The Study on Patterns of Prosthetic Restoration, by Age and Sex - Centering the medium and small city around Chollabuk-do - (성별과 연령에 따른 치과보철물 양상에 대한 조사연구 - 전자북도의 중소도시를 중심으로-)

  • The purpose of this study were to investigate the patterns of prosthetic restorations according to sex and age. We had surveyed the out-patents of dental clinics it Jeon-Ju, Iri and Gun roan area. The abtained result were as follow ; 1. The patterns of prosthetic restoration in female was higher than male and 20$\sim$30 age groups was higher than others. 2. The crown was higher in prosthetic restoration. 3. The procelain appliance area was high in male and female on 20$\sim$29 age. 4. The partial denture was higher than full denture in both sex. 5. The crown appliance area was not different in both sex. 6. The 3 unit bridge was higher than other bridge in both sex. 7. The porcelain appliance area was high in male and female on upper anterior. 8. The inlay appliance area of lower posterior was higher than upper posterior. 9. The upper posterior was high in male, and the lower posterior was high in female in partial denture. 10. The full dentur was high in old age group.

  • The black triangle is a common clinical finding in aged people with gingival recessions. Among other prosthetic treatment molalities such as composite resin filling and laminate venners, the all ceramic restoration procedure can lead to most successful result. With improved bonding strength and ceramic properties, the tooth preparation design for all-ceramic crown can be modified to minimize the reduction of sound tooth structure without loosing properties of conventional preparation design. Case selection is an important factor in acheiving succesful prosthesis. In this case report, the leucite reinforced pressable ceramic, $Authentic^{TM}$ [Ceramay, Germany] was used to fabricate the prosthesis.

Development of a Smooth Colon Surface Restoration Method for Electronic Colon Cleansing (전자적 장세척을 위한 부드러운 장표면 복원 방법 개발)

  • Virtual colonoscopy is favored over conventional colonoscopy because its non-invasive procedure can avoid complications that may happen in a conventional approach and because it can cleanse colon electronically instead of uncomfortable conventional colon cleansing. Electronic Colon Cleansing(ECC) has to deal with not only removing tagged fecal material but also recovering Partial Volume Effect(PVE) due to tagging material. This paper proposes an ECC method restoring inherent natural PVE while previous approaches focused only on reducing PVE due to tagged fecal material. The proposed method reduces PVE using 3-dimensional adaptive density correction and then replaces tagged fecal material into air. Next, it generates natural PVE for the replaced air adjacent to soft tissue and finally makes smooth transition of gray values for soft tissue adjacent to the replaced air. The proposed method applied to eleven patient data, and showed promising results.

  • A protected area means a space designated and protected by law from development pressure and environmental pressure. It is mainly designated to protect specific ecosystems, natural landscapes, and cultural resources from irrational development (or damage), and involves policies of the public sector such as central and local governments. The United Nations Educational, Scientific and Cultural Organization (UNESCO) has conducted conservation and restoration projects for preserving natural ecosystems and genetic resources. In order to conserve the ecosystem in the protected area, national and public organizations purchase private land and use it ecologically; in addition, ecological restoration project is carried out for the purpose of creating waterside ecological belt or preserving ecosystem. Land acquisition refers to the land where highly influenced by the water quality and need to restore, and purchased by negotiating with the landlord. Although the nation and public institution carried out ecosystem restoration project for partial purchase land in order to conserve ecosystem, it is below the expected effect due to lack of comprehensive management system and have some problems in restoration project and unification of management institutions. Land acquisition in Donggang River Basin Ecological Conservation area is initiated in 2005 for creating income of local residents and ecological restoration. However, the lack of overall management and awareness resulted in poor vegetation growth and poor response by local residents due to terrain exposure. As such, there is insufficient research on the current situation and systematic integrated management although the number of land acquisition is increasing year after year. Futhermore, overall recognition and follow-up monitoring of eco-restoration are still inadequate. Therefore, the survey on the awareness of the purchase land ecosystem restoration project is necessary for the efficient restoration project and establishment of the management strategy for land acquisition in the future. Therefore, in this study, we provide fundamental materials on further research projects by carrying out research on the awareness of ecological restoration projects in the Donggang River basin ecological preservation area.

