부산시 일부지역 중년남녀의 암 조기검진 수검 실태 조사 (A survey on cancer screening among the middle-aged in Pusan area)

  • Objectives: Cancer is the most frequent cause of death in Korea. Cancer screenings can save lives through early detection and their effect can be enhanced by regular repeat adherence rather than one-time screening. The aim of this study was to investigate major cancer screening rates and the reasons for not having screening for providing the basic data required. Materials and Methods: The study sample were recruited from the parents of students in 3 different middle and high schools in Pusan. 428 participants(l93 of males, 214 of females) completed a structured self-administered questionnaire from Dec. 21 to 31, 2001 and the response rate was 73.8%. Data were analyzed using descriptive statistics with SPSS Win 10.0. Results: The cancer screening rates of the subjects(male and female respectively) who have had one or more in their life-time were about 36.3% and 34.6% in gastroendoscopy for stomach cancer, about 11.1 % and 8.5% in stool hemocult test and colonoscopy for colon cancer, 13.5% and 9.3% in prostate-antigen test and rectal digit exam for prostate cancer, 36.4% in mammograpy for breast cancer, and 59.3% in Pap smear test for cervical cancer. And the higher proportions of having regular screening were 36.0% in Pap smear test for cervical cancer and 11.7% in mammograpy for breast cancer. The reasons related to not having screening tests were found that ‘seem to be healthy’ was 44.8%∼58.9% and the most common reason and the following was ‘not having opportunity for check-up’. The most common reason related to not having regular screening tests were ‘for the finding of previous check-up was normal’.

  • Purpose : Cancer is th e most frequent cause of death in Korea. Cancer screening can save lives through early detection. This study was to investigate major cancer screening rates and the reasons for not having been screening. Method : 210 participants of registered nurses from 5 general hospitals in Busan, Korea were completed a structured self-administered questionnaire. Data were analyzed using descriptive statistics with SPSS WIN 10.0. Results : The cancer screening rates of the subjects were 53.8% in Pap smear test for cervical cancer, 41.0% in gastroendoscopy for stomach cancer, 15.7% in mammograpy for breast cancer, and 3.8% in colonoscopy. And the higher proportions of having regular screening were 9, 5% in Pap smear test and 2.9% in gastroendoscopy. The primary reason related to not having a screening test was 'seems to be healthy'. Conclusion : The findings showed the necessity of recognizing and educating Korean nurses to have a regular cancer screening for their health management.

Psychometric Evaluation of Uterine Cervical Cancer Screening Embarrassment Questionnaire among Korean Women: Complementary Use of Rasch Model

  • Purpose: Cervical cancer can be prevented by regular cervical screening. Embarrassment has been reported as one important barrier to cervical screening uptake. The absence of appropriate instrumentation, however, has limited our understanding of the links between embarrassment and health care outcomes. The purpose of this study was to evaluate psychometric attributes of the uterine cervical cancer screening embarrassment questionnaire. Methods: A convenience sample for field study was recruited from four gynecological clinics in Gyeongju, Korea. Within a cross-sectional descriptive design, 339 women who had cervical screening completed self-administered measures of embarrassment including a visual analogue, general medical embarrassment, dispositional embarrassment, and Pap smear related negative emotion. Results: Rasch analysis of items demonstrated the evidence of one-dimensional construct and good 7-point rating scales functioning. Factor analysis revealed that uterine cervical cancer screening embarrassment was comprised of two domains-bodily manifestations and perceiving an undesirable social face. Construct validity was demonstrated by a high subscale-to-subscale correlation. Convergent and discriminant validity was evidenced by significant correlations with a 100 mm VAS scale, general medical embarrassment, and Pap smear related negative emotion. Known-group validity was established by comparing women with high versus low trait embarrassment. Both two sub-scales and overall scale demonstrated good internal reliability. Conclusion: The Uterine uterine cervical cancer screening questionnaire is a reliable and valid instrument suited to assessing the manifestations of embarrassment during screening. The use of instrument can be extended to understand the client's embarrassment undergoing health examinations which require the exposure of their private parts.

