구개근 절제술을 이용한 상악 제일 대구치의 치료 (Palatal root resection of compromised maxillary first molars)

  • Purpose: Root resection can be a valuable procedure when the tooth in question has a high strategic value. The prognosis of root resection has been well documented in previous studies, but the results focused on the palatal root resection have not been discussed in depth. I represent here the short term effectiveness of palatal root resection of maxillary first molars. Methods: Palatal root resection was performed on maxillary first molars of three patients. All the palatal roots were floating state on the radiographic finding and showed full probing depth and purulent exudation at initial examination. Reduction of palatal cusp and occlusal table was performed concomitantly. Endodontic therapy was completed after root resection. Results: Compromised maxillary first molars were treated successfully by palatal root resection in 3 cases. The mobility of resected tooth was decreased a little bit. The probing pocket depth of remaining buccal roots was not increased compared to initial depth. All the patients satisfied with comfort and cost effective results and the fact they could save their natural teeth. Conclusions: Within the above results, palatal root resection is an effective procedure treating compromised maxillary first molar showing advanced palatal bone loss to root apex with or without pulp involvement when proper case selection is performed.

Radix mesiolingualis and radix distolingualis: a case report of a tooth with an unusual morphology

  • Variation in the root and canal morphology of the maxillary first molars is quite common. The most common configuration is 3 roots and 3 or 4 canals. Nonetheless, other possibilities still exist. The presence of an additional palatal root is rather uncommon and has been reported to have an incidence of 0.06 - 1.6% in varying populations studied. Whenever two palatal roots exist, one of them is the normal palatal root, the other is a supernumerary structure which can be located either mesiolingually (radix mesiolingualis) or distolingually (radix distolingualis). This case report describes successful endodontic treatment of a maxillary first molar with radix mesiolingualis and radix distolingualis. Identification of this variation was done through clinical examination along with the aid of multiangled radiographs, and an accurate assessment of this morphology was made with the help of a cone-beam computed tomography imaging. In addition to the literature review, this article also discusses the epidemiology, classifications, morphometric features, guidelines for diagnosis, and endodontic management of a maxillary first molar with extra-palatal root.

소아에서 발생한 치성 기원 구개 농양의 치험례 (TREATMENT OF PALATAL ABSCESS OF ODONTOGENIC ORIGIN IN CHILDREN: CASE REPORTS)

  • 구강내 대부분의 감염은 치성 기원으로 치성 감염은 초기 감염부위로부터 저항이 제일 적은 경로를 따라 확산된다. 상악에서 치근단과 피질골 사이의 두께가 구개측보다 협측에서 더 얇기 때문에 감염이 구개측 보다는 협측으로 더 쉽게 확산되며, 구개 치근보다 협측 치근이 좁아서 근관치료의 실패가 협측 치근에서 많이 발생한다. 따라서 구개 농양의 발생은 협측 농양에 비해 흔치 않다. 구개 농양은 구개부에 발생하는 비치성 기원의 양성 또는 악성 타액선 신생물, 양성 신경 종양, 낭종 등과 감별진단이 어렵다. 따라서 소아에서 구개종창이 관찰될 경우 치성 기원의 구개 농양을 조기에 진단해 감염이 전신적으로 확산되는 것을 방지 해야 한다. 본 증례에서는 유치의 통증과 구개부 종창을 주소로 내원한 환아에서 치성 기원의 구개 농양이라고 진단하여 해당치아를 발치하고 항생제를 처방하였다. 치료 후 구개 종창이 해소되어 보고하는 바이다.

