파킨슨병에 동반된 수면장애의 한약 치료에 대한 임상 연구 동향 : 무작위 대조연구를 중심으로 (Efficacy of Herbal Medicine on Sleep Disorders in Parkinson's Disease: A Review of Randomized Controlled Trials)

  • Objectives: This study reviewed randomized controlled trials (RCTs) investigating the efficacy of herbal medicine on sleep disorders associated with Parkinson's disease and suggests a better research process. Methods: We searched for RCTs for herbal medicine treatments for sleep disorders related to Parkinson's disease on July 31, 2023 using eight databases (PubMed, Embase, the Cochrane library, China National Knowledge Infrastructure [CNKI], the Research Information Service System [RISS], Science ON, the Oriental Medicine Advanced Searching Integrated System [OASIS], and the Korea Citation Index [KCI]). Cochrane's risk of bias tool was used to assess the quality of the RCTs. Results: A total of 16 RCTs met all the inclusion criteria, and in most reports, the treatment group showed a significant improvement in sleep disorders compared to the control group. Total effective rate (TER), Pittsburgh Sleep Quality Index (PSQI), Unified Parkinson's Disease Rating Scale (UPDRS), TCM Symptom Score (TSS), Parkinson's Disease Sleep Scale (PDSS), etc., were used as evaluation indicators. Conclusion: Herbal medicine is a potential treatment for sleep disorders associated with Parkinson's disease. However, the selected RCTs were of poor quality, and it is necessary to perform more systematic studies.

  • Parkinson's Disease is a well known degenerative disease which result's in the depletion of dopamin-producing neurons in the pars compacta of the substantia nigra. This is a chronic, progressive disorder characterized by progressive muscular rigidity, pill-rolling tremor, flexed posture, shuffling gate and akinesia. This study was designed to evaluate the effects of an oxygen therapy with various scales on symptoms of Parkinson's disease. We treated his disease with Oxygen treatment , including herbal medicine acupuncture, and trigger point therapy, and we evaluated the clinical progress of the patient using UPDRS and H- Y stage. After treatment for about 1-month, the chief complaints and tremor symptoms were improved. This study suggests that oxygen therapy is significantly effective in the treatment of Parkinson's disease.

파킨슨병에 대한 국내 한의학 관련 실험 연구 동향 (A Review on Experimental Studies of Parkinson's Disease in Korean Medical Journals)

  • Objectives: The purpose of this study is to review experimental studies of Parkinson's Disease in Korean medical journals, which have been published since 1990. Methods: Experimental studies related to Parkinson's disease published in Korean medical journals, registered with Korea research foundation, were searched. Articles were grouped into journal of publication, subject, and publication year. Experimental study subjects and models were analysed. Results and Conclusions: Twenty-two articles about experimental studies of Parkinson's Disease in Korean medical journals were included in our review. There were 13 articles in which herbs were used for treatment, 4 articles in which bee venom was used for treatment, 4 articles in which acupuncture was used for treatment, and 1 article in which pharmacopuncture was used for treatment. Most of the studies used experimental models of MPTP-induced Parkinson's disease and MTT assay was used to analyse cell toxicity and cell viability. Pole test, rotarod test, Morris water maze test, and forced swimming test were commonly used to analyse MPTP-induced behavioral deficits in animals. Analysis on experimental studies of Parkinson's disease was categorized into the subjects of dopaminergic cell protective effect, cell-apoptosis protectiveness, anti-inflammatory effect, and anti-oxidative effect.

특발성 파킨슨병 환자에 대한 한의치료의 효과 : 후향적 연구 (Effects of Korean Medicine on Patients with Idiopathic Parkinson’s Disease: A Retrospective Study)

  • Objectives: The purpose of this study was to investigate the effects of Korean medicine on patients with idiopathic Parkinson’s disease.Methods: The charts of 47 patients diagnosed with idiopathic Parkinson’s disease in the Department of Internal Korean medicine, Stroke and Neurological Disorders Center, Kyung Hee University Hospital, Gangdong between August 2012 and July 2016 were reviewed. The Unified Parkinson’s disease rating scale (UPDRS) was administered before and after treatment with Korean medicine.Results: Thee average UPDRS Ⅱ+Ⅲ, UPDRS Ⅱ, and UPDRS Ⅲ of the 47 patients decreased significantly from 22.26±15.15, 10.19±7.53, and 12.06±8.35, respectively, pretreatment to 16.96±13.63, 7.47±6.44, and 9.49±7.73, respectively, post-treatment. The average postural instability and gait difficulty (PIGD), tremor, and bradykinesia also significantly improved post-treatment.Conclusions: These results provide evidence that Korean medicine can improve the activities of daily living and motor function of patients with idiopathic Parkinson’s disease. In particular, Korean medicine may be effective for the treatment of PIGD, tremor, and bradykinesia.