Usefulness of p16INK4a Immunocytochemical staining for the Differentiation between Atrophy and ASCUS in Diagnosis of Uterine Cervical Cancer

  • A Pap smear is the most important screening test for the diagnosis of cervical cancer. However, subjective judgment by the operator cannot be excluded, and replicability may greatly be reduced if uncertain specimens are examined. Examiners often experience difficulties in differentiating atrophy with inflammatory changes and ASCUS when diagnosing squamous epithelial lesions from a pap smear. Reports often vary between cytologists and pathologists, and misdiagnosis may result in delayed follow-ups and advanced diseases. Hence, auxiliary examinations are necessary when confusing results between atrophy and ASCUS are obtained. The importance of p16INK4a activation due to HPV infection, which is an important factor in the outbreak of cervical cancer, has been highlighted. Recent studies have reported that p16INK4a immunocytochemical staining and HPV high-risk type tests using liquid-based cervical specimens are effective to detect the presence of lesions of grade HSIL or higher in patients with ASC-H. However, no research exists on the utility of HPV and p16INK4a tests on the differential diagnosis of atrophy and ASCUS. This study focused on whether p16INK4a immunocytochemical staining and HPV tests can help diagnose borderline lesions between atrophy and ASCUS. The results reported that p16INK4a activation can significantly (P<0.001) differentiate atrophy from ASCUS in atrophic lesions infected with High risk-HPV. Therefore, it may be concluded that p16INK4a immunocytochemical staining is an effective auxiliary test in lesions infected with HR-HPV when atrophic lesions are difficult to differentiate by morphology. Such results are expected to help decide on adequate follow-up and treatment.

Age Specific Cytological Abnormalities in Women Screened for Cervical Cancer in the Emirate of Abu Dhabi

  • Background: Cervical cancer is the second most common cancer in women worldwide, with about 500,000 new cases and 270,000 deaths each year. Globally, it is estimated that over one million women currently have cervical cancer, most of whom have not been diagnosed, or have no access to treatment that could cure them or prolong their lives. In the United Arab Emirates (UAE) cervical cancer is the third most common cancer in women. Materials and Methods: A population-based cross-sectional retrospective survey of cervical smear abnormalities was conducted in the Emirate of Abu Dhabi, UAE, from January 2013 to December 2013 by collecting consecutive liquid-based cytology samples from the Department of Pathology at the SKMC Hospital in Abu Dhabi city. Results: The total number of women screened for cervical cancer for the year 2013 at SKMC was 4,593, with 225 (4.89%) abnormal smears. The majority of the abnormal smear results were atypical squamous cells of undetermined significance (ASCUS) 114 (2.48%). This study showed 60% increase in the rate of abnormal cervical smears in the UAE over the last 10 years. In this study the highest incidence of high grade abnormalities were seen in women above the age of 61years (1.73%), this might be due to the fact that this group of women missed the chance of screening of cervical cancer earlier in their lives or could be explained by the well-known second peak of HPV infection seen in many prevalence studies. Conclusions: We conclude that the rate of abnormal cervical smear in the screened Abu Dhabi women is not different from the rate in developed countries. A notable increase in both low and high grade abnormalities has occurred within the last decade.

Early Diagnosis Behavior in Turkish Women with and without a Family History of Cervical Cancer

  • Background: This study was planned as comparative and descriptive in order to measure and evaluate the knowledge and attitudes regarding early diagnosis of women with and without a family history of cervical cancer. Materials and Methods: The study sample consisted of the relatives of female patients (N=253) who were admitted to Istanbul University of Medicine. Women with a family history of cervical cancer formed the case group, while those without family history of cervical cancer constituted the control group. Two distinct data collection tools, a questionnaire and the Miller Behavioral Style Scale (MBSS), were used in order to obtain data for evaluation with SPSS for Windows 20.0 statistics package program. Results: It was found that 61.0% of the case group with family history of cervical cancer and 19.0% of the control group without family history of cervical cancer were using early diagnostic methods. Thus the presence of an individual with cervical cancer in the family affected the attitudes towards early diagnosis. It was further found that the level of knowledge on cervical cancer and PAP smear test was higher in the case group, which was more sensitive with regard to being informed about cervical cancer as compared to general society. However, the average MBSS scores were not significantly different compared to the control group. Conclusions: It was noted that, women participating this study knowledgeable, but this did not necessarily transform into better behavior.

