Paradoxical Deterioration of Intramedullary Spinal Tuberculomas during Antituberculous Therapy

  • "Paradoxical deterioration" during antituberculous therapy is generally defined as the clinical or radiological worsening of pre-existing tuberculous lesions or the development of new lesions in a patient who initially improves. The phenomenon of paradoxical deterioration in intramedullary tuberculoma of the spinal cord is rare and is a less established entity. The authors present an unusual case of paraparesis as a result of paradoxical deterioration of intramedullary tuberculoma despite adequate antituberculous therapy. Here, we review the relevant literatures and discuss its possible pathogenic mechanisms.

결핵흉막염 치료 중에 발생한 역설적 반응에 대한 연구 (Paradoxical Response Developed during the Antituberculous Treatment in Tuberculous Pleurisy)

  • 연구배경: 결핵환자에서 항결핵제 치료 중 초기에 병변의 호전 이후 방사선학적, 임상적으로 이전의 병변이 악화 되거나 새로운 병변이 발생하는 경우를 역설적 반응이라 정의한다. 결핵흉막염에서의 역설적 반응 발생과 이와 관련된 인자에 대해서 알려진 바는 많지 않다. 방 법: 2004년 1월부터 2006년 12월까지 결핵흉막염으로 진단받고 4개월 이상 항결핵제를 복용한 환자를 대상으로 하였다. 항결핵제 투약 전과 투약 후 흉부 방사선의 변화를 연속적으로 살펴보고 치료시작 2주 후에 이전의 병변의 악화나 새로운 병변이 생긴 경우를 역설적 반응이 발생한 것으로 판정하였다. 역설적 반응군이 발생하였던 환자군과 발생하지 않았던 환자군 간의 임상적인 특성과 흉수, 혈액 검사를 비교하였다. 결 과: 대상환자 77명 중 역설적 반응을 보인 환자는 16명(21%)이었다. 역설적 반응을 보인 시기는 항결핵제 사용 후 평균 38.6일이었으며, 병변의 크기가 줄어드는데 걸리는 시간은 평균 32.1일이었다. 역설적 반응은 젊은환자에서(30.5세 vs 39.0세), 그리고 흉수의 백혈구 중 림프구의 백분율이 높았던 경우에서(82.1% vs 69.6%) 많이 발생하였다. 결 론: 결핵흉막염의 항결핵제 치료 중에 발생하는 역설적 반응은 드물지 않았으며 환자의 연령과 흉수의 백혈구 중 림프구의 백분율이 역설적 반응의 발생빈도와 관련이 있었다.

  • 크립토코쿠스 수막염 면역재구성 염증증후군은 역설적 크립토코쿠스 수막염 면역재구성 염증증후군과 증상이 드러나는 크립토코쿠스 수막염 면역재구성 염증증후군의 두 가지 형태가 있다. 역설적 크립토코쿠스 수막염 면역재구성 염증증후군과 크립토코쿠스 수막염 재발을 구별하는 것은 중요하다. 왜냐하면 면역재구성 염증증후군 없는 크립토코쿠스 수막염보다 면역재구성 염증증후군이 있는 크립토 코쿠스 수막염의 사망율이 더 높고, 역설적 크립토코쿠스 수막염 면역재구성 염증증후군과 크립토코쿠스 수막염 재발은 다른 치료를 요구하기 때문이다. 우리는 인간면역결핍바이러스 감염환자의 3년의 추적기간 동안 임상 소견과 자기공명영상 소견이 잘 대조된 역설적 크립토코쿠스 수막염 면역재구성 염증증후군의 사례와 감별에 도움이 되는 새로운 자기공명영상 소견을 보고하고자 한다.

Sonographic observation of the paradoxical masseteric bulging and clinical implication of functional compartment

  • Masseter are commonly botulinum neurotoxin targeted muscle for facial contouring in aesthetic field. However, paradoxical masseteric bulging is common adverse effect that has not been discussed with ultrasonographic observations. Retrospective study has been conducted from October, 2021 to January, 2023, out of 324 patients have done blinded botulinum neurotoxin injection in the masseter at the middle and lower portion of the masseter with each side of 25 units (letibotulinum neurotoxin type A), 3 patients demonstrated paradoxical masseteric bulging has been reported and the image observed by ultrasonography by physician. Based on the observations made, we can infer that the function of the moving muscle involves twisting of the muscle fibers during contraction, along with the twisting of the deep inferior tendon, which causes the muscle to be divided into anterior and posterior compartments rather than into superficial and deep compartments of masseter. In ultrasonographic observe the skin surface of a patient with paradoxical masseteric bulging, it is observable that either the anterior or posterior part contracts significantly. The functional units of anterior and posterior compartment are observable as muscular contraction of inward movement of the muscle from either the anterior or posterior functional unit.

