Effects of Cardiotonic Pills® on Cerebrovascular CO2 Reactivity and Erythrocyte Deformability in Normal Subjects: A Pilot Study

  • Backgrounds and objectives: Cardiotonic Pills® (CP) are used for vascular diseases such as coronary diseases, atherosclerosis, and cerebral infarction. This study aimed to determine the transient effects of CP on cerebrovascular CO2 reactivity (CVR) and erythrocyte deformability in normal subjects. Methods: This study had a crossover design and included 10 participants who were randomly allocated to 2 groups. The experimental group was given CP with water, while the control group was given only water. CVR was measured by hyperventilation-induced CVR of the middle cerebral artery (MCA) using transcranial Doppler (TCD). Erythrocyte deformability was measured using a Rheoscan-D microfluidic ektacytometer. All measurements were performed prior to and 1, 2, and 3 hours after CP or water administration. Blood pressure and heart rate were also measured before and after administration. Results: CP significantly improved CVR 3 hours after administration in the experimental group compared to the control group (p = 0.042). The corrected blood flow velocity at partial pressure of end-tidal carbon dioxide (PETCO2) = 40mmHg (CV40) was also significantly improved 2 and 3 hours after administration in the CP group compared to the control group (p = 0.036 and p = 0.021, respectively). CP significantly improved erythrocyte deformability 3 hours after administration in the experimental group compared to the control group (p = 0.027). Mean heart rate and mean blood pressure showed no change. Conclusions: This study demonstrated that CP increases CVR and erythrocyte deformability. These results suggested that CP improves cerebral microcirculation which provide evidence for the future use of CP for prevention of ischemic stroke and neurodegenerative diseases.

지진 토모그래피를 이용한 한반도의 과거진원지역의 특성 연구 (Investigation of Post-seismic Sites Using Local Seismic Tomography in the Korean Peninsula)

  • 한반도의 3차원적인 지각구조와 진원의 특성을 P파와 S파의 주행시간 토모그래피 기법을 이용하여 연구하였다. 이 연구의 주요 목적은 진원, 분지 및 심부 구조지역에서의 Vp/Vs 비율의 이상 대를 찾기 위함이다. 이것은 한반도의 보다 세부적인 지진 구조력 데이터의 예측을 고려했다. 지진 토모그래피를 이용한 본 연구의 결과에서 진원에서의 높은 Vp/Vs 비율의 차이를 수직성분에서 찾을 수 있었다. 퇴적 분지와 추가령 구조곡 아래에서 높은 Vp/Vs 비율이 나타났다. 이는 마그마 분출 후 마그마가 고체화되고, 원생수(connate water)로 포화된 여러 파쇄대와 단층에 의한 것으로 추정되어진다. 대부분의 진원에서 높은 Vp/Vs 비율 차이(VRD)를 찾을 수 있었으며, 영월, 경주, 홍성, 대흥, 대관, 랑님 지진의 경우 상부지각에서 속리산, 사리원, 지리산 지진 등은 하부지각 또는, 모호 불연속면 근처에서 나타났다. 특히 대관, 대흥 및 운산에 있는 온천 지역과 높은 VRD가 일치함을 볼 수 있었다. 이것은 또한, 이 지역 하부지각에 용융체(partial melting)의 존재 가능성을 시사한다. 높은 Vp/Vs 비율 차이(VRD)는 단층과 파쇄대에 존재하던 액체 (connate water)가 지진발생 후 탈수화 작용(dehydration)이 발생하여, 강성률이 감소함을 나타낸다. 이러한 것은 진원이 부피의 변화, 전단 파쇄대(shear fracture), 장성률(rigid)의 변화를 포함하는 모멘트 텐서(moment tensor) 에 의해 표현된다는 사실에 기인한다는 것이다. 높은 Vp/Vs 비율의 차이(VRD)는 또한, 백두산의 40km 깊이 아래에서 찾을 수 있는데 이는 강성률을 감소시키는 마그마 방(magma chamber)가 존재함을 암시해준다.

