Aprotinin, a serine protease inhibitor, has been used to ameliorate the inevitable consequences, including blood component injury after cardiac surgery with cardiopulmonary bypass (CPB). However, there are many arguments on its dosage or usage. We assessed whether administration of low dose of aprotinin in only priming solution has any beneficial effect or reduces its side effects on cardiac surgery. Thirty patients scheduled for elective cardiac surgery were randomly assigned to aprotinin group (n=15) which received aprotinin in priming solution (two million kallikrein inhibitory unit, KIU) and added one million KIU at 1 hour after the beginning of CPB or control group (n=15) which did not receive it. Hematological and biochemical variables, cytokines and cardiac marker levels, and postoperative outcomes were compared between two groups at before, during or after operation. Platelet count in aprotinin group was higher than that of control group at postoperative 24 hr. Activated partial thromboplastin time in aprotinin group was longer than that of control group at intensive care unit (ICU). Troponin-I level and postoperative blood loss volumes in aprotinin group were lower than those of control group at ICU. There were no significant differences between the two groups on the others. These results showed that low dosage of only priming solution during cardiac surgery with CPB reduced platelet destruction and postoperative bleeding, and attenuates myocardial damage. However, further studies need to be carried out with more population or pediatric patients for evaluating various aprotinin usage.
To search of more selective vasculogenic relaxation activity, the antioxidant activity of silkworm male pupae was determined by measuring its radical scavenging effect on 1, 1-diphenyl-2-picrylhydrazyl (DPPH) radicals, and anticoagulant activity of them was measured clotting time in both activated partial thromboplastin time (aPTT). Because, most of cGMP-enhancing agent such as, sildenafil, promotes thrombin-induced platelet aggregation, developed unexplained thrombic conditions including heart attack. To search more suitable and safe drug for vasculogenic relaxation, we purified silkworm pupae male extract. The ethyl acetate extract of silkworm male pupae showed strong scavenging activity in both DPPH and aPTT anticoagulant activity. The antioxidant activity potential of the individual fraction was in order of ethyl acetate > n-butanol > chloroform > n-hexane. The ethyl acetate soluble fraction exhibiting strong anti-oxidant and anticoagulant activity was further purified by repeated silica gel and Sephadex LH-20 column chromatography. Cholesterol was isolated as one of the active principles from ethyl acetate fraction, together with, minor portion, ${\beta}-sitosterol$.
This study was undertaken to know the effect of fat diet (for eight weeks) on changes of inflammatory markers [tumor necrosis factor (TNF-${\alpha}$) and prostaglandin $E_2$ ($PGE_2$)] and blood coagulation system [platelet aggregation function (PAF), prothrombin time (PT), activated partial thromboplastin time (aPTT)] in rats. Serum TNF-${\alpha}$, $PGE_2$, biochemical markers, PAF, PT, aPTT, and body weight were measured and compared between the control (normal diet-rats) and the fat group (fat diet-rats). The weights in the fat group were higher than those of the control group. TNF-${\alpha}$, $PGE_2$, glucose, aspartate aminotransferase (AST), alanine aminotransferase (ALT), and creatinine levels were greater in the fat group compared with the control group. The degree of platelet aggregation was lower, whereas PT and aPTT levels were longer in the fat group than in the control group. These findings have shown that fat diet may cause inflammatory response, diabetes, liver and renal dysfunction, and disturbances of fibrinolysis and coagulation system.
Effects on thrombin by an amphipathic cation, dequalinium, which has been recognized as an anticarcinoma agent were investigated with small chromogenic substrates such as Na-benzoyl-DL-argininep-nitroanilide (BApNA), H-D-phenylalanyl-L-pipecoyl-L-arginine-p-nitroanilide (S-2238), and Na-p-tosyl-L-arginine methyl ester (TAME). Among them, only TAME hydrolysis due to an esterase activity of the enzyme was significantly activated to 81% at 20 ${\mu}M$ dequalinium in the absence of NaCl. This stimulation became even higher in the presence of 0.2 M NaCl to 3.5-fold at 60 ${\mu}M$ dequalinium. This specific activation of thrombin was well correlated with the results of in vitro coagulation tests measuring the activated partial thromboplastin time (APTT) and the prothrombin time (PT) It is pertinent. therefore, to suggest that the esterase activity should be examined in addition to the effects on 5-2238 hydrolysis when especially any regulators not directed to an active site of thrombin need to be studied. We also expect that dequalinium could be a useful tool for studying structure-function relationship of thrombin and blood coagulation.
Cordyceps is a genus of ascomycete fungi and is well known as one of the important medical fungi in Chinese, Korea, and other Asian countries, because of its various beneficial effects on human health. The pharmacological activities of Cordyceps extract are mainly focused on anti-cancer, anti-metastatic, and immune modulating effects. In the present study, we investigated whether the antiplatelet effect of ethanol extract of cultured Cordyceps militaris (CMEE) with FeCl3-induced arterial thrombosis model. We observed that CMEE exhibited a significant inhibitory effect against ADP and collagen-induced platelet aggregation. However, there were no significant differences in prothrombin time (PT) and activated partial thromboplastin time (aPTT). These results suggest that antithrombotic activity of CMEE is related to antiplatelet effect rather than anticoagulation effect, and CMEE may be a positive effect on improving blood circulation against vessel injury and occlusion.
