Purpose : To evaluate the effects joint mobilization and McKenzie exercise on the cervical range of motion and tenderness through cervical range of motion(CROM), algometer. Methods : The subjects consisted of thirty five patients. Eighteen underwent McKenzie exercise, seventeen did Joint Mobilization. The joint mobilization group received joint mobilization exercise for about 15 minutes, and McKenzie exercise group received extension, side-flexion, flexion, elevation, exercise for 15 minutes respectively. The test period of each group took place three times a week during 6 weeks. All measurements for each the subject took the following treatment: pre-treatment, treatment in 2 weeks, treatment in 6 weeks, post-treatment in 2 weeks. Results : The flexion, extension and side-flexion CROM of both groups were statistically significant increase within treatment period(p<.05), and also a significant difference within pre-treatment and post-treatment in a 2 week period(p<.05), within treatment in 2 weeks and post-treatment in 2 weeks by checking the extent of effect in the treatment period(p<.05). The trapezius tenderness threshold of both groups were statistically significant increase within treatment period(p<.05), and also significant difference within pre-treatment and post-treatment, within treatment in 2 weeks and post-treatment by checking the extent of effect in the treatment period. Conclusion : Joint mobilization and Mckenzie exercise improved cervical range of motion and tenderness threshold.

Previous reports have suggested that physical and psychological stresses may trigger fibromyalgia (FM). Stress is an important risk factor in the development of depression and memory impairments. Antidepressants have been used to prevent stress-induced abnormal pain sensation. Among various antidepressants, tianeptine has been reported to be able to prevent neurodegeneration due to chronic stress and reverse decreases in hippocampal volume. To assess the possible effect of tianeptine on FM symptoms, we constructed a FM animal model induced by restraint stress with intermittent cold stress. All mice underwent nociceptive assays using electronic von Frey anesthesiometer and Hargreaves equipment. To assess the relationship between tianeptine and expression levels of brain-derived neurotrophic factor (BDNF), cAMP response element-binding protein (CREB), and phosphorylated cAMP response element-binding protein (p-CREB), western blotting and immunohistochemistry analyses were performed. In behavioral analysis, nociception tests showed that pain threshold was significantly decreased in the FM group compared to that in the control group. Western blot and immunohistochemical analyses of medial prefrontal cortex (mPFC) and hippocampus showed downregulation of BDNF and p-CREB proteins in the FM group compared to the control group. However, tianeptine recovered these changes in behavioral tests and protein level. Therefore, this FM animal model might be useful for investigating mechanisms linking BDNF-CREB pathway and pain. Our results suggest that tianeptine might potentially have therapeutic efficacy for FM.
Background: Following peripheral nerve injury, rats will show a tactile allodynia and hyperalgesia. But the mechanism of allodynia is still obscure. Previous studies have shown this allodynia was reversed by intrathecal alpha-2 agonists and NMDA antagonists, but not by morphine. In formalin test, either the pretreatment of NMDA antagonist or morphine prevents the hyperalgesia. The present studies, using rats rendered allodynic by ligation of the left L5 and L6 nerves, aimed to investigate the effects of pretreatment of MK-801 and morphine on the development of tactile allodynia. Methods and Material: Male Sprague-Dawley rats (100~150g) were anesthetized with halothane, the left L5 and L6 spinal nerves were ligated tightly by 6-0 black silk. For sham operation muscle dissection was performed but the spinal nerve was not ligated. For pretreatment of drugs, MK-801 (NMDA antagonist; 0.3 mg/kg). CNQX (non-NMDA) antagonist; 0.3 mg/kg), morphine (1 mg/kg) or saline (placebo) was administered subcutaneously 30 minutes before operation. A second dose was administered subcutaneously 24 hours after operation and further doses were given daily for 2 days further. The volume of injection was 5 ml/kg. To assess the mechanical allodynia, paw withdrawal thresholds of ipsilateral limb were determined using 8 von Frey hairs. Results: Within 2 days saline, CNQX or morphine injected rats developed tactile allodynia (paw withdrawal threshold was about 2g), and persisted for over 2 weeks. Pretreatment of MK-801 delayed the development of tactile allodynia for 3 days comparing to that of saline injected rat. Conclusion: NMDA receptor in the central nerve system plays an important role in the development of tactile allodynia induced by peripheral nerve injury. But the mechanism may be different from hyperalgesia developed in formalin test.

