PASTELS project - overall progress of the project on experimental and numerical activities on passive safety systems

  • Nuclear accidents such as Fukushima Daiichi have highlighted the potential of passive safety systems to replace or complement active safety systems as part of the overall prevention and/or mitigation strategies. In addition, passive systems are key features of Small Modular Reactors (SMRs), for which they are becoming almost unavoidable and are part of the basic design of many reactors available in today's nuclear market. Nevertheless, their potential to significantly increase the safety of nuclear power plants still needs to be strengthened, in particular the ability of computer codes to determine their performance and reliability in industrial applications and support the safety demonstration. The PASTELS project (September 2020-February 2024), funded by the European Commission "Euratom H2020" programme, is devoted to the study of passive systems relying on natural circulation. The project focuses on two types, namely the SAfety COndenser (SACO) for the evacuation of the core residual power and the Containment Wall Condenser (CWC) for the reduction of heat and pressure in the containment vessel in case of accident. A specific design for each of these systems is being investigated in the project. Firstly, a straight vertical pool type of SACO has been implemented on the Framatome's PKL loop at Erlangen. It represents a tube bundle type heat exchanger that transfers heat from the secondary circuit to the water pool in which it is immersed by condensing the vapour generated in the steam generator. Secondly, the project relies on the CWC installed on the PASI test loop at LUT University in Finland. This facility reproduces the thermal-hydraulic behaviour of a Passive Containment Cooling System (PCCS) mainly composed of a CWC, a heat exchanger in the containment vessel connected to a water tank at atmospheric pressure outside the vessel which represents the ultimate heat sink. Several activities are carried out within the framework of the project. Different tests are conducted on these integral test facilities to produce new and relevant experimental data allowing to better characterize the physical behaviours and the performances of these systems for various thermo-hydraulic conditions. These test programmes are simulated by different codes acting at different scales, mainly system and CFD codes. New "system/CFD" coupling approaches are also considered to evaluate their potential to benefit both from the accuracy of CFD in regions where local 3D effects are dominant and system codes whose computational speed, robustness and general level of physical validation are particularly appreciated in industrial studies. In parallel, the project includes the study of single and two-phase natural circulation loops through a bibliographical study and the simulations of the PERSEO and HERO-2 experimental facilities. After a synthetic presentation of the project and its objectives, this article provides the reader with findings related to the physical analysis of the test results obtained on the PKL and PASI installations as well an overall evaluation of the capability of the different numerical tools to simulate passive systems.

국소부신피질호르몬제 사용 중단 후 나타난 홍피성 건선의 한방 치험 1례 (A Case of Psoriasis aggravated to Erythrodermic Psoriasis after Stopping Topical Steroids treated with Traditional Korean Medicine)

  • Objective : The aim of this study is to report the effect and safety of Traditional Korean Medicine on the treatment of erythrodermic psoriasis which appeared after stopping high potency topical steroids. Methods : A patient with psoriasis stopped topical steroids after the first outpatient care and was treated with herbal medicine, acupuncture, moxibustion for eight months. The severity of psoriasis was assessed with Psoriasis Area and Severity Index. Liver and renal functions were tested to observe the hepatic and renal toxicity of the treatment. Results : PASI score were 6.2 on the first visit and three months after it increased to 30 and the patient showed symptoms of erythrodermic psoriasis. And seven months after the first visit, it decreased to 0.6. There was no hepatic and renal toxicity of the treatment. Conclusion : These findings suggest that Traditional Korean Medicine might be effective and safe for the treatment of erythrodermic psoriasis.

건선에 대한 가미윤부음의 임상연구 (Clinic Study of Gami-Yunbueum Efficacy on the Psoriasis)

  • Psoriasis is a common inflammatory disease of the skin marked by excessive scaling associated with inflammation, affecting 1-2% of the population. Although their etiologies remain unknown, the finding of CD4+ and CD8+ lymphocytes at sites of psoriasis has led many reports to hypothesize that psoriasis is a T lymphocyte-mediated disease directed against unknown autoantigens. The oriental medicine understands the cause of psoriasis as low humidity of skin tissues and functional deteriorations of the liver and lung. Gami-Yunbueum (GY) is a mixture of two previous prescription, which were used to the protection of liver and lung. We had prescribed the GY to the three psoriasis patients for 11 weeks and examined the effectiveness of the GY by PASI (Psoriasis Area and Severity Index) score. In our experiment, GY improved the symptoms of psoriasis in all cases. These results indicated that GY can apply to the treatment of psoriasis. Furthermore, the process for treatment of psoriasis is possible to provide humidity to skin and control the functions of liver and lung.

