맥파 재현장치 개발 동향 (A review on development trends of arterial pulse simulators)

  • Objectives : Recently, for over ten years, various arterial pulse simulators have been developed for training of pulse palpation technique. In this paper, we summarized development trends of those apparatuses. Materials and methods : To search the cases of development, we mainly used China Knowledge Infrastructure (CNKI), National Discovery for Science Leaders (NDSL), Korean Traditional Knowledge Portal, Patent Search by Baidu (百度專利搜索), and some general search engines. Some domain cases introduced were collected based on personal experience. Results : It was found that there were two types of arterial pulse simulators. The first type uses fluids and pumps, while the second type uses actuators without fluid circulation parts. Conclusion : From year 2000, various arterial pulse simulators have been developed. In general, they could be classified two types. Further effort toward Maixiang (脈象) simulation and validation in field is needed.

감염 근관에서의 주요 병인균과 임상증상간의 관계에 관한 연구 (A STUDY ON THE RELATIONSHIP BETWEEN THE CLINICAL SYMPTOMS AND THE PREDOMINENT PATHOGENIC MICROFLORA IN INFECTED ROOT CANALS)

  • In the Infected root canals, the majority of the isolated bacteria are either strict anaerobes or microaerophilic organisms. Among thease, Black-pigmented Bacteroides species are the most important and have relation with clinical symptoms. The purpose of this study were to investigate on the 7 different types of bacteria which include Black-pigmented Bacteroides - Black--pigmented Bacteroides, Actinobacillus actinomycetemcomitans, Streptococcus, Capnocytophaga, Eiknella corrodens, Fusobacterium, Actinomyces - and the interrelationship between these 7 bacterical species, and to compare Black-pigmented Bacteroides with the clinical symptoms in infected root canals. The canal contents of 15 necrotized teeth with 8 clinical symptoms were sampled and cultured aerobically and anaerobically for growth in 7 selective agar plates for 7 bacterial species. Black-pigmented Bacteroides and Streptococcus subspecies were identified by biochemical tests. The results were as follows; 1. 70.51% of the bacteria isolated were anaerobes and 29.49% were aerobes. 2. B. loescheii, B. intermedius, B. denticola were isolated in 8 cases, Streptococcus was isolated in 8 cases, Fusobacterium was isolated in 6 cases, Actinomyces was isolated in 5 cases and Eiknella corrodens, Actinobacillus actinomycetemcomitans, Capnocytophaga were not isolated. 3. There were mutual inhibition of growth between Black-pigmented Bacteroides and Streptococcus. (P<0.01) But Actinomyces and Fusobacterium showed mutual aids for growth. (P<0.05) 4. Black-Pigmented Bacteroides was found to be related both to sinus tract formation and to apical sensitivity to palpation.(P < 0.05).

동결견 환자에 대한 물리치료사의 진단 및 치료방법 결정기준 (An analysis of determining factor for frozen shoulder patients diagnosis and treatment of physical therapists)

  • The purpose of this study was to determine the diagnosis and treatment patterns for frozen shoulder patients. It was used Delphi survey techniques and the subjects were 35 expect physical therapists who agreed participated in the study. The data were analyzed by the x2-test at 0.05 level of statistical significance. The results were as follows : 1. In accordance with open ended question, 11 diagnosis and 14 treatment are collected in the first investigation. 2. In accordance with close ended question, they were selected 5 diagnosis and 5 treatments criteria from the results of the first replied questionnaire. 3. Finally, the third close ended questionnaire, they were selected 3 main driteria of the diagnosis and treatments from the results of the second investigation. The physical therapists prefer to 1) History 2) ROM test 3) Palpation & Inspection in the diagnostic field, and 1) EST and Convex-Concave rule 2) Codman's exercise and MFR 3) Deep friction massage in the treatment field.

