A Case Report of a Mucinous Adenocarcinoma Patient Treated with Korean Medicine after Wedge Resection of the Lung (점액성 선암종으로 폐의 쐐기절제술을 시행한 환자에 대한 한방치료 1례)

  • The purpose of this study is to report and demonstrate the effect of Korean medical treatment on a wedge resection of a lung patient with mucinous adenocarcinoma. The patient was an 84-year-old female who was diagnosed with mucinous adenocarcinoma and underwent wedge resection. The patient was treated with acupuncture and herbal medicine (Banhahubak-tang and Gamiondam-tang). The EORTC Quality of Life Questionnaire, Core 30 (EORTC QLQ-C30), EORTC Quality of Life Questionnaire, Lung Cancer 13 (EORTC QLQ-LC13), numeric rating scale (NRS), and the Global Assessment (G/A) were used to assess the changes in symptoms, including operation site pain and dyspnea. After treatment, the patient showed improvement in operation site pain and dyspnea, based on the NRS and G/A. However, dyspnea and pain were aggravated based on the EORTC QLQ-C30 and EORTC QLQ-LC13. Therefore, Korean medicine may have a substantial benefit for patients with mucinous adenocarcinoma after wedge resection of the lung.

  • Objective : To report a case of cervical block vertebra with neck pain successfully treated with conservative Korean Traditional Medicine treatment with Thoracic Chuna Manipulation. Methods : A patient diagnosed with block vertebra was treated with Thoracic Chuna manipulation, acupuncture, pharmacoacupuncture, and herbal medicine. Numeric Rating Scale(NRS) and Neck Disability Index(NDI) scores were collected before and after treatment for comparison. Results : There was a significant decrease in NRS and NDI scores. The mean NRS score decreased from 8 to 1, and NDI from 50 to 8. Conclusion : Conservative Korean Traditional Medicine treatment appears to be effective for treatment of cervical block vertebra.

Radiographic Analysis of Instrumented Posterolateral Fusion Mass Using Mixture of Local Autologous Bone and b-TCP (PolyBone$^{(R)}$) in a Lumbar Spinal Fusion Surgery

  • Objective : Although iliac crest autograft is the gold standard for lumbar fusion, the morbidity of donor site leads us to find an alternatives to replace autologous bone graft. Ceramic-based synthetic bone grafts such as hydroxyapatite (HA) and b-tricalcium phosphate (b-TCP) provide scaffolds similar to those of autologous bone, are plentiful and inexpensive, and are not associated with donor morbidity. The present report describes the use of Polybone$^{(R)}$ (Kyungwon Medical, Korea), a beta-tricalcium phosphate, for lumbar posterolateral fusion and assesses clinical and radiological efficacy as a graft material. Methods : This study retrospectively analyzed data from 32 patients (11 men, 21 women) who underwent posterolateral fusion (PLF) using PolyBone$^{(R)}$ from January to August, 2008. Back and leg pain were assessed using a Numeric Rating Scale (NRS), and clinical outcome was assessed using the Oswestry Disability Index (ODI). Serial radiological X-ray follow up were done at 1, 3, 6 12 month. A computed tomography (CT) scan was done in 12 month. Radiological fusion was assessed using simple anterior-posterior (AP) X-rays and computed tomography (CT). The changes of radiodensity of fusion mass showed on the X-ray image were analyzed into 4 stages to assess PLF status. Results : The mean NRS scores for leg pain and back pain decreased over 12 months postoperatively, from 8.0 to 1.0 and from 6.7 to 1.7, respectively. The mean ODI score also decreased from 60.5 to 17.7. X-rays and CT showed that 25 cases had stage IV fusion bridges at 12 months postoperatively (83.3% success). The radiodensity of fusion mass on X-ray AP image significantly changed at 1 and 6 months. Conclusion: The present results indicate that the use of a mixture of local autologous bone and PolyBone$^{(R)}$ results in fusion rates comparable to those using autologous bone and has the advantage of reduced morbidity. In addition, the graft radiodensity ratio significantly changed at postoperative 1 and 6 months, possibly reflecting the inflammatory response and stabilization.

