The purpose of this research was to determine whether or not a education program of low back pain would have benificial effect on nurses. Nonequivalent control group pre-post test research was designed. Subjects for this study were 50 selected from nurses who work at a general hospital in Taegu. Of them, twenty seven were assigned to experimental group, twenty-three to control group. The data were collected during the period from June 27 to August 31, 1994. The instruments used in this study were Visual Analog Scale for low back pain, Sit-and-reach test for trunk flexibility, and 1 min sit up test for abdominal muscle endurance. Data related to general chracteristics and factors of low back pain were collected with questionnaire developed by this reseacher. The data were analyzed by $X^2-test$, t-test and paired t-test, using the SAS package program. The results of this study are summerized as follows : 1. The numbers of the experimental group with low back pain were not a statistically significant difference from pre to posttesting($X^2=0.77$, p=.379), and the numbers of the control group with low back pain were also not a statistically significant difference from pre to posttesting($X^2=0.09$, p=.767). 2. The severity of low back pain of the experimental group was not a statistically significant difference from pre to posttesting(t=-0.55, p=.5857), and the severity of low back pain of the control group was also not a statistically significant difference from pre to posttesting(t=-0.70, p=.3101). 3. The trunk flexibility of the experimental group was not a statistically significant difference from pre to posttesting(t= 1.08, p=.2835), and the control group's trunk flexibility was also not a statistically significant difference from pre to posttesting(t=0.85, p=.4026). 4. The abdominal muscle endurance of the experimental group was a statistically significant difference from pre to posttesting(t=-2.42, p=.0190), but the control group's abdominal muscle endurance was not a statistically significant difference from pre to posttesting(t=-0.06, p=.9556) This study suggest that replication of study with more sample and more rigid treatment should be needed. And furthermore, the longitudinal research is needed to determine the effectiveness of education program.
Objectives : This studies were performed to investigate the correlation between the treatment effects and the changes of the thermal differences at the pre and post-treatment by using the digital infrared computer thermographic imaging (D.I.T.I) to the irritable colon syndrome 30 patients. Methods : The extracts of the gwakhyangjunggisan were administerd to the patients by oral method three times per day during four weeks. we selected the Chung-Wan($CV_{12}$), Chunchu($S_{25}$). Hapkok($LI_4$), Naegwan($P_6$), Yongdo($H_4$) acupoints and treated the patients two times per week during four weeks. One week later, we investigated the changes of the pressure pain around left side Pokkyol($SP_{14}$), the symptoms. and the skin temperature changes on Pokkyol($SP_{14}$) and Kimun($LR_{14}$) around, by using the D.I.T.I. Results : In the correlation between the severity of the pressure pain around left side Pokkyol($SP_{14}$) and the thermal differences on Pokkyol($SP_{14}$) and Kimun($LR_{14}$) around, before the treatment, the more the pain was severe, the more the skin temperature was differential significantly. In the correlation between the severity of the symptoms and the thermal differences on Pokkyol($SP_{14}$) and Kimun($LR_{14}$) around, before the treatment, the more the symptom was severe, the more the skin temperature was differential significantly. In the changes of the pressure pain around left side of Pokkyol($SP_{14}$), the improved were significantly higher than the unimproved. In the changes of the symptoms, the improved were significantly higher than the unimproved. In the correlation between the changes of the pressure pain around left side Pokkyol($SP_{14}$) and the changes of the thermal differences at the pre and post- treatment, the more the pain was improved, the more the thermal change was widened significantly. In the correlation between the changes of the symptoms and the changes of thermal differences at the pre and post-treatment, the more the symptom was improved. the more the thermal change was widened significantly. Conclusions : Above the results, the treatments was effective and the digital infrared computer thermographic imaging (D.I.T.I) was useful to prove the improvements of the pressure pain and the symptom with the irritable bowel syndrome patients.

This study aimed to evaluate the effects of stretching management on musculoskeletal joint pain in community indwelling elderly farmers. The study included 28 residents who lived in a rural community. All participants were assigned randomly to either the stretching group (n=13, $59.67{\pm}4.77$ year-old) or the control group (n=15, $61.44{\pm}10.41$ year-old). Respondents were interviewed by means of a structured questionnaire. Pain severity of 6 body areas caused by symptoms of work-related musculoskeletal disorders and pain severity on day time variations were measured by the visual analogue scale on a self-assessed questionnaire. The stretching group was administered stretching therapy (45 min, 2 sets, warming up and cool down, main exercise; total 19 stretching kinds of subset 5 fields) for 12 times for 4 weeks. There were no significant difference between the two groups in general social and pain characteristics. After 4 weeks of stretching, the stretching group showed significant improvements at almost all joint pain scores except arm/elbow, and day time variation scores of pain compared to both baseline scores, and with control group scores. These results showed that stretching therapy is one of the most useful modalities to manage musculoskeletal pain in community-based elderly farmers.