Purpose: To determine whether upregulation of inducible nitric oxide synthase (iNOS) transcription and translation is related to radicular pain in a model of lumbar disc herniation. Also, to investigate the temporal changes of mRNA expression of iNOS and the identity of iNOS and transient receptor potential vanilloid (TRPV) 1 channel expression cells in dorsal root ganglion (DRG) of a model of lumbar disc herniation. Methods: A lumbar disc herniated rat model was developed by implantation of the autologous nucleus pulposus, harvested from the coccygeal vertebra of each tail, on the left L5 nerve root just proximal to the DRG. Rats were tested for mechanical allodynia of the plantar surface of both hind paws 2 days before surgery and 1, 5, 10, 20 and 30 days postoperatively. Reverse transcription polymerase chain reaction (RT-PCR) was used to follow iNOS mRNA expression. To stain iNOS and TRPV1 in DRG, an immunohistochemical study was done 10 days after surgery. Results: A significant drop in mechanical withdrawal threshold on the ipsilateral and contralateral hind paws was observed 1 day after surgery and was prolonged to 30 days in rats with lumbar disc herniation. The expression of mRNA for iNOS peaked at postoperative day 10 on both sides of the DRG. iNOS-positive sensory neurons in the DRG varied in size from large to small diameter cells. A majority of small and intermediate sensory neurons were TRPV1-positive cells. Double immunofluorescence staining for TRPV1 and iNOS revealed that most intermediate TRPV1-positive sensory neurons co-localized with iNOS-positive neurons. Conclusion: Nucleus pulposus-induced mechanical allodynia can be generated without mechanical compression. This pain is related to temporal changes in expression of iNOS mRNA in the DRG. Co-localization of TRPV1 and iNOS in intermediate neurons of the DRG is correlated with pain modality and intensity.

The purpose of this study was to evaluate the analgesic effect on pulp induced by pulsed Nd : YAG laser according to the power density, irradiated time and the method of application (Non-contact vs contact) in vivo. Experiments were carried out on 90 volunteers who have sound 4 canines. Each canine was grouped by the intensity and the time of irradiation of pulsed Nd : YAG laser was measured. Mean EPT response(units) was measured at 5 min, 30 min, and 60 min after irradiation with various intensity(1.5-3W, 15-30Hz), irradiated time (1.5min, 3min), and application method (Non-contact and contact). The results were as follows: 1. 5 minutes after irradiation, the mean EPT response(units) was increased by 32 % in the non-contact method and by 35% in the contact method of application. Which showed significant difference in mean EPT response(units) compared to not-lased canine in each groups(P<0.05). 2. The mean EPT response(units) with irradiating condition according to the various laser intensity, and the irradiation time stastically did not show significant difference in each groups as time elapsed. 3. There was no significant difference between the non-contact and contact method in the mean EPT response(units). 4. The mean EPT response(units) returned to the baseline value within 30 miuntes. In regard to the above results, the analgesic effect on pulp induced by pulsed Nd : YAG laser resulted a slight increase of pulpal pain threshold, but it was not as high as to replace the role of drug analgesia as whole. This technique can be used for inducing pulpal analgesia as adjunct to other pain control methods and is needed to develop better analgesic effects.

Background: Effect of nitric oxide on the hyperalgesia induced by inflammation is controversial. From our previous experiment, NOS inhibitor, L-NAME given during the induction period decrease mechanical hyperalgesia occured by Freund's complete adjuvant induced inflammation. In addition, we attempted to analyze the effects of L-NAME on mechanical hyperalgesia after the development of inflammation by Freund's complete adjuvant in rat paw. Methods: Male Sprague Dawley rats were divided into four groups; control (normal saline), and three different doses of L-NAME (0.1 mg, 1 mg, 10 mg). Inflammation was induced in rats by injecting 0.15 ml of Freund's complete adjuvant (FCA) intraplantarly. Rats showed typical hyperalgesia within twelve hours after injection and maintained this for about one week. Tests were done 2 days after injection of FCA. After the baseline test either L-NAME or saline was injected under light halothane anesthesia. Effect of L-NAME on hyperalgesia was assessed by measuring mechanical hyperalgesia at 15, 30, 60, 90, 120 minutes. Same experients were repeated on normal rats. Results: When injected at the site of inflammation, L-NAME caused dose dependent decrease in mechanical hyperalgesia. However, normal rats also showed increased mechanical threshold after L- NAME injection. Conclusions: Although L-NAME reduces FCA induced mechanical hyperalgesia, this result may solely be due to inhibition of nitric oxide production and need to be further determined.