Paradoxical Transtentorial Herniation Caused by Lumbar Puncture after Decompressive Craniectomy

  • Although decompressive craniectomy is an effective treatment for various situations of increased intracranial pressure, it may be accompanied by several complications. Paradoxical herniation is known as a rare complication of lumbar puncture in patients with decompressive craniectomy. A 38-year-old man underwent decompressive craniectomy for severe brain swelling. He remained neurologically stable for five weeks, but then showed mental deterioration right after a lumbar puncture which was performed to rule out meningitis. A brain computed tomographic scan revealed a marked midline shift. The patient responded to the Trendelenburg position and intravenous fluids, and he achieved full neurologic recovery after successive cranioplasty. The authors discuss the possible mechanism of this rare case with a review of the literature.

  • Tuberculous Iliopsoas muscle abscess is a rare manifestation in patient with extrapulmonary tuberculosis and hardly observed in developed country. Paradoxical response to anti-tuberculous medication could make difficult therapeutic decision to clinicians. The authors report a case of tuberculous iliopsoas muscle abscess with multiple intraabdominal and thoracic abscesses in 9 year-old-boy who presented paradoxical response to anti-tuberculous treatment.

  • Merits, attributes and challenges associated with the application of analytical modeling in electronics and photonics materials science are addressed, based mostly on the author's research during his tenure with Bell Labs, University-of-California, Portland State University, and small business innovative research (SBIR) ERS Co., USA. The emphasis is on practically important, yet often paradoxical, i.e., intuitively non-obvious, material behaviors. It is concluded that when material reliability is crucial, ability to effectively quantify it is imperative, and that analytical modeling is the most suitable, although never straightforward, technique to understand, explain and quantify material behaviors, especially in extreme, extraordinary and paradoxical situations.

Unraveling the Paradoxical Action of Androgens on Muscle Stem Cells

  • Androgens act in almost all tissues throughout the lifetime and have important roles in skeletal muscles. The levels of androgens increase during puberty and remain sustained at high levels in adulthood. Because androgens have an anabolic effect on skeletal muscles and muscle stem cells, these increased levels of androgens after puberty should lead to spontaneous muscle hypertrophy and hyperplasia in adulthood. However, the maintenance of muscle volume, myonuclei number per myofiber, and quiescent state of satellite cells in adulthood despite the high levels of androgens produces paradoxical outcomes. Our recent study revealed that the physiological increase of androgens at puberty initiates the transition of muscle stem cells from proliferation to quiescence by the androgen-Mindbomb1-Notch signaling axis. This newly discovered androgen action on skeletal muscles underscores the physiological importance of androgens on muscle homeostasis throughout life. This review will provide an overview of the new androgen action on skeletal muscles and discuss the paradoxical effects of androgens suggested in previous studies.

Radiological Follow-up of a Cerebral Tuberculoma with a Paradoxical Response Mimicking a Brain Tumor

  • We report a case of a paradoxical response of a tuberculoma in the brain mimicking a brain tumor. A 76-year-old woman presented with a 2 week history of headache, dysarthia, and orthopnea. Brain magnetic resonance images (MRI) revealed two rim-enhancing lesions on the pons and occipital lobe, and chest computed tomography showed randomly distributed miliary nodules. The tentative diagnosis was tuberculosis (TB) of the brain and lung. She complained of right hemiparesis and worsening general weakness after taking the anti-TB medication. On the monthly follow-up images, the enhanced lesions were enlarged with increased perfusion and choline/creatinine ratio, suggesting a high grade glioma. A surgical resection was completed to diagnose the occipital lesion, and the tuberculoma was pathologically confirmed by a positive TB-polymerase chain reaction. The anti-TB medication was continued for 13 months. A follow-up MRI showed decreased size of the brain lesions associated with perilesional edema, and the clinical symptoms had improved. Brain tuberculoma could be aggravated mimicking brain malignancy during administration of anti-TB medication. This paradoxical response can be effectively managed by continuing the anti-TB drugs.

  • 결핵 환자들에서 역설적반응이란 항결핵제 치료 중 초기에 병변의 호전 이후 방사선학적, 임상적으로 이전의 병변이 악화되거나 새로운 병변이 발생하는 경우로 정의될 수 있다. 전세계적으로 지금까지 다양한 종류의 역설적 반응이 보고되어 왔다. 그러나 국내 소아에서의 보고는 거의 드물다. 이에 저자들은 17세 남아에서 결핵성 흉막삼출로 진단되어 결핵약물에 좋은 반응을 보여 적절한 치료 후 흉수가 호전된 후에, 첫 치료 3주 후에 새로이 발생한 폐병변이 기존의 항결핵제를 계속하면서 결국에는 호전된 케이스를 경험하였기에 보고하는 바이다.
