Clinical Trial of Nasal Flumazenil Administration (플루마제닐의 경비 투여)

  • Flumazenil is a competitive antagonist of benzodiazepines. It is usually administered intravenously. However, if the intravenous route is not available then other routes of drug administration should be considered. This study was designed to evaluate the reversal effects of flumazenil after nasal administration. Twenty-five young, healthy adult volunteers participated in this clinical trial. The dosage of 0.08mg/kg midazolam was administered intravenously to induce deep sedation. Ten minutes after midazolam administration, 0.5mg of flumazenil was dropped nasally, over a period of one minute. Blood samples were taken to measure the concentration of midazolam and flumazenil at 0, 5, 10, and 20min after nasal administration of flumazenil, using High Performance Liquid Chromatography. The degree of sedation was evaluated with sedation score and bispectral index (BIS), Statistical analysis was performed by multivariate ANOVA and correlation analysis (P<0.05). Peak serum flumazenil concentration was reached in 10min. Sedation score decreased after midazolam administration and showed a significant increase after flumazenil administration. However, BIS decreased during the first 10min after midazolam administration and then no significant changes after flumazenil administration. There were two instances representing rapid and complete reversal of midazolam after intranasal administration of flumazenil. In conclusion, intranasal flumazenil administration may be effective in some patients when intravenous route is not available in condition of benzodiazepine overdose.

Comparisons in Food Quality of Anchovy Snacks and Its Changes during Storage (멸치스낵제품의 품질비교 및 저장안정성)

  • For the effective utilization of anchovy as a food source, this work was undertaken the com-parison in food quality of anchovy snacks and its changes during storage at room temperature ($24\pm4^{\circ}C$). Chopped anchovy was mixed with soft flour($340.0\%$, w/w), corn starch($10.0\%$, w/w), sodium chloride($2.5\%$, w/w), monosodium glutamate($0.1\%$, w/w), sodium bicarbonate ($2.5\%$, w/w), water($5.6\%$, w/w), onion powder($0.3\%$, w/w), garlic powder($0.3\%$, w/w), red pepper powder($0.3\%$, w/w) and sodium erythorbate($0.2\%$, w/w), The mixture were rolled, aged, co沇ed, dried and finally parched or deep-fried at $190\pm10^{\circ}C$. The anchovy snacks were packed in the casted polypropylene film bag ($16cm{\times}14cm$), The changes in moisture contents, water activity, pH, volatile basic nitrogen, contents of amino acid and color values of products were negligible during storage. The results of TBA value and peroxide value showed that lipid oxidation can be retarded by adding antioxidant and spices. Judging from contents of amino acid and mineral, the products were more nutritive than the sold shrimp snack on the market. From the results of sensory evaluation and chemical experiments, the product prepared with sodium erythorbate could be preserved in good quality during storage of 120 days.

  • The members of the Korean Association of Pediatric Surgeons conducted a retrospective study of two hundred and twenty-two cases of intestinal atresia for the period from January 1, 2007 to December 31, 2009. Seventeen hospitals were involved. There were 76 duodenal, 65 jejunal, and 81 ileal atresias (3 colonic). The male to female ratio was 0.85:1 in DA and 1.34:1 in JIA. Ninety-four patients(43.3 %) were premature babies (DA 40.3 %, JA 64.6 %, IA 28.8 %), and 70 babies (32.0 %) had low birth weight (DA 38.7 %, JA 44.4 %, IA 16.0 %). Antenatal diagnosis was made in 153 cases (68.9 %). However, 27 infants (17.6 %) with antenatal diagnosis were transferred to the pediatric surgeon's hospitals after delivery. Maternal polyhydramnios was observed in 81 cases (36.59 %) and most frequent with proximal obstruction. In forty-four cases (19.8 %), only simple abdominal film was taken for diagnostic study. The associated malformations were more frequently observed in DA - 61.8 % in DA and 22.6 % in JIA. Meconium peritonitis, small bowel volvulus and intussusception were more frequently associated with ileal atresia. The overall mortality rate was 3.6 %. The Microalgal Attachment and its Growth on the Artificial Surfaces Immersed in Seawater: I. Attachment and Micro-succession (해수에 잠긴 인공기질 표면에서 미세조류의 부착과 성장: I. 부착 및 천이)

