개의 췌장염에 의한 만성 화농성 지방조직염증 (Chronic Suppurative Steatitis Secondary to Pancreatitis in a Dog)

  • A Maltese was presented with a subcutaneous mass at right flank, depression and hyperthermia. Ultrasonography revealed a hypoechoic mass with hyperechoic foci, a defect of abdominal wall and peritonitis. In cytology, fat necrosis was found. This dog was diagnosed as chronic suppurative steatitis secondary to pancreatitis and pancreatic herniation through surgery and histopathologic examination. Steatitis is recurrent inflammation and necrosis of adipose tissue. Also steatitis associated with pancreatic diseases such as pancreatitis and pancreatic tumor was reported and direct fat necrosis by pancreatic enzyme was supposed as the etiology. Steatitis secondary to pancreatitis was rarely reported in dogs and there was difficulty in diagnosis and treatment. This case has a limitation that pancreatitis was not diagnosed as underlying cause to steatitis before surgery. However, a defect of abdominal wall and peritonitis were observed on ultrasonography. Good prognosis could be achieved through prompt surgery and medical treatment.

PET/CT 검사에서 췌장암 판별을 위한 SUVm 경계값 연구 (Study of SUVm Cut-off Value for the Distinction of Pancreatic Cancer In PET/CT Exam)

  • 본 연구는 PET/CT 장비로 $^{18}F--FDG$ 방사성 의약품 주입 후 60분에서 검출된 췌장질환자 60명의 SUVm 데이터 분포를 정규분포 화 시키는 가설을 설정하여 PET/CT 이중시간 검사에서 지연 검사 시행을 결정할 수 있는 기준으로 제시하였다. 비정규분포를 나타내는 SUVm 데이터를 BOX-COX 변환과 Johnson 변환을 통해 정규분포 화 하였다. 정규 분포 95% 신뢰구간 하단에서 지연검사를 결정할 수 있는 경계값 SUVm 2.52를 제시하였다. 통계적 검증을 통해 제안된 경계값은 췌장암 구별을 위한 지연시간 검사에서 (120분, 180분)두 실험군 모두 적용할 수 있는 유의한 상관관계를 보였으며, 통계적으로 유의하다(p<0.05). C-Value는 췌장암 군과 췌장염 군을 감별에서는 췌장암 판별에 통계적으로 유의하다 (p<0.01). C-Value graph를 이용해서 지연시간 120분 검사가 지연시간 180분 검사보다 유용성이 있음을 확인하였다. 경계값 SUVm 2.52를 기준으로 이중시간 검사를 결정했을 때, 현재 대다수의 핵의학과 개설병원에서 시행하고 있는 단일 시간 PET/CT 검사보다, 췌장암 조기 발견과 감별에 도움을 줄 수 있을 것으로 사료된다.

Arsenic Trioxide Inhibits Cell Growth and Invasion via Down-Regulation of Skp2 in Pancreatic Cancer Cells

  • Arsenic trioxide (ATO) has been found to exert anti-cancer activity in various human malignancies. However, the molecular mechanisms by which ATO inhibits tumorigenesis are not fully elucidated. In the current study, we explored the molecular basis of ATO-mediated tumor growth inhibition in pancreatic cancer cells. We used multiple approaches such as MTT assay, wound healing assay, Transwell invasion assay, annexin V-FITC, cell cycle analysis, RT-PCR and Western blotting to achieve our goal. We found that ATO treatment effectively caused cell growth inhibition, suppressed clonogenic potential and induced G2-M cell cycle arrest and apoptosis in pancreatic cancer cells. Moreover, we observed a significant down-regulation of Skp2 after treatment with ATO. Furthermore, we revealed that ATO regulated Skp2 downstream genes such as FOXO1 and p53. These findings demonstrate that inhibition of Skp2 could be a novel strategy for the treatment of pancreatic cancer by ATO.

  • The blood glucose and plasma immunoreactive insulin(IRI) levels were measured glucose tolerance test in 7 healthy subjects and 6 patients with chronic liver diseases. The glucose tolerance was impaired in 5 of the 6 patients and normal in 1. Plasma IRI responses were markedly increased and delayed in all patients, suggesting insulin resistance. Patients with more glucose intolerance showed less increase in plasma IRI than the group with less intolerance. It is suggested that some insulin antagonists may decrease the peripheral insulin sensitivity and stimulate compensatory hyperactivity of pancreatic islets. If the compensatory hyperactivity is inadequate due to genetic predisposition to diabetes mellitus or exhaustion of ${\beta}$-cells of pancreatic islsts, the glucose intolerance and overt diabetes mellitus may ensue.

