Psoas Compartment Blockade in a Laterally Herniated Disc Compressing the Psoas Muscle - A Case Report -

  • A psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and analgesia for post-operative pain. Currently, this block is advocated for relieving pain in the lower extremity and pelvic area resulting from various origins. We report a case of a 69-year-old male patient who had gait abnormality with posterior pelvic and hip pain, which were both aggravated by hip extension. From the magnetic resonance image, the patient was found to have a laterally herniated intervertebral disc at the L2/3 level, which compressed the right psoas muscle. This was thought to be the origin of the pain, so a psoas compartment block was performed using 0.25% chirocaine with triamcinolone 5mg, and the pain in both the pelvis and hip were relieved.

A Study on Correlation between Dong-si Acupoints and the 14 Meridian Acupoints -Location and Efficacy of Acupoints on the Lower Extremity (정경혈(正經穴)과 동민기혈(童民奇穴)의 관계에 대한 고찰;하지부(下肢部)의 혈위(穴位) 및 주치(主治)를 중심으로)

  • Objectives : Dong-si acupuncture therapy is being widely used because of good clinical result. The purpose of this study was to compare the location and efficacy of Dong-si acupoints and 14 meridian acupoints. Conclusions : 1. Dong-si acupoints on the lower extremity total 83. Among them, 16 acupoints are the same as 14 meridian acupoints. 2. Between the same location points, the efficacy of each Dong-si acupoints is similar to that of each of the 14 meridian acupoints in cases of musculoskeletal pain diseases, paralytic diseases, urogenital diseases, and gastrointestinal diseases. 3. Dr. Dong studied deeply into the 14 meridian acupoints and created Dong-si acupuncture therapy. He said that the distribution of acupuncture points was closely related to the 14 meridians. 4. I think that we are able to apply Dong-si acupuncture therapy to clinical use widely by comprehending the character of the 14 meridian acupoints and each meridians.

Anomalous Retro-Psoas Iliac Artery: A Case Report (허리근뒤 이상 온엉덩동맥: 증례 보고)

  • The anomalous retro-psoas iliac artery is an extremely rare congenital iliolumbar vascular anomaly. A 51-year-old woman presented to our emergency department with worsening right lower extremity pain and weakness for 3 months. CT angiography of the right lower extremity showed no evidence of stenosis in the lower extremity arteries and the incidental finding of an anomalous right retro-psoas iliac artery. Herein, we report a rare case of anomalous retro-psoas iliac artery. Surgeons and clinicians need to be aware of this rare congenital anomaly to avoid severe complications during pelvic or orthopedic surgery.

A Case Report of Lower Extremity Paresthesia with Polyneuropathy Treated with Traditional Korean Complex Treatment (양하지 이상감각을 호소하는 다발신경병증 환자에 대한 침구-한약 복합한방 치료 치험 1례)

  • Objectives: This report presents the case of a 21-year-old woman with lower extremity paresthesia in both feet due to polyneuropathy. Methods: The patient was treated using a Korean traditional complex treatment approach that included herbal medication, acupuncture, electroacupuncture, and moxibustion. We evaluated the improvements in the pain symptoms in both feet using the numeric rating scale (NRS); we also assessed for gait disturbance and used Digital Infrared Thermographic Imaging (D.I.T.I.) to evaluate minute changes in body temperature in diseased areas. Results: After administering the Korean traditional complex treatment, we observed a decrease in pain levels in both feet based on the NRS scores. We also observed improvements in gait disturbance and D.I.T.I. Conclusions: This case showed that the use of a Korean traditional complex treatment approach, consisting of herbal medication, acupuncture, electroacupuncture, moxibustion, had a positive effect on decreasing polyneuropathy symptoms.

  • The purpose of this study was to investigated the muscle activity difference of lower extremity by abdominal co-contractive activation. 30 healthy women without pain in the low back and lower extremity were conducted. Muscle activities of lower extremity were measured with the active straight leg raise (ASLR) in 20cm raised from the floor without bending the knee at rest, abdominal drawing-in maneuver (ADIM) and abdominal bracing maneuver (ABM), respectively. Hip adductor longus (HadL), rectus femoris (RF), and biceps femoris (BF) muscle activity in the Abdominal bracing maneuver were statistically significantly higher, also gluteus medius (Gmed) and tibialis anterior (TA) muscle activity in the ADIM were higher. We provide the evidence to the positive effects of individual abdominal co-contractive activation training which can enhance postural stability and lower extremity strengthening with volitional preemptive abdominal contraction.

