Lymph Node Ratio Assessment of Brain Metastasis in Early Breast Cancer Cases

  • Background: Ten to 30% of early breast cancer (EBC) patients develop brain metastasis (BM) during their follow-up. In this study, we aimed to evaluate importance of the lymph node ratio (LNR) in development of BM in EBC cases. Materials and Methods: Ninety patients whom had axillary metastases in lymph nodes at their initial diagnosis and developed BM during 5-year follow-up were detected in 950 EBC patients. LNR values were calculated for all patients and after categorization into 4 molecular sub-types as luminal A, luminal B HER-2 (+), HER-2 overexpressing and basal- like. Comparison was with control group patients who had similar characteristics. Results: In the comparison of all molecular sub-types of LNR, 54.9% and 28.4% values were found in patients with and without BM respectively (p<0.001). In the comparison of the LNR with control groups, a statistically significant differences were found with luminal A with BM (p=0.001), luminal B HER-2 (p=0.001), HER-2 overexpressing (p=0.027) and basal-like groups (p<0.001). In the evaluation of patients with BM, the highest ratio was found in the basal-like group (67.9%) and there was a statistically significant difference between this group and the others (p=0.048). Conclusions: EBC patients developing BM within 5 years follow-up had significantly higher LNRs for all molecular sub-types, especially in the basal-like group. Larger scale studies are now needed for evaluating LNR prognostic importance for EBC regarding BM development.

Effect of Oligosaccharides on Quality Characteristics and Antioxidant Activities of Prunus persica Batsch var. davidiana Max. preserved in Sugar (올리고당을 활용한 당절임 야생 복숭아의 품질특성과 항산화 효과에 미치는 영향)

  • In order to investigate the quality characteristics of Prunus persica Batsch var. davidiana Max. preserved in sugar, the quality characteristics were examined by dividing them into four groups: PS-A (P. persica preserved in sugar mixed with sucrose and oligosaccharides at a ratio of 100:0, w/w), PS-B (P. persica preserved in sugar mixed with sucrose and oligosaccharides at a ratio of 60:40, w/w), PS-C (P. persica preserved in sugar mixed with sucrose and oligosaccharides at a ratio of 40:60, w/w) and PS-D (P. persica preserved in sugar mixed with sucrose and oligosaccharides at a ratio of 0:100, w/w). The results were as follows: pH levels and soluble solid contents of PS-D sample were lower than those of the others, however total acidity value were higher. In case of Hunter's value, the 'L', 'a' and 'b' values of all samples were affected by addition of oligosaccharides. No great difference occurred among samples in reducing sugar contents. Organic acid content was significantly decreased with the lower addition of sucrose. The predominating organic acid components analyzed in all samples were lactic acid 156.15 g/100 g, tartaric acid 72.75 g/100 g and citric acid 32.90 g/100 g. The highest contents of total phenol and flavonoid, and DPPH and ABTS radical scavenging activity were $10.79{\pm}0.55mg\;GAE/mL$, $8.71{\pm}0.06mg\;CE/mL$ and 71.8%, 71.0% for PS-D, respectively. The results of this study indicated the use of oligosaccharides could increase the quality and antioxidant potentials of P. persica preserved in sugar.

A Study on the Guided Search Method for Transcoding MPEG2 P frame to H.263 P frame in a Compressed Domain (압축상태에서 MPEG2 P 프레임을 H.263 P 프레임으로 변환하기 위한 가이드 탐색 방법 연구)

  • The purpose of the paper is to enable a format transcoding between a heterogeneous compression format in a real time, and to enhance the compression ratio using characteristics of the compressed frame. In this paper, for the heterogeneous format transcoding, we tried to transcode from MPEG2 having a lower compression ratio to H.263 having a higher compression ratio. After analyzing MPEG 2 bit stream and H.263 bit stream of the same original video, we found that the number of intra coded macro blocks in MPEG 2 data is much higher than the number of the intra coded macro blocks in H.263 data. In the process of P frame generation, a intra coded macro block is generated when a motion estimation value representing the similarity between the previous frame and current frame does not meet a threshold. Especially the intra coded macro block has a great impact on the compression ratio. Hence the paper, we tried to minimize the number of intra coded macro blocks in H.263 data stream which is transcoded from MPEG 2 in a compressed domain. For the purpose, we propose a guided search method for transcoding the INTRA coded block into INTER coded block using the information about motion vectors surrounding the intra macro block in order to minimize the complexity of the motion estimation process. The experimental results show that the transcoding of MPEG 2 into H.263 can be done in a real time successfully.