A METHOD OF CAST CROWN RESTORATION FOR EXISTING PARTIAL DENTURE CLASP AND A CASE REPORT OF IT (기존 Clasp에 적합한 주조 금관 형성법과 임상응용 1 예)

  • Sometimes in clinic, we have found that cast crown construction is necessary to fit the existing partial denture clasp, when the abutment is involved with caries, a failing restoration, periodontal disease, or fracture. This is important not only to protect the abutment tooth from missing, but to restore retention, bracing and support for partial denture. For constructing the cast crown, several methods have been reported by many of previous authors. The number of techniques reported during the past are ideal but have uncertain results However, with more improved and practical technic developed by me, the problem becomes more successful. One method in which does not need the laboratory technecian is the wax pattern has been made in chairside waxing by doctor himself. The other in which requires the laboratory technician is to use special impression method and articulator for laboratory waxing. In one case of the reported here, we have successful results in restoring the abutment with cast crown for original denture clasp.

  • Prosthodontic treatment planning for various edentulous arches is not an easy issue for dentists. Especially, in case of removable partial denture, we must have a knowledge of overall treatment procedures, and a careful approach is needed. Recently, interest of dental implant and case reports are increasing, the decrease of the removable partial denture is true, but dental implantation takes longer treatment period and it is more expensive. Also, there are still some limitations like lack of available alveolar bone, patient's general condition, and chronic periodontitis. Therefore, sometimes implantation is impossible. Finally, implantation cannot be adapted to every single patient. Currently, the clasp type removable partial dentures are used routinely, giving patients many unesthetic and functional difficulties. With better laboratory technique, removable partial denture with attachment and removable partial denture in rigid type increase patient's happiness level much more than predicted. The case presented in this article, clinically demonstrate the efficiency of using a telescopic system to improve esthetic and functional recovery for patients who lose multiple teeth.

  • With the rapid development of aesthetic ceramic materials and resin adhesives, the transition from metal ceramic crown to all-ceramic restoration is being actively carried out. When making porcelain restorations, the advantages can be maximized only when appropriate forms and techniques are used. Recently, an esthetic partial-coverage restoration method with supra-gingival margins was introduced by applying a minimally invasive tooth removal method that preserves enamel as much as possible and enhances adhesion efficiency. Through this, various advantages such as minimization of tooth structure removal and ease of excess cement removal can be obtained. In addition, since this manufacturing method does not require cord packing, it is possible to save time for impression taking and reduce patient discomfort. Furthermore, the margins can be clearly obtained without the intervention of gingival fluid or blood, making it possible to manufacture more accurate restorations. In the following cases of patients with partial tooth defects, esthetic ceramic partial-coverage restorations with supra-gingival margin was applied using minimally invasive tooth preparation method according to the concept of adhesive dentistry. Therefore, unnecessary tooth abutment preparation was prevented and residual tooth structure could be preserved as much as possible. After delivery of the ceramic partial-coverage restorations, favorable outcome was obtained.

PHOTOELASTIC STRESS ANALYSIS OF IMPLANT SUPPORTED FIXED PROSTHESES WITH DIFFERENT PLACEMENT CONFIGURATIONS IN MANDIBULAR POSTERIOR REGION (하악 구치부에서 임플랜트 배열방식에 따른 임플랜트지지 고정성 국소의치의 광탄성 응력 분석)

