Liver is an oragan which has various functions. For this functions in liver disease change variously, many methods of examination of them are invented. Each liver disease impairs these functions diversly rather than uniformly, thus it is possible to differentiate each disease by using various methods of testing it. Liver has high reproductive capacity and despite partial impairment of its function localized disease may not show any abnormality in liver function test due to compensation of remaining intact tissue of the liver. Thus the result of liver function test should be interpreted under adequate clinical investigation with other test, not depending on the liver function test only. This thesis deals with the composition, distribution in living body and clinical significance of liberating enzyme, GOT, GPT, and LDH which are included in serum enzyme that show abnormality in liver disease.
Objectives: The purpose of this study is to report the effect of Korean Herbal Medicine (KHM) on a Renal Cell Carcinoma with multiple liver metastases patient. Methods: One renal cell carcinoma with multiple liver metastases patient was treated by KHM in conjunction with targeted anticancer agent (Afinitor). The effect of KHM was measured by scanning with Computed Tomography (CT), Blood Test, Visual Analogue Scale (VAS) and Eastern Cooperative Oncology Group scale. Results: Multiple hepatic tumors were reduced after the treatment during 5 months (Partial Remission, PR). As treatment was performed, complications induced by targeted anticancer agent (Afinitor) were alleviated. Conclusions: This case provides us a possibility that Korean Herbal Medicine offers potential benefits for renal cell carcinoma with multiple liver metastases patient.
Liver transplantation is widely accepted as an effective therapeutic modality for a variety of irreversible acute and chronic liver diseases for which no satisfactory therapy is available. Following the first unsuccessful efforts at human liver transplantation in 1963, development of the procedure evolved at first slowly and steadily for 20 years and then rapidly over the past two decades. The growth of liver transplantation was facilitated by the conclusion of the national institutes of health consensus development conference in 1983 that liver transplantation is not an experimental procedure but an effective therapy that deserves broader application. The number of liver transplantations increased 2.4-fold(from 1.713 to 4.058) from 1988 to 1996. but the number of patients on the UNOS(united network of organ sharing) liver list increased 12.1-fold(from 616 to 7,467); as would be expected, the number of deaths of listed patients increased 4.9-fold(from 195 to 954), The current supply of donor livers is insufficient to meet this need, and organ donation has been stagnant or increased by only a few percent in recent years. These facts underscore the importance of the appropriate selection of candidates for liver transplantation and the development of operative procedures, such as living donor liver transplant, split liver transplant and auxiliary partial liver transplant.
The effects of ginseng saponins, G-Rbl and G-Rc on the rat liver LDH A-gene transcnptional activity was investigated during pro-replicative phase of rat liver after partial hepatectomy. Changes in LDH A-mRNA levels in regenerating rat liver after intraperitoneal administrations of G-Rbl of G-Rc were tested by slot blot hybridization methods. The results showed that G-Rbl (1 mg/100g B.W) and G-Rc (1 ma/100g B.W) caused marked increases of LDH A-mRNA contents by respectively 1.9- and 1.5-fold in rat liver at 5·hours after partial hepatectomy. Dose dependent effect of G-Rbl and G-Rc (1-25 mg/100g B.W) on the LDH A-mRNA levels on regenerating rat liver were also analyzed. The maximal in- creases of liver LDH A-mRNA levels were observed with the doses of 1 mg for G-Rbl and 5 mg for G-Rc However, when the administration doses of G-Kbl and G-Rc were increased to 20 mg, G-Rbl caused a marked decrease of LDH A-mRNA level to 61% of those in sham-operated rat liver In contrast, G-Rc slightly decreased the liver LDH A-mRNA contents by 30% as compared to those of the maximum value but still maintained 22% higher LDH A-mRNA levels then those of sham-operated rate liver. On the basis of these experimental results, we conclude that ginseng saponin, G-Rb 1 and G·Rc have stimulatory effect at the lower concentration (1 mg/100g B.W) and inhibitory effect at the higher concentration (20 moi loos 5.W) on the LDH A-gene transcription during regeneration of rat liver, Additionally we also investigated the stimulatory effects of ginsenosides on the protein and DNA synthetic activities in hepatocyte primary cell cultures isolated from regenerating rat liver. Both of G·Rc and -Re increased the synthetic rates of hepatocytes proteins and DNA at the administration doses of 50 ug and 100 ug/3 ml/dish respectively representing 1.3-1.6 fold increases. From these results we postulate that G-Rc and -Re may have a mitogen enhancer activity for the hepatocyte proliferation during rat liver regeneration period. A 12-year-old spayed, female Siberian Husky dog weighing 40 kg was referred for sudden collapse without a traumatic history. A splenic mass and hemoperitoneum were identified by ultrasonography and abdominocentesis at a local animal hospital. Emergency exploratory laparotomy was performed once the patient was stabilized. A splenic mass and engorged hepatic mass in the torsed left liver lobe were identified. Partial hepatic lobectomy of the left lateral lobe was performed using a laparoscopic linear self-cutting stapler and the spleen was also removed using an ultrasonic scalpel. Histopathologic examination revealed a diagnosis of splenic hemangiosarcoma with metastasis to the liver. Chemotherapy was not performed because the client did not give consent. The patient was checked regularly until death on POD 324. The metastatic nodules had spread to the lungs and other liver lobes and had gradually increased in size. Liver lobe torsion is a rare condition in dogs, especially accompanying neoplasia. This case supports a potential relationship between liver lobe torsion and neoplastic liver mass in dogs.
