미양(微量) Morphine 지주막하(蜘蛛膜下) 주입(注入)에 의(依)한 항문각위술후(肛門脚圍術後) 제통효과(除痛效果)에 대(對)한 임상연구(臨床硏究) (Subarachnoid Morphine for Perianal Postoperative Pain Control)

  • In order to control the pain after hemorrhoidectomy and anal fistulectomy, 6 mg of 0.5% hyperbaric tetracaine without(control, group I) or wilt 0.3 mg(group II) or 0.5 mg (group III) of 0.1% morphine was injected with a 22 gauge spinal needle into the subarachnoid space through L 3-4 interspace of patients in lateral position. About 30 minutes in Fowler' sposition after injection, operation was performed in lithotomy position. All the patients who ha4 morphine showed remarkable relief of postoperative pain for an average of 27 hours. However, the dosage(0.3 or 0.5 mg) of morphine administered did not affect the duration of pain relief. Blood pressure, pulse rate and pupil size were unchanged in all patients. Dysuria after block developed for on average of 5,6, 13.2 and 14.6 hours in group I, II and III respectively. Most of these cases required urethral catheterization. Minor complications such as nausea, vomiting, itching, fever, burning sensation and paresthesia were observed 16.7, 20 and 20% of cases in group I, II and III respectively; however, no treatment was required.

Assessment of the Nature and Severity of Pain Using SF-MPQ for Cancer Patients at the National Institute of Oncology in Rabat in 2015

  • Background: Cancer is a worldwide health problem and pain is among the most common and unpleasant effects affecting well-being of cancer patients. Accurate description of pain can help physicians to improve its management. Many English tools have been developed to assess pain. Onkly a limited number of these are applied in Arab countries. Our aim was to assess the quality, the nature and the severity of pain using the short McGill Pain Questionnaire (SF-MPQ) on cancer patients in the National Institute of Oncology (NIO) in Rabat, Morocco. Materials and Methods: The tool used is the SF-MPQ inspired from the Arabic version of the MPQ. The subjects were cancer patients (N=182) attending the NIO, from 24th October 2015 to 8th January 2016, aging ${\geq}18$ years old, experiencing pain and coming to have or to update their pain medication. Results: The rate of participation was 96.3%. Eight patients had difficulties to express their pain using descriptors, but could use the Visual Analogue Scale (VAS) and the body diagram. The most frequent sensory descriptors were 'Throbbing', 'Shooting', 'Hot-Burning'. The most used affective descriptor was 'Tiring-Exhausting'. The mean VAS was 6.6 (2.4). The mean score of all items was 11.9 (7.8). The patients were suffering from severe pain. The internal consistency of the form was s acceptable. Conclusions: The findings indicate that most of the patients attending the pain center of the NIO could use the descriptors of the SF-MPQ to describe their pain. They indicate the usefulness of the SF-MPQ to assess the nature and the severity of pain in cancer patients. This tool should be tested in other Moroccan and Arabic contexts associated with other tools in clinical trials.

  • Low-level laser therapy (LLLT) is the application of light to pathology to promote tissue regeneration, reduce inflammation, and relieve pain. LLLT has a photochemical effect whereby the light is absorbed and exerts a chemical change. The clinical applications of LLLT include improvement in wound and bone healing processes, control of pain and tooth hypersensitivity, modulation of periodontal inflammation, the prevention and treatment of cancer therapy-induced oral mucositis, management of burning mouth syndrome, and improvement in temporomandibular disorder symptoms. Further research is needed to better elucidate the cellular mechanisms of LLLT and provide a solid scientific basis for the clinical application of LLLT in dentistry.

작열통의 치료 경험 (Clinical Experiences of Causalgia -Two Cases Report-)

  • Causaliga is a syndrome of sustained burning pain, allodynia and hyperpathia after a traumatic nerve lesion, often combined with vasomotor and sudomotor dysfunction and later trophic changes. Various treatments of causalgia contain sympathetic blockade, sympathectomy, transcutaneous electrical nerve stimulation, physical therapy, cryotherapy and psychotherapy. Repeated stellate ganglion blocks with 6ml of 0.25% bupivacaine provided good results for 2 patients. We recommand sympathetic blocks for treatment of causalgia.