The effect of fivrinolytic enzyme produced from Bacillus subtilis K-54 on the thrombosis and stress in vivo was investigated. Each partially purified fibrinolytic enzyme of 4 protein casein unit was administered orally for 3 days before intravenously injection with collagen and epinephrine. In the mice group administered with the enzyme and increased life span of mice was observed in comparison with that of control. The result suggest that the enzyme may prevent the formation of thrombos in vivo. Administration of the enzyme did not influence to stress itself because 5-hydroxyindoleacetatic acid concentration of brain in the mice group with stress did not decreased after the administration of the enzyme. The value of lipid peroxide (LPO) of the liver and brain cells in the group treted with the enzyme was lower than that of control. However, protein degradation (PDP value showed no significant difference between treatment and control groups. In addition, the value of activated partial thromboplastin time (APTT), protrombin time (PT0 and antiplasmin in blood were higher in the stress group than that of the enzyme treated group.
The present experiments were designed to investigate the effects of BangPoongSan on the cardiovascular system in the experimental Animals. And thus the change of blood pressure, auricular blood flow, artery contraction, death rate, platelet aggregation repression, plasma coagulation factor activity, plasma antithrombin activity, whole blood viscosity and plasma viscosity were studied. The result were summarized as the followings: 1. BangPoongSan dropped the blood pressure in the spontaneous hypertensive rat. 2. The drug increased the auricular blood flow in rabbit. 3. The drug relaxed the artery contraction by pretreated norepinephrine in white rat. 4. The drug inhibited the death rate of mouse which was led to thromboembolism by serotonin and collagen. 5. The drug inhibited the platelet aggregation in rat. 6. The drug prolonged the prothrombin time and activated partial thromboplastin time on the test of plasma coagulation factor activity in rat, but was not valuable. 7. The drug presented the antithrombin activity in rat. 8. The drug reduced the whole blood viscosity and plasma viscosity in rat, but the latter was not valuable. According to the results, Bangpoongsan increased the blood flow and dropped the blood pressure by dilatation of blood vessel smooth muscle. And the drug presented the antithrombin acivity, inhibited the platelet aggregation and reduced blood viscosity. Therefore these effects are assumed to improve the cardiovascular circulation disorder and prevent thrombosis.
Elsholtzia splendens, which grows on moist soil of mountainous regions, is widely distributed at all regions of Korea, especially at Mountain Ji ri. It is categorized as a Labiatae plant which is dried aerial part. It has the following medicinal properties; removal of fever, alleviation of pain, a good antiphlogistic agent as well as antibacterial effects. However, the effects of E. splendens on thrombosis and platelet activation are not precisely understood. We performed this study to develop antithrombotic agents from oriental medicine herb extracts. E. splendens inhibited platelet aggregation induced by arachidonic acid and U46619 in a concentration dependent manner. E. splendens did not show an effect on anticoagulation as determined by prothrombin time (PT) or activated partial thromboplastin time (aPTT). We also tested the effects of E. splendens using a carotid artery thrombosis rat model induced by 35% $FeCl_3$ treatment. E. splendens significantly inhibited thrombus weight compared with the control group. These results show that E. splendens may be developed as a potential antiplatelet activity agent for treatment of cardiocerebrovascular disease and atherosclerosis.

Lindera obtusiloba has been used in traditional herbal medicine for the treatment of blood stasis and inflammation. The leaves of Lindera obtusiloba have been reported to exhibit various physiological activities. However, there is little information available on their antiplatelet and antithrombotic activities. Thus, the present study aimed to evaluate the effect of Lindera obtusiloba leaf extract (LLE) on platelet activities, coagulation and thromboembolism. In a platelet aggregation study, LLE significantly inhibited various agonist-induced platelet aggregations in vitro and ex vivo. Furthermore, LLE significantly inhibited collagen-induced thromboxane A2 (TXA2) production in rat platelets. In addition, oral administration of LLE was protective in a mouse model of pulmonary thromboembolism induced by intravenous injection of a mixture of collagen and epinephrine. Interestingly, LLE did not significantly alter prothrombin time (PT) and activated partial thromboplastin time (aPTT). This study indicates that the antithrombotic effects of LLE might be due to its antiplatelet activities rather than anticoagulation. Taken together, these results suggest that LLE may be a candidate preventive and therapeutic agent in cardiovascular diseases associated with platelet hyperactivity.
Objective : Meningioma is a benign tumor which has a high occurrence rate of postoperative hematomas. The purpose of this study is to analyze risk factors for postoperative hemorrhages after meningioma surgery. Methods : One hundred and fifty three patients with intracranial meningiomas, operated at the Department of Neurosurgery, National Medical Center, between January 1995 and December 2003 were included in this retrospective study. Risk factors considered to be related with postoperative hematomas were age, sex, preoperative pharmacological anticoagulants for medical co-morbidity, tumor location, histological type of the meningioma, infiltration of dural sinus and arachnoid, removal range of tumors, and the perioperative coagulation status including prothrombin time, partial thromboplastin time, and platelet count. Results : Patients' aged more than 70 years with a platelet count of less than $150{\times}10^9{\ell}^{-1}$ after surgery had statistically significant relations to the occurrence rate of postoperative hematomas. The other factors had no statistical significance. Conclusion : Various and intensive preoperative examinations for coagulation factors of patients, especially of older age, and proper transfusion before meningioma surgery are necessary for preventing postoperative hematoma.
