A Study on the Dental Anomalities and Site of Cleft Associated with Cleft Lip and/or Palate (순ㆍ악ㆍ구개파열 환자의 치아수의 이상과 악파열 부위에 대한 연구)

  • The purpose of this study is to investigate possible correlation between the dental anomalies and site of cleft in cleft lip and palate. In this study, 142 patients who had cleft lip and/or cleft palate were examined. The results are as follows. 1. The incidence of missing tooth was high in the permanent dentition as compared to the incidence in the deciduous dentition. 2. There was not much difference of incidence of supernumerary tooth between deciduous and permanent dentition in the group of patients who had cleft lip and jaw with or without cleft palate. 3. In the group of patients who had cleft lip and jaw with or without cleft palate, the frequency of incidence of cleft sides was higer in unilateral than bilateral cases. And, incidence of left sides was higher than right sides. 4. The type of cleft between central incisor and canine with missing lateral incisor was most frequent in permanent dentition and the type of cleft between central and lateral incisor was most frequent in deciduous dentition. 5. The type of cleft associated with tooth position in deciduous dentition was not almost the same in the succeeding permanent dentition.

Delayed Unilateral Soft Palate Palsy without Vocal Cord Involvement after Microvascular Decompression for Hemifacial Spasm

  • Microvascular decompression is a very effective and relatively safe surgical modality in the treatment of hemifacial spasm. But rare debilitating complications have been reported such as cranial nerve dysfunctions. We have experienced a very rare case of unilateral soft palate palsy without the involvement of vocal cord following microvascular decompression. A 33-year-old female presented to our out-patient clinic with a history of left hemifacial spasm for 5 years. On postoperative 5th day, patient started to exhibit hoarsness with swallowing difficulty. Symptoms persisted despite rehabilitation. Various laboratory work up with magnetic resonance image showed no abnormal lesions. Two years after surgery patient showed complete recovery of unitaleral soft palate palsy. Various etiologies of unilateral soft palate palsy are reviewed as the treatment and prognosis differs greatly on the cause. Although rare, it is important to keep in mind that such complication could occur after microvascular decompression.

  • Velopharyngeal insufficiency (VPI) is a phenomenon that can occur due to anatomical or neurological causes of the soft palate. VPI can make the patient difficult to articulate through hypernasality and nasal emission. There has been needed the customized treatment, as VPI can occur for many causes. We present the case of 21-year-old male who took palate plastic surgery 20 years ago for congenital submucosal cleft palate. As he had poor contraction of both lateral side of velopharynx, he was diagnosed with coronal type VPI. Through sphincter pharyngoplasty, he can obtain improvement of articulation accuracy. We would like to share this challenged case.

THE PREVALENCE OF CLEFT LIP AND/OR CLEFT PALATE IN KOREAN MALE ADULT (한국인 성인 남자에게 구순열 및 구개열의 유병률에 관한 연구)

  • Cleft lip and/or palate(CLP) is the most common congenital deformity in the craniofacial region. For a practical public health system for these patients it is necessary to have an understanding on the prevalence of CLP. However, it was difficult to estimate the exact number of CLP patients due to problems in sample selection, higher miscarriage and still birth rate, difficulty in classification, and adoptions to foreign countries. Therefore this study was to estimate the prevalence of CLP and the rate of orthodontic treatment, which is usually necessary in cleft lip and/or palate patients. The samples consisted of 218,322 Korean male adults from Seoul, Kwangju, Taegu and Pusan, all born in 1979. The screening method for recognizing the CLP patients was proceeded in steps and the results are as follows. 1. The prevalence of cleft lip and/or palate in Korean male adults born in 1979 was 0.65 out of the 1000 samples. 2. In the anteroposterior aspect of the 1000 samples, the prevalence of cleft lip, cleft lip and palate, and cleft palate was 0.26, 0.36 and 0.03 respectively. 3. In the transverse aspect of the 1000 samples, the prevalence of left, right and bilateral cleft was 0.35, 0.16, 0.12 respectively. The cleft in the left showed a much higher prevalence than in the rirht, while bilateral cleft showed a lower prevalence than unilateral cleft. 4. The orthodontic treatment rate of Korean male adults among cleft lip and/or palate Patients was $35\%$, and it was in the order of cleft lip and Palate, cleft lip, and cleft Palate, being $67\%,\;29\%\;and\;29\%$ respectively. The orthodontic treatment rate in patients with the more severe cleft lip and palate was higher than in patients solely having cleft lip or cleft palate.

  • A 20 year-old cleft lip and palate patient came for occlusal rehabilitation, but the constricted maxilla and early loss of posterior teeth called for an unusual treatment modalities. Distraction osteogenesis in the edentulous areas followed by artificial bone graft, dental implant along with orthodontic tooth movement were planed. Multidisciplinary treatment enabled both esthetic and functional oral rehabilitation of this patient.

  • The authors had treated a patient, 54years korean male, with malignant melanoma, egg sized, in the upper left molar region and hard palate. In the patient, radical operation was made by subtotal maxilectomy and prophylactic neck dissection. We could observe complete healing in the patient with malignant melanoma involving upper left molar region and hard palate.

  • The purpose of this study was to investigate whether CPAP therapy was effective for reducing the degree of hypernasality in individuals with repaired cleft palate and whether the efficacy of CPAP therapy was maintained. Five individuals with cleft palate participated in an 8-week home-based CPAP program. Results from perceptual evaluation of hypernasality and nasalance scores before and after CPAP therapy and at the follow-up speech evaluation were compared. The results of the study showed that the responses of the CPAP therapy were various among individuals. Three individuals exhibited reductions in the degree of perceived hypernasality, while nasalance scores in all individuals decreased after the therapy. The results showed that the effect of CPAP therapy was generally maintained until approximately three months after the completion of CPAP therapy.

  • Some children with Cleft Palate have shown a speech disorders after repaired surgical operation. A diagnostic evaluation of speech in children with cleft palates is important in preventing speech disorders. However, standard speech evaluation form for children with cleft palates has not yet developed in Korea.   • Congenital palatal fistulas are rare, and few cases have been reported. Most reported cases present with a submucous cleft palate. In terms of etiology, whether the fistula is congenital or acquired has been debated. Moreover, there is not a generally accepted surgical procedure for repair of palatal fistulas. We present a case of a congenital palatal fistula with a submucous cleft palate that was successfully treated with a Furlow double - opposing Z - plasty. We discuss palatal fistulas with a review of the literature.
