  • Diet-control has had an important role in cases of over weight syndrome and obesity, but in the most of the cases diet-control hasn' t been successful. In view of oriental medicine, those were regarded as deficiency syndrome. Recently I experienced a 41 years old woman who had arthralgia with over weight and whose condition was improved through Lizhong-tang used insufficiency of the spleen-yang.

A Study on the Three-Year Weight Changes of Workers at a Manufacturing Company and the Comparison of Metabolic Syndrome Diagnosis Components: Focused on the Data of Korean National Health Screening (2015~2017) (일개 제조업 근로자의 3년간 체중변화와 대사증후군 진단 구성요소의 비교 분석: 국가건강검진(2015~2017년) 자료를 중심으로)

  • Purpose: This study aims to provide preliminary data for weight management, and prevention and management of cardiovascular and cerebrovascular diseases. We examined the effect of changes in the weight of workers at a manufacturing company over three years on their metabolic syndrome and metabolic syndrome diagnosis components. Methods: Necessary data were collected from the questionnaire and the results of the Korean National Health Screening of 2015 and 2017, which included 228 workers at a manufacturing company in G region. The collected data were analyzed using the SPSS/WIN 23.0 program. ANCOVA was used to examine the differences in the metabolic syndrome diagnosis components according to weight change. In addition, multiple logistic regression analysis was used to obtain the odds ratios of metabolic syndrome and metabolic syndrome analysis component, based on the weight changes in the normal weight group and the obesity group. Results: Waist measure, systolic blood pressure, and blood pressure were found to have significant effects based on participants' weight change over three years. These factors increased with a larger increase in weight at a statistically significant level. This study analyzed the weight changes of the normal weight group and the obesity group considering the data from the National Health Screening of 2015, and found that the risk of metabolic syndrome increased at a statistically significant level as body weight increased; thus, the obesity group showed a higher risk in this regard. It was also found that waist measure, fasting blood sugar, and high-density low cholesterol increased at a statistically significant level as body weight increased. Conclusion: Health administrators need to recognize the importance of workers' weight management, select an intensive management group based on a time series analysis of weight changes, and develop and implement programs to manage the metabolic syndrome diagnosis components.

The Effect of Mildronate on Serum Alcohol Concentration and Hangover Syndrome (Mildronate가 혈청 알코올 농도와 숙취에 미치는 영향)

  • This study was conducted to determine whether Mildronate, an analogue of carnitine, influences blood alcohol concentration and hang-over syndrome in rabbits and healthy college male students. In the animal study, ten rabbits were randomly divided into two groups. One hundred mg per 1kg body weight of Mildronate was injected twice into five rabbits before injecting 50% ethanol; the rest of rabbits were injected with saline. The human study was performed with two sections. Each section of the study was conducted by a two-phase cross-over design with a four day wash-out period. All volunteers took Mildronate in one phase, and took a placebo in the next phase. The difference between the two sections was related to the time of taking the Mildronate pill and the amount of alcohol consumed. Blood alcohol concentrations were not significantly different between in those taking Mildronate and in those taking the placebo in both the human and the animal study. However, the concentration of serum aspartate aminotransferase(AST, GOT), the indicator of liver cell damage, was lowered in those taking Mildronate, compared to those taking the placebo. Also, headache and heartburn among hang-over syndrome patients were less severe with Mildronate. In conclusion, taking Mildronate prior to drinking alcohol can somewhat reduce liver cell damage and hang-over syndrome without stimulating alcohol metabolism.

A Study on the Analysis of Physical Function in Adults with Sarcopenia (근감소증 성인의 신체 기능 분석)

