의료급여 관절염환자의 복약순응향상을 위한 통합중재프로그램의 효과 (Effects of an Integrated Case Management Program on Medication Adherence, Pain, Physical Function and Depression among Korean Medical Aid Beneficiaries with Osteoarthritis)

  • Purpose: The purpose of this study was to test the effectiveness of an eight session integrated case management program for improvement of medication adherence, physical function, pain, and depression among medical aid beneficiaries with osteoarthritis. Method: A nonequivalent control group pre-posttest design was employed. Participants were 55 medical aid beneficiaries who agreed to participate in this study, and were assigned to an experimental group (n=28) or control group (n=27). The framework of this research derived from Cox's Interaction Model of Client Health Behavior guided the overall intervention and the components. The program led by a case manager with a medication calendar, motivating interviewing and coaching strategies and collaboration with a pharmacist. Analysis included change in scores, ${\chi}^2$-test, and t-test. Result: The results showed significant increase in medication adherence, physical function and decrease joint pain, joint stiffness and depression in the experimental group compared to the control group. Conclusion: The eight session integrated case management program indicated an effect on medication adherence, pain, physical function, and depression. Partnership with a pharmacist is recommended for medication adherence and conduct of further studies will be needed in order to determine the long-term effect of an extended integrated program on health outcomes.

외상 후 파괴된 주관절 또는 진구성 탈구에서의 주관절 전치환술 (Total elbow arthroplasty for posttraumatic destroyed or unreduced elbow joint)

  • Purpose: To evaluate the results in patients who received total elbow replacement for posttraumatic destroyed or unreduced elbow joint. Materials and Methods: Six patients with posttraumatic destroyed, or unreduced elbow joint, who were nearly impossible to move actively and had pain and grossly unstable joint, were followed up average 42 months. 3 cases were soft tissue injuries and bone defects which were caused by severe comminuted fracture, 1 was a nonunion with comminuted fracture, and 2 were unreduced elbow joint. Total elbow replacement was performed average 10 months after the injury. All the cases were used by semiconstrained prosthesis, and the results were estimated by Mayo elbow perfomance score. Results: Pain was decreased in all the cases postoperatively. Average ranges of motion were improved with active extension 20° and flexion 120°. Mayo elbow performance scores were pain 42.5 points, range of motion 17.5 points, stability 8.3 points, function 19.2 points and totally 87.5 points, and final results were 3 excellent and 3 good. Loosening of prosthesis was not found in all the cases by final follow-up radiograph. Conclusicon: Semiconstrained TER can be used as a effective treatment improving pain and active ranges of motion caused by posttraumatic destroyed or unreduced elbow joint, however, long term follow-up is needed because early loosening of TER can be occurred due to severe bone defects.

근막이완술과 Mulligan 기법이 경부성 두통환자의 통증과 기능회복에 미치는 영향 (Effects of Myofacial Release and Mulligan Technique on Pain and Disability for Cervicogenic Headache Patients)

  • Background: Headaches arising from cervical musculoskeletal disorders are common. Conservative therapies are recommended as the choice of first treatment. Evidence to the effects of manual therapy is inconclusive and available only during the short term. There is no evidence to exercise, and no study has investigated the effect of combined therapies for cervicogenic headache. Methods: In this study, 30 participants who met the diagnostic criteria for cervicogenic headache were randomized into two groups: experimental group and control group. The experimental group was performed myofascial release (MFR), Mulligan technique and self stretching exercises. The control group was performed electrical therapy and self stretching exercises. Assessments were performed to 30 participants before and after 4 weeks therapy. The components of assessments were headache intensity(HI), headcahe duration (HD), neck pain (NP) and neck disability index (NDI). Results: After 4 weeks therapy, HI, HD, neck pain and NDI were significantly reduced in both groups (p<.01). The HI, HD, neck pain and NDI were significantly reduced in experimental group more than in the control group (p<.01). Conclusions: Manual therapy could reduce the symptoms of cervicogenic headache.

