이하선관과 안면신경의 협근지 사이의 해부학적 관계 분석 (Analysis of Anatomical Relationship between Stensen's Duct and Buccal Branch of Facial Nerve)

  • Background: When using the anterior approach for performing superficial parotidectomy, the first thing to do is to find the buccal branch of the facial nerve and the parotid duct. The buccal branch usually runs transversely with the parotid duct from the anterior border of the parotid gland. We wanted to check the relationship between the two structures during the operation and to get clinically helpful information. Methods: Twelve patients with parotid mass were treated with superficial parotidectomy between May 2012 and August 2012. The outline of superficial and deep lobes of the parotid gland, parotid duct, and the buccal branch of the facial nerve were drawn on the transparent film by tracing the structures intraoperatively. Results: In 7 (58.3%) of 12 cases, the buccal branch of the facial nerve was located more caudally than the parotid duct at the anterior border of the superficial lobe of the parotid gland. In 3 cases (25%), the buccal branch was located more cephalically than the parotid duct. The mean distance between two structures were $2.54{\pm}1.48$ mm. In 11 cases, the parotid duct was located deeper than the buccal branch. Conclusion: The buccal branch of the facial nerve tends to be located more caudally than parotid duct and runs more superficially than parotid duct in all cases. We identified the relationship between the parotid duct and the buccal branch of the facial nerve during the operations on living subjects, not from the cadavers, so it would be a clinically helpful study which supplied more accurate anatomical information.

A better facial contour accomplished by parotid duct preserving superficial parotidectomy

  • Depression of facial contour after parotidectomy is still challenging to many of surgeons. A 68-year-old man presented with a 4-month history of a painless swelling in both parotid area. The mass was multiple and fixed at the parotid region. We conducted a parotid duct preserving bilateral superficial parotidectomy by one-stage operation to remove the multiple tumors. A lazy S incision was made in both preauricular area and the peripheral branches of the facial nerve were identified using surgical landmark. After dissecting the branches of the facial nerve and parotid duct, main parotid duct was preserved but only small fine ductules from the superficial lobe were ligated. Parotid gland was excised from its anterior aspect with about 1 cm of normal parotid tissue margin. The patient was followed up for 6 years to evaluate postoperative parotid gland function and the computed tomography (CT) was taken. Patient was satisfied with no significant complication such as sunken changes in facial contour, facial nerve function. As far as we know, it is the first study to compare long-term soft tissue contours of soft tissue of duct preserving superficial parotidectomy with duct sacrificing superficial parotidectomy by means of CT findings.

Treatment of intractable parotid sialocele occurred after open reduction-fixation of mandibular subcondylar fracture

  • A sialocele is a subcutaneous cavity containing saliva, most often caused by facial trauma or iatrogenic complications. In subcondylar fractures, most surgeons are conscious of facial nerve injury; however, they usually pay little attention to the parotid duct injury. We report the case of a 41-year-old man with a sialocele, approximately $5{\times}3cm$ in size, which developed 1 week after subcondylar fracture reduction. The sialocele became progressively enlarged despite conservative management. Computed tomography showed a thin-walled cyst between the body and tail of the parotid gland. Fluid leakage outside the cyst was noted where the skin was thin. Sialography showed a cutting edge of the inferior interlobular major duct before forming the common major duct that seemed to be injured during the subcondylar fracture reduction process. We decided on prompt surgical treatment, and the sialocele was completely excised. A duct from the parotid tail, secreting salivary secretion into the cyst, was ligated. Botulinum toxin was administrated to block the salivary secretion and preventing recurrence. Treatment was successful. In addition, we found that parotid major ducts are enveloped by the deep lobe and extensive dissection during the subcondylar fracture reduction may cause parotid major duct injury.

Cystic Salivary Duct Carcinoma Penetrated by Facial Nerve

  • Salivary duct carcinoma is a rare malignant salivary gland tumor that mainly has solid features. When it occurs in the parotid gland, it can invade the facial nerve and cause facial nerve paralysis. However, in our case, the salivary duct carcinoma exhibited cystic features on computed tomographic imaging, and the facial nerve passed through the cyst. Total parotidectomy with level-I to -III dissections was performed and nerve passing through the tumor was sacrificed. The patient received postoperative radiotherapy and was clinically and radiologically followed-up for every 3 months. Recurrence or distant metastasis was not reported. To the best of our knowledge, this is the first case involving a salivary duct carcinoma with cystic features and facial nerve invasion. Here, we report a first case of cystic salivary duct carcinoma of the parotid gland which uncommonly undergo cystic change and penetrated by facial nerve and successfully resected without causing facial nerve injury.