  • Statement of problem. More than 70% of patients who need the implant supported restoration are parially edentulous. The principles of design for implant supported fixed partial denture in mandibular posterior region are many and varied. Jurisdiction for their use is usually based on clinical evaluation. There are several areas or interest regarding the design of implant supported fixed partial denture in mandibular posterior region. 1) Straight and tripod configuration in implant placement, 2) Two restoration types such as individualized and splinted restorations. Purpose. The purpose of this study was to compare the amount and distribution of stress around the implant fixtures placed in the mandibular posterior region with two different arrangements and to evaluate the effects of splinting using the photoelastic stress analysis. Material & methods. 1) Production of study model: Mandibular partially edentulous model was waxed-up and duplicated with silicone and two models were poured in stone. 2) Fixture installation and photoelastic model construction: Using surveyor(Ney, USh), 3 fixtures(two 4.0 $\times$13 mm, one 5.0$\times$10 mm, Lifecore, USA) were insta)led in straight & tripod configurations. Silicone molds were made and poured in photoelastic resin (PL-2. Measurements group, USA). 3) Prostheses construction: Four 3-unit bridges (Type III gold alloy, Dongmyung co., Korea) were produced with nonhexed and hexed UCLA abutments and fitted with conventional methods. The abutments were tightened with 30 Ncm torque and the static loads were applied at 12 points of the occlusal surface. 4) Photoelastic stress analysis : The polarizer analyzer system with digital camera(S-2 Pro, Fujifilm, Japan) was used to take the photoelastic fringes and analysed using computer analysis program. Results. Solitary hexed UCLA restoration developed different stress patterns between two implant arrangement configurations, but there were no stress transfer to adjacent implants from the loaded implant in both configurations. However splinted restorations showed lesser amount of stresses in the loaded implants and showed stress transfer to adjacent implants in both configurations. Solitary hexed UCLA restoration with tripod configuration developed higher stresses in anterior and middle implants under loading than implants with straight configurations. Splintied 3 unit fixed partial dentures with tripod configuration showed higher stress development in posterior implant under loading but there were no obvious differences between two configurations. Conclusions. The tripod configuration of implant arrangement didn't show any advantages over the straight configuration. Splinting of 3 unit bridges with nonhexed UCLA abutments showed less stress development around the fixtures. Solitary hexed UCLA restoration developed tilting of implant fixture under offset loads.

Arthroscopic Partial Repair of Massive Contracted Rotator Cuff Tears

  • Typically, massive rotator cuff tears have stiff and retracted tendon with poor muscle quality, in such cases orthopaedic surgeons are confronted with big challenging to restore the cuff to its native footprint. Furthermore, even with some restoration of the footprint, it is related with a high re-tear rate due to less tension free repair and less tendon coverage. In this tough circumstance, the partial repair has yielded satisfactory outcomes at relatively short follow-up by re-creating the transverse force couple of the rotator cuff. Through this partial repair, the massive rotator cuff tear can be converted to the "functional rotator cuff tear" and provide improvement in pain and functional outcomes in patient's shoulder.

Effect of Zusanli (ST36) Electroacupuncture Stimulation on Delayed Gastric Emptying in Rats with Partial Pyloric Obstruction (족삼이혈(足三里穴) 전침 자극이 유문부 부분폐색이 된 흰 주의 위(胃) 배출능에 미치는 효능)

  • Objective : The aims of this study were to observe how body weight and gastric morphology were changed and whether gastric emptying was impaired in rats with partial pyloric obstruction. and to evaluate whether electroacupuncture was able to restore delayed gastric emptying. Methods : Partial pyloric obstruction was induced by wrapping a nonabsorbable rubber ring around the 1st portion of the duodenum for 2 weeks. Body gain and morphologic changes of stomach were investigated and compared with normal intact rats. Gastric emptying was measured by numbering expelled glass of beads in rats. Rats were divided into 4 groups(non-acupuncture, manual acupuncture. 3Hz-electroacupuncture. 60Hz-electroacupuncture). Stimulus intensity in two electroacupuncture groups was 1.2 times of pain threshold. Results : Partial pyloric obstruction produced a significant loss of body weight and induced a significant increase of gastric surface area. The 60Hz electroacupuncture-stimulated group significantly restored the delayed gastric emptying compared to the other groups of rats with partial pyloric obstruction. Conclusion : 60Hz electroacupuncture stimulation on Zusanli(ST36) showed significant restoration of delayed gastric emptying in rats with partial pyloric obstruction.
