A Retrospective Study on 15 Patients of Chronic Prostatitis/ Chronic Pelvic Pain Syndrome (만성 전립선염/만성 골반통증 증후군 환자 15례에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the clinical effect of herb medicine (Indongsoyeom-bang) and acupuncture therapy on Chronic Prostatitis/Chronic Pelvic Pain Syndrome. Methods : 15 patients diagnosed with Chronic Prostatitis/Chronic Pelvic Pain Syndrome were enrolled in this study. They were treated with herb medicine (Indongsoyeom-bang) and acupuncture therapy. To evaluate the therapeutic effect, before and after we measured Extract Prostatic Secretion (EPS), National Institutes of Health Chronic Prostatitis Symptom Index (NIHCPSI) and International Prostatitis Symptom Score (IPSS). Result : After treatment WBC numeral index by EPS test and NIH-CPSI and IPSS decreased. Conclusion : Herb medicine (Indongsoyeom-bang) and acupuncture therapy are effective in treating Chronic Pelvic Pain Syndrome. Further study of mechanism and efficacy is needed.

Treatment of the chronic pelvic pain and complications of post-ovarian cystectomy with laparoscopy : a case report (난소낭종제거수술 후 내원한 만성골반통 환자 1례에 대한 증례보고)

  • Purpose : Chronic pelvic pain(CPP) is a common gynecologic symptoms. Recent research has been directed towards understanding the pathology of CPP, but many questions have existed. And the study for treating CPP has been tried by various manners. A Korean medicine(KM) also try to treat a various methods for treatment of CPP. It is often that a pelvic pain is continue a post-hysterectomy or a operating of gynecology. We have a case of CPP treatment by KM that is a patient continue pain after post-ovarian cystectomy with laparoscopy. So we report a case on pelvic pain and complication of post-ovarian cystectomy with laparoscopy. Methods : A 44 years old woman, who suffered from chronic pelvic pain, dizziness, nausea, dyspepsia, general weakness, insomnia after ovarian cystectomy, was enrolled in this study. She received KM therapies such as herbal medicine, moxibustion, acupuncture for 4 weeks. Results : CPP and symptoms after ovarian cystectomy were reduced by KM therapies. Conclusion : 'The present study suggests that KM therapies have a significant effect on CPP and complications of post-ovarian cystectomy with laparoscopy.

Effects of trunk Muscles Endurance, Hip Joint Muscular Strength, and Pelvic Alignment on Mild Low Back Pain

  • Objective: Low back pain easily becomes chronic and has a high recurrence rate. Therefore, it is most important to prevent chronicity and reduce the risk of recurrence in the early stages of back pain or at the stage with mild pain. Therefore, this study was conducted to compare hip joint muscle strength, trunk muscle endurance, and pelvic alignment between subjects with mild low back pain and subjects without back pain. Design: Crossed-sectional study Methods: The study was conducted by recruiting 30 students in their twenties who are enrolled in K University in Gyeongsangnam-do, and classifying them into 15 patients with mild back pain and 15 patients with normal. The subjects who participated in the experiment were measured for hip flexor and extensor muscle strength, trunk flexion and extension muscle endurance, and pelvic alignment. To measure hip joint muscle strength, biodex was used, and muscle endurance of the trunk was recorded at the end range of the trunk flexion and extension. And pelvic alignment was measured using Formetric 4D. Results: There were no significant differences in hip joint muscle strength, pelvic alignment, and trunk extension muscle endurance. The retention time was found to be significantly shorter in the mild low back pain group than in the normal group for trunk flexion muscle endurance. Conclusions: In the early stages of back pain or in the mild pain stage, training to increase muscle endurance of the flexor muscles may be helpful.

  • Chronic prostatitis/Chronic pelvic pain syndrome (CP/CPPS) is a common disorder to men that involved urinary symptoms and sexual activity-related discomfort. As a pathophysiology of the diseases has not been established, CP/CPPS is diagnosed on the basis of symptoms, principally pain or discomfort in the pelvic region. The purpose of this study is to report the clinical effects of Korean medicine, Sibimijihwang-tang(十二味地黃湯), bee venom pharmacopuncture, and acupuncture treatment on 6 cases of CP/CPPS. 6 patients were treated with Korean medicine, Sibimijihwang-tang and acupuncture and bee venom pharmacopuncture was given into Hoeeum(CV1) et al. National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) was used to assess treatment effect. After treatment, NIH-CPSI decreased significantly all the patients. Sibimijihwang-tang, bee venom pharmacopuncture and acupuncture treatment was shown fairly effective to CP/CPPS.

