FIBROMA ON THE PALATE : A CASE REPORT (구개의 섬유종 : 증례보고)

  • The fibroma is the most common benign soft-tissue tumor found in the oral cavity and most commonly occurrs in sites predisposed to irritation or trauma. Treatment is conservative surgical excision. Seldom does the lesion recur. A 14-year old girl was referred to the Department of Pediatric Dentistry for evaluation and management of a mass on the palate. The patient's parents reported the slow growth of this soft tissue lesion over a 6-month period. A history of trauma or irritable habits was denied. Clinical examination revealed an asymptomatic, soft, pedunculated elevation of mucosa, measuring 3cm in diameter. Excisional biopsy of the lesion supported a diagnosis of fibroma. There have been no signs of recurrence during follow up of 4 years after surgical intervention. The clinical picture, pathology and management of the case are described, and the differential diagnosis and treatment are discussed.

Aorto-Coronary Bypass Graft -A Case Report- (관상동맥 회로술 치험 1예)

  • Occlusive coronary artery disease presents a potential threat to a significant population in the United States. According to many case reports, the increasing incidence of coronary artery disease due to atherosclerosis is noted in Korean, recently. Operative vascular procedures have increased steadily in number over the past 20 years in the United States. There have been many isolated case reports about coronary artery surgery, but these had little clinical impact. Nowadays, major three coronary bypass surgery has developed principally at three cardiac centers in the United States since 1967. Among three coronary bypass operation, the aorta-coronary artery saphenous vein bypass graft was first demonstrated by Favolaro and Effler at the Cleveland Clinic in 1967. We experienced one case of coronary artery disease, which was treated successfully by aorta-coronary saphenous vein graft with mild hypothermia under extracorporeal circulation in May, 1977. Saphenous vein was removed from above the knee and was 2.5 mm in diameter. The left ventricle was not vented for the left ventricle was not overdistended. Temporary artificial pace-maker-Medtronic-was implanted for the prevention and treatment of post-operative arrhythmia and heart block in post-operative first day. He is a 57 year old male businessman who had been suffered from hypertension [200 mmHg in systolic pressure] since 4 years ago, who had intermittent conservative treatment at local clinic. He had been afflicted with severe chest pain with choking sensation for 50 days. This symptom was aggravated exposing cold weather, or cold water, but was respond to rest. Pre-operative ECG revealed no any other ischemic sign except sinus bradycardia. Significant S-T segment depression was noted at lead II, AVF after double 5 minutes exercise, indicating positive Master`s test. Serum cholesterol was slight elevated to 253 mg/dl. Final pre-operative diagnosis was made by coronary arteriogram, which showed about 1.0-cm segmental 90 % occlusive atherosclerotic lesion in the proximal part of right coronary artery above the origin of acute marginal artery. Left coronary artery revealed good patency and there was no collateral circulation between right and left coronary artery .Hospital course was not eventful. He was discharged with good result on the post-operative day. He has been free from chest pain for longer than 2 years. And also the arterial flow in the coronary bypass graft is auscultated with the pocket-sized ultrasonic velocity detector, which shows the patency of the coronary bypass graft good.

  • The prevalence and interest of dental erosion seems to be rising in children all over the world. Thus, This study was performed to investigate the prevalence and severity of dental erosion in the primary molars at terminal stage nearing exfoliation, and associated risk factors. An examination was performed on 788 children using modified Linkosalo & Markkanen system, and questionnaires were gained. Association between dental erosion and the risk factors were statistically analyzed with chi-squared test and logistic regression analysis at a significance level of p < 0.05. As a result, 213 children (27%) showed dental erosion, and the mandibular left first primary molar was the most influenced tooth. According to the analysis of risk factors, frequent intake of carbonated drinks and fruit juices showed significant development of erosion (p < 0.05), and using straw for drinking fruit juices showed significant a reduction of erosion (p < 0.05).

Correlates of Depressive Symptoms among College Students on Jeju Island in Korea

  • Objectives: The aim of the present study was to determine the prevalence depressive symptoms, and the correlates of depressive symptoms, and to propose countermeasures for individual depression risk factors among college students. Methods: One university and three colleges in Jeju-do were selected as the study population and a total of 1000 students were selected by multiphasic cluster sampling to represent each department and year within each college. Of the 945 total respondents, 421 were men and 524 were women. The Beck Depression Inventory was used to evaluate depression and the BDI STEN score was used to assess depressive symptoms. Multiple logistic regression analysis was performed for comparisons. Results: 1. The prevalence of depressive symptoms among men was 14.4%, while among women the level rose to 26.8%. 2. Those with poor life satisfaction scores were significantly more likely to have depressive symptoms than those with good life satisfaction (odds ratio (OR) = 2.62; (95% confidence interval (95%CI), 1.483-4.629). 3. Students over 23 years old were significantly more likely to have low levels of depressive symptoms than those under 18 (OR = 0.439; 95%CI, 0.247-0.779). 4. Groups with poor body image satisfaction had higher levels of depressive symptoms when compared to respondents who had good body image (OR = 2.847; 95%CI, 1.341-6.045). 5. Students with poor perceived health status were significantly more likely to have high levels of depressive symptoms than those with good perceived health status (OR = 2.029; 95%CI, 1.157-3.557). Conclusion: Universities and colleges should provide education programs, such as health education that facilitates the development of a healthy body image. In addition, they should focus on various scholarships that could improve satisfaction with student life, especially for alienated students. Furthermore, colleges should help direct students towards engagement in more productive work, and lead social policies away from the dominance of appearance.