Feasibility of Pediatric Low-Dose Facial CT Reconstructed with Filtered Back Projection Using Adequate Kernels (필터보정역투영과 적절한 커널을 이용한 소아 저선량 안면 컴퓨터단층촬영의 시행 가능성)

  • Purpose To evaluate the feasibility of pediatric low-dose facial CT reconstructed with filtered back projection (FBP) using adequate kernels. Materials and Methods We retrospectively reviewed the clinical and imaging data of children aged < 10 years who underwent facial CT at our emergency department. The patients were divided into two groups: low-dose CT (LDCT; Group A, n = 73) with a fixed 80-kVp tube potential and automatic tube current modulation (ATCM) and standard-dose CT (SDCT; Group B, n = 40) with a fixed 120-kVp tube potential and ATCM. All images were reconstructed with FBP using bone and soft tissue kernels in Group A and only bone kernel in Group B. The groups were compared in terms of image noise, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR). Two radiologists subjectively scored the overall image quality of bony and soft tissue structures. The CT dose index volume and dose-length product were recorded. Results Image noise was higher in Group A than in Group B in bone kernel images (p < 0.001). Group A using a soft tissue kernel showed the highest SNR and CNR for all soft tissue structures (all p < 0.001). In the qualitative analysis of bony structures, Group A scores were found to be similar to or higher than Group B scores on comparing bone kernel images. In the qualitative analysis of soft tissue structures, there was no significant difference between Group A using a soft tissue kernel and Group B using a bone kernel with a soft tissue window setting (p > 0.05). Group A showed a 76.9% reduction in radiation dose compared to Group B (3.2 ± 0.2 mGy vs. 13.9 ± 1.5 mGy; p < 0.001). Conclusion The addition of a soft tissue kernel image to conventional CT reconstructed with FBP enables the use of pediatric low-dose facial CT protocol while maintaining image quality.

A Hardening and Strength Properties of Magnesium Phosphate Mortars for Rapid Repair Materials (급속 보수용 마그네슘 인산염 모르타르의 경화 및 강도특성)

  • Damage to the pavement system due to various causes will be required rapid repair work for reopening the vehicle traffic. The magnesium oxide phosphate composite(MPC) has a short curing time and is capable of early compressive strength development, is suitable for rapid repair materials. The aim of this study was to evaluate the hardening and compressive strength characteristics of MPC according to the water-binder (W / B) ratio and magnesium-phosphate(M / P) ratio in order to develop repair materials consisted with light burned magnesia and potassium dihydrogen phosphate. In order to ensure the workability in the field application, the difference of mechanical properties according to standard sand and ordinary sand and performance of retards were evaluated. The mix proportion with W/B ratio was about 35% and the M/P ratio was about 1.0 ~ 1.2 has a superior perfomance with strength and hardening condition. Especially, the strength of composite at only 1 day curing with W/B ratio of 0.35 and the M/P ratio of 1.2 was shown the higher than 25.0 MPa. Boric acid as a retarder was found to be suitable for ensuring the working time, and the purity of magnesium oxide was about 90 ~ 95%, which is effective for ensuring curing time and strength.