Comparison of the Effects of Pelvic Compression Using Instruments on Trunk Muscle Endurance and Balance Ability in Subjects in Their Twenties With or Without Low Back Pain (기구를 이용한 골반 압박이 20대의 요통 경험자와 비경험자의 체간 근지구력과 균형 능력에 미치는 영향 비교)

  • Background: Low back pain (LBP) is a representative disease, and LBP is characterized by muscle dysfunction that provides stability to the lumbar spine. This causes physical functional problems such as decreased posture control ability by reducing the muscular endurance and balance of the lumbar spine. Pelvic compression using instruments, which has been used during recent stabilization exercises, focuses on the anterior superior iliac spine of the pelvis and puts pressure on the sacroiliac joint during exercise, making the pelvis more symmetrical and stable. Currently, research has been actively conducted on the use of pelvic compression belts and non-elastic pelvic belts; however, few studies have conducted research on the application effect of pelvic compression using instruments. Objects: This study aimed to investigate whether there is a difference in trunk muscular endurance and dynamic and static balance ability levels by applying pelvic stabilization through a pelvic compression device between the LBP group and the non-LBP group. Methods: Thirty-nine subjects currently enrolled in Daejeon University were divided into 20 subjects with LBP group and 19 subjects without LBP (NLBP group), and the groups were compared with and without pelvic compression. The trunk muscular endurance test was performed with 4 movements, the dynamic balance test was performed using a Y-balance test, and the static balance test was performed using a Wii balance board. Results: There was a significant difference the LBP group and the NLBP group after pelvic compression was applied to all tests (p < 0.05). In the static and dynamic balance ability test after pelvic compression was applied, there was a significant difference in the LBP group than in the NLBP group (p < 0.05). Conclusion: These results show that pelvic compression using instruments has a positive effect on both those with and without LBP and that it has a greater impact on balance ability when applied to those with LBP.

A Clinical Report on Two Patients with Chronic Pelvic Inflammatory Disease (만성 골반염 환자 2례의 임상증례보고)

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatments on a chronic pelvic inflammatory disease (CPID). Methods: We treated 2 cases of chronic pelvic inflammatory disease patients with Korean herbal medicine, acupuncture and moxibustion. One patient is a 50 years old woman, who suffered from uterine bleeding, lower abdominal pain, right pelvic pain. She was treated with Gwichuligyeong-tang-gami and Samul-tang-gami. Other patient is a 40 year old woman, who appealed for pain such as pelvic pain, lower abdominal pain, urinary frequancy, dysuria. She was treated with Cheongpochugeo-tang. Both patients are taken acupuncture at Junggeuk (CV3), Gwanwon (CV4), Chagung (CA111), Sameumgyo (SP6), Eumneungcheon (SP9) and moxified at Gwanwon (CV4). Results: After treatments, their symptoms were remarkably improved. Conclusions: This study suggests that Korean medicine treatments have significant effect on reducing symptoms of chronic pelvic inflammatory disease.

Relationship between Leg Length Discrepancy and Radiological Parameters of Lumbosacrum and Pelvis in Patients with Chronic Low Back Pain (만성 요통 환자의 하지 길이 부전과 요천추부 및 골반의 방사선학적 지표와의 관계)

  • Objectives : The biomechanical relationship of leg length discrepancy(LLD), Lumbar lordosis, pelvic variance and degenerative scoliosis is one of the most important parameters when treat musculoskeletal disorders, however the reports are still controversial. The purpose of this study was to compare the relationships between the parameters and bothersomeness in subject with chronic ow back pain. Methods : Sixty female and eight male adults with non specific low back pain over 3 months were recruited. LLD was measured by tape measure method. Lumbar lordosis, lumbosacral angle and related pelvic parameters were measured using simple radiologic films of lumbosacral view. Results : Lumbar lordosis was significantly correlated to the lumbosacral angle, pelvic incidence and difference of the both iliac widths. Pelvic incidence had significant correlation with difference of the both iliac widths. And difference of both iliac widths was related with LLD by radiologic film. There was also significant correlation between the LLD by radiologic film and tape measurement. Visual analogue scale(chronic low back pain) of normal lordosis group was greater than hyperlordosis group. Conclusions : There were close biomechanical relationships between lumbar, pelvis, and lower extremity. But in order to determine the effect of structure on the chronic low back pain, global balance of musculoskeletal structure seems to be worth further researching.