  • Put four different kinds of Jangdongsi, Pasi, Gamsi, and Taebongsi etc. into glass bottle(Fig. 1) of 1150ml and after modulated constitution of air girding this, by means of $CO_2$ the keep in cold storage on $0{\sim}5^{\circ}C$. The results are as follows : 1. $CO_2$ non-addition group was not changed it's own color to four months regardless of kind of persimmons 2. $CO_2$ added test group by double quantity of container was not more rapidly discoloration. 3. One - half quantity of test group added double, same, one-half and one of fifth quantity of container was the most effect, $CO_2$ as the same kind in S. T. P. 4. Taebongsi of test group of four different kinds added one-half quantity of $CO_2$ was maintained effectually possible it's natural color till seven months. 5. In taste, non-addition of $CO_2$ existed astringency taste but addition of $CO_2$ showed sweet taste like a Gamsi since two months of storage.

아토피피부염 및 건선 환자 증례를 통한 상한론 백호탕 조문의 '면구(面垢)'에 대한 새로운 해석 (New Interpretation on 'MyeonGu' of Baekho-tang Text of Shanghanlun through Case Reports)

  • This paper suggested MyeonGu(面垢), symptom of Baekho-tang(白虎湯) in text 219 of Shang-Han-Lun(傷寒論), could be reinterpreted. There were 4 cases, 1 case on atopic dermatitis and 3 cases on psoriasis. All of them were diagnosed by Ying-Ming-Bing(陽明病) symptom. After taking Baekho-tang, SCORAD index of atopic dermatitis in case 1 improved 62.8 to 12.8 and PASI score of psoriasis in case 2, 3, and 4 improved 13.0 to 2.4, 10.8 to 0.8, and 8.6 to 1.0 respectively. Since Baekho-tang was effective for certain skin disease with Ying-Ming-Bing(陽明病) symptom, MyeonGu could be related on skin problem. MyeonGu was mainly translated in 'someone's face seems to be covered with dirt'. However if Myeon(面) is translated in 'surface of the body' and Gu(垢) is translated in 'inflammation', it means the surface of the body suffered from papules, erythema, white scales, etc. These are features of chronic inflammatory cutaneous disease like atopic dermatitis and psoriasis. This study is meaningful in case of giving specific clinical case of MyeonGu of Baekhotang text of Shanghanlun.

Persicaria senticosa Ameliorates Imiquimod-induced Psoriasis-like Skin Lesions in Mice via Suppression of IL-6/STAT3 Expression and Proliferation of Keratinocytes

  • Psoriasis is one of the most common inflammatory skin disorders, with a global prevalence of 2% - 3%. It is an autoimmune skin disorder characterized by excessive generation of plaques on the skin with typical long-lasting red, itchy, and scaly lesions. In this study, we aimed to elucidate the anti-psoriatic effect of the methanolic extract of Persicaria senticosa (PS), a bioactive edible plant extract used in traditional medicine, using a mouse model of imiquimod (IMQ)-induced psoriasis. The daily topical application of IMQ could induce human psoriasis-like lesion. The extract ameliorated IMQ-induced psoriasis. Furthermore, hematoxylin and eosin staining and the Psoriasis Area and Severity Index (PASI) scores indicated that topical application of PS led to an improvement in erythema, scaling, and thickness scores of the mouse dorsal skin and a considerable decrease in the epidermal thickness of the ear and dorsal skin in the IMQ-induced psoriatic mouse model. We also studied the effect of PS on the proliferation of keratinocytes using HaCaT cells. The extract inhibited cell proliferation and IL-6 and pSTAT3 expression induced by M5 cocktail (comprising interleukin [IL]-1α, IL-17A, IL-22, oncostatin M, and tumor necrosis factor-α) in HaCaT cells. Thus, PS might serve as a potential therapeutic agent for the treatment of psoriasis.

The Amelioration Effect of the Ethanolic Extract of Cnidium officinale in Mice with Imiquimod-induced Psoriasis-like Skin Lesion

  • Psoriasis is an auto-immune skin disease, which is characterized by the excessive generation of plaques on the skin with typically a long-lasting red, itchy and scaly symptoms. Imiquimod, which has been used for the treatment of external genital warts, actinic keratosis, and superficial basal cell carcinoma, induced of psoriasis-like skin disorders with skin erythema and thickness in mice. In the present study, we tried to find the bioactive herbal extract against imiquimod-induced psoriasis-like skin disorder in mice. During the searching of the herbal extract with anti-psoriatic effect, the ethanolic extract of Cnidium officinale ameliorated imiquimod-induced psoriasis-like skin disorder in mice. The morphological evaluation, H&E staining and Psoriasis Area and Severity Index (PASI) score showed that ear and back thickness, and erythema induced by imiquimod were significantly reversed after the treatment of the cream of the ethanolic extract of C. officinale. The overexpressed myeloperoxidase (MPO) and keratin 6A levels were decreased by the treatment of C. officinale cream. Also, $IFN-{\gamma}$, c-fos and $I{\kappa}B-{\alpha}$ mRNA levels, which are related to the progression of psoriasis, were reduced by C. officinale cream. Thus, the ethanolic extract of C. officinale ameliorated psoriasis-like skin disorder induced by imiquimod and might be the therapeutic agent for psoriasis.