  • Purpose : This study used a sarcopenia diagnostic algorithm proposed by the Asia working group in adults over 50 to diagnose sarcopenia and analyze body function. The purpose of this study is to prepare basic data for the management and prevention of sarcopenia. Methods : We performed a diagnostic evaluation of sarcopenia in 97 adults over the age of 50 years with the cooperation of the Seongnam senior experience complex in Seongnam-si, Gyeonggi-do. As a result of the diagnostic process, 24 subjects were placed into the sarcopenia group, while 73 subjects were placed into the normal group. We measured each subject's body, performed the timed up and go test to evaluate functional mobility, and conducted a questionnaire on the pre-symptom of locomotive syndrome and locomotive syndrome. Results : There were statistically significant differences in height, weight, and skeletal muscle mass between the two groups. There was also a statistically significant difference in the timed up and go test, which confirmed the difference in functional mobility between the two groups. In addition, there was a statistically significant difference between the two groups in the proportion and the mean score of subjects with pre-symptom of locomotive syndrome and locomotive syndrome. In the correlation analysis, grip strength was statistically significantly correlated with height, weight, skeletal muscle mass, waist circumference, timed up and go test, pre-symptom of locomotive syndrome and locomotive syndrome. Gait speed was significantly correlated with the timed up and go test and locomotive syndrome. Appendicular skeletal muscle index was significantly correlated with height, weight, waist circumference, hip circumference, and the pre-symptom of locomotive syndrome. Conclusion : In conclusion, sarcopenia is closely related to height, weight, skeletal muscle mass and functional mobility, as well as the pre-symptom of locomotive syndrome and, locomotive syndrome. In consideration of this, the prevention and management of sarcopenia should be made accordingly.

Study of Relationship between Metabolic Syndrome and Bone Mineral Density on Post Menopausal Chronic Low Back Pain Patients Under Oriental Medicine Treatment (요통으로 한방치료중인 폐경 후 여성의 대사증후군 요인이 골밀도에 미치는 영향에 관한 연구)

  • This study was performed from April, 2007 to August, 2012 with female patients who were being treated for and suffering from chronic lumbar pain for periods of 6 months and over. The 53 female patients were diagnosed with osteoporosis by having a T-Score of <-2.5 in a bone mineral density(BMD), as well as showing signs of metabolic syndrome. This was deduced by taking measurements of blood pressure, carrying out blood-chemical examinations and physical measurements such as weight, height, waist measurement and body mass index(BMI). After 5 minutes rest, the patient's blood pressure, height and weight were measured. BMI was calculated using the equation BMI = weight (Kg)/height ($m^2$). The patients had their blood taken in a fasted state(more than 12hours), the fasting blood sugar, total cholesterol, triglyceride, HDL-cholesterol were measured. The average BMD and T-score were calculated by measuring BMD(mg/cc) of L1-L3 using QCT. In a correlation analysis of the physical examinations, clinical character of metabolic syndrome and T-score, the result showed that age and T-score had a negative correlation(r=-0.699, p<0.01) as did triglyceride and T-score (r=-0.047, p<0.01), where as weight(r=0.239, p<0.05) and height(r-=0.329, p<0.01) and T-score had a positive correlation. There was no significant correlation with total cholesterol, HDL cholesterol, blood sugar, blood pressure and T-score. This study showed that there are significant correlations with age, weight, height and T-score. But there are no significant correlations with total cholesterol, HDL cholesterol, blood sugar, blood pressure and T-score and that these did not influence bone density. Further research with more subjects is required to determine whether there is a correlation of clinical character of metabolic syndrome and T-score.

Bone Spur and Over Weight in Painful Heel Syndrome and Tenderness, Underlying Cause Analysis (종골부 동통 증후군의 원인분석)

  • Chiefly, painful heel syndrome is observed at old age. Many possible reasons were bursitis, plantar fasciitis, calcaneal periostitis around the calcaneus and achilles tendon. But the exact cause was not clearly identified due to complexity of subcalcaneal pain mechanism. The incidence of calcaneal spur and overweight were significant in painful heel syndrome. Our purpose of this study was analysis of underlyng cause and correlation about bony spur and overweight in painful heel syndrome. The author used incidence of heel spur on painful heel syndrome and body mass index to evaluate overweight. The material is 55 cases of painful heel syndrome patients and 60 cases of control group. Bony spur was one of the cause of painful heel syndrome. Body weight and calcaneal spur was developing factors on painful heel syndrome. And the result was as follows. 1. Spur formation incidence is 35 cases (63.6 %) in painful heel syndrome, 8 cases (13.3%) in normal control group. So, patient's group is significantly high (p<0.01). 2. Body mass index is 26.48 in painful heel syndromes, 21.75 in normal control groups. Overweight above index 27 is 22 cases (40%) in painful heel syndromes, 3 cases (5%) m normal control groups. So, patient's group is significantly high (p<0.01). 3. In painful heel syndrome, tenderness site is 46 cases (83.6%) in medial calcaneal tuberosity, 4 cases (7.3%) in central calcaneal tuberosity, 1 cases (1.8%) in both site. 4. Underlying causes of painful heel syndrome is 19 cases (34.5%) in plantar fasciitis, 16 cases (29.1%) in calcaneal periostitis, 11 cases (20%) in bursitis, 4 cases (7.3%) in tendinitis, 2 cases (3.6%) in entrapment neuropathy.