대상포진 환자 치험 1례 (A Case Report on Herpes Zoster)

  • Clinical symptoms of herpes zoster include red rash, burning pain, tingling or extreme sensitivity in the affected area, usually limited to one side of the body. There might be a fever or a headache. The pain of acute herpes zoster may be severe, but it is usually temporary. Some of old patients are likely to develop postherpetic neuralgia. We treated a 73 year-old male patient who had severe pain and a headache after acute herpes zoster. In the point of Differentiation of Syndromes(辯證), this subject was diasgnosed as wind-heat syndrome(風熱證) and was administered Bangpungtongsung-san. For the purpose of making the pain easier, we used the western medication as well. After six days of treatment, pain and the other symptoms improved. Besides postherpetic neuralgia dissipated. Based on this experience, both oriental medicine and western medicine have a good effect on acute herpes zoster.

복강신경총 차단후 하지마비 -증례 보고- (Paraplegia Following Celiac Plexus Block -A case report-)

  • Paraplegia complicating a block of the celiac plexus with alcohol for recurrent stomach cancer is described. A 33 year old male patient was admitted to control for severe epigastric pain. With the patient in prone position, the needle was advanced further than 2 cm on the anterior margin of $L_1$ vertebral body under fluoroscopy. 3 ml of 1% lidocaine and 5 ml of conray were injected through each needle, and the diffusion of the contrast medium was checked, This was followed by 7 ml of pure alcohol and 8 ml of 50% alcohol for each needle. At that time, the patient was very satisfied with loss of abdominal pain. About 30 minutes after injection of alcohol, suddenly patient complained of severe burning pain on back and both extremities. Thereafter, loss of sensation and paralysis in both extremities were developed slowly. The senstivity to cold recovered 3 days after block. By the 33rd day after the block, sensation had recovered in both extremities, the bladder and rectum. Movement of the right ankle joint and left great toe was also possible.

Proposal of Applying the Exercise Program for the Prevention of Work-related Chronic Low Back Pain

  • Objective: The purpose of this research is to provide exercise programs for the prevention of work related chronic back pain. Background: In order to prevent musculoskeletal disease, including proper medical care health promotion programs are needed. Method: This is a research of musculoskeletal disease looking at 618 workers working at a car engine manufacturing factory from April to July of 2008. Through questionnaire specific areas of musculoskeletal diseases experienced by the workers were identified and preventative exercise program for chronic low back pain was recommended. Result: Research showed that of the musculoskeletal disease experienced by the workers, 197 presented with low back pain, 171 presented with shoulder pain, 64 presented with neck pain and 44 presented with knee pain. The symptoms of low back pain included stiffness(143), twinge and burning sensation(24) and absence of sensation(19). Using this result 4 types of exercise programs were recommended for prevention of chronic low back pain. Conclusion: Preventative exercise programs recommended for the workers in this research is easily accessible for the workers. Use of the suggested exercise programs will inevitably decrease work related low back pain. Also 2 other recommendations were made: 1) Internal structural change may be necessary using ergonomics. 2) More exercise programs to be used to increase adaptation and tolerance of joints and muscles that are constantly used for repetitive work. Application: This study can be used to provide for the prevention of work-related Chronic Low Back pain.