Continuous Caudal Block for Intractable Pain (지속적(持續的) 미추차단(尾椎遮斷)에 의한 동통관리(疼痛管理) 3예(三例))

  • We experienced 3 cases of continuous caudal block. The first case had suffered from severe pain of the external genitalis after urethral injury from a car accident and this was controlled by continuous caudal block. The other 2 cases were a metastaric malignant tumor of the lumbar vertebra from cancer of the cervix and histiocytoma of the breast, and both had suffered from intractable pain of the lower extremity. But lumbar epidural block was impossible because of radiation fibrosis and previous operation scar of the spine. So a continuous caudal block was performed and the pain was controlled effectively. The longest duration was 50 days and there were no problems related catheter indwelling. Pain in the area of the lumbar and sacral nerve distribution can be controlled by continuous caudal block. Here in we reported 3 cases and reviewed the literature.

A Case Report of Sequela of Operation of Hallux Valgus (족부 무지외반증 수술 후 후유증 치험 1례)

  • Objectives : This study aims to report the effect of Korean medicine treatments on a sequela of operation of hallux valgus. Methods : The patient was treated with Dangkisoosan-gami, Acupuncture treatment including Herbal-Acupuncture and physical therapy. We evaluated treatment effects by circumference of foot and ankle, Edema index score by Body Composition Analyzer, Ankle-Brachial Index (ABI) and Visual Analogue Scale (VAS). Results : After 25 days of treatment, the symptoms such as lower extremity edema, foot pain and ankle pain were improved. Conclusions : In this case, Korean medicine treatments for a sequela of operation of hallux valgus was effective. But further studies are required to confirm the effect of these methods.

  • Complex Regional Pain Syndromes (CRPS) type I and type II are neuropathic pain conditions that are being increasingly recognized in children and adolescents. The special distinctive features of pediatric CRPS are the milder course, the better response to treatment and the higher recurrence rate than that of adults and the lower extremity is commonly affected. We report here on a case of pediatric CRPS that was derived from ankle trauma and long term splint application at the left ankle. The final diagnoses were CRPS type I in the right upper limb, CRPS type II in the left lower limb and unclassified neuropathy in the head, neck and precordium. The results of various treatments such as medication, physical therapy and nerve blocks, including lumbar sympathetic ganglion blocks, were not effective, so implantation of a spinal cord stimulator was performed. In order to control the pain in his left lower limb, one electrode tip was located at the 7th thoracic vertebral level and two electrode tips were located at the 7th and 2nd cervical vertebral levels for pain control in right upper limb, head, neck and right precordium. After the permanent insertion of the stimulator, the patient's pain was significantly resolved and his disabilities were restored without recurrence. The patient's pain worsened irregularly, which might have been caused by psychological stress. But the patient has been treated with medicine at our pain clinic and he is being followed up by a psychiatrist. (Korean J Pain 2007; 20: 60-65)

Effect of Balance Ability of Knee Osteoarthritis with Lower Extremity Strengthening Exercise (퇴행성 슬관절염 환자의 하지 근력 강화운동이 균형능력에 미치는 영향)

  • Purpose : The main purpose of this study was to investigate the influence of lower extremity strengthening on balance ability of knee osteoarthritis. Methods : The walking exercise group with modality treatment and strengthening exercise group with modality treatment. The walking exercise and strengthening for 40 minutes per day and three times a week during 6 weeks period. Short Form McGill Pain Questionnaire(SFMPQ) was used to measure patient's pain level. Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. BPM was used to measure sway area. Global Perceived Effect Scale(GPES) was used to measure recovery or worse of patient's condition. Results : 1. SFMPQ was strengthening exercise group showed significantly decreased more than walking exercise group(p<.05). 2. PSFS was strengthening exercise group showed significantly increased more than walking exercise group(p<.05). 3. Sway area was strengthening exercise group showed significantly limited area more than walking exercise group(p<.05). 4. GPES was strengthening exercise group showed significantly increased more than walking exercise group(p<.05). Conclusion : This study will be used as exercise method of patient with osteoarhtritis.

  • The foot or lower extremity is rarely the initial site in adult-onset dystonia, whereas dystonia in children often begins in the foot. Isolated lower extremity dystonia in adults is rarely on a primary basis. Oral anti-dystonic medications have been found unsatisfactory in providing adequate symptomatic relief. On the other hand, botulinum toxin injections have been reported as beneficial. It is also known that personalized orthosis can be an effective solution for patients of dystonia. The purpose of this report is to demonstrate a case of primary focal foot dystonia that was effectively treated with botulinum toxin injection and the custom-made ankle-foot orthosis.