  • Purpose : This study is to examine the characteristics of the elderly and their family caregivers. to study the main factors affecting Nursing Home utilization, and to help frame a policy about Nursing Home's demand. management and welfare facilities. Method: Data for this present study were collected via questionnaires randomly distributed to family caregivers of the over 60-year-old patients at general hospital and university hospital in Seoul. Kyung-Ki Do and In-Chon from 26 June to 20 July, 2000. Questionnaires were issued to 512 people and 479 were returned. The data was analyzed by frequency, $x^2$-test, t-test, ANOVA. factor analysis. correlation coefficients analysis and Stepwise multiple regression analysis using SPSS 9.0. Result First. Instrumental Activities of Daily Living(IADL), duration of hospitalization, sex, marital status, behavior problems, home ownership, and cognitive disorder about elderly patients affect family caregivers burden. one of need factors. Secondly, marital status. religion. health status, sex, education and age in the family caregiver predisposing factors are main factors on Nursing Home utilization. Third, in need factors, care burden. time burden, family relation burden, physical burden and mental burden have an effect on Nursing Home utilization. Finally, the model the most important factors that affect Nursing Home utilization is composed of six of eleven totally, care burden, religion, time burden, health status, marital status and education. When the family caregivers get care burden and time burden. are highly educated, have no religion, have health problems, and have no spouse, it is possibility for them to utilize Nursing Homes. Conclusion: The government should decrease a family caregivers burden and seek to find how to support Nursing Homes. Furthermore. Social support program for the family caregivers should be required. Thus, the family caregivers need consultation and need to meet to talk about their patients. how to care them, get information, which are the crucial field in advancing the research in nursing science.

Compression of the Superficial Radial Nerve by Schwannoma: A Case Report (신경초종에 의한 표재요골신경의 압박)

  • Purpose: Schwannoma, a benign peripheral nerve tumor, is slow-growing, encapsulated neoplasm that originates from the Schwann cell of the nerve sheath. Schwannoma most frequently involves the major nerve. Schwannoma occurring in the superficial radial nerve rare. This is a report of our experience with schwannoma arising from the superficial radial nerve with neurologic symptom. Methods: A 55-year-old woman presented with eight-month history of progressive numbness and paresthesia in dorsum of the thumb and index finger. Physical examination revealed a localized mass on the midforearm. Sonographic examination showed an ovoid, heterogenous, hypoechoic lesion, located eccentrically in related to the superficial radial nerve. The lesion was mobile in the transverse but not in the longitudinal axis of the nerve, which was thought to favour schwannoma rather than neurofibroma. At operation, a $20{\times}15mm$ ovoid, yellowish grey mass was seen arising from the superficial radial nerve. The tumor present as eccentric masses over which the nerve fibers are splayed. Using operating microscope, the tumor was removed, preserving the surrounding nerve. Results: Histology confirmed that the mass was a benign schwannoma. There were no postoperative complications. After two months the patient had no clinically demonstrable sensory deficit. Conclusion: An unsusual case of a schwannoma of the superficial radial nerve is presented. In case with neurologic symptom, prompt surgical decompression must be made to prevent further nerve damage and to restore nerve function early.

Systemic Pemphigus Vulgaris: A Case Report (전신성 심상성 천포창의 치험례)

  • Purpose: Pemphigus vulgaris, a rare autoimmune blistering disease of the skin and mucous membranes remains a challenging disease to treat. Management is focused on immunotherapy against autoimmune antibodies that target keratinocyte cell adhesion molecules, and antibiotics preventing secondary infections. There is no established dressing protocol and skin is usually manipulated the least amount possible in order to minimize irritation. The authors suggest that early initiation of aggressive bathing and debridement of skin lesions, with nutritional support, is essential in accelerating resolution. Methods: A 40 year-old male previously diagnosed with pemphigus vulgaris was admitted due to exacerbation of mucocutaneous lesions involving the epidermis and mucosa of the whole body. Steroids, immunosuppressants, intravenous immunoglobulin and antibiotics were administrated, but infection and de-epithelialization progressed, while his general condition deteriorated with a weight loss of over 20 kilograms. The plastic surgery department intervened with daily bathing, debridement of unhealthy debris and non-traumatizing coverage of growing epithelium. Total parenteral nutrition and mobilization with rehabilitation therapy was initiated as early as possible. Results: After bathing, healthy epithelium gradually covered the patient's entire body, while his general condition improved with a corresponding weight gain of 14 kgs. Conclusion: Treatment of pemphigus vulgaris focuses on immunotherapy and infection control. However, an equal amount of attention should be laid on early intervention with daily dressings including bathing and irrigation, nutritional support, and exercise as this accelerates resolution of existing infections, promotes healthy epithelialization and leads to faster recovery.