The Use of Locally Applied Vibration to Minimize Pain during Fractional CO2 Laser Therapy in Living Liver-Donor Scar Management

  • Background Fractional $CO_2$ laser is an effective treatment for scars, but most patients complain about sharp burning pain, even after the application of lidocaine ointment. This study analyzed the impact of a vibrating device to nonpharmacologically reduce the acute pain of laser treatment, in accordance with the gate control theory of pain management. Methods This is a prospective study performed from May 2013 through March 2014. Fifty-three patients (mean age, 26.7 years; range, 16-44 years) who had donated livers for liver transplantation were treated with a fractional $CO_2$ laser (10,600 nm; model $eCO_2$, Lutronic Corp) for their abdomen scars. Laser treatment was applied 4 months after surgery. A commercially available, locally applied vibrating device (model UM-30M, Unix Electronics Co. Ltd.) was used, in an on-and-off pattern, together with the $CO_2$ laser. A visual analogue scale (VAS; 0, no pain; 10, most severe pain) of pain sensation was assessed and statistically analyzed using a paired t-test. Results The average VAS score for pain with the vibrating device was 4.60 and the average VAS score without the vibrating device was 6.11. The average difference between scores was 1.51 (P=0.001). Conclusions A locally applied vibrating device was demonstrated to be effective in reducing pain when treating with a fractional $CO_2$ laser. Vibration treatment could be helpful when treating scars with fractional $CO_2$ laser in pain-sensitive patients, particularly children.

신경병증성 통증에 대한 전침자극 효과의 연구 (Effects of Electroacupuncture on Neuropathic pain in Rats)

  • Objcetive : Neuropathic pain sometimes arises from a partial peripheral nerve injury. This kind of pain is usually accompanied by spontaneous burning pain, allodynia and hyperalgesia. It has been well known that acupuncture is effective to the pain control from ancient time in Asia. However, it is not clear whether acupuncture can control neuropathic pain. The aim of the present study is to examine if acupuncture stimulation may be effective to the mechanical allodynia in a rat model of neuropathic pain. Methods : To produce neuropathic pain, under sodium pentobarbital anesthesia, the right superior caudal trunk was resected between the S3 and S4 spinal nerves. After the neuropathic surgery, we examined if the animals exhibited the behavioral signs of mechanical allodynia. The mechanical allodynia was assessed by stimulating the tail with von Frey hair (bending force : 2.0g). three or 6 weeks after the neuropathic surgery, acupuncture stimulation was delivered to Houxi (SI 3) as the following parameters (2HZ frequency, 0.07mA intensity and 3msec duration) for 30 minutes. Results : The stimulation of Houxi (SI 3) acupoint relieved the behavioral signs of mechanical allodynia. Conclusion : Our results suggest that acupuncture can control the mechanical allodynia of neuropathic pain.

타액분비저하에 따른 구강작열감증후군 환자의 임상적 특징 비교 (Comparison of the Clinical Characteristics of Patients with Burning Mouth Syndrome due to Hyposalivation)

  • Objectives: The aim of this study was to investigate the clinical characteristics of patients with Burning Mouth Syndrome (BMS) due to hyposalivation (HS). Methods: We reviewed the clinical records of 39 BMS patients who visited the Department of Digestive Diseases of Kyunghee Korean Medicine Hospital from March 1st, 2020 to July 31st, 2020. The subjects were classified according to the presence or absence of hyposalivation and both groups were compared for the proportion of coated tongue, heart rate variability (HRV), Ryodoraku, and the numeral rating scale (NRS) score of tongue pain results. Results: The BMS with Hyposalivation (HS group) and the BMS without Hyposalivation (Non-HS group) showed a significant difference in the proportion of coated tongue and the NRS score for tongue pain. The NRS score was significantly higher in the Non-HS group and the proportion of coated tongue was lower. However, no significant differences were noted in several HRV parameters between the two groups. The most frequent accompanying symptoms were xerostomia and dyspepsia. Conclusions: The results of this study suggest that hyposalivation might be one of the main causes of tongue pain, the key complaint in BMS patients. Sympathetic/parasympathetic imbalance might not be a main contribution of hyposalivation in BMS. Instead, factors such as the number of medications taken seem to correlate with hyposalivation in BMS. This results could be useful in the management of BMS patients with hyposalivation in clinical practice.
