The purpose of this research is to develop a gait assistive device to enhance the gait stability and training efficiency of stroke patients. The configuration of this device is mainly composed of a motored wheel and a single cane whose lower end is attached to a motored wheel frame. A patient can feel haptic information from continuous ground contact from the wheel while walking through the grip handle. In addition, the wheeled cane can avoid using excessive use of the patient's upper limb for weight support and motivate the patient to use a paralyzed lower limb more actively. Moreover, the proposed device can provide intuitive and safe user interaction by integrating a force sensor and a tilt sensor equipped to the cane frame, and a switch sensor at the cane's handle. The admittance control has been implemented for the patient to change the walking speed intuitively by using the interaction forces at the handle. A hemi-paretic stroke patient participated in the walking assistive experiments as a pilot study to verify the effectiveness of the proposed haptic cane system. The results showed that the patient could improve walking speed and muscle activations during walking with a constant speed mode of the haptic cane. Moreover, the patient could maintain the preferred walking speeds and gait stability regardless of the magnitude of resistance forces with the admittance control mode of the haptic cane. The proposed robotic gait assistive device with a simple and intuitive mechanism can provide efficient gait training modes to stroke patients with high possibilities of widespread utilizations.
A laboratory study was conducted to evaluate if two different age groups(young vs. old) had differences in walking velocity and heel contact velocity and, furthermore, if these gait characteristics could adversely influence initial friction demand characteristics(i.e. RCOF) and the likelihood of slip-initiation. Twenty eight(14 younger and 14 older adults) participated in the study. While wearing a safety harness, all participants walked at their preferred gait speed for approximately 20 minutes on the linear walking track(1.5m× 20m) consisting of two floor-mounted forced plates. During subsequent 20 cameras, respectively. The results indicated that older adults walked slower(i.e., slower whole body center-of-mass velocity), exhibited lower heel contact velocity, and produced lower initial friction demand characteristics (i.e. RCOF) in comparison to younger adults. However, ANCOVA indicated that the diferences in heel contact velocity between the two age groups were due to the effects of walking velocity. The bivariate analysis further suggested that walking velocity was correlated to RCOF and heel contact velocity, while heel contact velocity was not found to be correlated to RCOF. In conclusion, could be a better indicator for predicting initial friction demand characteristics(i.e. RCOF) not hel contact velocity.
The purpose of this study was to investigate the effects of Hoehn-Yahr scale on the activation of lower-extremity muscles during walking. Electromyography (EMG) analysis was carried out on 36 patients with Parkinson's disease in the off phase of the medication cycle. We recorded EMG signals of the tibialis anterior (TA), medial gastrocnemius (MG), lateral gastrocnemius (LG), soleus (SOL), rectus femoris (RF), vastus lateralis (VL), semitendinosus (ST) and biceps femoris (BF) using Noraxon 16 channels EMG system during walking at preferred speed. Rectified EMG signals were normalized to reference voluntary contractions (RVC) over a gait cycle at the preferred speed, allowing for an assessment of how the activity was distributed over the gait cycle. Compared to the H & Y Scale 1, H & Y Scale 3 exhibited greater activation of the vastus lateralis during mid-stance and greater activation of the medial gastrocnemius during terminal swing. Compared to the H & Y Scale 1, H & Y Scale 2 and 3 exhibited less activation of the tibialis anterior during initial swing. We conclude that the more Hoen & Yahr Scale increase, the more abnormal lower-extremity muscles activation.
Objective: The purpose of this study was to investigate the relationship between heart rate (HR), self-awareness of exercise intensity (rating of perceived exertion, RPE), and 5-meter walk test (5MWT) of persons affected by stroke during backward walking according to the preferred stride frequency (PSF), PSF+3 and PSF+6 conditions. Design: Cross-sectional study. Methods: A total of 11 persons with stroke (9 males, 2 females) participated voluntarily. All patients underwent backward walking under the PSF, PSF+3, and PSF+6 conditions in underwater and ground environments, and each condition was performed for 5 minutes. The HR, RPE, and walking speeds were measured during walking, and the measured values from underwater and ground environments were compared. Results: The HR and RPE in the ground environment were significantly increased (p<0.05), and although the 5MWT showed an increase in speed, it was not significant. The HR and RPE in the underwater environment were also significantly increased (p<0.05), however, although the 5MWT results was increased, it was not significant. The HR and RPE were significantly increased in the PSF+6 condition (p<0.05). Conclusions: The results of this study showed that backward gait training underwater can provide an appropriate exercise intensity for stroke survivors and suggests that exercises performed in an underwater environment is more effective compared to the ground environment.
