This study purposed to analyze the preference of names in Korean hospitals. Names were analyzed in terms of hospital's characteristics such as hospital type, location, ownership. Also, multivariate regression analysis was applied to assess how the amount of information measured from their names was varied by the hospitals' internal characteristics. Yearbook of Korean Hospital Association in 2008 was used to collect the study data. The full name of hospitals was divided into meaningful words to investigate the preferred words. The amount of information in hospital names was measured based on the number of words extracted. The study results showed that hospitals preferred different words depending on their characteristics. The amount of information contained in hospital names tends to decrease between before and after year 2000. Korean hospitals increased the uses of Hangeul and foreign words for their names in recent years. These changes were considered as the responses of Korean hospitals to increase competitive advantages by establishing unique brand power.
This study purports to examine the current management and information technology related strategy of Korean hospitals and suggest the effective management strategy in the 21st century when is digital era. Specifically the study tries to analyze the changing trends of strategic orientation and investigate the general management and information technology strategy of Korean hospitals. Self-administered Questionnaires were distributed to 721 hospitals nationwide and finally 98 Questionnaires were analyzed for the study. The results of the study are as follows : 1) Half of the respondent hospitals reported that they have an analyzer orientation in 2000, whereas 19.4% were prospectors, reactors 16.4%, and defenders 14.3%. However, the respondent hospitals intended to have a prospector orientation in the future (2002), while 29.6% planned on being analyzers, 17.3% reactors, and 3.1% defenders. 2) Hospital services for improving patient satisfaction were the most common. strategy for the respondent hospitals, followed by cost containment, organizational restructuring, employee education, purchasing system change, specialization of clinical services, quality improvement of medical care, strengthening the networking with the stakeholders, public relations and marketing strategy, diversification, and installing the information system. However, the strategies of annual salary system, retrenchment of unprofitable services, merit payment based on performance were still not popular for the respondent hospitals. 3) As for the strategies related with information technology, most hospitals have not implemented actively, except for the establishment of home-pages, order communication systems, and insurance claims through electronic data interchange system. 4) There were significant differences in the level of strategy implementation in terms of the ownership, bed size, financial performance, and the top managers I knowledge of information technology. The larger bed size, the higher financial performance, the better knowledge of information technology the top managers have, the more strategies the respondent hospitals implemented. The managerial and political implications for Korean hospitals in digital era were also discussed.
The purpose of this study is to reform the productive industrial relations in hospital. Because the industrial relations of hospital is antagonistic each other and this problem cause a low productivity. In Korea, hospitals maintain their ownership structure, strategy is controlled by government, short process of management, and culture of grade. The executive of hospital doesn't gain the confidence of union. This circumstances is related with the antagonistic industrial relations. This research is to analyze hospital management system through experiential and literature study, and is to present a reform strategies for cooperative industrial relations of improving productivity in hospital. As a results, if the hospital manager hopes to improve productivity, they should introduce expert management system rather than ownership and management system. And it is need to execute the hospital initiative strategy rather than to depend on the government. Moreover, the hospital can achieve productive industrial relations as a middle-long term visions in management process and horizontal organization culture. Although this study is limited by literature research, it will contribute to settle productive industrial relations in hospital and to accomplish the additional study.
