중환자실 완화의료 모델과 간호사의 역할 (Palliative Care Models in Intensive Care Units and Nurses' Roles in the Models)

  • Purpose: In Korea, about 30,000 people die in intensive care units annually. However, their quality of life at the end-of-life seems very low. The purpose of this study was to describe palliative care models that could be applied in intensive care units and examine nurses' roles in the models. Methods: A conventional literature review was performed focusing on palliative care in intensive care units and nursing roles in the approaches identified. Results: There are two different models regarding the implementation of palliative care approaches in the intensive care unit. Those are the consultative model and the integrative model. Each model has advantages and disadvantages. Therefore, an appropriate model must be chosen according to the setting. Nurses' roles could vary depending on the setting. Conclusion: Palliative care in the intensive care unit is important to improve patients' quality of life. Moreover, nursing roles are important in providing comprehensive palliative care in intensive care units.

Factors Influencing Compassion Fatigue among Hospice and Palliative Care Unit Nurses

  • Purpose: This descriptive study aimed to explore nursing workplace spirituality, end-of-life care stress, and resilience as factors influencing compassion fatigue among nurses working in hospice and palliative care units. Methods: Data were collected using a self-report questionnaire completed by 146 nurses at 14 hospice and palliative care institutions across South Korea who had worked in a hospice and palliative care institution for at least 6 months and had experience providing end-of-life care. Data were collected from February 25, 2019 to April 12, 2019, and analyzed using SPSS for Windows version 18.0. As appropriate, descriptive statistics, the t-test, analysis of variance, the Scheffé test, Pearson correlation coefficients, and stepwise multiple regression were used. Results: The survey results showed that factors influencing compassion fatigue were resilience, subjective health status, current satisfaction with the hospice ward, and end-of-life care stress. Higher levels of resilience, a subjective health status of "healthy", high levels of current satisfaction with the hospice ward, and lower levels of end-of-life care stress were associated with lower levels of compassion fatigue, explaining 42.9% of the total variance. Conclusion: The results of this study suggest that resilience is an important factor mitigating compassion fatigue among nurses at hospice and palliative care institutions. Therefore, intervention programs should be developed to reduce compassion fatigue.

Hospice and Palliative Care for Patients in the Intensive Care Unit: Current Status in Countries Other than Korea

  • Although most patients prefer dying at home, patients whose condition rapidly becomes critical need care in the intensive care unit (ICU), and it is rare for them to die at home with their families. Therefore, interest in hospice and palliative care for patients in the ICU is increasing. Hospice and palliative care (PC) is necessary for all patients with life-threatening diseases. The following patients need palliative care in the ICU: patients with chronic critical illnesses who need tracheostomy, percutaneous gastrostomy tube, and extracorporeal life support; patients aged 80 years or older; stage 4 cancer patients; patients with specific acute diseases with a poor prognosis (e.g., anoxic brain injury and intracerebral hemorrhage requiring mechanical ventilation); and patients for whom the attending physician expects a poor prognosis. There are two PC models-a consultative model and an integrative model-in the ICU setting. Since these two models have advantages and disadvantages, it is necessary to apply the model that best fits each hospital's circumstances. Furthermore, interdisciplinary decision-making between the ICU care team and PC specialists should be strengthened to increase the provision of hospice and palliative care services for patients expected to have poor outcomes and their families.

Current Status of the Pediatric Palliative Care Pilot Project in South Korea Based on the Experience of a Single Center

  • Purpose: We evaluated the status of patients enrolled in South Korea's pediatric palliative care pilot project based on the experience of a single center. This study examined factors related to end-of-life services and differences in medical costs. Methods: The medical records of 120 patients referred by a pediatric palliative care team were analyzed retrospectively. Data from July 1 to February 28, 2022 were collected and analyzed using the chi-square test and the Mann-Whitney U test. Results: Volunteer programs and psychological support (100%), family support and education (99.2%), and financial support through institutional linkage (62.5%) were provided to the participants. In the deceased group, there were no significant differences in general characteristics, which included age, gender, primary disease, religion, duration of hospitalization in an intensive care unit (ICU) and non-intensive care unit (non-ICU). However, the ICU group had fewer opportunities to access individual pain and physical symptom management than the non-ICU group and there were limitations in linking with external resources. Medical expenses were significantly different for the ICU group, with a 3-times higher average cost than the non-ICU group. Conclusion: Although an individualized approach is needed for each patient in pediatric palliative care, psychosocial care is essential. In addition, if early intervention for end-of-life pediatric patients is available from a palliative care team, the cost burden of medical care for patients and their families should be minimal.

