Arterial pulse palpation is an important diagnostic method in Oriental Medicine, particularly for obtaining information about a patient's health conditions or illness, or for confirming a diagnostic conclusion based on the patient's pulsation. The pulse analyzer is both a leading Oriental Medical equipment and a promising tool with such a strong industrial ripple effect that it was selected as one of the four strategic tools for world Oriental Medical instrument market domination at a recent survey. Although various pulse analyzers had been developed, however, most of these were not widely used for clinical diagnosis, due perhaps to lack of the appliance's reliability caused by its inability to reflect the requirements of the clinicians. Thus, in this thesis, the clinical requirements for the pulse analyzer were identified and analyzed by conducting a questionnaire survey among Oriental Medicine clinicians. By looking into the basic functions of a pulse analyzer, the required measurement time, and the medical insurance fee required were determined and among others, the appliance's specific requirements were determined. Moreover, by investigating on the latest patent trend, the technical elements that are needed for the development of a next-generation pulse analyzer were identified. Through these processes, the flow of the technology that must be developed for the pulse analyzer was determined, and the direction for the development of the specific pulse analyzer hardware, sensor, and diagnostic algorithm was identified and proposed.
This study was conducted to compare the expression pattern of the specific factors associated with pregnancy and angiogenesis during early pregnancy in Hanwoo. Synchronized female Hanwoo ($4{\sim}6$ year-old) were inseminated artificially. After 10 weeks after artificial insemination (AI), the pregnancy was tested by rectal palpation method. Three pregnant and non-pregnant Hanwoo were used in this experiment, respectively. The plasma progesterone level was measured by ELISA. Western blot analysis was performed to detect the expression of pregnancy associated glycoprotein (PAG) or angiogenic factors (VEGF, B-FGF, ANP-1, and TIE-2). The plasma P4 level was increase gradually in pregnant group and maintained high level. The concentration of PAG was significantly higher from $5^{th}$ weeks in pregnant group compared to that of non-pregnant group (p<0.05). The concentrations of the VEGF (p<0.05), B-FGF (p<0.05), and ANP-1 (p<0.05) were significantly increased from $6^{th}\;or\;7^{th}$ week after AI in pregnant group, respectively. And the intensity of TIE-2, ANP-1 receptor, was well matched with ANP-1 (p<0.05). Taken together, it can be postulated that the blood vessels connected with fetus and dam were formed dramatically around 40 days after AI, because the expression levels of the angiogenic factors were increased significantly from this time in pregnant Hanwoo.

Objective : To evaluate the potential effects of risedronate (RIS) which shows a higher anti-resorptive effect among bisphosphonates, after a posterolateral lumbar intertransverse process spinal fusion using both autograft and allograft in a rat model. Methods : A totoal of 28 Sprague-Dawley rats were randomized into 2 study groups. A posterolateral lumbar intertransverse process spinal fusion was peformed using both autograft and allograft in a rat model. Group I (control) received 0.1 mL of steril saline (placebo) and Group II (treatment) received risedronate, equivalent to human dose (10 ${\mu}g$/kg/week) for 10-weeks period. Results : The fusion rates as determined by manual palpation were 69% in the group I and 46% in the group II (p = 0.251). According to radiographic score, the spinal segment was considered to be fused radiographically in 7 (53%) of the 13 controls and 9 (69%) of the 13 rats treated with RIS (p = 0.851). The mean histological scores were 5.69 ${\pm}$ 0.13 and 3.84 ${\pm}$ 0.43 for the control and treatment groups, respectively. There was a significant difference between the both groups (p = 0.001). The mean bone density of the fusion masses was 86.9 ${\pm}$ 2.34 in the control group and 106.0 ${\pm}$ 3.54 in the RIS treatment group. There was a statistical difference in mean bone densities of the fusion masses comparing the two groups (p=0.001). Conclusion : In this study, risedronate appears to delay bone fusion in a rat model. This occurs as a result of uncoupling the balanced osteoclastic and osteoblastic activity inherent to bone healing. These findings suggest that a discontinuation of risedronate postoperatively during acute fusion period may be warranted.
Cellulitis is a infectious disease characterized by acute purulent inflammation clinically manifested by erythema, pain or heating sensation caused by palpation, chilling sign and mild to moderate fever. in western medicine most part of the treatment is focused on injecting routine antibiotics hoping for the pathogen(in the case bacteria) to be treated but instead causing the bacteria to be resistant to antibiotics and consequently leads to longer admission. In oriental medicine Cellulitis belongs to the cathegory of Ong(癰), Bal(發) or Dandok(丹毒) on symptoms. The chief cause of acute Cellulitis is the evil of wet and heat blended and the postraumatic infection evil. We experienced a case of acute cellulitis defined as Bicheonbal(비천발) and Chokbaebal(足背發). We attempted to use both methods using acupuncture, herbal medicine(Gamidangkwuijeomtong-tang(加味當歸拈痛湯)) as well as routine antibiotic treatment and as a result we have achieved remarkable results in laboratory tests though there was no difference in shortening the curing process compared to the average time that it take to cure when admitted to western medical center.