  • 송풍식건조과정(送風式乾燥過程) 중의 말쥐치육의 건조기구(乾操機構)와 수분활성(水分活性)과의 관계를 검토하기 위하여 $47.5^{\circ}C$에서 풍속과 공기의 상대습도를 달리하여 실험한 결과를 요약하면 다음과 같다. 전체 건조과정은 정속건조기(定速乾燥期)와 감속건조기(減速乾燥期)로 구분되었다. 정속건조기(定速乾燥期)는 건조표면(乾燥表面)이 수분활성(水分活性) 1.0을 유지하는한 계속되었으며, 온도와 상대습도가 일정할 때 정속건조속도(定速乾燥速度)는 공기의 속도(速度)의 제곱근에 비례하였다. 감속건조기(減速乾燥期)는 건조기구(乾操機構)가 서로 다른 제(第)1 및 제(第)2감속건조기(減速乾燥期)로 구분되었다. 제(第)1감속건조기(減速乾燥期)는 모세관 응축수의 건조표면(乾燥表面)으로의 이동이 불충분한 불포화표면건조기(不飽和表面乾燥期)였으며, 이 때의 건조속도(乾燥速度)는 공기의 온도가 일정할 때 상대습도에 크게 좌우되었다. 제(第)1감속건조기(減速乾燥期)와 제(第)2감속건조기(減速乾燥期)의 변환점에서 표면경화현상(表面硬化現象)이 시작되었다. 상대습도의 변화에 따라 제(第)2감속건조기(減速乾燥期)가 시작될 때의 수분활성(水分活性)과 수분함량(水分含量)은 각각 다른 값을 나타내었다. 제(第)2감속건조기(減速乾燥期)는 다시 건조기구(乾操機構)를 달리하는 두 개의 건조기(乾燥期)로 구분되었다. 제(第)2감속건조기(減速乾燥期)의 1단계는 말쥐치육 내부의 모세관 응축수가 건조표면(乾燥表面)으로 확산, 증발하여 건조가 진행되었으며, 이때의 수분의 확산계수는 $47.5^{\circ}C$에서 $2.89{\cdot}10^{-10}m^2/sec$였다. 표면경화현상(表面硬化現象)은 말쥐치육의 수분활성(水分活性)이 0.7에 이를때까지 계속되었다. 제(第)2감속건조기(減速乾燥期)의 2단계는 수분활성(水分活性) 0.45에서 시작되었다. 이 때의 건조는 말쥐치육 내부에 다분자층(多分子層)으로 흡착(吸着)한 결합수의 건조표면(乾燥表面)으로의 확산, 증발에 의하여 진행되었다. 흡착수분(吸着水分)의 분자층(分子層)의 수는 4였으며, $47.5^{\circ}C$에서의 확산계수는 $4.38{\cdot}10^{-11}m^2/sec$였다.

성인 선천성 심기형에 동반된 삼첨판막 폐쇄부전의 판막륜 성형술 후 중기성적 (Intermediate-term Result of Tricuspid Annuloplasty for Tricuspid Regurgitation Associated with Congenital Heart Disease in Adult)

  • 성인 선천성 심기형에 동반된 삼첨판막 폐쇄부전에 대한 판막륜 성형술의 중기 성적 및 잔존폐쇄부전의 위험인자를 분석하였다. 대상 및 방법: 1989년 8월부터 2001년 6월까지 총 73명의 성인환자가 선천성 심기형에 동반된 삼첨판막 폐쇄부전으로 판막륜 성형술을 받았다. 환자의 성비는 51:22로 여자가 많았고 연령은 16∼73세(평균 43세)였다. 진단은 심방중격결손(55), 심실중격결손(6), 부분 폐정맥 환류이상(4), 기타(8)의 순 이였다. 수술 전, 후 판막 폐쇄부전의 평균 혈류 속도는 각각 3.25 m/sec, 2.56 m/sec이었고, 판막 성형술의 방법은 De Vega 형(43), Kay 형(18), Ring Annuloplasty 형(12)의 순 이었다. 외래 추적기간은 2,347 patient-month (평균: 32.6개월)이었고, 이 기간 중 총 134예의 심초음파가 시행되었다. 초음파상 III/IV 이상의 잔존 판막 폐쇄 부전이 있는 경우 판막 성명술 실패로 간주하였다. 결과: 7명의 환자(9.6%)에서 판막 성명술 실패의 소견을 보였고, 이중 1명에서 삼천판막 치환술이 시행되었다. 잔존 폐쇄부전의 위험인자로는 심방 중격 결손 이외의 진단, 폐쇄부전이 우심실 압력 과부하에 의한 경우, 수술 후 높은 판막 폐쇄부전 혈류 속도 등으로 해석되었고. 판막 성형술의 방법은 잔존 폐쇄부전과 무관하였다. 결론: 선천성 심기형에 동반된 삼첨판막의 폐쇄부전이 우심실 압력 과부하에 의한 경우 판막 성형술 시 주의를 요하며, 수술 후 우심실 압부하가 소실되지 않는 경우 잔존 폐쇄부전의 위험이 높다.