당뇨병과 동반된 참굴큰입흡충증 2례 (Two cases of Gymnopholloides seoi infection accompanied by diabetes mellitus)

  • 참굴큰입흡충(Gymnophalloides seoi)은 참굴이 매개하는 우리 나라 고유의 인체기생 장흡충의 하나로서, 전남 신안군 도서 지방에서 큰 유행지가 발견된 바 있다. 이 흡충은 췌장 질환과의 관계가 의심되어 의학적 관심의 대상이 되고 있다 이번 증례 보고에서는 당뇨병과 동반된 흥미있는 G. seoi감염 2례를 발견하여 보고하고자 한다. 환자들은 당뇨병 치료를 위해 내원하였으며 내원시 대변검사에서 참굴큰입흡충류(gymnophallid)의 충란이 검출되었다 프라지콴텔 및 하제를 투여한 후 충체를 수집한 결과 G. seoi 충체 772마리 및 10마리가 각각 검출되었다. 증례 I은 62세 남자로 G. seoi의 유행지 부근인 목포에 거주하고 있었고, 증례 II는 인천에 거주하고 있으면서 참굴을 날로 즐겨 먹었다고 하였다. 이 증례들을 볼 때 G. seoi감염이 당뇨병과 어떠한 상관관계를 가지고 있을 가능성을 시사하였다.

The inhibitory effects of Nardostachys jatamansi on alcoholic chronic pancreatitis

  • Nardostachys jatamansi (NJ) belonging to the Valerianaceae family has been used as a remedy for gastrointestinal inflammatory diseases for decades. However, the potential for NJ to ameliorate alcoholic chronic pancreatitis (ACP) is unknown. The aim of this study was to examine the inhibitory effects of NJ on ACP. C57black/6 mice received ethanol injections intraperitoneally for 3 weeks against a background of cerulein-induced acute pancreatitis. During ACP, NJ was ad libitum administrated orally with water. After 3 weeks of treatment, the pancreas was harvested for histological examination. NJ treatment increased the pancreatic acinar cell survival (confirmed by amylase level testing) and reduced collagen deposition and pancreatic stellate cell (PSC) activation. In addition, NJ treatment reduced the activation but not death of PSC. In conclusion, our results suggest that NJ attenuated ACP through the inhibition of PSC activation.

A Rare Cause of Recurrent Acute Pancreatitis in a Child: Isovaleric Acidemia with Novel Mutation

  • Recurrent acute pancreatic attacks is a rare clinical condition (2-5% of all acute pancreatis) in children and is mainly idiopathic in most cases. Sometimes it may be associated with congenital anomalies, metabolic diseases or hereditary conditions. Isovaleric acidemia (IVA) is a rare autosomal recessive amino acid metabolism disorder associated with isovaleryl coenzyme A dehydrogenase deficiency presenting the clinical findings such metabolic acidosis with increased anion gap, hyperammonemia, ketonemia, hypoglycemia, "the odor of sweaty feet," abdominal pain, vomiting, feeding intolerance, shock and coma. Recurrent acute pancreatitis associated with IVA have been rarely reported. Herein; we report a child who admitted with recurrent acute pancreatic attacks and had the final diagnosis of IVA. Mutation analysis revealed a novel homozygous mutation of (p.E117K [c.349G>A]) in the IVA gene. Organic acidemias must kept in mind in the differential diagnosis of recurrent acute pancreatic attacks in children.

Agonist (P1) Antibody Converts Stem Cells into Migrating Beta-Like Cells in Pancreatic Islets

  • Tissue regeneration is the ultimate treatment for many degenerative diseases, however, repair and regeneration of damaged organs or tissues remains a challenge. Previously, we showed that B1 Ab and H3 Ab induce stem cells to differentiate into microglia and brown adipocyte-like cells, while trafficking to the brain and heart, respectively. Here, we present data showing that another selected agonist antibody, P1 antibody, induces the migration of cells to the pancreatic islets and differentiates human stem cells into beta-like cells. Interestingly, our results suggest the purified P1 Ab induces beta-like cells from fresh, human CD34+ hematopoietic stem cells and mouse bone marrow. In addition, stem cells with P1 Ab bound to expressed periostin (POSTN), an extracellular matrix protein that regulates tissue remodeling, selectively migrate to mouse pancreatic islets. Thus, these results confirm that our in vivo selection system can be used to identify antibodies from our library which are capable of inducing stem cell differentiation and cell migration to select tissues for the purpose of regenerating and remodeling damaged organ systems.

Ectopic insulinoma in a dog with insulin-induced hypoglycemia: a case report

  • A 7-year-old spayed female Shih Tzu dog was presented for evaluation of recurrent hypoglycemia. Serum insulin levels during hypoglycemia were 35.3 µIU/mL. Ultrasonography and computed tomography showed a mesenteric nodule between the kidney and the portal vein, but no pancreatic mass was observed. During surgery, the nodule had neither anatomical adhesions nor vascular connections to the pancreas. Pancreatic inspection and palpation revealed no abnormalities. Hypoglycemia improved after resection of the nodule. Histopathological examination confirmed the nodule to be an islet cell carcinoma. Although extremely rare, ectopic insulinoma should be considered as a possible cause of insulininduced hypoglycemia in dogs.