  • The effects of acupuncture are complex and how it works is not entirely clear. Research suggests that the needling process, and other modalities used in acupuncture, may produce their complex effects on a wide variety of ways in the brain and the body. For example, it is theorized that stimulated nerve fibers transmit signals to the spinal cord and brain, thus activating parts of what is called the central nervous system. The spinal cord and brain then release certain hormones responsible for making us feel better overall and, more specifically, feel less pain. In fact, a study using images of the brain confirmed that acupuncture increases our pain threshold, which may explain its ability to produce long-term pain relief. Also, acupuncture may increase blood circulation and body temperature. It may also affect white blood cell activity (responsible for our immune function).

Helicobacter pylori 감염과 소아 복통 (Helicobacter pylori infection and abdominal pain in children)

  • The relationship between H. pylori(Hp) infection and recurrent abdominal pain(RAP) in children is not clear. Current data in the literature regarding a causal relationship between Hp infection and childhood RAP are conflicting. However, meta-analysis and most of the recently published studies have not supported an association between Hp infection and an increased prevalence of abdominal pain. Most published studies have some problems and have been criticized on methodological grounds. Most of the existing studies have not been designed to include double-blind placebo controls. And, because of the low incidence of Hp infection in children, they have a problem of obtaining sufficient numbers of the Hp infected children. There is also a concern about the heterogeneity of testing methodology in the studies and diagnostic accuracy problems of the non-invasive tests such as low sensitivity of the serology tests and false positive results of the UBT in children. And conflicting results may be explained, in part, by the poor definition of RAP or dyspepsia for the study subjects. To date, no consensus exists for the treatment of Hp-infected children with RAP. Some authors suggest the clinical efficacy of Hp. But others suggest that eradication of Hp is not associated with improvement of RAP. The problem of the most treatment studies is that they had usually no control groups and the number of the subjects was small. In order to make reasonable decisions about treatment of Hp infection in children with RAP, long-term, randomized, double blind, well controlled studies in large numbers of children will be required.

Fractured styloid process masquerading as neck pain: Cone-beam computed tomography investigation and review of the literature

  • Historically, Eagle syndrome is a term that has been used to describe radiating pain in the orofacial region, foreign body sensation, and/or dysphagia due to a unilateral or bilateral elongated styloid process impinging upon the tonsillar region. Because elongated styloid processes-with or without associated Eagle syndrome-can present with various symptoms and radiographic findings, it can be challenging for healthcare practitioners to formulate an accurate diagnosis. Abnormal styloid anatomy can lead to a multitude of symptoms, including chronic orofacial/neck pain, thus masquerading as more commonly diagnosed conditions. In this report, we describe a patient who presented to our department with styloid process elongation and fracture. A careful history, physical examination, and a cone-beam computed tomography (CBCT) investigation led to the diagnosis. The patient was then referred for appropriate care. This case report demonstrates the utilization of CBCT in differentiating a fracture site from a pseudo-joint that might mimic a fracture.

초산부의 간호요구에 관한 연구 (A Study on Nursing Needs during Labor Pain)

  • The purpose of this study was to explore the structure of nursing needs during labor pain as perceived by women who have given birth. The phenomenological perspective of qualitative research theory guided the approach to the study. The sample consisted of 20 women who had undergone normal labor and had delivered a healthy baby at term. Collection of data was by means of in-depth interviews conducted 1-2 days after delivery in the admission room from March to may 1998. On average, the interviews lasted for about 30 minutes. Interviews were taken with the consent of the subjects. Data were analyzed by means of Giorgi's method and categorized according to the similarities of their contents. The investigator read the data repeatedly to identify themes and categories. Four categories were : 1) a sense of security 2) self esteem 3) willingness to learn 4) a sense of comfort. Under these categories there were twelve themes. I. A sense of security : (a) presence (b) conversation (c) touching II. Self esteem : (a) praise (b) encouragement (c) treating the women with respect (d) caregiver with good character III. Willingness to learn : (a) teaching (b) information IV. A sense of comfort : (a) self controlled pain relief (b) artificially controlled pain relief (c) skillfulness of caregiver The findings should sensitize nurses to the various needs of women in labor who are under their care. Further research should focus on developing instruments to assess the nursing needs of parturients. Researchers also need to identify ways to assess women's satisfaction with nursing needs.