Maxillary Posterior Crossbite Appliance의 적용시 응력 분포에 관한 광탄성법적 연구 (Photoelastic evaluation of Maxillary Posterior Crossbite Appliance)

  • 본 연구는 협측 이소맹출된 상악 제2대구치의 협측 반대교합을 개선하기 위해 사용되는 maxillary posterior crossbite appliance에 의해 상악 제2대구치의 치근단과 그 주위의 치조골에 발생되는 응력분포를 알아보기 위해 광탄성법을 이용하여 분석하였다. 상악의 치조골을 재현하기 위해 PL-3형의 epoxy resin과 PL-3보다 경질인 레진치아를 사용하여 협측 이소맹출된 상악 제2대구치를 광탄성모형으로 재현하였다. 광탄성 모형상에 maxillary posterior crossbite appliance를 적용하고 힘을 가하기 전과후의 응력분포를 알아보기 위해 원형편광기를 사용하여 모형의 전후방에서 관찰하였다. 이상의 연구를 통해 얻어진 결과는 다음과 같다. 1. 상악 제2대구치의 구개면에 힘을 가한 경우, 협측과 구개측 치근첨에는 무늬차수 0.6차와 치경부에는 무늬차수 0.9차의 응력이 집중이 되어 나타났고 회전중심이 구개측 치근은 치근첨에, 협측 치근은 치근 1/4부위에 발생하였으며 이로 인해 제2대구치에 구개측으로의 비조절성 경사이동 및 회전력이 발생하였다. 2. 상악 제2대구치의 협측면에 힘을 가한 경우에는 구개면에 힘을 가한 경우 보다 협측과 구개측의 치근첨에 더 많은 무늬차수 2.2차의 응력이 발생하였다. 또한 치근하방의 치조골 부위에 응력의 발생과 함께 구개측 치근의 회전중심은 사라지고 협측 치근은 치근첨으로 이동하여 이로 인해 구개측으로의 조절성 경사이동 및 함입력이 발생하였다. 3. 상악 제2대구치의 협측이나 설측에 힘을 가한 경우, 고정원인 제1대구치는 치근첨부위에 소량의 응력이 발생하였고, 편측의 제2대구치에만 힘을 가한 경우보다 양측의 제2대구치에 동시에 힘을 가한 경우에서 더 많은 양의 응력이 제1대구치에 발생하였다. 이상의 결과는 협측 이소맹출된 상악 제2대구치의 구치부 협측 반대교합을 개선하기 위해 사용되는 maxillary posterior crossbite appliance를 적용할 때, 제1대구치의 고정원 상실을 최소화하기 위해서는 편측의 제2대구치를 먼저 개선시키고 그 후 반대측 제2대구치의 개선을 도모하는 것이 바람직하다는 것을 의미한다. 또한 구개측면보다는 협측면에서 견인력을 가하여 제2대구치에 함입력과 구개측으로의 조절성 경사이동을 발생시키는 것이 구치의 정출 및 회전에 의해 야기될 수 있는 교합장애를 방지할 수 있으리라 사료된다.

  • 이 임상증례의 목적은 상악 제2대구치의 해부학적 형태를 computed tomography (CT)를 이용해 확인하여 근관치료에 활용한 증례이다. 이번 증례에서는 두개의 구개치근을 갖는 상악 제 2대구치에서 성공적인 근관치료를 위해서 CT를 활용하여 해부학적 형태를 확인하여 활용하였다. Cone beam computed tomography (CBCT)의 사용은 구개치근과 협측치근이 겹쳐보이는 치근단 방사선 사진의 한계를 극복할 수 있게 한다.

  • Rapid palatal expansion(RPE) with the tooth-born appliance is not sufficient to apply to the patients with periodontal problem or insufficient tooth anchorage, and it leads to tipping of the anchorage teeth and increasing teeth mobility and root resorption. To avoid these disadvantages, we present the case using palatal screws and custommade palatal expander. A 23-year-old patient underwent surgically assisted rapid maxillary expansion with the Hyrax expansion using 4 tent screws. The study models were used to measure the pre-/-post surgical width of the anterior and posterior dental arches with a digital sliding caliper. In the result, the custom-made palatal expander with 4 tent screws is suitable for delivering a force to the mid-palatal suture expansion. And it is low cost, small sized and simply applied. The results indicated that maxillary expansion with the custom-made palatal anchorage device is predictable and stable technique without significant complications in patients.

