하지운동과 탄력압력스타킹 적용이 일종합병원 간호사의 하지부종과 하지통증에 미치는 영향 (Effects of the Application of Lower Extremities Exercise and Elastic Compression Stockings on Edema and Pain of Lower Extremities in Nurses)

  • 본 연구는 하지운동과 탄력압력스타킹 적용이 간호사의 하지부종 및 통증에 미치는 효과를 검증하기 위한 비동등성 대조군 전 후시차설계를 이용한 유사실험연구이다. 연구대상자는 E시에 소재하는 일개종합병원에 근무하는 간호사 30명으로 탄력압력스타킹 적용군 10명, 하지운동 적용군 10명, 대조군 10명으로 구성되었다. 자료수집은 2015년 8월 10일부터 2015년 8월 31일까지 이루어졌다. 실험처치는 실험군인 탄력압력스타킹 적용군은 근무 중 탄력압력스타킹을 착용하며 하지운동 적용군은 근무 전 하지운동을 적용하였다. 종속변수인 하지부종은 줄자를 사용하여 측정하였고, 하지통증은 숫자척도를 사용하여 측정하였다. 자료분석을 위해 SPSS WIN 21.0 프로그램을 이용하였다. 연구결과, 하지운동 적용군, 탄력압력스타킹 적용군과 대조군간의 하지부종에 대한 차이에 대한 비교는 지지되었으며(오른쪽 종아리 F=0.645, p=.000, 왼쪽 종아리 F=21.941, p=.000), 하지운동 적용군, 탄력압력스타킹 착용군과 대조군간에는 하지통증에 대한 차이에 대한 비교는 지지 되었다(오른쪽: F=7.320, p=.000 왼쪽: F=10.623, p=.000). 또한 하지운동 적용군과 탄력압력스타킹 적용군은 근무 후 발목둘레와 종아리둘레가 감소하였으나 대조군의 경우는 발목둘레와 종아리둘레가 오히려 증가하는 결과를 보였다. 이에 하지운동 적용과 탄력압력스타킹 적용은 간호사와 같이 장시간 서서 일하는 직업 종사자들의 하지부종 감소 및 통증완화를 위해 효과적인 중재방법임을 알 수 있다.

  • Objectives The purpose of this study was to report a case of nodules on lower extremities with pain which was treated by herbal medicine treatment and acupuncture.Methods One male patient who had nodules on lower extremities with pain was treated by Yangdokbaekho-tang and Hyungbangsabak-san, and acupuncture according to the result of Sasang constitutional diagnosis.Results and Conclusions The patient's pain and accompanied symptoms were improved after using Yangdokbaekho-tang and Hyungbangsabak-san. The Numeric Rating Scale(NRS) which was used to evaluate pain intensity was decreased from 9 to 2 without using Western medications.

통나무 도구를 이용한 발목 펌프 운동이 수술실 간호사의 하지 부종 및 하지 통증에 미치는 효과 (A Study on the Effects of Ankle Pump Exercise in Reducing Lower Limbs Edema and Pain of Operating Room Nurses)

  • Purpose: The purpose of this study was to identify the effects of Ankle Pump Exercise on lower limb edema and pain of operating room nurses. Methods: This research was arranged on the basis of the nonequivalent control group pre-post test design. The participants were 41 nurses working in the operating room in 'S' hospital in Seoul and 'H' hospital in Guri City, Korea. Data were collected from August 20 to September 30, 2012. Edema of the lower extremities was measured in terms of the girths of ankles and calves. Pain of the lower extremities was measured in terms of VAS (Visual Analogue Scale) for pain. The data were analyzed by frequency, percentage mean, and standard deviation, using the chi2(p) and t-test, repeated measures ANOVA, and a paired t-test. The SPSS Win 20.0 program was used for analysis. Results: Hypothesis 1 - "There are significant differences in lower extremity edema between the experimental group and the control group." - was partially supported. Hypothesis 2 - "There are significant differences in lower extremity pain between the experimental group and the control group." - was supported. Conclusion: The results of this study demonstrate positive practical effects of the Ankle Pump Exercise in reducing the circumference of both ankles, reducing the circumferential edema of both calves, and alleviating the pain of nurses in the operating room. Based on these findings, application of the Ankle Pump Exercise can be considered as an effective method to prevent nurses' health problems in the lower extremities that can be induced by their work during surgical operations.

