뇌졸중 환자의 무릎관절과 발목관절에 비탄력테이핑 적용이 근활성도와 균형 및 보행에 미치는 영향 (Effects of Incorporating Non-elastic Taping into PNF Techniques on Muscle Activities, Balance, and Gait in Patients with Chronic Stroke)

  • PURPOSE: The study examined the effects of non-elastic taping on the knee and ankle joints of stroke patients to increase muscle activity, balance and gait ability. METHODS: In this study, 10 stroke patients were assigned to experimental group (knee and ankle joint non-elastic taping and PNF technique) and control group (PNF technique). The experimental group applied the PNF technique for 30 minutes after attaching the inelastic tape, and the control group performed the PNF technique for 30 minutes. five times a week for a total of four weeks (20 times). RESULTS: The muscle activity of Vastus Medialis and Tiblialis Anterior showed significant differences between the experimental and control groups. BBS and 10MWT also showed significant differences between the experimental and control groups. CONCLUSION: In this study, the muscle activity, balance and walking on the paraplegic side of stroke patients using inelastic taping, and applied inelastic taping on two joints in a different way from the previous study, Therefore, it will have the advantage of increasing paralysis side muscle activity and improving balance and walking ability.

뇌졸중환자를 대상으로 한 PNF 적용 치료사들의 중재 내용 분석 (An Analysis of Intervention Contents in PNF Therapists for the Patient with Stroke)

  • Purpose : The purpose of this study was to record, analyze and describe in detail, content of PNF intervention for recovery of function after stroke using SPIRIT intervention recording tools. Methods : A convenience sample of 21 physical therapist from a rehabilitation hospital recorded the intervention for 69 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the intervention. Results : A convenience sample of 21 physical therapist from a rehabilitation recorded the interventions for 64 patients with stroke who treated during 30 minutes. The content of the treatment sessions was recorded using the Stroke Physiotherapy Intervention Recording Tool(SPIRIT). Descriptive statistics assessed for describe the interventions accurately and investigate the difference according to time since stroke. Conclusion : Facilitation techniques were the most frequently used intervention(n=1226,35.6%), practice(n=967, 28.0%) and exercise(n-670, 19.4%) in clinical practice of physical therapist. In contrast, Teaching careers and health care professionals to do or assist category is not frequently used intervention(n=5, 0.1%). Therefore, We need to be careful again with category of teaching careers and health care professionals.

고유수용성신경근촉진법이 뇌졸중 환자의 연하기능에 미치는 효과 (The Effect of Proprioceptive Neuromuscular Facilitation(PNF) on Swallowing Function of the Stroke Patients)

  • 본 연구는 뇌졸중 환자를 대상으로 고유수용성신경근촉진법이 연하기능에 미치는 효과를 알아보고자 하였다. 이를 위해 뇌졸중 환자 중 실험군 9명, 대조군 9명을 대상으로 주 3회 30분~50분씩 총 12회 동안 고유수용성신경근 촉진법을 이용한 프로그램으로 중재하였다. 연구 결과 첫째, 연하곤란 정도의 변화는 고유수용성신경근촉진법 중재 후에 실험군의 연하곤란 척도 점수가 감소하여 유의한 차이가 있었다. 둘째, 구강 인두기능 점수는 고유수용성신경근 촉진법 중재 후에 실험군에서 유의한 차이를 나타내었다. 셋째, 연하속도의 변화는 실험군에서 중재 후에 1회 연하시간, 총소요시간이 감소하여 유의한 차이를 보였다. 따라서 고유수용성신경근촉진법이 뇌졸중 환자의 연하기능을 향상시키는데 효과가 있는 것으로 나타났으며, 앞으로도 다양한 중재가 결합된 운동프로그램을 개발하여 뇌졸중 환자의 연하기능 향상에 기여할 수 있는 연구들이 진행되어져야 할 것으로 사료된다.