등압타액선조영촬영법을 이용한 정상성인의 이하선에 관한 연구 (A ROENTGENOGRAPHIC STUDY OF NORMAL PAROTID GLANDS USING ISOBARIC SIALOGRAPHY)

  • The aim of this study was to establish the diagnostic criteria of normal parotid glands in adults revealing the anatomical shape, its variations and the postitional relationships of the gland. Materials included 96 lateral and anterior-posterior sialograms of selected person from 23 to 28 years of age. Results were as follows: 1. The average length and lateral displacement of main duct was 48.43㎜ and l6.88㎜. The mean lumen diameter of that was 0.91㎜ in distal end and 1.40㎜ in hilar end in parotid glands. 2. The average angle of main duct to the inferior border of mandib was 34.32 degree. In configurations of main duct, modified curvilinear type was. most prevalent and followed by curvilinear, reverse sigmoid, sigmoid type. 3. The mean caliber of parotid gland was the longest in superior-inferior. 4. The interlobar ducts showed relatively well defined in all cases, its average number was 5.72. Arrangement of these ducts showed at random. Accessory lobe showed 87.5% in the all cases, its average number was 1.7. 5. There were no difference between the well and poorly defined acinar fillings in the glandular parenchyme. 6. There were no differences between right and left parotid glands in size and shape of main duct and parenchymal portion, but there were great variations in each individuals.

이하선관에 발생한 MALT 림프종 1예 (A Case of MALT Lymphoma in Parotid Gland Duct)

  • Purpose: Primary malignant lymphomas of the salivary glands are uncommon. The parotid gland was most frequently involved, followed by the submandibular gland, minor salivary gland and sublingual gland. The most common subtype is mucosa - associated lymphoid tissue(MALT) lymphoma. We experienced a case of salivary MALT lymphoma involving parotid gland duct, so report a case with a review of the literature. Methods: A 65 year old female presented with a palpable mass on the left side of her cheek. There was no clinical or laboratory evidence of pre - existing autoimmune disease. Preoperative facial and neck CT with contrast showed $2.1{\times}1.7cm$ sized, ill defined, homogeneous low density mass near left masseter muscle, and no evidence of other enlarged lymph nodes. Results: At operation, a yellowish oval shaped mass was found slightly adhered to middle portion of the parotid gland duct, meaduring $2{\times}1.5{\times}0.7cm$. Microscopic finding showed that centrocyte - like cells, monocyte B cells and plasma cells were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. Conclusion: We report that very rare case of MALT lymphoma involving parotid gland duct in 65 year old female patient was experienced with clinical characteristics, histologic features and references.

타석에 대한 방사선학적 연구 (A RADIOLOGIC STUDY OF SALIVARY STONES)

  • The author examined fifty cases of sialolithiasis diagnosed in the Dept. of Oral Radiology in SNUH by conventional radiography and sialography, and analyzed patient's age, sex, location, radio density, numbers, shapes, and relation with ducts and parenchymas. 1. The results of this study were as follows: 1. The average age of patients was 38.6 years in submandibular sialolithiasis, and 39.2 years in parotid sialolithiasis. 2. There was slightly higher incidence in males (58.0%) than in females (42.0%). 3. Salivary stones were found to be much more in the submandibular gland and duct (82.0%) than in the parotid gland and duct (18.0%). 4. Of 62 submandibular salivary stones, 33 (53.2%) occurred in the main duct, 25 (40.3%) occurred in the hilum, and 4 (6.5%) occurred in the parenchyma. Of 18 parotid salivary stones, 9(50.0%) occurred in the main duct, 5(27.8%) occurred in the parenchyma, and 4(22.2%) occurred in the hilum. 5. Of the submandibular salivary stones, the number of radiopaque type was 45 (75.8%), and the number of radiolucent type was 17 (24.2%). Of the parotid salivary stones, the number of radiopaque type was 12 (66.7%), and the number of radiolucent type was 6(33.3%). 6. The single type was 30 cases (73.2%) in submandibular gland, 6 cases (66.7%) in parotid gland, and the multiple type was 11 cases (26.8%) in submandibular gland, and 3 cases (33.3%) in parotid gland. 7. Round shape was 35 cases (43.8%), ovoid shape was 22 cases (27.5%), irregular shape was 17 cases (21.3%), and cylindrical shape was 6 cases (7.5%).

우연히 발견된 무증상의 이하선 기종 (Incidentally Diagnosed Asymptomatic Pneumoparotid)

  • Pneumoparotid is a rare cause of parotid gland swelling, and is caused by retrograde air reflux from the oral cavity, into the parotid gland via Stensen's duct. Most patients complained of painless swelling in the parotid region. Herein, we report a rare case of pneumoparotid, incidentally diagnosed without symptoms on CT, for follow-up of parotid lymph node enlargement.

  • 건성 각결막염(KCS)은 임상에서 많이 접하게 되는 개의 안과 질환이다. 눈물샘으로부터 수성층의 눈물생산이 억제되거나 정지되어 각막 표면 눈물층의 양적변화와 함께 KCS가 유도된다. 본 연구는 KCS로 잠정진단을 받아, 본 대학병원에 의뢰된 개를 대상으로 조직 병리학적 변화를 검사하고, KCS의 진단 및 내과적 치료 방법들을 검토하였으며, 내과적 치료에 반응하지 않은 예를 외과적 처치인 parotid duct transposition(PDT)으로 치료하는 방법을 서술하고자 한다.

  • The salivary duct cyst is a rare disease and usually found in the parotid gland. A 55-years-old man presented swelling in the left buccal area and pathological diagnosis was a salivary duct cyst. Though its recurrence has been reported rare, the presented case showed recurrent swelling after enucleation. The recurrent lesion was successfully treated by incision and drain insertion.