측두하악 관절 장애의 평가 (Clinical Assessment of Temporomandibular Joint Dysfunction)

  • The Temporomandibural joint(TMJ) is one of the most frequently used joint in the body as $1,500{\sim}2,000$ times per day for the activities of chewing, swallowing, talking, yawing and sneezing. The TMJ are formed by condylar process of mandible and mandible fossa of temporal bone, separated by an articular disc. This articular disc divides into two cavities as upper cavity and lower cavity. The gliding movement occurs in the upper cavity of the joint, whereas hinge movement occurs in the lower cavity. The movements that are allowed at the TMJ are opening, closing, protrusion, retraction and lateral movement. A cause of TMJ dysfunction are capsulitis, internal derangement, osteoarthritis, rheumatoid arthritis, infection and inflammation near the joint, trauma on joint, ankylosis, subluxation or dislocation of joint, injury of articular disc, myositis, muscle contracture or spasm, myofascial pain dysfunction syndrome, dyskinesia of masticatory muscles, developmental abnormality, tumor, connective tissue disease, fibrosis, malocclusion, swallowing abnormality, wrong habits such as bite nail or hair, bruxism, psycological stress and Costen syndrome etc. Assessment of TMJ dysfunction consist of interview, observation, functional examination, palpation, reflex test, joint play test, electromyography and radiologic examination and behavioral and psycological assessment etc.

3D-printing Bone Model for Surgical Planning of Corrective Osteotomy for Treatment of Medial Patellar Luxation in a Dog

  • A 2-year-old, castrated male Chihuahua dog was referred for revision surgery for reluxation of the patella following surgery for medial patellar luxation (MPL) of the left stifle joint. On general inspection, the patient showed bilateral hindlimb weight-bearing lameness. On physical examination, bilateral non-reducible MPL was detected through palpation. Radiographs revealed bone deformities of both hindlimbs. Computed tomography (CT) was applied for a three-dimensional (3D) printing bone model to establish an accurate surgical plan. The bone plate was pre-contoured over the 3D-printing bone model after execution of corrective osteotomy and sterilized prior to use in surgery. Corrective osteotomy was performed through a staged, bilateral procedure. The patient showed improvement of limb function following surgery without reluxation of the patella. The use of 3D-printing bone model for accurate surgical planning of corrective osteotomy appears to be effective in increasing the accuracy of surgery. That may lead to successful surgical outcomes.

Mycobacterium avium subsp. avium infection in a lineolated parakeet (Bolborhynchus lineola)

  • A 2-year-old lineolated parakeet (Bolborhynchus lineola) was presented with abdominal distention and respiratory distress for two months. The bird was poorly fleshed and the liver was enlarged on coelomic palpation. Plain and contrast radiographic examinations exhibited hepatomegaly and distended intestinal loop, which compromised the air sacs. Multifocal hyperechogenecity was observed in the liver on ultrasonography. Postmortem gross examination revealed hepatomegaly with numerous pinpoint tan foci in the hepatic parenchyma and distended small intestine filled with adult ascarids. Microscopically, granulomatous hepatitis and enteritis infected by intrahistiocytic acid-fast bacilli were evident. Polymerase chain reaction indicated that the acid-fast bacilli were Mycobacterium avium subsp. avium.

편마비 환자의 견갑골 대칭적 정렬상태가 선 자세에서 체중부하지지율에 미치는 영향 (The Effects of Symmetrical Scapular Alignment on Weight Bearing of Hemiplegic Patients During Standing Position)

  • Purpose: This study examined the effects of the symmetrical scapular alignment on the weight bearing of hemiplegic patients in the standing position. Methods: PALM (PALpation Meter) test and Gaitview AFA-50 were used to measure the skeletal alignment of the scapula and the weight bearing loaded on the affected and unaffected sides before and after training. The data was analyzed using a paired t-test on the SPSS 12.0 program for descriptive statistics. A p value <0.05 was considered significant. Results: The mean difference in the scapular alignments on sup. & mid. & inf. area in the hemiplegic side before and after training was $0.93\pm0.50cm$ and $0.58\pm0.43cm$, respectively. There was a significant decrease after training rather than that before (p<0.05). The mean weight bearing on the foot area in the hemiplegic patients before and after training was $9.12\pm5.51N/cm^2$ and $4.36\pm4.11N/cm^2$, respectively,. There was a significant decrease after training rather than that before (p<0.05). The scapuar alignments and weight bearing data on the standing position were grouped around the average (to central point) in the distribution graph. Conclusion: These findings suggest that the symmetrical scapular alignment can serve as an effective means of improving the weight bearing ability of hemiplegic patients.