유산소운동과 근력운동이 노인의 하지근력과 통증, 균형에 미치는 영향 (The Effects of Aerobic and Muscle Strength Exercises on Pain, Balance, and Muscular Strength in the Lower Extremities among Elderly People)

  • Purpose : The purpose of this study was to improve the quality of life of people aged 65 and over by reducing musculoskeletal pain and increasing balance and muscular strength in their lower-extremities through aerobic exercises and the use of Thera-Bands and gym balls. Method : Fifteen out of 30 women who used a welfare center for the elderly in C City performed exercises using Thera-Bands and gym balls, and the remaining 15 elderly women did aerobic exercises. Both groups performed their respective exercises three times a week over a four-week period, for 50 minutes each time. Pain (VAS), balance (TUG), and lower extremity muscular strength were measured as evaluation tools. The data were analyzed using the statistical software SPSS 18.0 for Windows. The Wilcoxon signed-rank test was performed to compare the differences within each group, and the Mann-Whitney U test was used to compare the two groups in terms of their differences both before and after the participants' respective exercise programs. Results : No differences were found between the two groups, although there were some differences within each group in terms of levels of pain, muscular strength, and balance. Conclusion : The aerobic and muscle strength exercise programs had positive effects on all variables, including pain, balance, and muscular strength in the lower extremities. Therefore, the combination of aerobic and muscle strength exercises may be effective in improving the quality of life of elderly people.

요각통 환자의 12 경맥 전위측정 연구(6) (Differences in Electric Potential of Meridian System(6) -Comparing electrical potentials of patients with waist lesions-)

  • Objectives:Assuming that the characteristic of meridian system has been similar to that of electric potentials in human body and that measurements of electric potential at well(井穴) and sea(合穴) points in branches of the twelve meridians(WSBTM) will be representative of measurements of the twelve meridians, to measure the electric potentials of 17 patients with loin lesions, and pain of loin and lower extremities (腰脚痛, LWLE), to find out the characteristic of meridian system in patients with loin lesions. Methods:Electric potentials of well and sea points in the meridians in 17 patients with LWLE diagnosed by both CT or MRI and self view on pain at loin and lower extremities were repeatedly measured by physiograph(PowerLab). Measurements of those electrical potentials were analyzed by factor analysis. Results The electric potentials of WSBTM at the left side were divided into four factors. On the other hand those at the right side wert divided into three factors.

Sympathetic Block for Treating Primary Erythromelalgia

  • Primary erythromelalgia is a rare condition that’s characterized by erythema, an increased skin temperature and burning pain in the extremities. The pain is often very severe, and treating erythromelalgia is frustrating and difficult. We report here on the case of a 12-year old girl with primary erythromelalgia in both lower extremities. The pain was refractory to medical treatment, but a bilateral sympathetic block with lidocaine and triamcinolone resulted in relief from the pain. Our experience with this disease demonstrates that sympathetic blocks are effective in improving the symptoms and they may be attempted on erythromelalgia patients who do not respond to other treatments, including medication and epidural blocks.

Newly Developed Urinary Retention and Motor Weakness of Lower Extremities in a Postherpetic Neuralgia Patient

  • During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

산후 하지무력 환자 1례 임상경과 보고 (Clinical study for the one case complaining both lower extremities weakness after parturition)

  • Purpose : The purpose of this study is to report the effect of TKM to both lower extremities weakness after parturition who is diagnosed as Subacute inflammatory demyelinating peripheral neuropathy. Methods : The patient was a 31-year-old woman who couldn't walk at all by herself because of both lower extremities weakness after parturition. The patient was demonstrated as shin-yang-heo, and treated by Pal-mi-ji-hwang-hwan, Acupuncture treatment of Shin-jeong-gyeok, and Moxa treatment of Gwan-won. And the progress of symtoms was evaluated by observation of her walking status, cheking the change of VAS, and inspecting the DITI. Results : The patient could walk by herself. The feeling of both lower extremities weakness improved to VAS 2. The paraesthesia of the soles of feet improved to VAS 4. And the DITI state slightly got better. Conclusion : TKM is expected to have positive effect on puerperium disease especially the symptoms of pain, weakness, paraesthesia, etc.

Buerger환자 43명의 치료 경험 (Treatment of 43 Patients with Buerger's Disease)

  • Buerger's disease is a nonatherosclerogic occlusive inflammatory disease of medium and small arteries, and veins, of unknown cause. It occurs predominantly in young males who are habitual tabacco users. These patients often complain of painful ulcerations of their digits. The care of this disease is very difficult when the treatment is delayed. Consequently, early treatments are most important to patients with Buerger's disease. This disease can be treated with sympathetic block such as stellate ganglion block for upper extremities and lumbar epidural block, and lumbar sympathetic block for lower extremities. Intravascular regional sympathetic block can be another method of treatment. However, discontinuation of smoking is the most basic and essential treatment for Buerger's disease. We treated 43 Buerger's disease patients with stellate ganglion block and laser therapy. The treatment was not effective for three patients who definitely required amputation.