This study was conducted to compare the severity of cannulation pain in hemodialysis patients after topical application of EMLA cream and local injection of lidocaine and evaluated side effects and problems accompanied by the former. Twenty patients, who were on hemodialysis from September 1 to October 15, 1994 at the Kangnam St. Mary's Hospital, Catholic University Medical College, were divided into two groups of ten. To conduct a cross over study, two groups were placed on four repeated methods with lidocaine followed by four repeated methods with EMLA cream and vice versa, respectively, while the severity of cannulation pain was being measured according to a Visual Analogue Scale with each methods. The results are follows : 1) The scale of pain was recorded as $4.56{\pm}1.38$ and $2.05{\pm}1.36$ points for methods with lidocaine and EMLA cream, respectively, indicating the less severe pain with EMLA cream. 2) Local side effects such as itching(4 cases, 5.0%)and pallor (5 cases, 6.3%)were observed with methods with EMLA cream but disappeared before the completion of hemodialysis. 3) Problems associated with local lidocaine were pain at the injection of anesthetic (27cases, 16.9%)and fear for needle insertion(6 cases, 3.8%). The most frequent problems with EMLA cream application were an inconvenience in use (11 cases, 6.9%)and tedious long pretreatment time(11 cases, 6.9%), those associated with inconvenience in cream applying procedures. 4) Twelve out of twenty patients(60.0%) responded with yes to a continued use of EMLA cream in spite of problems with cream application and economical difficulties in purchasing. These results indicate that 5% EMLA cream used as a local anesthetic in hemodialysis significantly reduces cannulation pain and lacks side effects, thus serving as a suitable method for the alleviation of cannulation pain and inconvenience in hemodialysis and the relief of psychological stress of nurses.

This case study is to report the effectiveness of Korean medicine in the treatment of musculoskeletal chest pain in Parkinson's patient. The Parkinson's patient with chest pain was treated with Jungsongouhyul Pharmacopuncture for 15 days. The severity of chest pain was assessed using Face Pain Scale-Revised, Short Form McGill Pain Questionnaire(SF-MPQ) and King's Parkinson's Disease Pain Scale(KPPS). Additionally, duration of the pain and standing state was evaluated. After treatment, chest pain was removed. SF-MPQ score was decreased from 6 to 0, and KPPS score 21 to 0. As a result this case showed the effect of Jungsongouhyul Pharmacopuncture on musculoskeletal chest pain in Parkinson's disease.
The Purpose of this Study was to examine the anatomic differences in the upper airway according to severity of respiratory disturbance index of habitual snorers. Forty-three male habitual snorers, aged 28-68, were examined by polysomnography and divided into 4 groups according to severity determined by polysomnography. Anatomic differences in the upper airway were assessed by lateral cephalograms in upright position, and epidemiological surveys were done by using questionnaires. The obtained results were as follows : 1. All subjects were habitual snorers and 35 patients (81.4%) complained the loudness of snoring as severe as be heard outside of the room. 2. According to the results of polysomnography, the number of the primary snoring patients was 7(16.3%), mild obtrusive sleep apnea 7(16.3%), moderate 7(16.3%), and severe 22(51.2%). 3. The respiratory disturbance index (RDI) of subjects was 39.5$\pm$24.4 and the body mass index(BMI) was 26.2$\pm$2. 4. More inferiorly positioned hyoid bone according to the degree of respiratory disturbace index (RDI) was observed. (p<0.001) 5. The width of superior oropharyngeal airway space was according to the degree of RDI. (p<0.001)
Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of reflux esophagitis. Methods: A 62-year-old male patient had reflux esophagitis that occurred one year after a proximal gastrectomy for gastric ulcer in 2020. The patient underwent drug treatment in 2021 and inpatient treatment in 2022 at Mokpo Hankook Hospital, but he did not improve. We treated him with Korean medical treatments, including herbal medicines (Pyungjingunbi-tang-gami), acupuncture, and moxibustion. His symptom severity was assessed with a daily visual analog scale (VAS) for heartburn, upper abdomen pain, and acid reflux. Results: After treatment, the patient's symptoms were improved. The severity of heartburn and acid reflux was reduced from VAS 7 to VAS 1, and his upper abdomen pain was improved from VAS 6 to VAS 1. The symptom frequency was also reduced. Conclusion: Korean medicine treatment could be an effective and quick treatment for reflux esophagitis.