A Diagnostic Significance of Pleural Fluid Cholesterol Level (흉막액 Cholesterol 농도에 대한 진단적 의의)

  • The purpose of this study is to investigate the utility of the pleural fluid cholesterol level in separating the exudates from the transudates, and in differentiating tuberculous exudates from non-tuberculous exudates, 52 patients with pleural effusion were involved in this prospective study. By predefined criteria, 40 of these effusions were classified as exudates (Group I) and 12 as transudates (Group II). Group I was subdivided into tuberculous exudates (Group A) and non-tuberculous exudates (Group B). The followings are parameters used in separating the exudates from the transudates; pleural protein (P-PROT) 3.0 g/dl, pleural protein/serum protein ratio (P/S PORT) 0.5, pleural LDH (P-LDH) 200 IU, pleural LDH/serum LDH ratio (P/S LDH) 0.6, pleural cholesterol (P-CHOL) 50 mg/dl and pleural cholesterol/serum cholesterol ratio (P/S CHOL) 0.4. Mean values of the parameters in each group were compared, and then misclassified rate and the dignostic efficiency for each parameter were calculated. The results were as follows; 1) Mean P-CHOL ($94.98{\pm}73.86\;mg/dl$) in Group I was higher than that ($36.5{\pm}26.5\;mg/dl$) in Group II (p<0.05), but there was no significant difference in mean P-CHOL between Group A and Group B. 2) Mean P/S CHOL ($0.64{\pm}0.39$) in Group I was also higher than that ($0.27{\pm}0.15$) in Group II (p<0.01), but no difference was observed in mean P/S CHOL between Group A and Group B. 3) Misclassified rates for each parameter in separating the exudates from the transudates were as follows; P-PROT 1.9%. P/S PROT 3.8%. P-CHOL 9.6%, P/S CHOL 11.5%, P/S LDH 11.5%, and P-LDH 17.3%. 4) Diagnostic efficiencies for each parameter in separating the exudates from the transudates were as follows; P-PROT 98.1%, P/S PROT 96.2%. P-CHOL 90.4%. P/S CHOL 88.5%, P/S LDH 88.5%, and P-LDH 82.7%. In conclusion, we think that the pleural fluid chloesterol level could be used as a supportive parameter in separating the exudates from the transudates, but could not be used as a parameter in differentiating tuberculous exudates from non-tuberculous exudates.

Replacement of sucrose with other sweeteners and high methoxyl pectin in low caloric pectin gels (대체 감미료와 high methoxyl pectin을 이용한 저열량 pectin gel의 제조)

  • Replacement of sucrose with sugar alcohols in high methoxyl pectin(HMP) jellies were studied. HMP jellies were sweetened with 100% sucrose, 75% sucrose and 25% maltitol, 50% sucrose and 50% maltitol, and 50% sucrose, 25% maltitol, 12.5% sorbitol and 12.5% mannitol. The effect of sugar alcohols in HMP jellies were investigted. There was only slight differences in sweetness intersity in HMP jellies. Color(p<0.05), texture(p<0.01) and preference(p<0.01) of 100fl sucrose jelly were significantly higher than other Jellies sweetened with sugar and sugar alcohols mixtures. Hunter's L(lightness), a(redness), b(yellowness) values of 100%, sucrose jellies and jellies substituted with maltitol up to 25% were not signignificantly different. But as the ratio and the number of sugar alcohols used for substition were increased, L, a and b values had been lowered signigicantly(p<0.01). Instru-mental characteristics of jelly texture were examined. Hardness of 100% sucrose jelly sweetened with sucrose and 3 kind of sugar alcohols were higher than others(p<0.05). As substitution ratio was higher, springiness and chewiness of HMP jellies were lowered(p<0.01). Springiness of HMP jellies indicated positive correlation with general acceptance of sensory scores at 5% significant level. Brittleness, gum-miness and cohesiveness in all types of jellies were not significantly different.

A Study on the Preference of Hobakjook upon Material & Mixing ratio Change (호박죽의 재료와 배합비 변화에 따른 기호도 연구)