병동형 호스피스 대상자를 위한 전인적 호스피스 간호중재 프로그램의 개발 (Development of Wholistic Hospice Nursing Intervention Program for In-patient of Hospice Palliative Care Unit)

  • People in the end of life and their families suffer in their physical disease and other aspects as a whole person. They need hospice care to palliate their total suffering in physical, emotional, social and also spiritual aspect through professional hospice team. To care their whole personal needs, hospice team must be a multi-discipline team which consists of medical doctors, nurses, social workers, pastors and volunteers. Recently those who die in hospice palliative care unit have trend to increase more than in home year by year. So it is necessary to develop the nursing intervention program to be performed by multi-discipline team approach for in-patient of hospice palliative care unit. The purposes of this study were to develop of wholistic hospice nursing intervention program for inpatient of hospice palliative care unit. The subjects of study were collected from 30 patients those who were over 18 years old and admitted in hospice palliative care unit of S hospital in P city with agreement in hospice palliative care in their terminal disease. The period of data collection was from December 15, 2003 to March 15, 2004. The result were as follows : 1. The result of Wholistic Hospice Nursing Program's development was as follow : A Wholistic Hospice Nursing Program was developed by me in this study is one of the service program for hospice palliative care unit. It was named as ‘Rainbow Program’ to be approached easily by hospice patients. The purposes of it are to improve the quality of life of the terminal patients with their dignity, to help them live in abundant and meaningful in their lives, to care them in peaceful in dying process with understanding them in whole personal, and also to palliate the grief and suffering of the bereaved. It was provided by hospice professionals(nurses, medical doctors, social worker, pastors, art therapists) and volunteers those who were educated in hospice for multi-diciplinary team approach to collaborate with each role play I 20-30 minuters of each through visiting their rooms individually and a place of hospice palliative care unit of S hospital in P city. The subjects of it were the terminal patients those who admitted hospice palliative care unit and their familes. with agreement in hospice palliative care in their terminal disease. The characteristics of it were multi-disciplinary team approach, whole personal care, individual care and total care according to their needs in their condition. The contents of it were pain control, symptom control, counseling patient, counseling family, hair cutting, hair shampooing, bed bath, recreation, taking a walk, event of culture(screen, recital, festival of praises, exhibition and so on), pastoral counseling, ritual service in bed, praying, service in bed, sing a worship praise, listening to the music, sharing remembrance of life, individual visiting music service(sing and praying), meditation Bible, art therapies(dance and drawing), social worker's counselling, confessing and sharing love and thanksgiving. The experimental group subjects participated in Wholistic Hospice Nursing Program which takes 120 minutes per session, total 10 sessions(total 1,200 minutes) altogether. In conclusion, this Wholistic Hospice Nursing Intervention can be used actively for whole personal well-being of the patients in hospice palliative in hospice palliative care unit and also applied in hospice practice as an useful model of multi-disciplinary team approach by hospice professionals.

종합병원 호스피스 병동부 단위공간에 관한 연구 (A Study on the Unit Spaces of Hospice Unit within a General Hospital)

  • The hospice movement evolved as an alternative to hospitals as places to die. Recently, the palliative care for dying and hospice has been developed rapidly and placed itself as one of the medical systems. But the studies on hospice program and establishment method in Korea are few or no in comparison with developed countries. Hospice unit within a general hospital is more efficient hospice type than other terminal care establishments, therefor it will be developed rapidly. With this in mind, this study puts elements of architectural planning on the hospice unit for the space requirements. Also, It is investigated the architectural conditions of hospice unit within a general hospital, analyzed the unit space and spatial composition in hospice unit. The purpose of this study is to propose the fundamental data and unit space for architectural plan and design.

신생아중환자실 간호사의 완화간호에 대한 지식과 태도 및 죽음에 대한 인식 간의 관계 (Relationship Among Nurses' Knowledge, Attitude Towards Palliative Care and Perception of Death in Neonatal Intensive Care Units)

  • Purpose: The purpose of this study was to investigate the relationship among knowledge and attitude toward palliative care and perception of death for Neonatal Intensive Care Unit (NICU) nurses. Methods: A cross-sectional descriptive study was conducted with 110 nurses who work in NICUs in South Korea. The participants filled out a questionnaire regarding their knowledge, attitude towards palliative care and perception of death. The attitude scale was divided into 3 subscales: comfort level, nurses' role and nurses' involvement with family. Results: Comfort level regarding attitude towards palliative care was positively correlated with knowledge (r=.220, p=.016) and the perception of death (r=.194, p=.042). Nurses' role showed a positive correlation with perception of death (r=.395, p=.001). Conclusion: NICU nurses' knowledge of palliative care was below standard across the board, implying that there is a definite need for palliative care education for nurses. The education program for palliative care should include a section that focuses on fostering a positive perception of death as well as defining and delineating the role of nurses.