Objectives : The objective of this study was to investigate the relationship between psychological distress and pain in cancer patients. Methods : 249 patients with cancer who visited National Health Insurance Service Ilsan Hospital between April 2013 and March 2014 were evaluated with National Cancer Center Psychological Symptom Inventory(NCC-PSI) which consisted of Modified Distress Thermometer(MDT) and Modified Impact Thermometer(MIT). Each scale was divided into 3 subscales targeting separate symptoms: insomnia, anxiety, and depression. Psychological distress was defined as positive for those who scored above the cutoff values in at least one of all six subscales. The Numeric Rating Scale for Pain(NRS-Pain) was used to assess the subjective severity of pain. Logistic regression was performed to investigate the association between psychological distress and pain. Results : Univariate logistic regression analysis showed that pain, gender, compliance, and two subscale scores of Hospital Anxiety and Depression Scale(HADS) were significantly associated with psychological distress. Multivariate logistic regression analysis showed that pain and HADS anxiety subscale score maintained a statistically significant association with psychological distress adjusted for variables including age, gender, years of education, Eastern Cooperative Oncology Group performance status, cancer stage, Charlson Comorbidity Index, compliance, and HADS depression subscale score. One point increase in pain was 1.31 times more likely to cause psychological distress. In secondary analysis, pain was significantly associated with all subscales of NCC-PSI, except MIT-anxiety subscale. Conclusions : This study suggests that NCC-PSI, a screening tool for psychological distress, reflects pain. We recommend that physicians who treat cancer patients consider the examination of psychological distress which provides comprehensive evaluation of various factors regarding quality of life.
Background: The pathophysiological and neurochemical changes following spinal injury are not yet elucidated. This study was designed to evaluate the morphological changes of the dorsal horn of the spinal cord and profiles of pain behaviors following intraspinal injection of NMDA in rats. Methods: Rats were randomized into three groups: a sham-operated control group and groups where the rats received 10 mM or 100 mM N-methyl-D-aspatate (NMDA) injected into their spinal dorsal horn. Following injection, hypersensitivity to cold and mechanical stimuli and excessive grooming behaviors were assessed serially for four weeks. Morphological changes of the spinal cord were evaluated four weeks after intraspinal injection. Results: Few animals in the NMDA groups developed hypersensitivity to cold and mechanical stimuli. The number of groomers and the severity of excessive grooming were significantly higher in the 100 mM NMDA group than those values of the control and 10 mM NMDA groups. The size of the neck region (lamina III-IV) was significantly smaller in the 100 mM NMDA group than in the control and 10 mM NMDA groups. Conclusions: In conclusion, intraspinal injection of NMDA in rats leads to the pathological sequela in the spinal cord and to excessive grooming behavior. These results support the use of NMDA and excessive grooming behavior after excitotoxic SCI as a model to study chronic pain after SCI.
Purpose: Temporomadibular disorders (TMDs) can result in chronic pain, which is often associated with psychological and sleep disturbance. Increased levels of psychological and sleep impairments are often related with poor treatment outcomes. The purpose of the present study was to evaluate clinical features, psychological profiles and the prevalence of clinical insomnia in TMD patients with chronic pain and to suggest an effective treatment approach. Methods: A total of 200 TMD patients who had visited the Pusan National University Hospital dental clinic for treatment of their pain were recruited from June 2018 through December 2019. TMD patients were classified into an acute (n=100) and chronic (n=100) group and compared the clinical symptoms. The primary diagnosis of TMD were categorized as TMD with joint pain group (TMD_J), TMD with muscle pain group (TMD_M) and TMD with joint-muscle mixed pain group (TMD_JM). Self- report measures of sleep quality and psychological profiles were evaluated via the Insomnia Severity Index (ISI) and the Korean Stress Response Inventory (SRI). Independent t-tests, Mann-Whitney U-tests, and chisquared test were used for the statistical analysis. Results: Chronic TMD patients showed higher pain intensity, as well as higher prevalence of related symptoms (headache, musculoskeletal pain) and myogenous pain. They also had significantly higher scores in all SRI parameters and a higher percentage of clinical insomnia than acute TMD patients. Conclusions: Based on the above results, psychological profiles and sleep quality assessments are necessary to provide essential data that will allow for improved treatment of chronic TMD patients.