  • To understand the attachment of micro algae and their subsequent growths on artificial surfaces immersed in seawater, the relationship between attachment of diatoms on the immersed artificial substrates and species pool in the surrounding water was investigated. We used acryl slides for the study of diatom attachment and examined the surrounding water samples collected in Incheon Harbour from July 1995 to February 1997. Variations of species composition and abundances by exposure time in seawater were investigated during the early phase of biofilm formation on various substrates, e.g. glass, acryl, titanium, copper and antifouling paint-treated slides. Immigration rates of diatoms to acryl slides during spring and winter were significantly correlated with the abundance of benthic diatoms in surrounding water ($r^2$=0.78, p<0.01, n=42), suggesting that immigration rates were affected by variations of benthic diatom abundances in surrounding water. Immigration coefficient of monoraphid diatoms was 5 times higher than that of biraphid diatoms, but relative abundance of monoraphid diatoms was 3 times lower than that of biraphid diatoms on acryl slides in spring. In winter, immigration coefficient and relative abundance of centric diatoms were higher compared to other raphe forms. These results suggest that the attachment of diatoms seems to be caused by the abundance and immigration coefficients of benthic diatoms in surrounding water. Pennate diatoms predominantly attached to all artificial surfaces throughout all experimental periods. Interestingly, centric diatoms predominantly attached to all artificial surfaces in winter. Hantzschia virgata, Licmophora abbreviata and Melosira nummuloides appeared dominantly on antifouling paint-treated slides, probably being tolerant of the antifouling paint. During incubations, the abundance of attached diatoms increased exponentially on glass, titanium and acryl slides with exposure time. The maximum abundance was highest on glass slide, followed by acryl, titanium, copper and antifouling paint-treated slides. The growth rates of attached diatom community on all artificial surfaces were higher at temperature of $24-25^{\circ}C$ than that of $2-3^{\circ}C$. The growth rate of attached diatoms on glass slide was generally higher compared to other slides during the study period. Dominant morphotypes of observed species with exposure time in seawater were prostrate form Amphora coffeaeformis, fan shape Synedra tabulata, stalk type Licmophora paradoxa and chain type M. nummuloides. A micro-succession in the attached microalgal community was observed. The composition of dominant species seems to be the result of species-specific response to gradually limited space with development of microalgal film.

  • Processing conditions of retort pouched fried mackerel fish meat paste and quality stability during storage were investigated. The reasonable amounts of added ingredients to the frozen mackerel meat paste were $10\%$ of corn starch, $1\%$ of soybean protein, $1.5\%$ of sodium chloride, $0.6\%$ of monosodium glutamate, $0.3\%$ of alcoholic extract of red pepper, and $0.1\%$ of sodium erythorbate as an antioxidant and also added water corresponding to $10\%$ of the frozen mackerel meat paste. After grinding the defrosted mackerel fish meat paste with ingredients, the meat paste was molded in bar type and fried in soybean oil at $170-180^{\circ}C$ for 3 minutes. The fried mackerel meat paste was cooled, vacuum-packed in laminated plastic film bag (polyester/polyvinylidene chloride/unoriented polypropylene : $12{\mu}m/15{\mu}/50{\mu}m,\;14{\times}19cm$) and finally sterilized at $120^{\circ}C$ for 20 minutes in a hot water circulating retort. The pH, volatile basic nitrogen, moisture content, water activity, color, thiobarbituric acid value, peroxide value, texture and viable bacterial count of products were examined during 100 days of storage at $25{\pm}3^{\circ}C\;and\;5^{\circ}C$. The results showed that products could be preserved in good condition for 100 days at $25{\pm}3^{\circ}C$. Judging from sensory evaluation, the quality of products was not inferior to that of market products.

  • Topical fluoride application for children is a widely peformed procedure in the field of pediatric dentistry to prevent dental caries. However, it is recently recognized as having some unwanted effects on several esthetic restorative materials as it roughens the surface of the restorative materials. The aim of this study was to evaluate the surface changes in composite resins to topical fluoride. Composite resins(Z $250^{(R)}$, Ultraseal $XT^{(R)}$ Filtek $flow^{(R)}$ $Revolution^{(R)}$, $Denfil^{(R)}$) in topical fluoride agents were immersed and their surface roughness, weight loss and SEM were evaluated. The results were as follows : 1. The 4 minutes-immersion groups showed more roughened surface than 1 minute-immersion groups and the control groups showed the smoothest surface among all the materials, and there was statistically significant difference except the revolution between the groups. 2. There was no significant difference between the 1 minute-immersion groups and 4 minutes-immersion groups in weight loss. 3. The experimental group treated with topical fluoride gel showed the generally mere roughened surface than control group in the SEM findings.

  • A survey on the intestinal atresias was made among 34 members of Korean Association of Pediatric Surgeons. The response rate was 82.4 %. Two hundred and fifteen patients from the January 1, 1994 to December 31, 1996 were analyzed. The lesions were 73 cases of duodenum(DA), 72 cases of jejunum(JA), 71 cases ileum(IA) and 2 cases cecum and sigmoid colon respectively. There were 2 cases of combined anomalies (DA + JA + IA and DA + JA). Male to female ratio was 1:1 in DA, and 1.8:1 in JA. Seventy four cases(34.3 %) were premature babies(DA 35.2 %, JA:48.6 %, IA:19.2 %), and 62 cases(28.7 %) had low birth weight (DA:39.4 %, JA 33.0 %, IA:13.7 %). Antenatal diagnosis was made in 92 cases(43.6 %). However 22 cases (23.9 %) of them were transferred to pediatric surgeon after delivery. Maternal polyhydramnios was observed in 63 cases(28.9 %). Seventy· five cases(34.4 %) were taken only simple abdominal film for diagnostic studies. The associated malformations were observed in 54 aresia and were observed more frequently in DA(35 cases, 47.9 %). Meconium peritonitis due to intrauterine bowel perforation was more frequently associated with IA compared to DA and JA. The overall mortality rate was 30 %.