This study aims to analyze the effects of job satisfaction on turnover intention among the nurses in geriatric hospitals. In terms of study methods, this study developed structured questionnaires, and distributed them to nurses working in 11 geriatric hospitals in metropolitan area. A total of 418 questionnaires (response rate: 87.0%) were used for final analysis. We conducted multiple regression analyses to analyze the influence of job satisfaction on turnover intention, adjusting for the characteristics of the research subjects. The results of this study are as follows. First, both job satisfaction and turnover intention were statistically significantly related to individual and hospital characteristics. Second, turnover intention was lower in older age group, and higher in smaller hospitals. As job satisfaction increases, turnover intention decreases. Third, the results of the regression analysis adjusting for subject's characteristics and job satisfaction's sub-categories show that the turnover intention is lower in nurses who are older, day worker, and work in bigger hospitals. Fourth, the result of subgroup analysis by hospital volume show that both of two groups are influenced by hospital ownership and satisfaction about compensation. In addition, professionalism had a significant effect on decreasing turnover intention in smaller hospitals. However, there was no statistically significant relationship between job satisfaction's sub-categories and turnover intention in larger hospitals. The results of this study confirms that job satisfaction lowers turnover intention among nurses in geriatric hospitals. It is suggested that the administrators of those hospitals continue to monitor critical factors associated with job satisfaction, and thus, reduce turnover intention, which helps improve quality of services.
The purpose of this study is to analyze the influence of external environment, organizational characteristics on informatization level in hospital settings, and to provide suggestions to improve the informatizaiton level of strategic information system of hospitals. In order to achieve study purpose, causal model was developed and empirically tested. Data were collected from 716 managers of 89 hospitals keeping information system in Busan and KyungNam Province using self-administered structured questionnaire. Major results of this study were as follows; First, among hospitals which had information training programs, informatization capacity was significantly higher. Moreover, informatization capacity was significantly varied by ownership type, institutional level, and number of beds of hospitals. Second, environmental uncertainty, intra-industry competition intensity, decision-making concentration, work standardization variables and perceived benefits were found to be significant affecting factors on informatization capacity as a result of path analysis. Third, support of top-management and perceived benefits were found to be significant affecting factors on informatization capability as a result of path analysis. Especially, informatization capability was positively affected by informatization capacity. Finally, perceived benefits was found to be significant affecting factors on informatization competency as a result of path analysis. Especially, informatization competency was positively affected by informatization capability.
Purpose: This is a phenomenological study to improve the quality of nursing and understand its essence by comprehensively analyzing job experience of nursing managers in small and medium. Methods: This study focused on deriving the common empirical attributes of all the study participants, rather than their individual attributes. Data on the job experiences of nine nurse managers in small and medium-sized hospitals were collected and analyzed using Colaizzi's phenomenological method. Result: The job experiences of nurse managers in small and medium hospitals were classified after analysis into 14 theme cluster sets and 34 themes in five categories. The categories derived were 'A feeling of pressure as if taking responsibility for the entire hospital', 'Taking on the difficulties of hiring a nurse alone.' 'Difficulty in mediating conflicts within the organization', 'Struggling to endure', 'To take root in the field with a sense of ownership'. Conclusion: This study is meaningful in helping nursing managers in small and medium hospitals perform their duties more efficiently and stably by understanding their job experience.
This study was performed to identify factors affecting the profitability of private hospitals in Korea different and to make informations that could be very helpful to hospitals in improving profitability. Data used in this study were collected from 112 hospitals with complete general data of present conditions as well as financial statements(balance sheets, income statements). They were chosen from hospitals that passed the standardization audit undertaken by the Korean Hospital Association from 1998 to 2001 for the purpose of accrediting training hospitals. The dependent variables were used operating margin to total assets and operating margin to gross revenues as proxy indicators for profitability. The independent variables were ownership type, location, bed size, period of establishment, debt to total assets, current ratio, fixed ratio, total asset turnover, average length of stay, bed occupancy rate, admission ratio of outpatients, personnel costs per adjusted inpatient, and fiscal years. The factors had significantly positive effect on operating margin to total assets and operating margin to gross revenues were bed size, total asset turnover. And the factors had significantly negative effect on operating margin to total assets and operating margin to gross revenues were period of establishment, debt to total assets, average length of stay, personnel costs per adjusted inpatient. The adjusted $R^2$ of multiple regression equation was 25.2%, 21.4% respectively. It is very important for private hospitals to improve profitability by raising total asset turnover, and reducing debt to total assets, average length of stay, and personnel costs per adjusted inpatient using the rational and efficient business strategy.