본 연구의 목적은 트레드밀 보행 시, gait dynamics 측면에서 보행의 속도 변화와 인지과제 수행 시 보행 변인의 가변성(variability)을 알아보고자 하는 것이다. 실험은 인지과제의 동시수행 유/무에 따른 5가지 속도(선호속도의 80%, 90%, 100%, 110% and 120%)에 의한 보행 실험으로 구성되었다. 인지과제의 종류는 학습기능이 없는 인지과제(2-back task)를 수행하였다. 인지과제는 피험자의 트레드밀 보행 시, 3m 앞에 놓여진 스크린에 주어지고 무선마우스를 통해 응답하는 형태로 구성되었다. 실험의 모든 과정은 3차원 동작분석기를 통해 동작데이터를 획득하였다. 이를 통해, 5가지 보행 시간 변인과 3가지 공간 변인을 추출하였다. gait dynamics 측면의 분석을 위해, 가변성의 크기를 살펴볼 수 있는 방법인 분산계수(coefficient of variance)와 변동량의 구조적 자기 유사성을 추론할 수 있는 detrended fluctuation analysis (DFA)를 사용하였다. 그 결과 보행 속도 변화에 따라 보행 변인의 평균값과 분산계수에서 통계적 유의한 차이가 발생하였고, 인지과제의 수행 유/무에 따라서는 DFA에서 통계적인 차이가 발생하였다. 이는 인지과제의 수행에 의해 보행의 발생과 조절 능력에 영향을 끼쳤다고 추론할 수 있다. 본 연구 결과를 명백히 하기 위해 더 많은 수의 피험자 실험과 추가 실험이 필요할 것이다.
Objective: The purpose of this study was to investigate the acute effect of walking on high heels on the behavior of fascicle length and activation of the lower limb muscles. Methods: Twelve healthy inexperienced high heel wearers (age: $23.1{\pm}2.0yr$, height: $162.4{\pm}4.9cm$, weight: $54.4{\pm}8.5kg$) participated in this study. They walked in high heels (7 cm) and barefoot on a treadmill at their preferred speed. During the gait analysis, the lower limb joint kinematics were obtained using a motion analysis system. In addition, the changes in fascicle length and the level of activation of the medial gastrocnemius (MG) were simultaneously monitored using a real-time ultrasound imaging technique and surface electromyography, respectively. Results: The results of this study show that the MG fascicle operates at a significantly shorter length in high heel walking ($37.64{\pm}8.59mm$ to $43.99{\pm}8.66mm$) in comparison with barefoot walking ($48.26{\pm}9.02mm$ to $53.99{\pm}8.54mm$) (p < .05). In addition, the MG fascicle underwent lengthening during high heel walking with relatively low muscle activation while it remained isometric during barefoot walking with relatively high muscle activation. Conclusion: Wearing high heels alters the operating range of the MG fascicle length and the pattern of muscle activation, suggesting that prolonged wearing of high heels might induce structural alterations of the MG that, in turn, hinder normal functioning of the MG muscle during walking.
Objective: The purpose of this study was to compare the accuracy of stride time and stride length provided by a commercial APDM inertial sensor system (APDM) with the results of three dimensional motion capture system (3D motion) during treadmill walking. Method: Five healthy men participated in this experiment. All subjects walked on the treadmill for 3 minutes at their preferred walking speed. The 3D motion and the APDM were simultaneously used for extracting gait variables such as stride time and stride length. Mean difference and root mean squared (RMS) difference were used to compare the measured gait variables from the two measurement devices. The regression equation derived from the range of motion of the lower limb was also applied to correct the error of stride length. Results: The stride time extracted from the APDM was almost the same as that from the 3D motion (the mean difference and RMS difference were less than 0.0001 sec and 0.0085 sec, respectively). For stride length, mean difference and RMS difference were less than 0.1141 m and 0.1254 m, respectively. However, after correction of the stride length error using the derived regression equation, the mean difference and the RMS difference decreased to 0.0134 m and 0.0556 m or less, respectively. Conclusion: In this study, we confirmed the possibility of using the temporal variables provided from the APDM during treadmill walking. By applying the regression equation derived only from the range of motion provided by the APDM, the error of the spatial variable could be reduced. Although further studies are needed with additional subjects and various walking speeds, these results may provide the basic data necessary for using APDM in treadmill walking.

