Review of measurement of prostate specific antigen: in the aspect of insurance medicine (전립선암 선별을 위한 PSA 측정의 보험의학적 의미)

  • The measurement of prostate specific antigen (PSA) in screening for prostate cancer is recently performed as a routine check-up in clinical medicine and insurance medicine. Several factors may affect serum PSA levels. As prostate size increases with increasing age, the PSA concentration also rises. Increasing body mass index (BMI) is associated with a lower mean PSA concentration. Inhibitors of 5-alpha-reductase such as finasteride and dutasteride produce a 50 percent or greater decrease in serum PSA during the first three months of therapy, which persists as long as the drug is continued. Men who are regularly taking non-steroidal antiinflammatory drugs (NSAIDs) or acetaminophen have lower PSA levels. Emerging concepts regarding PSA testing that may help refine the interpretation of an elevated concentration include: PSA density, PSA velocity, and Free versus complexed or bound PSA. With many insurance companies, PSA level has become part of a standard battery of blood tests, along with HIV, cholesterol, liver enzymes, and other predictors of premature death. But, there is no clear proof of benefit, so we have to monitor the value of PSA test as a prostate cancer screening test in insurance medicine.

Association of Prostate Specific Antigen Concentration with Lifestyle Characteristics in Korean Men

  • We investigated the relationships between demographics, lifestyle characteristics, and serum total prostate specific antigen (PSA) concentration and examined the population-based distribution of total PSA by age among 2,246 Korean men with a median age of 45 years. We obtained data about demographic and lifestyle characteristics based on self-reporting using a quetionnaire. We also performed physical examinations, anthropometric measurements, and biochemical measurements. The PSA concentration increased with age and there was a significant difference in total PSA concentration between the age groups of 21-60 years and >60 years. Age >60 years, height ${\geq}1.8$ m, a low frequency of alcohol consumption, and taking nutritional supplements showed a significantly increased odds ratio for increased PSA when 3.0 ng/mL was chosen as the PSA cut-off level. Smoking status, BMI, percent body fat, diabetes mellitus, fatty liver, herbal medicine use, vitamin use, and diet were not significantly associated with total PSA regardless of the cut-off level. When interpreting a single PSA test, height, alcohol consumption, and nutritional supplement use should be considered, in addition to age.

Elution Behavior of Copper(II) and Iron(II) Ions by Phenol Sulfonic Acid on Chelating Resin (킬레이트 수지에서 Phenol Sulfonic Acid에 의한 구리(Ⅱ)와 철(Ⅱ) 이온의 용리현상)

  • The elution behavior of copper(Ⅱ) and iron(Ⅱ) ions by phenol sulfonic acid(PSA) as an eluent on chelating resin, Amberlite IRC-718 have been investigated. When copper(Ⅱ) and iron(Ⅱ) solutions were adsorbed on the resin and eluted with various concentration of PSA, two peaks of each ion were appeared in the elution curve. These two peak areas were changed according to the PSA concentration. Using these phenomena, the stability constants of complex formation between the two ions and PSA were calculated. The values are 7.0 for copper(II) and $4.5{\times}10^4$ for iron(II), respectively.

Diagnostic Role of Serum Free-to-Total Prostate Specific Antigen (PSA) Ratio in Prostate Cancer with Serum Total Concentration of PSA below 4 ng/mL

  • Purpose: To examine the effectiveness of serum free-to-total prostate specific antigen ratio (%fPSA) for the detection of prostate cancer (PCa) in men with different serum total PSA (tPSA) categories. Materials and Methods: From January 2010 to December 2013, a total of 225 patients with lower urinary tract symptoms (LUTS) underwent tPSA and %fPSA measurements. Histological examination with calculation of Gleason score and whole body bone scans were performed in identified cases of PCa. Results: PCa was diagnosed in 44 (19.6%) patients and the remaining 181 patients had benign prostate disease. PCa was detected in 5 (23.8%), 13 (8.7%) and 26 (47.3%) cases with tPSA level ranges ${\leq}4ng/ml$, 4 to 10 ng/ml and >10 ng/ml, respectively. The average Gleason score was $7.2{\pm}0.2$. Some 6 (13.6%) out of 44 PCa patients had bone metastases. The sensitivity was 80% and specificity was 81.3% at the cut-off %fPSA of 15% in PCa patients with a tPSA level below 4 ng/mL. A lower %fPSA was associated with PCa patients with Gleason score ${\geq}7$ than those with Gleason score ${\leq}6$ ($11.7{\pm}0.98$ vs. $16.5{\pm}2.25%$, P=0.029). No obvious relation of %fPSA to the incidence of bone metastasis was apparent in this study. Conclusions: The clinical application of %fPSA could help to discriminate PCa from benign prostate disease in men with a tPSA concentration below 4 ng/mL.