A Review of Recent Evidence on Trigeminal Neuralgia

  • This review aimed to update our knowledge of the classification, pathophysiology, prognosis, and treatment of trigeminal neuralgia (TN), with the intention of establishing better treatment protocols. The latest version of the International Classification of Headache Disorders uses an etiology-based approach to characterize TN patients, potentially contributing to the development of targeted treatment measures. Noticeable changes in the recent European Academy of Neurology guidelines for the management of TN include the use of magnetic resonance imaging for exclusion of secondary TN and differentiation of idiopathic and classical TN. Additionally, the use of botulinum toxin type A as an addon therapy for mid-term treatment of TN has also been included. Though there has been limited recent progress in the treatment of TN, previous studies emphasize the importance of customized, multidisciplinary management protocols that include drug therapy optimization; provision of continuous education and support; and timely referral of medically refractory patients for surgery in order to achieve favorable prognosis. Furthermore, slow but growing evidence on gene mutations will help elucidate the pathophysiology of TN and contribute to the development of targeted drugs that are effective and safe.

Toxicity Assessment of a No-Pain Pharmacopuncture Extract Using a Standard Battery of In Vitro Chromosome Aberration Tests

  • Objectives: Genotoxicity is evaluated through a chromosomal aberration test using cultured mammalian cells to determine the toxicity of no-pain pharmacopuncture (NPP), which has recently been used to treat musculoskeletal pain disorders in Korean medical clinical practice. Methods: An initial test was performed to determine the dosage range of the NPP, followed by the main test. In this study, NPP doses of 10.0, 5.0, and 2.5%, and negative and positive controls were tested. An in vitro chromosome aberration test was performed using Chinese hamster lung cells under short-term treatment with or without metabolic activation and under continuous treatment without metabolic activation. Results: Compared with the saline negative control group, NPP did not significantly increase the frequency of chromosomal abnormalities in Chinese hamster lung cells, regardless of the presence or absence of metabolic activation. Additionally, the number of cells with structural chromosomal abnormalities was significantly higher in the positive control group than that in the negative control group that received saline. Conclusion: Based on the above results, the chromosomal abnormality-producing effect of NPP was determined to be negative under these test conditions.

요양병원 입원환자의 요양시설 이동의사에 영향을 미치는 요인 (Factors Affecting of Long Term Care Hospital Patient's Intention of Transfer to a Nursing Home)

  • Purpose: To examine factors affecting long-term care hospital patients' intention of transfer to a nursing home. Method: A questionnaire survey was conducted in Aug. 2007 that included 655 patients from 49 long-term care hospitals. The survey aimed to assess the patients' health status, family status, cost and intention of transfer to a nursing home. Institutional characteristics were analyzed from the nationwide database of Health Insurance Review & Assessment Service. The affecting factors were examined by employing chi-square test and logistic regression using SAS 8.2. Result: Of the subjects, 32.4% had intention of transfer to a nursing home. The intention of transfer to a nursing home was affected by moderate or severe pain, living together with the primary carer, high cost uncovered by insurance, and recognition of nursing home. Conclusion; For appropriate service utilization. a higher level of care is needed to satisfy patients at nursing homes and a balanced fee schedule is needed between long term care hospitals and nursing homes. It is desirable to encourage transfer to a nursing home at which nurses support patients and their families by giving information, coordination, and to make efforts to establish a reference system.