다양한 강도에서 수행된 고유감각신경근육촉진 스트레칭과 정적 스트레칭이 뒤넙다리근의 유연성에 미치는 효과 (The Effects of Proprioceptive Neuromuscular Facilitation and Static Stretching Performed at Various Intensities on Hamstring Flexibility)

  • Background: To prevent or reduce the risk of strain injury, various approaches, including stretching techniques are currently being used. The effect of proprioceptive neuromuscular facilitation (PNF) and static stretching on flexibility has been demonstrated; however, it is not clear which one is superior. Objects: This study aimed to evaluate the differences between the effects of PNF and static stretching performed at various intensities on muscle flexibility. Methods: The maximum voluntary isometric contraction (MVIC) of the hamstrings using the PNF stretching technique was performed in the P100 group, while 70% of the MVIC was performed in the P70 group. The MVIC value obtained during the PNF stretching in both groups was used as a reference for setting the intensity of static stretching. Static stretching was performed at 130% (S130), 100% (S100), and 70% of the MVIC (S70). The active knee extension (AKE) values, defined as the knee flexion angle were measured before stretching (baseline), immediately after stretching (post), and at 3 minutes, 6 minutes, and 15 minutes. Results: PNF stretching produce a greater improvement in flexibility compared with static stretching. Specifically, the ΔAKE was significantly higher in the S100 and S70 groups than in the P100 group at Post. In the comparison of ΔAKE over time in each group, the ΔAKE at Post showed a significant decrease compared to the value at Baseline in the S130 group; however, no significant difference was observed at 6 minutes while a significant increase was noted at 15 minutes. Conclusion: This study found that PNF stretching is more effective than static stretching with respect to increasing and maintaining the flexibility of muscles. In addition, the increase in flexibility at maximal intensity was similar to that observed at submaximal intensity during both PNF and static stretching.

고유수용성신경근촉진법의 상하지 협응 운동이 만성 뇌졸중 환자의 균형 능력에 미치는 효과 (The Effects of Upper and Lower Limb Coordinated Exercise of PNF for Balance in Chronic Stroke Patients)

  • Purpose: This study aimed to examined the effects of upper and lower limb coordinated exercise of proprioceptive neuromuscular facilitation (PNF) on static and dynamic balance ability. Methods: The subjects of this study were 18 patients who had been diagnosed with a stroke and hospitalized and who had received rehabilitation treatment at D rehabilitation hospital located in J city. They were randomly and equally assigned to a PNF upper and lower limb coordinated exercise group and an ordinary central nervous system development and treatment group, and they conducted exercises for eight weeks. They carried out exercise three times per week, for 30 minutes per each time. After each four minutes of exercise, each participant was given a and rest for one minute after exercise for four minutes was given. In order to test the subjects' static and dynamic balance ability and their dynamic balance ability, frailty and injuries,: a cooperative studyies of intervention techniques (FICSIT-4) test, a four- step square test (FSST), a timed up and go test (TUG), and a TWT3M tests wasere conducted before and after the exercise. Descriptive statistics were taken for the general characteristics of the subjects, and an independent t-test was conducted both before and after the exercise in order to examine differences between the two groups before and after the exercise were conducted. The A statistical significance level was set at p= 0.05. Results: In all the tests (of FICSIT-4, FSST, TUG, and TWT3M), both the experimental group and the control group saw more improved results, but the experimental group's results were significantly higher than those of the control group, and the size of the effects was larger in the experimental group than in the control group, with statistically significant results. Conclusion: PNF upper and lower limb coordinated exercise applied to chronic stroke patients produced brought significant results in static and dynamic balance ability. PNF and is considered as an important intervention program to improve stroke patients' balance ability.

오십견 환자에 있어서 고유수용성신경근 촉진법과 일반적인 운동치료의 효과에 대한 비교연구 (A Study on Comparison of Proprioceptive Neuromuscular Facilitation and General Exercise in the Treatment of Frozen Shoulder)

  • Two techniques were compared to test their effectiveness of frozen shoulder. The study was 20 cases of frozen shoulder who were treated at the department of physical therapy at Kum ma clinic during 6 months from 1 July, 1994 to 30 December, 1994. The treatment methods were application Proprioceptive Neuromuscular Facilitation(PNF) and General Exercise(Pendulum exercise, Scratch exercise, Overhead pully, Shoulder wheel). The results obtained from these cases were as follows; 1. The subjects were 8(40 %) males and 12(60 %) females. The largest group was $50{\sim}59$ years of age 8 persons(40 %). 2. The majority of patients came for physical treatment within $7{\sim}10$ months after onset of shoulder pain(10 presons or 50 %) and 11 months after(4 persons or 20 %). 3. PNF cases recovered in average of 23 days and G - E cases 32 days. Also, the PNF method showed the best results in treatment of frozen shoulder better than the G-E.

  • Proprioceptive neuromusculat facilitation(PNF), scapular patterns are very important for orthopaedic and neurologic patients. It is an essential treatment techniques for motor developmental disorder, CVA, cervical disk, frozen shoulder and pain control of cervical, shoulder girdle and upper extremity. Scapular patterns of PNF has 4 different type of pattern. each of them in combining of movement plane and functional movement. Biomechanically, most of PNF patterns are a concentric contraction with third-claw lever. But the movement pattern have a technique of combination of isotonic that should make a eccentric contraction with second-claw lever.