Nurse staffing level is an important factor that influences the quality of health service and patient outcomes. This study was carried out to examine the current state of acute hospital nurse staffing and find out factors that affect the nurse staffing level. Nurse staffing of individual hospitals was measured using the number of registered nurses per 100 beds. Descriptive and multiple regression analyses were conducted using 592 acute care hospitals' data. Regression model included structure factors such as referral level, ownership, medical and general staffing, and financial outcome factors such as occupancy rate, inpatient and outpatient revenues. Market characteristics included strength of competition, supply of nurses, and income and health status level of consumers. The average number of nurses per 100 beds was 28 and showed a great variation according to the referral level. Regression model explained this variation as much as 76.87%. Hospital structure variables which affecting the hospital nurse staffing level positively were ICU bed ratio, the staffing level of specialist, training doctor and employees except doctor and nursing personnel, while the negative factor was nurse aid staffing level. General hospitals employed more nurses than hospitals. Among outcome characteristics, occupancy rate and the amount of health insurance inpatient revenue affected positively on the hospital nurse staffing level. The more supply of the new nurse and the higher consumer income and health status in the medical service markets, the more nurses were employed by the medical institutes. According to the study result, hospitals employed more nurses when they had more financial incentive by increasing nurses. This means appropriate hospital incentive policy and regulation policy, which hospital violate nurse staffing level have to pay penality, should be needed. Clarifying job description between nurses and nurse aids and the reentry program for unemployed experienced nurses will be helpful to increase nurse staffing level.
This study was attempted to identify the liquidity trends and determinants of private hospitals in Korea different. Data used in this study were collected from 98 hospitals with complete general data of present conditions as well as financial statements(balance sheets, income statements). They were chosen from hospitals that passed the standardization audit undertaken by the Korean Hospital Association from 1996 to 2000 for the purpose of accrediting training hospitals. The dependent variables in this study were used current ration and quick ratio as a proxy indicator for liquidity. The independent variables were ownership type, hospital type, location, bed size, period of establishment, short-term liabilities to total assets, long-term liabilities to total assets, borrowings to total assets, fixed asset ration, net profit to total assets, operating margin to gross revenue, growth rate of net worth to total assets, total asset turnover, and business risk(volatility of profit). The major findings of this study were as follows. Trends of liquidity(current ratio, quick ratio) had been continuously decreased. Especially, There were very distinct decreasing trends of personal hospitals and less than 300beds, which weakened liquidity. The factors had significant effect on current ratio were short-term debt to total assets(-), fixed asset ratio(-), business risk(+). High short-term debt to total assets, high fixed asset ratio and high business risk significantly decreased in liquidity. The factors that significantly affected on quick ratio were short-term debt to total assets(-), borrowings to total assets(+), fixed asset ratio(-), business risk(+).
Purpose: The purpose of this study was to analyze the migration patterns of new nurses and experienced nurses and to identify the factors influencing inter-regional migration for solving regional imbalances of clinical nurses in South Korea. Methods: This study involved a secondary analysis of data from the Health Insurance Review and Assessment Service (HIRA). Data were analyzed using descriptive statistics and multiple logistic regression analysis. Results: New nurses tended to migrate from Kyunggi to Seoul. However, experienced nurses tended to migrate from Seoul and Chungchung to Kyunggi. Significant predictors of inter-regional migration among new nurses were location and nurse staffing grade of hospitals. Significant predictors of inter-regional migration among experienced nurses were location, hospital type, nurse staffing grade, ownership of hospitals and age of nurses. Conclusion: Inter-regional migration occupied a small portion of total hospital movement among clinical nurses. The regional imbalances of nurses were not caused by the migration from non-metropolitan areas to Seoul. Nurse shortage problems in the small and medium hospitals of the non-metropolitan area can be solved only through improvement of work environment.