전인적 호스피스 간호중재 프로그램이 호스피스완화의료병동 입원 환자의 자아존중감과 영적안녕에 미치는 효과 (Effects of Holistic Hospice Nursing Intervention Program on Self Esteem and Spiritual Well-being for Inpatients of Hospice Palliative Care Unit)

  • 목적: 본 연구는 단일군 사전 사후 원시실험설계 연구로서 호스피스완화의료병동 입원 환자를 위한 전인적 호스피스간호중재 프로그램("무지개 프로그램")의 자아존중감과 영적안녕에 대한 효과를 검증하고자 하였다. 방법: 2004년 4월 6일부터 2005년 4월 20일까지 경북포항시 소재 선린병원의 호스피스완화의료병동에 입원한 만18세 이상의 성인 환자로서 의사소통이 가능하여 연구참여에 서면 동의한 27명을 대상으로 사전조사 후 2주간, 총 10회(회당 120분)로 구성된 전인적 호스피스 간호중재 프로그램 제공 후, 사후 조사를 실시하였다. 효과 검정을 위해 자아존중감 측정 도구로는 성인용으로 수정보완된 Self Esteem Questionnaire (SEQ), 영적안녕 측정도구로는 Spiritual Well-being Questionnaire를 사용하였으며, 자료분석은 SPSS/WIN 12.0 프로그램을 이용하여 Paired t-test로 분석하였다. 결과: 1. 가설 1 '전인적 호스피스 간호중재 프로그램을 제공받은 호스피스완화의료병동 입원환자(이하 실험군)는 실험 전보다 실험 후의 자아존중감 정도가 높을 것이다'는 지지되었다(t=11.554, P<0.000) 2. 가설 2 '전인적 호스피스 간호중재 프로그램을 제공받은 실험군은 실험 전보다 실험 후의 영적안녕 정도가 높을 것이다'는 지지되었다(t=6.387, P<0.000). 결론: 전인적 호스피스간호중재 프로그램은 호스피스완화의료병동 입원환자의 자아존중감과 영적안녕을 증진시키는 데 효과적이므로, 호스피스완화의료 병동에 입원한 말기환자를 위해 임상 실무에서 적용 가능하며, 호스피스 전문직의 다학제적 팀 접근 모델로 연구, 교육 측면에서도 유용하리라 생각한다.

간호사의 소아청소년 호스피스완화의료에 대한 지식 및 태도 (Nurses' Knowledge of and Attitudes toward Pediatric Palliative Care of Korea)

  • 목적: 본 연구는 말기아동 간호에 대한 교육자를 위한 교육프로그램인 ELNEC-PPC에 참여한 간호사의 소아청소년 호스피스완화의료(PPC)에 대한 지식 및 태도를 확인하기 위하여 수행되었다. 방법: 2012년 ELNEC-PPC 교육자를 위한 교육과정에 등록한 간호사 중 연구참여에 동의하고 서명날인 후 설문조사에 응답한 총 196명의 자료를 분석하였으며, 소아청소년 호스피스완화의료에 대한 지식과 태도는 20문항의 7점 척도로 측정하였다. 결과: 학력수준과 소아과병동 및 호스피스 근무경험에 따라 PPC에 대한 지식 정도에 차이가 있었다. 소아과 근무경험, PPC 경력 및 교육경험 이수 정도에 따라 소아 호스피스완화의료 태도에 차이가 있었으며, 기혼 간호사는 자녀의 완화의료서비스 결정에 대한 보호자의 권리에 대해 더 긍정적이었으며, 석사 이상군은 PPC에 대한 지식과 태도 정도가 높았다. 결론: PPC에 대한 간호사의 지식 및 태도를 높이기 위해서는 일반간호사를 대상으로 하여 표준화된 교육이 필요하다.

Characteristics Associated with Survival in Patients Receiving Continuous Deep Sedation in a Hospice Care Unit

  • Continuous deep sedation (CDS) is an extreme form of palliative sedation to relieve refractory symptoms at the end of life. In this study, we shared our experiences with CDS and examined the clinical characteristics associated with survival in patients with terminal cancer who received CDS. We conducted a chart audit of 106 consecutive patients with terminal cancer who received CDS at a single hospice care unit between January 2014 and December 2016. Survival was defined as the first day of admission to the date of death. The associations between clinical characteristics and survival were presented as hazard ratios and 95% confidence intervals using a Cox proportional hazard model. The mean age of participants was 65.2 years, and 33.0% (n=35) were women. Diazepam was the most commonly administered drug, and haloperidol or lorazepam were also used if needed. One sedative was enough for a majority of the patients. Stepwise multivariate analysis identified poor functioning, a high Palliative Prognostic Index score, hyperbilirubinemia, high serum ferritin levels, and a low number of sedatives as independent poor prognostic factors. Our experiences and findings are expected to be helpful for shared decision-making and further research on palliative sedation.