Induction of Local and General Analgesia by Electroacupuncture in Dogs (개에 있어서 침술에 의한 국소 및 전신마취에 관한 연구)

  • This study was performed to evaluate the possibility of inducing analgesia by electroacupuncture stimulation at single acupoint or combined acupoints and to examine the analgesic effects following the combination of premedication and electroacupunrture analgesia(EA). Analgesia was induced by EA with the current of 1-4 volts and the frequency of 10-45 Hz to the acupoints Uown to be related to analgesia on the head/necIL axial part thoracic and pelvic limb. In Yi Feng acupoint of head/neck part pain responses were not disappeared after electroacupunrture stimulation to the head/necIL thoracic limbo thoraxl abdomen, loin, rear and pelvic limb. Pain responses were remained after EA of Tian Men-Tian Ping and Shen Yu arupoints of axial park whereas hypoalgesia was observed after EA of Tian Ping-Bai Hui acupoint in all parts of body. There was no analgesic effects after EA stimulation of the brachial plexus and Wai Kuan acupoint, whereas after EA stimulation of San Yang Lo, pain responses were disappeared in headfnecll, thoracic limb and pelvic limbo and in the other parts of body hypoalgesia was shown. In EA stimulation of Tsu San Li acupoint pain responses were disappeared in pelvic limb and in San Yin Chiao acupoint pain responses were disappeared in head/necIL thoracic and pelvic limb, and hypoalgesia was shown in abdomen. On the combination of San Yang Lo Pli Men) and San Yin Chiao (Pu Yan6 acupoints, pain response in heauneck was decreased in 5 minutes, whereas analgesia in thoracic and pelvic limb was induced after 20-30 minutes and in abdomen was noted after 50 minutes. The more frequrncy was increased, the more rapid analgesic e11%t was induced. The analgesic effects wert not good in laparotomy under EA at the combination of San Yang Lo (Xi Men) and San Yin Chiao (Pu Yang) arufoints. Enteroanastomosis could not be continued under acrpromazine, xylazine and diazepam with EA. However, under EA followed by tiletaminetzolazepam, the operation could be completed without additional anesthesia and the analgesic effects were good. There were no changes in clinical signs, hematological and serological values after combination of the premeditation of tiletamine+zolaEepam and EA. It is considered that EA alone is not suitable for the main surgery, but the combination method of EA and sedatives can be utilized in practice.

The Effects of Pelvic Floor Stabilization exercise on Pain, Function, Psychosocial, EMG Activity on the Lower Back Pain with Postpartum Women (골반 안정화운동이 산후 요통 여성에게 통증, 기능장애, 심리사회적수준, 근활성도에 미치는 영향)

  • Background: This study investigated the effects of pelvic floor stabilization exercise of pain, disfunction, psychosocial, electromyography (EMG) activity on the lower back pain with postpartum. Methods: The study included 20 postpartum women who were randomly assigned to a sling exercise group (SEG, n=10) or a general physical therapy group (GPTG, n=10). Outcomes were assessed using to lower back pain with postpartum the quadruple visual analogue scale (QVAS), the Korean version of the Oswestry disability index (KODI), the fear avoidance belief questionnaire (FABQ), the inventory of functional status after childbirth (IFSAC), the edinburgh postnatal depression scale-Korean (EPDS-K), and trunk muscle activity before and after a 4-week exercise intervention. Statistical analysis were performed using a mean, standard deviation, crosstab test, paired t-test, independent t-test. Kolmogorove-Smirnov test was used for test of normality. Results: Compared to the GPTG, the SEG showed significant improvement in the QVAS, KODI, FABQ, IFSAC, and EPDK-K scores (p<.05) after 4 weeks. Conclusion: Postpartum pelvic strengthening exercise proved to have a positive effect.

Short-term Clinical Observation of the Early Postpartum Low Back Pain in the Korean Medical Hospital (한방치료를 받은 출산 초기 산모의 요통 경과 고찰)

  • Objectives: The purpose of this study was to observe the early postpartum low back pain and pelvic girdle pain in the Korean medical hospital for 2-weeks. Methods: Fifty-one postpartum women who admitted for postpartum care in Korean hospital from December 3rd, 2011 to January 31st, 2012 had written out pain measurement questionnaires(SF-MPQ, VAS, ODI) three times for 2-weeks. SPSS 18.0 for Windows was used to analyse the data and the independent samples t-test, paired T-test were used to verify the results. Results: 1. Evaluation of low back pain within the first week after birth was as follows: SF-MPQ sensory area was $5.06{\pm}4.41$ points; SF-MPQ emotional areas was $0.86{\pm}1.34$ points; VAS mean was $3.79{\pm}2.02$ points; and ODI mean was $10.27{\pm}6.69$. 2. Early postpartum low back pain and pelvic girdle pain were significantly decreased in SF-MPQ, VAS, and ODI after 2-weeks(p < 0.01). 3. Age, birth type, parity, weight, and BMI increased during pregnancy does not affect the postpartum low back pain and pelvic girdle pain. Conclusions: The top 10% of postpartum low back pain scale was 10.8 points or more in the sensory area of SF-MPQ, and 3 points or more in the affective area of SF-MPQ. In the case of VAS, it was more than 7 points, and ODI was more than 21.8 points. Postpartum back pain with oriental medical treatment was significantly improved.