결합조직 채득방법에 따른 치은퇴축 회복의 비교연구 (THE COMPARATIVE STUDY OF ROOT COVERAGE EFFECT OF DIFFERENT CONNECTIVE TISSUE OBTAINING METHODS)

  • A mucogingival grafting procedure has been developed to cover denuded root surfaces. The subepithelial connective tissue graft is composed of a free connective tissue graft and an overlying pedicle graft. The source of connective tissue graft were trap door approach and thining of a full thickness palatal flap. The purpose of this study was compare a two different connective tissue obtaining method. In this study, where palatal pocket was present, pocket elimination was performed, and the tissue normally discarded after thinning of the palatal flap was used as a grafting material. The results were as follows : 1. The mean difference between trap door approach and thinning procedure for root coverage were $2.1{\pm}O.lmm$, $2.2{\pm}O.2mm$. 2. The mean difference between trap door approach and thinning procedure for pocket depth change were $O.2{\pm}O.lmm$, $O.2{\pm}O.2mm$. 3. The mean difference between trap door approach and thinning procedure for attachment gain were $2.1{\pm}O.2mm$, $2.4{\pm}O.2mm$. 4. The esthetics in recipient site, both color match and tissue contour, were acceptable to the patient in all cases. 5. Therefore, thinning procedure were similar to trap door approach in root coverage effect.

Morphological characteristics of the mesiobuccal root in the presence of a second mesiobuccal canal: a micro-CT study

  • Objectives: This study investigated the internal morphology of mesiobuccal (MB) roots of maxillary molars with a second mesiobuccal (MB2) canal. Materials and Methods: Forty-seven maxillary first or second molars from Brazilians were scanned using micro-computed tomography. The following measurements were obtained from the MB roots: root thickness, root width, and dentin thickness of the buccal aspect of the first mesiobuccal (MB1) canal, between the MB1 and MB2 canals, and the palatal aspect of the MB2 and MB1 canals at 3 mm from the root apex and in the furcation region. For statistical analysis, the Student's t-test and analysis of variance with the post-hoc Tukey test were used (α = 0.05). Results: In maxillary molars with an MB2 canal, MB roots were significantly thicker (p = 0.0014) and narrower (p = 0.0016) than in maxillary molars without an MB2 canal. The dentin thickness of the palatal aspect of the MB1 canal was also significantly greater than that of MB roots without an MB2 canal at 3 mm from the root apex (p = 0.0007) and in the furcation region (p < 0.0001). In the furcation region of maxillary molars with an MB2 canal, the dentin thickness between the MB1 and MB2 canals was significantly smaller than that in the buccal and palatal aspects (p < 0.0001). Conclusions: The internal morphology of MB roots of maxillary molars with an MB2 canal revealed differences in dentin thickness, root diameter, and distance between the canals when compared with maxillary molars without an MB2 canal.

Endodontic management of a maxillary first molar with three roots and seven root canals with the aid of cone-beam computed tomography

  • Variation in root canal morphology, especially in maxillary first molar presents a constant challenge for a clinician in their detection and management. This case report describes the successful root canal treatment of a three rooted right maxillary first molar presenting with three canals each in the mesiobuccal and distobuccal roots and one canal in the palatal root. The clinical detection of this morphologic aberration was made using a dental operating microscope, and the canal configuration was established after correlating and computing the clinical, radiographic and cone-beam computed tomography (CBCT) scan findings. CBCT images confirmed the configuration of the canals in the mesiobuccal and distobuccal roots to be Al-Qudah and Awawdeh type (3-2) and type (3-2-1), respectively, whereas the palatal root had a Vertucci type I canal pattern. This report reaffirms the importance of careful examination of the floor of the pulp chamber with a dental operating microscope and the use of multiangled preoperative radiographs along with advanced diagnostic aids such as CBCT in identification and successful management of aberrant canal morphologies.