Korean Medicinal Combination Treatments for Failed Back Surgery Syndrome: A Retrospective Case Series (한의학적 복합치료에 의해 호전된 척추수술실패증후군(Failed Back Surgery Syndrome)환자 10례: 후향적 증례연구)

  • Objectives : This study is an evaluation of the effectiveness of treatments combined with Korean medical methods, such as relaxative Chuna and Gangchuk herbal medicines, for patients with failed back surgery syndrome. Methods : In this study, medical records of ten patients who experienced failed back surgery syndrome and participated in intensive Korean medical treatment were reviewed. All patients received relaxative Chuna and acupuncture along with Gangchuk herbal medicines during hospitalization. Numeric rating scale(NRS) in the degree of 0 to 10, pain-free walking distance(PFWD) and oswestry disability index(ODI) were measured before and after treatments. Results : The patients were hospitalized for an average of 22 days. The average NRS of lumbar pain in all patients decreased from $7.7{\pm}1.6$ to $3.2{\pm}2.2$ and the average NRS decline of leg pain was from $7.7{\pm}1.7$ to $3.2{\pm}2.2$, both of which indicate statistical significance(p < 0.05). PFWD in all patients increased from 113.3 m to 798.1 m on average. The average ODI in all patients significantly improved from $63.9{\pm}7.0$ to $43.9{\pm}6.9$(p < 0.05). Conclusions : The treatments combined with Korean medical methods for failed back surgery syndrome patients are clinically effective and viable.

Unstable Pathologic Vertebral Fractures in Multiple Myeloma : Propensity Score Matched Cohort Study between Reconstructive Surgery with Adjuvant Radiotherapy and Radiotherapy Alone

  • Objective : Although radiotherapy (RT) is recommended for multiple myeloma (MM) involving spine, the treatment of choice between reconstructive surgery with RT and RT alone for pathologic vertebral fractures (PVFs) associated with structural instability or neurologic compromises remains controversial. The purpose of this study was to evaluate the clinical efficacies of reconstructive surgery with adjuvant RT for treatment of MM with PVFs by comparing with matched cohorts treated with RT alone. Methods : Twenty-eight patients underwent reconstructive surgery followed by RT between 2008 and 2015 in a single institution, for management of PVFs associated with structural instability of the spine and/or neurologic compromises (group I). Twenty-eight patients were treated with RT alone (group II) after propensity score matching in a 1-to-1 format based on instability of the spine, as well as age and performance. Clinical outcomes including the overall survival rates, duration of independent ambulation, neurological status, and numeric rating scale (NRS) for back pain were compared. Results : Clinical and radiological features before treatment were similar in both groups. The median survival period was similar between the two groups. However, the mean duration of independent ambulation was significantly longer in group I (88.8 months; 95% confidence interval [CI], 66.0-111.5) than in group II (39.4 months; 95% CI, 25.2-53.6) (log rank test; p=0.022). Deterioration of Frankel grade (21.4% vs. 60.7%, p=0.024) and NRS for back pain (2.7±2.2 vs. 5.0±2.7, p=0.000) at the last follow-up were higher in the group II. Treatment-related complications were similar in both groups. Conclusion : In patients with unstable PVFs due to MM, reconstructive surgery may yield superior clinical outcomes compared with RT alone in maintaining independent ambulation and neurological status, as well as pain control despite similar median survival and complications.

Korean Medical Rehabilitation for Total Knee Replacement (슬관절 전치환술 후 한방재활치료의 효과 보고)

  • The objective of this study is to report the effectiveness of Korean medical rehabilitation for total knee replacement. Three female patients who had total knee replacement were treated by Korean medical rehabilitation with protocol. We evaluated the consequences by checking numeric rating scale (NRS), Lysholm knee score, physical examination of knee joint and walking state. After Korean medical rehabilitation, we find out that the pain and function were improved. NRS, Lysholm knee score, physical examination of knee joint, walking state of patients were also improved. Korean medical rehabilitation can be effectively used for total knee replacement patients. There is a limit that this study hasn't sufficient number of cases. Further studies are needed to set up a Korean medical rehabilitation protocol.

