantation Act (KOTA) in 2018 included hand/arm among the organs that can be transplanted. The first hand transplantation since the revision of KOTA took place in January 2021. A 62-year-old male patient experienced hand amputation on July 13, 2018, by a catapult injury. The patient first visited our institute 3 months after the injury. After serial interviews and an overall evaluation, the patient was registered on the hand transplantation waiting list in January 2020. On January 9, 2021, the patient underwent hand transplantation at the right distal forearm level. The total operation time was 17 hours 15 minutes, and the cold ischemic time was 4 hours 9 minutes. Postoperative immunosuppression was administered based on the protocol used for kidney transplantation. Two acute rejection episodes occurred, on postoperative days 33 and 41. Both rejection episodes were reversible with rescue therapy of a higher tacrolimus trough level, steroid pulse therapy, and topical immunosuppressants. Controlled passive range of motion exercise was started on postoperative day 10. Dynamic splint was applied on postoperative day 18. At 1 year, graft maintenance and functional improvement were satisfactory, and the patient showed a Disabilities of Arm, Shoulder and Hand score of 25.8. We successfully performed the first hand transplantation surgery under the KOTA amendment. It came from the organic and effective cooperation of plastic, orthopaedic, and transplantation departments and we believe it will guarantee the future ongoing success.

Variability in physical therapy protocols following total shoulder arthroplasty

  • Background: Physical therapy (PT) plays an important role in the recovery of function following anatomic total shoulder arthroplasty (aTSA). While several PT protocols have been published for these patients, there is no standardized protocol for aTSA rehabilitation. This lack of standardization may lead to confusion between patients and physicians, possibly resulting in suboptimal outcomes. This study examines how PT protocols provided by academic orthopedic surgery programs vary regarding therapeutic goals and activities following aTSA. Methods: PT protocols for aTSA available online from the Accreditation Council for Graduate Medical Education accredited orthopedic surgery programs were included for review. Each protocol was analyzed to evaluate it for differences in recommendation of length of immobilization, range of motion (ROM) goals, start time for and progression of therapeutic exercises, and timing for return to functional activity. Results: Of 175 accredited programs, 25 (14.2%) had protocols publicly available, programs (92%) recommended sling immobilization outside of therapy for an average of 4.4±2.0 weeks. Most protocols gave recommendations on starting active forward flexion (24 protocols, range 1-7 weeks), external rotation (22 protocols, range 1-7 weeks), and internal rotation (18 protocols, range 4-7 weeks). Full passive ROM was recommended at 10.8±5.7 weeks, and active ROM was 13.3±3.9 weeks, on average. ROM goals were inconsistent among protocols, with significant variations in recommended ROM and resistance exercise start times. Only 13 protocols (52%) gave recommendations on resuming recreational activities (mean, 17.4±4.4 weeks). Conclusions: Publicly available PT protocols for aTSA rehabilitation are highly variable. Level of evidence: IV.

경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술의 임상적 유용성 (The Clinical Usefulness of the Minimal Invasive Ulno-humeral Arthroplasty in the Patients with Mild to Moderate Elbow Arthritis)

  • 목적: 경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술의 임상적 유용성에 대하여 알아보고자 한다. 대상 및 방법: 2000년 1월에서 2008년 12월까지 척골 신경 증상이 없는 경도 및 중등도의 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술을 시행받은 환자 29예 중 1년 이상 추시가 가능하였던 24예를 대상으로 하였다. 남자 20예, 여자 4예였으며, 평균 연령은 53세 (31~69세)였고, 평균 수술 시간, 관절 운동 범위, 술 후 관절 운동 시작까지의 시간, 술 후 1년 추시에서 Mayo elbow performance score (MEPS)를 조사하였다. 결과: 술 후 평균 1.8일 (1~4일)째 수동적 및 능동적 관절 운동을 시작하였고, 평균 수술 시간은 38분 (25~55분)이었다. 관절 운동 범위는 술 전 평균 25~104도 (신전 0~70도, 굴곡 80~130도)에서 술 후 1년 추시 상 평균 14~133도 (신전 0~45도, 굴곡 90~150도)로 평균 40도 관절운동 범위가 향상되었으며, 술 후 관절 운동 시작까지의 시간은 평균 1.6일 (1~5일)이었다. 술후 1년 추시에서 MEPS는 우수 19예, 양호 5예이었다. 술 후 합병증으로 창상 치유지연 1예, 술후 부종 7예가 있었으나 자연적으로 호전되었다. 결론: 경도 및 중등도 주관절 관절염 환자에서 최소 침습적 척골-상완 관절 성형술은 수술 시간이 짧고 조기 관절 운동이 가능하며 통증이 적으므로 임상적으로 유용한 수술이다.