Evidence-based Screening, Early Diagnosis and Treatment Strategy of Cervical Cancer for National Policy in Low-resource countries: Example of India

  • Cervical cancer remains the most frequent cancer in women from the developing world. More than 88% of deaths occur in low-income countries, and it is predicted to climb to 91.5% by 2030. Although Pap-based screening programmes have shown to be effective in reducing the disease burden in high-resource countries, implementation and sustention of cytology-based programmes is quite challenging in low-resource settings. The present paper reviews evidence-based alternatives of screening methods, triaging algorithm, treatment of cervical precancerous lesions, and age-group at screening appropriate for low-income countries. Evidence shows that visual inspection methods using diluted acid acetic or Lugol's iodine, and HPV-DNA testing are more sensitive tests than the Pap-smear screening test. Visual inspection allows an immediate result and, when appropriate, may be immediately followed by cryotherapy, the so called "screen-and-treat" approach, achieved in a single visit, by trained nurses and midwives. Examples of cervical cancer prevention programmes in India and selected low-income countries are given.

  • Primary malignant melanoma of vagina is a rare tumor which is easily misinterpretated in routine cytologic examination. We lately experienced a case of primary malignant melanoma of the vagina with direct cervical extension diagnosed by Pap smear. The cervicovaginal smear showed variable sized clusters of epithelial cells or singly scattered abnormal epithelial cells. Most of the tumor cells had round hyperchromatic nuclei with prominent nucleoli and brownish pigments in cytoplasm. The cytologic findings are compared with histologic features of resected specimen.

군집을 이루는 자궁 경부암 세포 인식에 관한 연구 (A Study on Recognition of Clustered Cells in Uterine Cervical Pap-Smear Image)

  • PaP Smear 테스트는 자궁 경부암 진단에 가장 효율적인 방법으로 알려져 있다. 그러나 이 방법은 높은 위 음성률(false negative error, 15~50%)을 나타내고 있다. 이런 큰 오류율은 주로 다량의 세포 검사에 기인하여, 자동화 시스템의 개발이 절실히 요구되고 있다. 본 논문은 자궁 경부암의 특징인 군집을 이루는 암세포를 인식할 수 있는 시스템을 제안한다. 시스템은 두 부분으로 나누어진다. 첫 단계에서는 저 배율(100배)에서 간단한 영상처리와 최소 근접 트리(Minimum Spanning Tree)를 통해 군집을 이루는 세포를 찾는다. 두 번째 단계서는 고 배율(400배)로 확대하여 군집 세포들로부터 여러 가지 특징을 추출한 후 KNN(k-Neighbor) 방법을 통해 인식하는 단계이다. 50개의 영상 (640X 480, RGB True Color 25 개의 100배 영상 , 25개의 400배 영상)이 실험에 사용되었다. 한 영상을 처리하는데 약 3초 (2.984초) 소요되었으며, 이는 region growing(20초)나 split and merge(58초) 방법 보다 덜 소요되었다. 100배 영상에서 정상과 비정상의 두 그룹으로 나누었을 경우에는 96%의 높은 인식율을 나타내었으나 비정상을 다시 5개의 그룹으로 나누었을 때는 45%로 나타내었다. 이는 영역 추출(segmentation) 단계에서 오류와 트레이닝 데이터의 비정확성에 기인한다. 400배 영상에서는 각각 92%와 30%로 나타내었다. 이는 영역추출 단계에서 사용한 Watershed 방법의 오류로 기인한 것으로 본다.

Assessment of Cervical Cancer Risk in Women between 15 and 49 Years of Age: Case of Izmir

  • Background: The aim of the study was to determine risk factors for cervical cancer for women in Izmir. Methods: This cross-sectional, descriptive field covered a population of 4319 women of reproductive age (15-49) (household registration in the Mukhtar's office-2007). A total of 1,637 women were included in the sample given a four-part questionnaire through face-to-face interview by visiting the women in their homes in order to determine socio-demographic factors, obstetric history, genital hygiene and the use of family planning methods. In addition, during the data collection process, the women were given group training in order to raise awareness of cervical cancer. The number and percentage distributions of the data were calculated. Results: While the average age of the women was $31.9{\pm}9.77$ (Min: 15.00-Max: 49.00), education level of 43.4% of them was elementary school only. It was determined that 70.3% of the women experienced at least one pregnancy, 71.0% had vaginal delivery and 75.9% used a contraceptive method. In the study it was determined that among the cervical cancer related risks vaginal delivery, vaginal lavage and having three or more pregnancies had the highest rates, while having sexual intercourse before 16 years of age and having more than one sexual partner constituted lower rates. The rate of the women who stated not having a smear in the last three years was 82.4%. Conclusions: Considering the case in terms of having Pap smear test, women's awareness on the risk factors and early diagnosis of cervical cancer was found to be low. Due to this reason, awareness of women has to be raised through education.