The present experiment was performed to investigate the acute effects of cadmium on ultrastructural and biochemical changes in mouse kidney and compare these changes with liver damage. Mouse were injected with cadmium chloride at a dose of 5 mg/kg body weight. After treatment, mouse were sacrificed at time intervals of 6, 12, 24 and 48 hours. It was observed that ultrastructural changes in mouse kidney were composed of swelling of mitochondria, dilation in endoplasmic reticulum, wrinkling at basal infolded membrane, formation of autophagosome and partial loss of microvilli in brush. border, and that ultrastructural changes in liver were mitochondrial change, dilation and deterioration of rough endoplasmic reticulum and proliferation of smooth endoplasmic reticulum. Biochemical effects of cadmium were more severe on liver than kidney. Therefore, acutely injected cadmium caused not only liver damage, but also kidney damage.
Objective: To document the imaging findings of hepatic cavernous hemangioma detected in cirrhotic liver. Materials and Methods: The imaging findings of 14 hepatic cavernous hemangiomas in ten patients with liver cirrhosis were retrospectively analyzed. A diagnosis of hepatic cavernous hemangioma was based on the findings of two or more of the following imaging studies: MR, including contrast-enhanced dynamic imaging (n = 10), dynamic CT (n = 4), hepatic arteriography (n = 9), and US (n = 10). Results: The mean size of the 14 hepatic hemangiomas was 0.9 (range, 0.5-1.5) cm in the longest dimension. In 11 of these (79%), contrast-enhanced dynamic CT and MR imaging showed rapid contrast enhancement of the entire lesion during the early phase, and hepatic arteriography revealed globular enhancement and rapid filling-in. On contrast-enhanced MR images, three lesions (21%) showed partial enhancement until the 5-min delayed phases. US indicated that while three slowly enhancing lesions were homogeneously hyperechoic, 9 (82%) of 11 showing rapid enhancement were not delineated. Conclusion: The majority of hepatic cavernous hemangiomas detected in cirrhotic liver are small in size, and in many, hepatic arteriography and/or contrast-enhanced dynamic CT and MR imaging demonstrates rapid enhancement. US, however, fails to distinguish a lesion of this kind from its cirrhotic background.
Cholangiocarcinoma is a heterogeneous group of aggressive tumors that correspond to the second most common primary liver tumor. They can be classified according to their anatomical position concerning the biliary tree, and each subtype demonstrates different behavior and treatment. A 38-year-old male patient presenting solely right lumbar pain was diagnosed with a 7 cm hepatic tumor involving segments I, Iva, and VIII associated with involvement of the hepatic veins. He underwent a bloc resection of hepatic segments I, II, III, IV, partial V, partial VII, and VIII; right, middle, and left hepatic veins; and inferior vena cava segment, with perfusion of the remaining liver in situ with a preservation solution. As the patient had a large accessory inferior right hepatic vein draining the remaining liver, no reimplantation of hepatic veins was necessary. He remained clinically stable in outpatient follow-up, with excellent performance status-current survival of 2 years 6 months after surgical treatment.
This study was carried out to investigate the inhibitory effects of red ginseng extract on preneoplastic nodule formation in the rat liver using a rapid bioassay of hepatocarcinogenesis. Wistar male rats in test groups were fed diets containing 3.0, 0.3 or 003% of red genseng extract for 6 weeks after 2 week of dosing with diethylnitrosamine (DEAN, 200 mg/kg B.W., I.P.) or saline solution. The rats in the control groups were treated with DENA or saline alone. Three weeks after the beginning of the experiment, the two-thirds partial hepatectomy was performed on all animals. The inhibitory effects were investigated by measuring the number, total area and size of hyperplastic nodules enhanced by the 2/3 partial hepatectomy following induction with DENA. The results obtained were summarized as follows: 1. In general, the body weight gains of the rats fed diets containing red ginseng extract were higher than those of other groups.