Small Intestine Resection and Anastomosis for Epiploic Foramen Entrapment of a Horse (말의 그물막구멍포착 교정을 위한 소장 절제 및 문합 1례)

  • A 10-year-old gelding Warmblood weighing 560 kg was referred to J&C Equine Hospital with the history of hyperpnea, depression, pawing, and rolling for 7 hours. According to the results of clinical and ultrasound examination, it was considered that intestines were distended with thickened wall. The horse had been treated with lactated Ringers' solution (14 L, IV), flunixin meglumine (1.1 mg/kg, IV), and mineral oil (1 L, PO), but he did not show any responses to those treatments. Exploratory laparotomy was performed and identified incarcerated small intestine through the epiploic foramen. The horse received resection and anastomosis of the entrapped small intestine. After surgery, the horse was treated with intensive postoperative care of fluid therapy (5 L with 20 mEq/L KCl, every 2 hours), flunixin meglumine (1.1 mg/kg, IV, sid), antibiotics (penicillin 22,000 IU/kg, IV, qid and gentamicin 6.6 mg/kg, IV, sid), lidocaine constant rate infusion (bolus 1.3 mg/kg over 15 minutes then 0.05 mg/kg/minute), common nutritional supplements, nasogastric intubation every 2 hours and trunk bandage. Postoperative feeding program had started with small amount of hay every 4 hours and gradually increased to normal amount till 5 days. At 77 days after surgery, he showed sudden outbreak of colic and was euthanized. The causes of colic were small intestinal strangulation by passing through the mesenteric rents and postoperative adhesion between small intestines. According to the results, it is recommended to perform perioperative intensive care of horse with colic and to use several methods to prevent adhesions during abdominal surgery of horses.

Acute Kidney Injury after Dose-Titration of Liraglutide in an Obese Patient (비만 환자에서 리라글루티드 증량 과정에서 발생한 급성 신손상)

  • Liraglutide (SaxendaR) is prescribed to induce and sustain weight loss in obese patients. The starting dose of liraglutide is 0.6 mg/day for 1 week, which is increased by 0.6 mg/day every week until the full maintenance dose of 3 mg/day is achieved. Such dose titration is needed to prevent side effects, which primarily include gastrointestinal problems such as nausea, diarrhea, constipation, vomiting, dyspepsia, and abdominal pain. A 35-year-old, reportedly healthy obese man receiving liraglutide treatment for obesity visited the emergency room complaining of generalized weakness and dizziness accompanied by repeated diarrhea and vomiting. He reported over 20 episodes of diarrhea starting the day after liraglutide dose escalation from 1.2 mg/day to 1.8 mg/day. Laboratory findings suggested pre-renal acute kidney injury, including serum creatinine 4.77 mg/dl, blood urea nitrogen (BUN) 37 mg/dl, estimated glomerular filtration rate (eGFR) 15 ml/min/1.73 m2, and Fractional excretion of sodium 0.08. After volume repletion therapy, his renal function recovered to a normal range with laboratory values of creatinine 1.08 mg/dl, BUN 14 mg/dl, and eGFR 88 ml/min/1.73 m2. This case emphasizes the need for caution when prescribing glucagon-like peptide-1 receptor agonists, including liraglutide, given the risk of serious renal impairments induced by volume depletion and dehydration through severe-grade diarrhea and vomiting.

Traditional Korean Medicine for Treating Bechet's Disease Symptoms of the Upper Burner: A Case Report (베체트병으로 상초(上焦) 부위의 증상을 호소하는 환자에 대한 치험1례)

  • Objective: This case report presents the effects of traditional Korean medical treatment for a patient with symptoms of the upper burner due to Beçhet's disease. Methods: A 42-year-old male patient was treated with traditional Korean herbal medicine (herbal formulas and patent medicines), acupuncture, and cupping therapy. Neck, ocular, and right wrist pain and xerostomia were assessed daily with a numeric rating scale (NRS) and grading assessment tool for subjective symptoms. Results: For a total of 20 days over two hospitalizations, the patient's neck pain was relieved from NRS 4 to 1, the ocular pain from NRS 5 to 1, the right wrist pain from NRS 7 to 1, and the xerostomia from grade 8 to 1. Conclusion: These results show that Beçhet's disease symptoms of the upper burner can be relieved with traditional Korean medical treatment, although additional well-designed studies are required to confirm these findings.