건선 치료용 한약제제에 대한 임상시험 가이드라인 설명 및 제언 (Explanation and Elaboration of the Clinical Trial Guidelines for Psoriasis Using Herbal Medicine)

  • Objectives : We report on the contents and reviews of the guidelines for clinical trials of herbal medicine for psoriasis. Methods : After reviewing the existing reports, the guidelines were drafted by the writing committee on guidelines for Korean medicine clinical trial center in Kyung Hee university Korean medicine hospital. Since then, the guidelines have been consulted by the experts in diseases, clinical trials and pharmaceuticals. In addition, the guidelines have been certified by the Korean medicine clinical trial center association and Korean Medicine Ophthalmology & Otolaryngology & Dermatology Society. Results : The guidelines for clinical trials are divided into 8 categories: (1) general contents, (2) evaluation standards of effectiveness, (3) outcome valuables, (4) selection of study participants, (5) designs of clinical trials, (6) safety evaluations, (7) combination therapies in psoriasis treatment and (8) Korean medical considerations. Conclusions : There are 3 major contents for discussion: (1) obscurity of Korean medical considerations to differentiate from existing guidelines, (2) the absence of Korean version of PASI and DLQI, which are validity parameters and (3) realistic institutional problem using the herbal medicine.

다빈도 피부질환에 대한 국내외 한의학 논문에서 평가도구 사용현황에 대한 고찰 (A Review of Assessment Tools in Traditional Medicine on the High Frequent Skin Diseases)

  • Objectives : The purpose of this study is to establish the basis of the assessment tools that can be used for the diagnosis and assessment of skin diseases by reviewing the present status of the assessment tools used in domestic and overseas traditional medicine papers for the past five years. Methods : This study is based on analysis of papers on clinical trials about atopic dermatitis, psoriasis, acne, urticaria published from 1st July 2013 to 30th June 2018. The papers were searched from Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Pubmed, MEDLINE. Results : 40 articles on atopic dermatitis, 34 articles on psoriasis, 26 articles on acne and 10 articles on urticaria were selected. Among them, 38 articles(90%), 20 articles(85%), 22 articles(85%) and 5 articles(50%) used assessment tools in order. Conclusions : SCoring Atopic Dermatitis(SCORAD), objective SCORing of Atopic Dermatitis index(OSI), Eczema Area and Severity Index(EASI) were used in studies on atopic dermatitis. In the case of psoriasis, The Psoriasis Area and Severity Index(PASI) was used in most papers. In the study on acne, most of Korean papers used Korean Acne Grading System(KAGS), but in the overseas papers, various assessment tools were used. In the case of urticaria, the rate of use of the assessment tool was the lowest, but the assessment tool used was unified by Urticaria Activity Score(UAS). Since skin diseases can cause stress and a decrease in quality of life, the quality of life should be taken into account when evaluating treatment effects.

건선 한약제제 임상시험 가이드라인 개발을 위한 관련 국내외 가이드라인과 기존 건선치료 한약제제 연구와의 비교 (Comparison between domestic and foreign Clinical guidelines and previous researches on Korean medicine for psoriasis to develop the clinical trial guideline of psoriasis using Korean medicine)

  • Objectives : The objective of this study is to develop the strategies of the psoriasis clinical trials guideline on Korean medicine by comparison between Clinical guidelines and domestic and foreign clinical trials papers of psoriasis on Korean medicine. Methods : This study is based on analysis of papers on Clinical trials guidelines, Clinical practice guideline and clinical trials about Korean medicine. The papers were searched from Pubmed, Medline, Oasis(Oriental Medicine Advanced Searching Integrated System), Korean Traditional Knowledge Portal and Google portal database. Results : A total 8 Clinical practice guidelines and 2 Clinical trials guidelines were found. Moreover, there were 15 foreign papers about clinical trials and 29 internal articles about case studies. They suggested the diagnostic strategy, classification, effective outcome measure, severity measure, precaution of combination therapy, precaution and treatment period of clinical trials, safety evaluation, patterns of Korean Medicine, clinical specific features on psoriasis.Conclusions : The criteria of every item to provide the clinical trials guideline using Korean medicine on psoriasis were developed by apply the results. If we accumulate the more clinical articles on Korean medicine, it will be great help to develop the reliable standard of that guideline.