Non-suicidal self-injury (NSSI) has been noted as a significant risk factor for possible suicide attempts. According to the Interpersonal Psychological Theory of Suicide, individuals can attempt suicide after they have acquired the capability to commit it. The acquired capability for suicide can be defined as one's habituated level to the pain and fear associated with suicidal behaviors. This is obtained through constant exposure to painful and provocative events, NSSIs being a prime example. This article reviews prior related studies to determine the extent to which the acquired capability for suicide can be utilized as a predictive factor for fatal suicide attempts following NSSIs. Our review finds a total of 11 studies that directly or indirectly support the claim that the acquired capability for suicide should be considered as a relevant factor linking NSSIs and suicide attempts. Given that NSSIs are most frequently observed in clinical settings, our findings suggest that the acquired capability for suicide will be a useful indicator for clinicians to predict the risk of future suicide attempts by patients.
Objectives : This study was designed to evaluated the effect of electroacupuncture on Quality of life of patients with urinary incontinence Methods : Subjects were voluntarily recruited by newspapers and internet. Electroacupuncture was performed three times a week for 3 weeks. Acupuncture point for EA group was B32, Electrical stimulation frequency was 2Hz, duration 20 minutes, and intensity was up to pain threshold according to patients. The patients's symptoms were assessed before, after 3 weeks of treatment by QOL item of International Prostate Symptom Score(IPSS), Medical Outcomes Study(MOS) 36-Item Short-Form Health Survey(SF-36). Results : QOL score of IPSS were significantly improved after 3 weeks(p<0.05) compared to the pre-treatment. There were significant changes in Social functioning(SF), role-physical(RP), role emotional(RE), mental health(MH), bodily pain(BP) score of SF-36 after 3 weeks(p<0.05), but there were no significant changes in physical functioning(PF), vitality(VT), general health(GH) score of SF-36. Conclusion : This study suggests that electroacupuncture treatments can be applicable to improve symptoms in patients with urinary incontinence.
Sanpoongdan(SPD) has been known effective on infantile convulsive disorders in clinical field of oriental medicine. The purpose of this study was to investigate the anti-convulsive, sedation and analgesic effects of SPD in mouse. The anticonvulsive effect was evaluated In mice treated with pentylenetetrazol, stychnine, and picrotoxin. For the sedative effect, observations were made on the sleeping time induced by thiopental sodium and pentobarbital sodium following oral administration of SPD. Furthermore, reduction of spontaneous movements and ataxia using rota rod method were evaluated. Analgesic effects on the writhing syndrome induced by acetic acid and on hindlimb pain induced by pressure were also observed. The findings were as follows : 1. The solid extracts of SPD revealed no effect on convulsions induced by pentylenetetrazol, strychnine, and picrotoxin. 2. Thiopental sodium-induced sleeping time was prolonged by the administration of the solid extracts of SPD, but this result was devoid of statistical significance. 3. The oral administration of SPD enhanced the sleeping induced by pentobarbital sodium. 4. Spontaneous movements were significantly depressed following the oral administration of the solid extracts of SPD. 5. Ataxia was not shown in rota rod method following the oral administration of the solid extracts of SPD. 6. The solid extracts of SPD showed positive analgesic effects on the acetic acid-induced writhing syndrome. 7. The solid extracts of SPD raised the threshold of the hindlimb pressure pain, but the result was not statistically significant. From the results, it can be concluded that SPD has sedative and analgesic effects.
Objectives: This exploratory trial evaluated the possibility of quantitatively measuring several aspects during the "abdominal examination" of traditional Korean medicine. The main results of this study will be used to develop a new diagnostic device for abdominal examinations. Methods: Fifteen healthy volunteers were recruited for this study. Three certified Korean medical doctors assessed the existence of pressure pain, the elasticity of the abdominal muscle, and the tonus at CV12 as gold standards. Then, 2 well-trained investigators measured the pressure pain threshold (PPT), the indentation depth of a bar, and repelling force by pressing CV12. Each investigator measured the above 3 variables 2 times at 1 min intervals using a modified digital algometer. Reliability and validity tests of the 3 variables were performed. Results: There were statistically significant coefficients of intraclass correlation on the 3 variables both between and within the investigators (P<0.001). Also, PPT and repelling force showed statistically significant high sensitivity and specificity in a ROC curve. However, the indentation depth of the bar presented relatively low sensitivity. Conclusions: This new diagnostic method using a modified digital algometer could be a useful tool for quantitative measurement in "abdominal examinations". However, future rigorous clinical studies with a large population will be needed for the verification of its usefulness.