Milk samples(n = 78) were taken 19d, 20d, 21d, 22d after artificial insemination(AI) for early pregnancy diagnosis by using the Enzyme immunoassay(EIA) kit. The progesterone ($P_4$) concentration in the whole milk was measured on the same day of pregnancy diagnosis. Rectal palpation(RP) was accomplished between 60d and 70d after AI to estimate the ovary condition and pregnancy status. Milk progesterone concentrations measured by Radio-immunoassay(RIA) method, in the pregnant cows at 17d, 19d, 21d after insemination were $17.10{\pm}0.91$, $17.60{\pm}0.46$, and $18.43{\pm}0.79nmol/l$, whereas those in the not-pregnant cows were $6.57{\pm}1.03$, $2.63{\pm}0.29$, and $0.67{\pm}0.08nmol/l$, respectively. When the progesterone concentration was less than 7 nmol/l, the color of the EIA kit was lighter and when the progesterone concentration was ${\geq}16nmol/l$, the color of the EIA kit was darker compared to the standard color. The detection rates of error by judging the color differences were 5.1% and 20.7%, respectively. In the early pregnancy diagnosis by the EIA kit and RIA method, the accuracy rates in the pregnancy of cows were 82% and 87%, and those in not-pregnant cows were 86% and 91%, respectively. For ovarian status estimated by the RIA method and certified by RP, the accuracy rates of the ovarian atrophy, follicular cyst and luteal cyst were 80, 91 and 83% and the progesterone concentrations were 2.51, 2.03, and 26.7 nmol/l, respectively.

Background: We aimed to investigate the epidemiological, clinical, paraclinical, and treatment aspects of elastofibroma dorsi through a retrospective study of 76 patients who underwent surgery between January 2008 and December 2012 in our department. Methods: Our study is retrospective between January 2008 and December 2012. We admitted 79 patients with a subscapular mass, and only 76 patients had ED. The others (n=2) had high associated risk of anesthesia and were managed by a medical treatment and one patient had a subscapular sclerotic hemangioma. Results: The average age of the patients was 49 years (range, 38 to 70 years), with a female predominance (54 females and 22 males). Subscapular location was constant. The right, left, and bilateral form was noted in 41, 15 and 20 cases, respectively. The diagnosis was clinical in 60 cases. Ultrasound and computerized tomography scans confirmed the diagnosis of an ill-defined mass in a subscapular location in all cases. Surgical treatment consisted of complete resection of the mass. The clinical diameter of the mass remained significantly lower than that of the surgical specimen (7 cm versus 12 cm) because the major hidden part of the mass in the subscapular area was inaccessible to palpation. Complications were noted in 9 cases (11.8%), seroma in 8 cases (10.5%), infection of wound site in 4 cases (5%), and parietal textilome in one case (1%). No case of recurrence was noted. Conclusion: Surgery of elastofibroma is unique because of the subscapular location of the parietal tumor, whose histological fibrous nature makes it very adherent to the chest wall.
The present study examined the follicular populations prior to and during superovulation and investigated their relationship with superovulatory response in crossbred cattle. Eleven animals were administered i.m. 8 doses of Folltropin of 2.5 ml each (1.75 mg/ml) spread over 4 days beginning on Day 10 of oestrous cycle, and 30 and 20 mg Lutalyse was given alongwith the 5th and 6th injections of Folltropin, respectively, to induce luteolysis. The animals were artificially inseminated 48, 60 and 72 h after the first Lutalyse injection. The number of corpora lutea (CL) was recorded by palpation per rectum and embryos were recovered non-surgically on Day 6 (Day 0 day of superoestrus). The ovarian follicular population was examined by transrectal Ultrasonography 15 h prior to and 52 h after the first FSH injection, and then on the day of superoestrus and the day of flushing. The follicles were classfied on the basis of diameter as small (3-5 mm), medium (6-9 mm) and large (${\geq}10mm$). The total number of follicles increased significantly (p < 0.01) from $2.45{\pm}0.35$, 15 h prior to the first FSH injection to $8.09{\pm}1.12$, 52 h after the first FSH injection and then further to $13.27{\pm}1.89 $ on the day of superoestrus. A positive correlation was observed between the number of small follicles 15 h prior to the first FSH injection (r = 0.60, p < 0.05), the number of large follicles 52 h after the first FSH injection (r=0.59, p < 0.05) and the number of CL. The follicular population prior to and during superovulation was, however, not significantly different between high (> 6 CL) and low responders (${\leq}6CL$). The present study suggests that the follicular populations undergo dynamic changes during superovulation and that follicular populations prior to superovulation have a limited application as an indicator of the superovulatory response.