Therapeutic effect of intraductal irrigation of the salivary gland: A technical report

  • Purpose: Obstructive and inflammatory disease often occurs in the major salivary glands, and no predictive treatment has yet been developed for this condition. The aim of this report was to introduce an intraductal irrigation procedure and to illustrate its application to practical patient cases. Materials and Methods: Two patients complaining of pain and swelling in the parotid gland during meals who underwent sialography were diagnosed as having sialodochitis with sialadenitis. Intraductal irrigation was then performed on the parotid gland on the side of the complaint. The irrigation procedure was conducted in the same manner as the sialography procedure, except that saline was used as the filling solution. Symptom severity was evaluated with a numerical rating scale (NRS) at the initial visit and a month after the irrigation. Results: The initial NRS value of patient 1 was 10. The value decreased to 6 and then to 0 after 2 irrigation procedures. The NRS value of patient 2 regarding the symptoms involving the left parotid gland decreased from 4-5 to 1 after 4 irrigation procedures performed at 1-month intervals. Conclusion: Intraductal irrigation of the salivary gland may be a simple, safe, and effective treatment option for patients with obstructive and inflammatory disease of the salivary gland that is capable of resolving their symptoms.

  • Objectives: The purpose of this study was 1. To investigate correlation of subjective stress with patient's prognosis 2. To observe the effect of Kami-shoyo-san on anxiety and depressive disorder. Methods: We investigated 3 cases of in-patients from traffic accidents. Patients were treated with Kami-shoyo-san. The Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were used to assess anxiety and depression, respectively. We also used Qi stagnation test and Qi counterflow test according to Terasawa's criteria for diagnosis of anxiety and depression. Pain was evaluated by Numeric Rating Scale (NRS). Results: After treatment, the patient's symptoms improved considerably. Conclusions: 1. There were significant correlations between subjective stress and prognosis. 2. Kami-shoyo-san has good effect on anxiety and depressive disorder.

Open Versus Arthroscopic Technique in the Traumatic Recurrent Anterior Dislocation of the Shoulder (견관절 전방 재발성 탈구에 대한 관혈적 술식과 관절경적 술식의 결과 비교)

  • Purpose: We compared the results of open and arthroscopic Bankart repair in traumatic recurrent anterior dislocation ,3f the shoulder. Materials and methods: We analysed 7 cases underwent open Bankart repair (group I) and 13 cases underwent arthroscopic Bankart repair (group Ⅱ). The average follow-up period was 68.1 months (51-113 months) in group I and 41.1 months (16~57 months) in group Ⅱ. All patients in group I and Ⅱ were non-athletes. We analyzed statistically objective evaluation, such as the stability of shoulder joint, the range of motion, pain, impaired throwing, Bankart rating system by Rowe and subjective evaluation, visual analog scale (VAS) between two groups. Results: In terms of dominant and non-dominant shoulders, the age at initial episode of dislocation, the elapsed time from injury to surgery, the number of preoperative dislocations associated with susceptibility to apprehension. respectively, there was no statistically significant differences between two groups. In group I the average Rowe's scortls was 84.3 and 3 cases (43%) had excellent results,4 cases (S7cfo), good ones. In group H the average Rowe's scores was 87.3 and 7 cases (54%) had excellent results,6 cases, good ones. There was tendency to show more excellent results in group ll, but there was no statistically significant differences. The average VAS were 90.3 points in group I and 88 points in group Ⅱ, which showed also no statistically significant differences. Conclusion: Open and arthroscopic Bankart repairs had no significant difference and showed also good results in travinatic recurrent anterior dislocation of shoulder.

Inferior Capsular Shift Procedure for Multidirectional Instability of the Shoulder in Contact Athletes (접촉성 운동선수의 다방성 불안정 견관절의 하관절낭 이동술)

  • Initial treatment of multidirectional instability of the shoulder would be a thorough rehabilitation program. If rehabilitation fails to resolve a patient's symptoms, the most commonly performed surgical procedure remains the inferior capsular shift. Eleven patients who had disabling multidirectional instability of the shoulder were managed with the inferior capsular shift. All of the procedure were performed by means of an anterior approach and a laterally based capsular shift. All of the patients were the contact athletes. The purpose of our study was to evaluate the efficacy of the inferior capsular shift procedure in the contact athletic patients, to review the loss of external rotation postoperatively and to discuss their return to sports. Mean follow up duration was 14.5 months(range, 12 to 24). Pain was relieved postoperatively in all cases and most patients could get stability except one case of recurrent subluxation. The average loss of external rotation and forward flexion after the operation were Y and 30 at last follow up. By the rating scale from American Shoulder and Elbow Society, overall scores improved from 49 points to 85 points. The results were excellent or good in 9 patients(82%) out of 11 patients. In reference to return to sports, 10 patients(91 %) of 11 patients returned to their sports with 7 patients (64%) returning at the same levels of competitiveness. The inferior capsular shift procedure was considered to be a recommendable method for the management of the multidirectional instability of the shoulder.