고유수용성신경근촉진법 이완기법이 뒤넙다리근 유연성과 점프수행력에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation Relaxation Techniques on Hamstring Flexibility and Vertical Jump Performance)

  • Purpose: The purpose of this study was to assess the effect of the proprioceptive neuromuscular facilitation (PNF) relaxation technique (hold relax; HR, contract relax; CR) and static stretch (SS) on hamstring flexibility and vertical jump performance in healthy adults over a four-week period. Methods: Twenty-four healthy adults volunteered to participate in PNF and static stretch sessions. The subjects were divided into three groups as follows: 8 subjects in the SS group, 8 subjects in the HR group, and 8 subjects in the CR group. In the HR and CR groups, contractions lasted 15 seconds per trial and consisted of 5 sets of 15 seconds of hamstring contraction immediately followed by 15 seconds of passive static hamstring stretching. For the static group, the participants were asked to stretch by hanging a leg over a table for 30 seconds. Subjects in all groups performed the exercises three days per week for 4 weeks. The active straight leg raising (ASLR) test, active knee extension (AKE) test, and vertical jump test were performed before the intervention and after 1, 2, and 4 weeks. Results: The SS, HR, and CR techniques effected positive improvement in hamstring flexibility and vertical jump performances but neither of the stretching methods had any statistically significant different effects according to group, although there were interactions (between group and time) in the ALSR group from 1 week to 2 week and in the AKE test from pretest to 1 week. Conclusion: Based on the results of the current study, SS, HR, and CR were shown to affect hamstring flexibility and vertical jump performance in healthy adults. In particular, it was determined that within a short period, hamstring flexibility could be attributed more to CR than to SS.

Effects of Proprioceptive Neuromuscular Facilitation and Treadmill Training on the Balance and Walking Ability of Stroke Patients

  • Purpose: The purpose of this study was to investigate the effects of combined training using proprioceptive neuromuscular facilitation (PNF) patterns and treadmills on the balance and walking ability of stroke patients. Methods: Twenty-three stroke patients were randomized into a control group (n=11), receiving only treadmill training and an experimental group (n=12) receiving combined training. The use of both PNF exercise and treadmill were implemented in the combined training. Interventions were performed 5 times a week for 6 weeks. Balance ability was measured by a timed up and go (TUG) test. Walking ability was measured by a 10-meter walk test (10MWT) and a 6-minute walk test (6MWT). A paired t-test was used to compare differences between pre- and post-intervention and independent t-tests were used to compare between groups. Results: Changes in TUG, 10MWT, and 6MWT before and after interventions were significantly different for both the experimental group and the control group (p<0.05). In addition, within-group changes in the TUG, 10MWT, and 6MWT were more effective in the experimental group than in the control group (p<0.05). Conclusion: Combined training using PNF techniques and treadmills may be useful in improving the balance and walking ability of stroke patients.

넙다리뒤근육의 유지-이완기법이 무릎관절 각도 증가에 미치는 효과 (Effects of Hamstring HR Technique on Knee Joint Angle Increase)

  • Background: The purpose of this study was to investigate the effect of various relaxation techniques on various dysfunction problems caused by shortening of the sagittal muscles. Method: The subjects were 44(18 males, 26 females). The subjects were composed of 3 groups. The experimental group consisted of 14 patients with proprioceptive neuromuscular facilitation stretching (PNF) technique, control group A 15 patients with self myofacial release (SMR) ball exercise, and control group B with 15 patients with Sling exercise. After 3 weeks of relaxation on the hamstring muscle, the length of the hamstring muscle before and after the intervention was compared. Results: The results of relaxation exercise of the snake muscles applied to passive PNF group, SMR ball group, and Sling relaxation group are as follows. 1. In the passive PNF group, the muscle length of the hamstring muscle was significantly increased after the intervention. 2. The muscle length of the hamstring muscle was significantly increased after the intervention in the SMR ball group. 3. Sling relaxation group significantly increased the muscle length of the hamstring muscle after sling exercise intervention. 4. Passive PNF group showed the greatest change in muscle length before and after intervention than SMR ball group and Sling relaxation group. Conclusion: Passive PNF relaxation therapy, SMR ball relaxation therapy, and Sling relaxation therapy applied to the hamstring muscle were effective in increasing muscle length of the hamstring muscle. PNF relaxation therapy showed the most significant effect after 3 weeks intervention.