소 우정란의 Bisection과 이식에 관한 연구 (Studies on Bisection of Bovine Embryos and Embryo Tranfer)

  • These experiments were carried out to develop the practical technique for the production of identical twins in cattle. Morula and blastocyst stage embryos collected from superovulated donors were bisected into halves by micromanipulation. The resulting demi-embryos were transferred to the uterine horn ipsilateral to the corpus luteum of synchronous recipients. The viability of demi-embryos after splitting was also evaluated by culturing demi-embryos with and without a zona-pellucida. The results obtained in these experiments were summarized as follows : 1. Of total 132 embryos collected by superovulation from 29 donors, 37 embryos were morular and 30 at blastocyst stages. 2. Total 111 demi-embryos were produced from 67 embryos by bisection and 98% of those were normal in morphology. 3. The viability of the demi-embryos cultured with zona-pellucida ranged from 70 to 76.5% and that of the demi-embryos without from 53.8 to 69.2%. 4. The viability of demi-embryos obtained from morula was 63.6% and that of demi-embryos from blastocyst was 73.3%, respectively. 5. 35 demi-embryos were transferred to 21 recipients, 7 of which were confirmed to be pregnant by rectal palpation at 55∼60 days after embryo transfer. One of them produced a calf and 6 are still on pregnancy.

Formation of an intestine-cartilage composite graft for tracheal reconstruction

  • Purpose; Tracheal transplantation is necessary in patients with extensive tracheal stenosis, congenital lesions and many oncologic conditions but bears many critical problems compared with other organ transplantations. The purpose of this study was to make an intestine-cartilage composite graft for potential application for tracheal reconstruction by free intestinal graft. Methods; Hyaline cartilage was harvested from trachea of 2 weeks old New Zealand White Rabbits. Chondrocytes were isolated and cultured for 8 weeks. Cultured chodrocytes were seeded in the PLGA scaffolds and mixed in pluronic gel. Chondrocyte bearing scaffolds and gel mixture were embedded in submucosal area of stomach and colon of 3kg weighted New Zealand White Rabbits under general anesthesia. 10 weeks after implantation, bowels were harvested for evaluation. Results; We could identify implantation site by gross examination and palpation. Developed cartilage made a good frame for shape memory Microscopic examinations include special stain showed absorption of scaffold and cartilage formation even though not fully matured Conclusion; Intestine-cartilage composite graft could be applicable to future tracheal substitute and needs further Investigations.

슬개골 탈구 환견에서 지대 중첩, 활차구 성형 및 경골결절변위술의 복합수술 평가 (Evaluation of Combination of Retinacular Imbrication, Trochleoplasty and Tibia Tuberosity Transposition in Dogs with Patellar Luxation)

  • The purpose of this study is to evaluate the results obtained from three surgical techniques (retinacular imbrication, trochleoplasty and tibia tuberosity transposition), which were simultaneously applied for correction of patellar luxation. Eleven, (6 to 108 months) old dogs were presented. The dogs showed, medial patellar luxation (17 stifle joint) and lateral patellar luxation (2 stifle joint) ranging from grade 1 to 4. By palpation and radiographic view, all 11 dogs were diagnosed as patellar luxation. Bilateral patellar luxation was diagnosed in 8 dogs. After operation, the dogs were monitored daily for 7 days and on the 10th, 15th, 20th, 25th and 30th days. Lameness progressively decreased in all dogs around 30 days after surgery, and the patients were showed normal walking. Normal walking was possible in 2 dogs after 4 days, in a dog after 5 days, a dog after 7 days, 3 dogs after 10 days,2 dogs after 15 days, a dog after 25 days and a dog after 30 days (Mean$\pm$S.E: 12.27$\pm$2.57). The recovery took 14 days in dogs with bilateral patellar luxation, and 9 days in dogs with unilateral patellar luxation. Trend of decrease of lameness grade in bilateral patellar corrected dogs were similar to that in unilateral patellar corrected dogs. Swelling, pain and fever disappeared on 7 days. In conclusion, combination of tibial tuberosity transposition, retinacular imbrication and trochleoplasty is appropriate for dogs with patellar luxation.