Objective: The purpose of this study was to investigate the effect of the falls on the center of pressure (CoP) complexity during gait using non-linear approximate entropy (ApEn). Method: 20 elderly women with experience of falling ($age=72.55{\pm}5.42yrs$; $height=154.40{\pm}4.26cm$; $body\;weight=57.40{\pm}6.21kg$; $preferred\;gait\;speed=0.52{\pm}0.17m/s$) and 20 elderly women with no experience of falling ($age=71.90{\pm}2.90yrs$; $height=155.28{\pm}4.73cm$; $body\;weight=56.70{\pm}5.241kg$; $preferred\;gait\;speed=0.56{\pm}0.13m/s$) were recruited for the study. While they were walking at their preferred gait speed on a treadmill (instrumented dual belt treadmills, Bertec, USA) with a force plate CoP data were collected for the 20 strides. The complexity of the CoP was analyzed using the ApEn technique. Results: The ApEn of the medial-lateral CoP in the fallers showed smaller about 16% compared to the non-fallers (p<.05). The ApEn of the antero-posterior CoP of the fallers showed smaller about 12% compared to the non-fallers, but the difference was not statistically significant. Conclusion: Based on the results of this study, the reduction of the medio-lateral CoP complexity in the elderly gait would be an index to determine the potential fall.

We have not identified on what gender difference during gait at a pace of one's preferred velocity effects on the function of bilateral lower limb. This study was undertaken to investigate a difference of gait strategy by gender during gait at a one's preferred velocity of participants of adult male and female (n=20). Cinematographic data for motion analysis, ground reaction force (GRF) variables, and muscle volume of lower limb were analyzed. Significant difference of variables on movement of center of mass whole body, joint angle and moment of lower limb, and ground reaction force were tested by 2-way ANOVA analysis (P<0.05). Male group showed more muscle volume than female, and both showed more volume in dominant leg than non-dominant. Main effect by bilateral leg during gait showed higher difference in right than left leg in change of vertical position of center of mass (maximal, minimal). Main effect by gender in vertical change of position and velocity of center of mass showed higher difference in male than female (maximal, minimal). Hip joint showed more flexed and extended angle in male than female, and also dorsiflexion of ankle and flexion moment of knee and hip joint showed higher in male than female group. Therefore, this result was assumed that dominant showed furthermore more contribution for propulsive function than non-dominant leg. Gender difference showed in strategy controlling of biomechanical characteristics, and perhaps influenced by muscle volume.
Objective: The purpose of this study is to investigate whether the athletic knee show greater rotation and translation movement than non-athletic knee during the treadmill walking with their preferred speed in a complete gait cycle. Method: Thirty young and healthy male subjects participated in the study, fifteen handball players (mean age: 19.6 ± 1.4 years old, mean weight: 85 ± 11.9 Kg, mean height: 179.8 ± 4.7) and fifteen non-athletes (mean age: 22.8 ± 1.2 years old, mean weight: 74.5 ± 8.6 Kg, mean height: 175 ± 5.9). Three-dimensional positional coordinate of lower limb during treadmill walking were analyzed. Results: There were significant differences (t (22.014)=1.585, p=0.127 in the range of internal and external rotation with mean value for handball player (M=14.4513, SD=2.3839) was higher than non-athletes (M=13.3327, SD=1.337). The magnitude of the difference in the means (mean difference=1.11867, 95% CI: -0.34489 to 2.5822) was significant. There were also significant differences (t (17.956)=1.654, p=0.116 in the max abduction and adduction with mean value for handball player (M=5.7160, SD=2.49281) was higher than non-athletes (M=4.5773, SD=0.94667). The magnitude of the difference in the means (mean difference=1.138, 95% CI: -0.30805 to 2.58539) was significant. At significance level 0.05. Conclusion: Finding of this study suggest that to understand the actual characteristic of knee motion studies have to be done in different walking and running trial at variable speed.