Research purpose : Review of the systematized medical theory of Huangdi within the first series of conversations including questions and answers between Leigong and Huangdi. Conclusions : 1. "Huang-di-Nei-Jing" records the 5 theories of Huangdi's system of thinking (黃帝學派). Among these, the contents of the questions and answers with Leigong led to the establishment of the first of Huangdi's treatises. 2. In the questions and answers exchanged between Leigong and Huangdi, the Suwen (素問) Chapter deals with the areas of Yinyang, Zangfu, Cause and Mechanism of diseases, as well as medical ethics. Moreover, the "Lingshu (靈樞)" Chapter discusses distribution of meridian system, routes of circulation of energies, symptoms, acupuncture therapy and diagnosis, etc. 3. Characteristics of the medicine of Leigong and Huangdi can be summarized as follows: Firstly, 12 meridian systems of the "Jingmai (經脈)" section describes the meridian system that ceaselessly circulates in the shape of loops with the inner and outer aspects linked with each other. Secondly, regarding the methods of diagnosis, the "Yinyangleilun (陰陽類論)" section describes the method of diagnosis through palpation of pulse. In addition, the "Wuse (五色)" section describes visual diagnosis that uses five colors. The Wuhang combination of spleen, liver, kidney of "Shicongronglun (示從容論)" section also includes the Wuhang theory of "Jinwenshangshu (今文尙書)". The "Wuse" section discusses areas of the face and five colors, while the "Yinyangleilun" discusses the 'Yinyancixiongmaifa (陰陽雌雄脈法)', the "Jingmai" discusses the 'Yinyancunkoumaifa (人迎寸口脈法)', and the "Zhengsishilun" and "Fangchengshuailun" discuss 'chifuzhenfa (尺膚診法)'. Thirdly, Huangdi's school of thinking did not neglect bencao (本草) and tangye (湯液), which are discussed in the "Jiejingweilun" and "Shicongronglun" sections.
Objectives: To determine the tumor interstitial fluid pressure(TIFP) in patients with head and neck cancerand predict radiotherapy outcome.Materials and Methods: In 12 biopsy proven primary head and neck cancer patients with accessible by direct inspection and palpation, and of sufficient thickness(>1cm) to permit accurate needle placement, we measured TIFP at cervical lymph node before and during radiotherapy using a modified wick-in-needle technique. Tumor size was measured clinically and radiologically. Results: The mean preradiotherapy TIFP was 23.4mmHg. Preradiotherapy TIFP had significant relationship with tumor size(p=0.0009). Preradiotherapy TIFP was not different between complete response group and partial or less response group(p=0.114). Radiotherapy outcome was not different between group with above and group with below average TIFP(p=0.09). Conclusion: The mean TIFP was elevated with 23.4mmHg before radiation therapy. Preradiotherapy TIFP had significant relationship with tumor size. It is not definitive that TIFP could be prognostic indicator of radiation response.
Palpation of the pulse has been used in Korean traditional medicine since ancient times to assess physical health. Pulse wave contour may be obtained by measuring arterial pressure or blood volume change of skin. The latter is called as Photoplethysmography(PPG) or digital volume pulse(DVP). The PPG signal is measured by a device comprising an infrared light sourece and a photodetector. Although less widely used, this technique deserves further consideration because of its simplicity and ease of use. The contour of the PPG is formed as a result of a complex interaction between the left ventricle and the systemic circulation. It usually exhibits an early systolic peak and an early diastolic peak. the first peak is formed mainly by pressure trasmitted along a direct path from the left ventricle to the finger. The second peak is formed in part by pressure transmitted along the aorta and large arteries to sites of impedance mismatch in the lower body. The contour of the PPG is sensitive to changes in arterial tone and is influenced by ageing and large artery stiffness. Measurements taken directly from the PPG or from its second derivative can be used to assess these properties. In some mathematical approaches, the extraction of periodic components using frequency analysis was tried to analysis of the PPG. But we don't understand yet what kind of factor in the cardiovascular system or human body is related with the respective specific Fourier components of PPG. This review describes the background to measurement principles, representative contour, contour analysis and frequency domain analysis of PPG, and current and future.
