  • Lymphoepithelial carcinoma, also known as malignant lymphoepithelial lesion or lymphoepithelioma, is a rare tumor in salivary glands. Lymphoepithelial carcinoma has a characteristic histological findings comprising irregularly-shaped nests of malignant epithelial cells within a lymphocyte-rich stroma, occasionally forming lymphoid follicles. We recently experienced a case of fine needle aspiration cytology (FNAC) of lymphoepithelial carcinoma of parotid gland in a 61-year-old male. The FNAC yielded a hypercellular smear of many irregular clusters of malignant epithelial cells in the background of lymphoid stroma.

High grade carcinoma ex pleomorphic adenoma of parotid gland: a case report

  • Pleomorphic adenoma is the most prevalent benign tumor of the parotid gland, and shows potential malignancy. Carcinoma ex pleomorphic adenoma (CXPA) can occur in 3%-15% of pleomorphic adenoma cases. Owing to its clinical similarity to benign tumors, critical information related to CXPA can be easily overlooked, leading to frequent misdiagnosis of the condition. In this article, we report a rare case of CXPA found in the 55-year-old male patient with characteristic clinical, radiographic, and histological features, and subsequent treatment.

  • Sclerosing polycystic adenosis (SPA) is a rare, asymptomatic disease that occurs mainly in the salivary glands. We report the case of a 51-year-old man who presented with trismus and pain upon mouth opening. Magnetic resonance imaging revealed a 2-cm mass located in the anterior portion of the left parotid gland. SPA was diagnosed based on histopathological examination of the surgical specimen. In pathologic findings, there was a well-circumscribed multicystic nodule in the parenchyma. Dense fibrosis and chronic non-specific inflammatory cells were observed in the stroma. In 13 previous reports on SPA, the most preferred treatment was superficial or total parotidectomy. This report suggests that simple excision of SPA preserves facial nerve function and facial volume.

  • The differential diagnosis of lesions in buccal area include lipoma, neurofibromas, epidermoid cyst, salivary ducts calculus, hemangioma, lymphadenopathy. Accessory parotid glands is defined as salivary gland tissue adjacent to the parotid duct, but separated from the body of parotid and it may be found in approximately 20% of human parotid glands. The appearance of an accessory parotid tumor is rare, with a reported frequency of 7.7% of all parotid neoplasm. Angiomyoma, which is also termed angioleiomyoma, is a rare solitary subcutaneous tumors arising from the vascular smooth muscle. It often occur in the extremities and is rarely found in buccal area. We present 2 cases of rare tumor in buccal mass and resected surgically without facial nerve palsy.

Treatment of intractable parotid sialocele occurred after open reduction-fixation of mandibular subcondylar fracture

  • A sialocele is a subcutaneous cavity containing saliva, most often caused by facial trauma or iatrogenic complications. In subcondylar fractures, most surgeons are conscious of facial nerve injury; however, they usually pay little attention to the parotid duct injury. We report the case of a 41-year-old man with a sialocele, approximately $5{\times}3cm$ in size, which developed 1 week after subcondylar fracture reduction. The sialocele became progressively enlarged despite conservative management. Computed tomography showed a thin-walled cyst between the body and tail of the parotid gland. Fluid leakage outside the cyst was noted where the skin was thin. Sialography showed a cutting edge of the inferior interlobular major duct before forming the common major duct that seemed to be injured during the subcondylar fracture reduction process. We decided on prompt surgical treatment, and the sialocele was completely excised. A duct from the parotid tail, secreting salivary secretion into the cyst, was ligated. Botulinum toxin was administrated to block the salivary secretion and preventing recurrence. Treatment was successful. In addition, we found that parotid major ducts are enveloped by the deep lobe and extensive dissection during the subcondylar fracture reduction may cause parotid major duct injury.

안면신경 침범시의 수술적 처치 (Surgical Dilemma of Facial Nerve Invasion)

  • The most important concern to do parotidectomy is correct identification of the facial nerve and preservation of the nerve function. Many descriptions for the localization and branching types of the facial nerve trunk have existed. During the parotid surgery, it is necessary to have knowledges about the incidence and prognostic aspect of a invasion of the facial nerve by the parotid tumors. The method of the dissection and the surgical extent of the parotid gland would be decided not only by the anatomic variation of the facial nerve. but also the size and location of the tumor. Invasion of the facial nerve in parotid malignancies is the most significant factors affecting the prognosis, so radical parotidectomy which consists of the total extirpation of the parotid gland in conjunction with resection of the facial nerve is often required for proper management. Radical parotidectomy is advocated for the surgical treatment of high grade malignancies and in selective recurrent benign tumors intimately involving the facial nerve. Unfortunately, the morphologic and functional deficits created by sacrificing the facial nerve can be emotionally and physically traumatizing to the patient. Therefore, when the facial nerve is sacrificed, immediate reconstruction of the facial nerve should be necessary. Immediate nerve repair with direct anastomosis of the resected nerve ends or placement of a cable nerve graft provides the better cosmetic and functional results. Surgical resection remains the mainstay of treatment for cancer of the parotid gland, and there is general agreement that facial nerve should not be sacrificed unless the tumor is adherent to, or surrounds the nerve. The following statement is described general principles of troublesome management of the facial nerve during surgery for parotid tumor.