  • The elution behaviors of stannous ion by PSA(phenol sulfonic acid) as an eluent on chelating resin, Amberlite IRC-718 have been investigated. When 0.10 M stannous solution was adsorbed on the resin and eluted with various concentrations of PSA, the two peaks of stannous ion were appeared in the elution curve. These two peak areas were changed according to the PSA concentration. Using these results, the stability constant of the complex formation between Sn2+ and PSA was calculated. This value is 2.0 ${\times}$ 10-1.

Production of High Purity Oxygen by Combination of Membrane and PSA Methods (분리막과 PSA혼합법에 의한 고순도 산소의 제조)

  • There are growing needs to produce relatively high purity(99.0% or higher) oxygen at low cost. For small scale production, both pressure swing adsorption(PSA) and membrane process are competitive and less expensive or more convenient than well known cryogenic fractionation technology. A continuous membrane column(CMC) combined with a PSA oxygen generator can be employed to produce high purity oxygen continuously. The oxygen enriched gas generated by a PSA unit, with a concentration of 93~94%, is fed to the CMC that consism of three modules of poly(imide) hollow fibers. Several experiments were conducted by varying parameters, such as feed flow rate, transmembrane pressure drop, stage cut, and feed location in order to obtain a high oxygen concentration above 99.0%. A two-series unit mode was also employed with CMC operation to optimize the given membrane area.

  • Recent reports indicate that the clinical usefulness of prostate specific antigen (PSA), particulary in the differentiation of benign prostate hyperplasia from prostate cancer, can be improved by measuring the amount of free PSA in serum. Measuring free PSA is especially useful in attempts to improve diagnositc performance of PSA in the diagnostic gray zone of total PSA. The objective of this study was to develop free PSA assay kit using sandwich microplate enzyme immunoassay format. We chose a test format with polyclonal anti-PSA antibodies coated on the wells and monoclonal anti-free PSA antibodies for quantification to gain higher test sensitivity. We adpoted 10 uL of specimen and 2 hours of first incubation time with detecting antibody for free PSA EIA format using microplate. The within-day and between-day precision (%CV) in the high and low concentration ranges were below 4%. The correlation coefficient between in-house free PSA assay and commercial assay kit was r=0.9965 (slope=0.0984, y intercept=0.0173, N=27). No hook effect was found by 40 ng/mL and correlation coefficient (r) value of the fitted linear regression was over 0.995. The recovery tests were in the range of 98.9∼104.1% for free PSA. In conclusion, in-house free PSA enzyme immune assay is cost effective, simple and rapid and could be useful for the prognosis after theraphy as well as for the differential diagnosis between prostate cancer and benign prostate hyperplasia.

  • The influence of respiration on photosythetic electron transport were investigated in the Wid type and psaB mutants from Syneehocystis sp. PCC6803. The amount of glucose uptake in the wild type was proportional to the glucose concentration added in wild type and less than that of psaB mutants in the dark. It was suggested that psaB mutants more depend on the glucose than the wild type. It was investigated how the activities of isocitrate dehydrogenase(IDH) and glucose-6-phos-phate dehydrogenase(G6PDH) were changed. The activities of IDH were very low. While, the ac-tivities of G6PDH were much higher than that of IDH. These results agree to the reports that ex-ogenous glucose was dismilated aerobically via Oxidative Pentose Phosphate Pathway in heterotrophic cyanobacteria. PsaB mutants showed high G6PDH activity in the presence of glucose as well as in the dark and high respiratory activities especially in the dark. It was also investigated how photosynthetic electron transport activities were changed. PsaB mutants showed higher photosynthetic electron tranasport activities than wild type in the presence of glucose as well as in the dark. In the results, it was proposed that photosynthetic electron transport between PS I and PS U was complemented by respiratory electron transport through the NADPH generated by Dxidative Pentose Phophate Pathway in psaB mutant from Synechocystis sp. Economic and Performance Analysis for 2bed and 3bed Oxygen PSA Process (2탑 및 3탑식 Oxygen PSA 장치 운전결과 및 경제성 비교분석)

  • For oxygen PSA process development, adsorbed amount of oxygen and nitrogen on various adsorbents were measured corresponding Langmuir isotherm parameters were measured. A reasonable adsorbent for oxygen process was selected based on the effective adsorbed amount. The PSA process consists of adsorption, desorption, pressurization, purging and pressure equilization steps. Adsorption pressure was about 2 atm and desorption pressure was between 120 torr to 400torr. Cycle time of 2-bed PSA process was 80 seconds and that of 3-bed oxygen PSA process was 180 seconds. In order to compare and analyze operation characteristics and economic feasibilities of 2-bed and 3-bed oxygen PSA processes, productivity, oxygen concentration and recovery were compared and the effect of purge and pressurization steps on the performance of PSA processes were analyzed. For the commercial scale oxygen PSA process, capital and electricity cost were estimated. In the range of $O_2$ production less than $700Nm^3/hr$, the 2-bed process is conformed more feasible in economic view point.