MPTP(1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine)로 유도된 Parkinson's Disease 동물 모델을 이용한 향사양위탕의 신경 세포 보호 효과 (Neuroprotective Effects of Hyangsayangwi-tang in MPTP-induced Mouse Model of Parkinson's Disease)

  • Objectives To evaluate the neuroprotective effects of Hyangsayangwi-tang (HY), a Korean traditional medicinal prescription in a Parkinson's disease mouse model. Methods Four groups(each of 10 mouse per group) were used in this study. The neuroprotective effect of HY was examined in a Parkinson's disease mouse model. C57BL/6 mouse treated with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP, 30mg/kg/day), intraperitoneal (i.p.) for 5 days. Slow behavioral responses and memory disorder is the major clinical symptoms of PD. In order to investigate the effect of HY on recovery of behavioral deficits and memory, we examined the motor function and memory by using Morris water maze and Forced swimming test. Ischemic mouse brain stained with TTC(2,3,5 triphenyl tetrazolium chloride) in the MPTP-induced Parkinson's disease to find out ischemia and tissue damage in mouse. The convenient, simple, and accurate high-performance liquid chromatography (HPLC) method was established for simultaneous determination of neurotransmitters in MPTP-HY group. To measure the amount of dopamine in mice brain, striatum-substantia nigra, was examined by Bradford assay. Immunohistochemistry was examined in the MPTP-induced Parkinson's disease (PD) mouse to evaluate the neuroprotective effects of Hyangsayangwi-tang on hippocampal lesion, ST and SNpc. Results and Conclusions Hyangsayangwi-tang (HY) prevents MPTP-induced loss of serotonin, hippocampus and TH-ir cell.

파킨슨병 노인의 낙상에 영향을 미치는 신체적, 심리적 요인 (Physical and Psychological Factors Affecting Fall in Elderly Patients with Parkinson's disease)

  • 파킨슨병 노인은 낙상의 신체적, 정신적 위험요인을 가진 비율이 비 파킨슨병 노인에 비해 높아, 파킨슨병 노인의 낙상 경험 유무에 따라 낙상에 영향을 주는 신체적, 정신적 위험요인을 한국 노인 전체를 모집단으로 하는 국가 데이터를 이용하여 규명하였다. 2017년 보건복지부의 노인실태 자료조사를 이용한 2차 분석연구로, 파킨슨병 노인은 총 103명이었으며 결측값을 제외하고 총 96명의 대상자를 분석하였다. 파킨슨병 노인에게서 낙상에 가장 큰 영향을 미치는 요인은 IADL로 나타났으며, IADL은 운동 조절기능과 관련이 있다. 운동 조절기능의 저하는 일상생활에 필수적으로 요구되는 신체 움직임에 제약을 가하며 위급상황에서의 자기 보호적 행동까지도 영향을 미쳐 낙상에 영향을 미친다. IADL이 낙상에 영향을 줄 수 있다는 본 연구결과를 바탕으로 파킨슨병 노인의 낙상 예방 중재를 위한 다양한 운동요법을 제안할 수 있을 것이다.

Diagnosing Parkinson's Disease Using Movement Signal Mapping by Neural Network and Classifier Modulation

  • Parkinson's disease is a growing and chronic movement disorder, and its diagnosis is difficult especially at the initial stages. In this paper, movement characteristics extracted by a computer using multilayer back propagation neural network mapping are converted to the symptoms of this disease. Then, modulation of three classifiers of C4.5, k-nearest neighbors, and support vector machine with majority voting are applied to support experts in diagnosing the disease. The purpose of this study is to choose appropriate characteristics and increase the accuracy of the diagnosis. Experiments were performed to demonstrate the improvement of Parkinson's disease diagnosis using this method.

소양인 파킨슨병 환자의 한양방 병행치료 장기 경과관찰 보고 3례 (Long-term Clinical Efficacy on Three Soyang Patients with Parkinson's Disease Improved by Combined Treatment of Korean-Western Medicine)

  • Objective This case study was about a long-term clinical effects after using Korean medicine treatment and western treatment in three Soyangin patients with Parkinson's disease. In this study, we reported significant improvement of non-motor and motor symptoms of these patients after Sasang constitutional medicine treatments for more than a year. Method The patients were identified as Soyangin and treated with Yangguksanhwa-tang, Hyeongbangdojeok-san and acupuncture. The Hoehn & Yahr scale, Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (UPDRS) were used to assess the overall function of the patients. And the Global Assessment Scale (GAS) was used to assess the change of chief complaints after the treatments. Result and Conclusion The non-motor and motor symptoms of Parkinson's disease improved after Sasang constitutional medicine and western medicine treatments for more than a year. Furthermore these patients' chief complaints improved without increasing of amount of western medicine (levodopa). In conclusion, this study showed that Sasang constitutional medicine with western medicine can be effective treatment for long-term clinical care of Parkinson's disease.

대뇌 기저핵의 기능과 파킨슨 질환 (The Function of Basal Ganglia & Parkinson's Disease)

  • Parkinson's disease(PD) is a progressive neurodegenerative disease that affects the functioning of the basal ganglia, a brain area that contributes to the control of movement. The disease is caused by the death of nerve cells in the brain that produce dopamine, a chemical messenger. The cells affected usually produce a neurotransmitter(a chemical that transmits nerver impulses) called dopamine, which acts with acetylcholine, another neurotransmitter, to fine-tune muscle control. In Parkinson's disease, the level of dopamine relative to acetylcholine is reduced, adversely affecting muscle control. When the supply of dopamine is depleted, the function of the basal ganglia is disrupted and its ability to control movement-deteriorates. The result is that PD patients experience moderate rigidity, difficulty in initiating movements and slowness in executing them, and a rhythmical tremor at rest. Although the cause of Parkinson's disease is not known, genetic factors may be involved. About 3 in 10 people with the disorder have an affected family member. About 1 in 100 people over the age of 60 in the US have Parkinson's disease. And Parkinson's disease is slightly more common in men. The course of the disease is variable, but drags may be the best effective in treating the symptoms and improving quality of life. But, The doctor may arrange physical therapy to help with physical mobility problems. It is important to continue to exercise and take care of your general health. Try to take a walk each day. Stretching exercises can help you maintain your strength and mobility. So, This papers will serve about the information of PD for clinical physical therapist. Finally, The aim of review is increasing approach method and technique for PD patients by the view of physical therapy.