주타액선에서 발생한 선양낭세포암종의 방사선치료성적 (Treatment Results for Adenoid Cystic Carcinoma of the Major Salivary Gland)

  • Purpose: This study was to investigate clinical feature of adenoid cystic carcinoma (ACC) in the major salivary gland. Materials and Methods: A retrospective analysis was undertaken for 23 patients with adenoid cystic carcinoma of the major salivary gland, who completed postoperative radiotherapy at the Yonsei Cancer Center, Yonsei University Hospital between May 1981 and December 1999. The male to female ratio was 1.1:1. Median age was 50. Follow up periods were 12-180 months with the median follow-up time of 59 months. Results: Parotid gland, submandibular gland and sublingual gland were 17, 5 and 1 respectively. Overall failure rate was 26%. Local failure was observed in two patients with parotid gland origin. Five cases showed distant failure. Overall and disease free survival rate were 68% and 72% respectively. Conclusion: Major pattern of failure was distant failure. Resection margin status had not prognostic significance. Postoperative radiation treatment is an effective to control in the adenoid cystic carcinoma of the major salivary gland.

심장 전이와 비후성 골관절증을 동반한 재발성 이하선 선방세포암 (Recurrent acinic cell carcinoma in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy)

  • Acinic cell carcinoma (ACC) is an uncommon malignant tumor of the salivary glands that is difficult to diagnose. It grows slowly and shows distant metastasis rarely. We experienced a case of recurrent ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy. The 29-year-old man had been suffering from severe multiple bones and joints pain for 2 months. Ten years earlier, he underwent superficial parotidectomy due to a right subauricular mass. The mass was diagnosed with ACC. After surgery, the tumor recurred twice. Then the patient was diagnosed with cardiac metastasis via positron emission tomography-computed tomography and trans-thoracic echocardiography. He also had hypertrophic osteoarthropathy with multiple bone metastasis. He was given palliative radiotherapy and conservative treatment. ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy has not yet been reported in literature. From this case, it is recommended to evaluate multiple distant metastasis in the ACC of the parotid gland when joint and bone pain are present.

  • Epidermoid cysts presents as a nodular and fluctuant subcutaneous lesion beneath the skin and are most common in acne-prone areas of the head, neck and back. This cyst often arises after localized inflammation of the hair follicle and occasionally after the implantation of epithelium following trauma and surgery including a biopsy procedure. It is often associated with Gardner syndrome, particularly before puberty. The lesion is normally treated by a surgical excision or enucleation, and recurrence is uncommon. A 27 year old woman complained of a swelling of the left parotid gland when she visited our clinic. A cystic lesion was found in the left parotid gland from the high signal intensity on the MR images. Ultrasonography showed that the cystic lesion was heterogeneous echogenic. Six months earlier, botulinum toxin was injected in her left masseter muscles six months earlier and progressive swelling of the left parotid area was noticed four months after treatment. The lesion was surgically removed. It was encapsulated by a thin wall and filled mainly with keratin. The final diagnosis was an epidermoid cyst.

다형성 선종으로 오인된 이하선 상피-근상피암종 3례 (Epithelial-Myoepithelial carcinoma of parotid glands: 3 cases misdiagnosed as pleomorphic adenoma)

  • Epithelial-myoepithelial carcinoma (EMC) is a rare type of low-grade malignant tumor that account for approximately 0.5% to 1% of salivary gland neoplasm and arises most commonly in the parotid gland (80%). We introduce three cases of parotid EMC arose as painless cystic mass in male patients over 70 years old. All patients were diagnosed as benign tumors (pleomorphic adenoma) by image and pathologic study (fine needle aspiration) before surgery, but the final histopathologic results were EMC. All three patients underwent parotidectomy without adjuvant radiotherapy. There were no complications such as facial paralysis. No complications or recurrences were observed during follow-up for 6 to 9 months. Since the reports of EMC are still relatively few, we report our three cases with the clinical and pathological review.