  • Thiocarbamate wool은 $CS_2$ 및 기타의 SH-reagent로 쉽게 수식(修飾)할 수 있을 뿐만 아니라 가역적(可逆的)으로 사용(使用)이 가능(可能)하며 일반적(一般的)인 조건(條件)에서도 효소(酵素)와 쉽게 결합(結合)하여 재사용(再使用) 및 제거(除去)에 용이(容易)하므로 thiocarbarmate wool의 특이(特異)한 반응성(反應性)을 이용(利用), 효소(酵素)를 고정(固定)하여 공업적(工業的)인 이용(利用) 목적(目的)으로 연구(硏究) 검토(檢討)하였다. $DTC-wool-{\alpha}-amylase$의 일반적(一般的)인 성질(性質)은 $Nat-{\alpha}-amylase$에 비(比)하여 최적온도(最適溫度), 최적(最適) pH, 내열성(耐熱性), 동력학적(動力學的) 동태(動態) 및 활성화(活性化) 에너지 등(等)이 다르게 나타났다. 1) 반응시간(反應時間)은 70분(分)에서 환원탕(環元糖)이 12.2 mg/ml로서 최대(最大)의 활성(活性)을 나타내었고 2) 효소량(酵素量)은 3.0g 으로서 13.5 mg/ml의 환원탕(還元糖)으로 최대(最大)의 활성(活性)을 나타내었으며 3) Michaelis constant $(K_m^*)$는 5.6 mg/ml 였고 $V_{max}^*$$370.37\;{\mu}g/ml{\cdot}min^{-1}$이었다. 4) 최적온도(最適溫度)는 $60^{\circ}C$였고 이때의 환원탕(還元糖)의 양(量)은 16 mg/ml 였고 5) 최적(最適) pH는 pH 7.0이었고 95% 이상(以上)의 활성(活性)을 나타낸 pH 안정성(安定性)은 pH $6.0{\sim}7.0$ 이었다. 6) 내열성(耐熱性)은 $Nat-{\alpha}-amylase$ 보다 강(强)하였으며 7) 활성화(活性化) 에너지는 16.6 Kcal/mole 이었다. thiocarbamate wool은 가역(可逆), 재사용(再使用) 및 제거(除去)의 편리(便利)에 큰 의의(意義)를 둘 수 있으며 쉽게 조제(調製)할 수 있는 용이점(容易點)이 있을 뿐 아니라 이 DTC-wool에서 효소(酵素)의 고정(固定)이 室溫(室溫)에서도 짧은 시간(時間) 내(內)에 이루어지며 상당히 높은 고정율(固定率)을 기대할 수 있다. 또 최적온도(最適溫度)가 $60^{\circ}C$이고 최적(最適) pH는 7.0이므로 반응장치(反應裝置) 및 운용(運用)에 별다른 어려움이 없고, 내열성(耐熱性)이 강(强)하므로 쉬운 조건하(條件下)에서 경제적(經濟的)으로 공업적(工業的) 이용(利用)에 유리(有利)하다고 판단(判斷)되어진다.

12CO 분자선 관측 자료를 이용한 오리온 분자운 복합체내 북쪽 필라멘트의 운동학 연구 (Kinematics of the Northern Filament in Orion Molecular Clouds Complex Using 12CO Molecular Observation Data)

  • 우리는 오리온 분자운 복합체의 북부 필라멘트(이하 NF)에 대하여 $^{12}CO$ (J=1-0) 분자선의 자료를 이용하여 은하 평면이 분자의 운동과 운동학에 미치는 영향을 연구하였다. 6 m 서울대학교 전파망원경(Seoul Radio Astronomy Observatory, SRAO)을 이용하여 2arcmin 공간분해능으로 은하면으로부터 먼 순서로 NF1, NF2, NF3 세 곳을 총 270시간 동안 관측된 자료를 사용하였다. 은하면과 OMC NF는 $^{12}CO$ (J=2-1) 경우 3% 밀도에서, 티끌의 경우 9% 밝기 수준에서 자기장을 따라 서로 연결되어 있었다. $^{12}CO$ (J=1-0), $^{12}CO$ (J=2-1), 성간 티끌 관측결과를 비교해본 결과, 세 경우 모두 NF3에서는 고루 분포했지만, NF1과 NF2에서는 비교적 밀도가 높은 특정 영역에서만 함께 나타났다. NF는 단일 구조를 보였으며, NF1에서는 부분 수축 운동을, NF2에서는 하단에서 회전 운동이 나타났고, NF3에서는 유일하게 명확히 자기장에 연관된 나선형 회전이 보였다. 위치-속도 분석 결과, $^{12}CO$ (J=1-0)를 비롯한 물질들은 NF2와 NF3을 따라 은하면을 향하여 흐를 가능성이 있음을 확인할 수 있었다. 은하면을 향하여 물질이 흐르는 명백한 원인을 이번 연구결과에서 볼 수 없었지만 추후의 더 정교한 관측결과가 NF1과 NF2 상단부의 회전 운동을 확인 할 수 있겠다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases.