  • Determining the optimal mixing ratio of pumpkin, water, glutinous rice powder, red bean, kidney bean in preparing Hobakjook has been attempted and on the basis of it, Hobakjook with glutinous rice powder was set to be A group, and the one that glutinous rice powder was replaced by brown glutinous rice powder for the purpose of enhancing preference and nutrition was set to be B group. A group and B group with additional 0%, 5%, 10%, 15% perilla powder of pumpkin weight was labeled as Al-A4 and Bl-B4 respectively, and then analyzed nutritionally. Optimal material mixing ratio and preference was investigated by use of sensory evluation and instrumental test. As a result, the optimal material mixing ratio was determined as pumpkin 400 g, water 60 cc, glutinous rice powder 40 g, red bean 30 g, kidney bean 30 g, salt 4 g, sugar 10 g. In sensory evaluation significant difference (P < 0.05) was shown among the samples in color., flavor.1, falvor. II and overall quality. And A3 in viscosity, A2 in color, A4 in sweetness, B2 in flavor 1, Al and B2 in flavor 11, and B2 in overall quality was preferred most. Instrumental measurement shows that B4 in viscosity and A4 in sweentness was highestly recorded and showed significant difference (p<0.05). As the addition of perilla powder increased, the viscosity and sweetness was increased. In color measurement A4 in L value, A3 in a value, and Bl in b value was highestly recorded. And as perilla powder was added, L value and a value was increased while b value was decreased. Color in sensory evaluation was significantly correlated to b value in instrumental measurement. In summary, Hobakjook with brown glutinous rice was superior to the one with glutinous rice in preference and proximate composition, and as the addition of perilla powder was increased, proximate composition was increased but preference was rather decreased, and Hobakiook with 5% addition of perilla powder to brown glutinous rice powder was preferred most.

Effect of Molecular Weights of Polyethyleneimine on the Polyplex Formation with Calf Thymus DNA

  • In this study we investigated the spectral properties, including electric absorption, circular and linear dichroism (CD and LD), and fluorescence emission, of DNA in a DNA-branched polyethyleneimine (BPEI) polyplex at various polymer molecular weights (M$\_$w/) and BPEI-amine-to-DNA-phosphate ratios (N/P ratios). All BPEIs exhibited a common N/P dependence in their absorption and CD spectra. At N/P ratios < 1.0, we observed some hyperchromism in the absorption spectrum, red-shifts in CD bands, and decreases in LD intensity and fluorescence intensity of intercalated ethidium. At intermediate N/P ratios, complete collapse of all spectra occurred. As the N/P ratio increased further, the polyplex dissolved in water. From its characteristic CD spectrum obtained under these conditions, we conclude that the DNA exists in a B-like form. The fluorescence and LD intensities never recovered even at high N/P ratios- which indicates that the dissolved polyplex possesses positive charges and the DNA in the polyplex is condensed despite its B-form CD spectrum. The N/P range in which the absorption and CD signals collapsed was wider when the BPEIs M$\_$w/ decreased. In the case where the BPEIs M$\_$w/ was 0.8 k, recovery of the absorption and CD spectral properties at a high N/P ratio was never achieved, which suggests that the molecular weight of the polymer plays an important role in its dissolution at a high N/P ratio.

Salivary soluble receptor activator of nuclear factor kappa B ligand/osteoprotegerin ratio in periodontal disease and health

  • Purpose: The receptor activator of nuclear factor kappa B (RANK)/RANK ligand (RANKL)/osteoprotegerin (OPG) system plays a significant role in osteoclastogenesis, activation of osteoclasts, and regulation of bone resorption. This study aimed to evaluate the use of the salivary soluble RANKL (sRANKL)/OPG ratio as a diagnostic marker for periodontitis in nonsmokers. Methods: Twenty-five patients with chronic periodontitis and 25 individuals with a healthy periodontium were enrolled in this study. Samples containing 5 mL of unstimulated saliva were obtained from each subject. Salivary sRANKL and OPG concentrations were determined using a standard enzyme-linked immunosorbent assay. Statistical analysis was performed using SPSS ver. 18.0. Results: The levels of sRANKL and OPG were detectable in all of the samples. Positive relationships were found between the plaque index and clinical attachment level and both the salivary concentration of sRANKL and the salivary sRANKL/OPG ratio (P<0.05). The salivary concentration of sRANKL and the sRANKL/OPG ratio were significantly higher in the periodontitis group than in the healthy group (P=0.004 and P=0.001, respectively). In contrast, the OPG concentration showed no significant differences between the groups (P=0.455). Conclusions: These findings suggest that the salivary sRANKL/OPG ratio may be helpful in the screening and diagnosis of periodontitis. However, longitudinal studies with larger populations are needed to confirm these results.