The Evaluation of a Health Coaching Program on Metabolic Syndrome Patients (대사증후군 대상자들의 건강코칭프로그램 평가)

  • Objectives: We assessed the feasibility of health coaching for health coaching program on metabolic syndrome. Methods: We developed a 6 month health coaching program on metabolic Syndrome. We recruited people with metabolic syndrome according to modified NCEP-ATP III. The participants were 9 men over 30 years of age who had taken a health screening at general hospital. We collected data such as demographics, BMI, body fat, blood pressure, HDL-cholesterol blood sugar and triglyceride. The program was analyzed by using Wilcoxon signed rank test. Results: Participants showed significantly decreased BMI, weight, waist circumference, body fat after 6 month program. They talked the awareness about their own behavior. They changed into better for eating habits, physical activities, and self management. Their discipline increased and eating habits became regular. They were satisfied to this program and showed strong confidence about their own change. Conclusions: Coaching did not direct certain behavioral change but guided self awareness and practice. Health coaching program showed long maintained effect to participants. We suggested health coaching as a helpful individual program to intervene risky health behavior especially for metabolic Syndrome.

Clinical Usefulness of Aripiprazole Treatment in a Girl with Prader-Willi Syndrome and Psychosis

  • Prader-Willi syndrome (PWS) is a quite rare multi-systemic genetic disorder strongly associated with psychiatric illness in adults, especially psychosis. This report presents a 16-year-old female with PWS and symptoms of brief psychotic disorder with a complete resolution of symptoms under aripiprazole medication. However, an exacerbation occurred after aripiprazole reduction. Apart from a weight gain of about 2 kg over the course of two years, no adverse effects could be found. This first report on the use of aripiprazole in a subject with PWS and psychosis suggests that aripiprazole might be a promising treatment approach in this distinct group of patients.

Cancer Cachexia in Pancreatic Cancer Patients: Recent Advances and New Therapeutic Approach

  • About 80% of all pancreatic cancer patients suffer from a wasting syndrome defined as the cancer cachexia characterized by abnormally low weight, weakness, and loss of skeletal muscle mass, which directly impacts physical activity, quality of life and overall survival. Over the past decades, we have gained new insights into the underlying mechanism of cachexia associated with pancreatic cancer. The aim of this review was to explore recent findings about cancer cachexia pathophysiology and describe the current pharmacologic approach. Pancreatic cancer cachexia is a multifactorial syndrome mediated by mechanical factors, inflammatory cytokines, neuropeptides, hormones and tumor-derived factors. The treatment of cancer cachexia remains controversial but is currently an active area of research. Several new targeted drugs are under investigation, and we hope to open a new prospect in the management of cancer cachexia in the future.

Effects of Individualized Intensive Physical Therapy for a Child with MECP2 Duplication Syndrome: A Case Study

  • PURPOSE: High-intensity physical therapy may help improve some gross motor developmental delays through an increased treatment frequency. This study describes an increase in physical therapy frequency and intensity over an eight-week time frame for a child with a rare genetic neurodevelopmental disorder. METHODS: A single-subject research was performed. The subject was a nine-year-old boy with MECP2 Duplication Syndrome. The outcome measure consisted of one time before and after the intervention. An intensive physical therapy program was applied to this subject. The treatments included reciprocal crawling, high kneeling, tailor sitting, weight-bearing and shifting training to facilitate ankle balance strategies and training to walk without assistance. RESULTS: After the eight-week follow-up, the child achieved a gradual improvement in the gross motor function measures, and the mother reported that the child frequently engaged in tailor sitting and independently walking. CONCLUSION: This case study highlights that intensive physical therapy improved the overall motor function of a child with MECP2 duplication syndrome.
